Effect of ulinastatin combined with anti-infective therapy on systemic inflammatory response and oxidative stress response in patients with acute severe pneumonia

Objective: To study the effect of ulinastatin combined with anti-infective therapy on systemic inflammatory response and oxidative stress response in patients with acute severe pneumonia. Methods: A total of 50 patients with acute severe pneumonia who were treated in the hospital between September 2014 and December 2016 were collected and divided into control group and observation group according to the random number table method, 25 cases in each group. Control group received conventional therapy, and the observation group received ulinastatin combined with conventional therapy. The differences in inflammatory response and oxidative stress response in serum were measured before treatment and 10 d after treatment. Results:Before treatment, differences in serum levels of inflammatory response indexes and oxidative stress indexes were not statistically significant between the two groups. After treatment, serum IL-2, IL-6, PCT, HMGB1, CRP, IL-4, IL-10, IL-13, MDA, AOPP and ROS levels of both groups of patients were lower than those before treatment while SOD, T-AOC, GSH-Px and LHP levels were higher than those before treatment, and serum IL-2, IL-6, PCT, HMGB1, CRP, IL-4, IL-10, IL-13, MDA, AOPP and ROS levels of observation group were lower than those of control group while SOD, T-AOC, GSH-Px and LHP levels were higher than those of control group. Conclusion: Ulinastatin combined with anti-infective therapy can effectively reduce the systemic inflammatory response and oxidative stress response and optimize the overall condition in patients with acute severe pneumonia.

BACKGROUND Rectal carcinoma(RC),one of the most common malignancies globally,presents an increasing incidence and mortality year by year,especially among young people,which seriously affects the prognosis and quality of life of patients.At present,dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI)parameters and serum carbohydrate antigen 19-9(CA19-9)and CA125 Levels have been used in clinical practice to evaluate the T stage and differentiation of RC.However,the accuracy of these evaluation modalities still needs further research.This study explores the application and value of these methods in evaluating the T stage and differentiation degree of RC.AIM To analyze the diagnostic performance of DCE-MRI parameters combined with serum tumor markers(TMs)in assessing pathological processes and prognosis of RC patients.METHODS A retrospective analysis was performed on 104 RC patients treated at Yantai Yuhuangding Hospital from may 2018 to January 2022.Patients were categorized into stages T1,T2,T3,and T4,depending on their T stage and differentiation degree.In addition,they were assigned to low(L group)and moderate-high differentiation(M+H group)groups based on their differentiation degree.The levels of DCE-MRI parameters and serum CA19-9 and CA125 in different groups of patients were compared.In addition,the value of DCE-MRI parameters[volume transfer constant(Ktrans),rate constant(Kep),and extravascular extracellular volume fraction(Ve)in assessing the differentiation and T staging of RC patients was discussed.Furthermore,the usefulness of DCE-MRI parameters combined with serum CA19-9 and CA125 Levels in the evaluation of RC differentiation and T staging was analyzed.RESULTS Ktrans,Ve,CA19-9 and CA125 were higher in the high-stage group and L group than in the low-stage group and M+H Group,respectively(P<0.05).The areas under the curve(AUCs)of the Ktran and Ve parameters were 0.638 and 0.694 in the diagnosis of high and low stages,respectively,and 0.672 and 0.725 in diagnosing moderate-high and low differe

Neural operators have been explored as surrogate models for simulating physical systems to overcome the limitations of traditional partial differential equation(PDE) solvers. However, most existing operator learning methods assume that the data originate from a single physical mechanism, limiting their applicability and performance in more realistic scenarios. To this end, we propose the physical invariant attention neural operator(PIANO) to decipher and integrate the physical invariants for operator learning from the PDE series with various physical mechanisms. PIANO employs self-supervised learning to extract physical knowledge and attention mechanisms to integrate them into dynamic convolutional layers.Compared to existing techniques, PIANO can reduce the relative error by 13.6%–82.2% on PDE forecasting tasks across varying coefficients, forces or boundary conditions. Additionally, varied downstream tasks reveal that the PI embeddings deciphered by PIANO align well with the underlying invariants in the PDE systems, verifying the physical significance of PIANO.

A time-inconsistent linear-quadratic optimal control problem for stochastic differential equations is studied.We introduce conditions where the control cost weighting matrix is possibly singular.Under such conditions,we obtain a family of closed-loop equilibrium strategies via multi-person differential games.This result extends Yong’s work(2017) in the case of stochastic differential equations,where a unique closed-loop equilibrium strategy can be derived under standard conditions(namely,the control cost weighting matrix is uniformly positive definite,and the other weighting coefficients are positive semidefinite).

Purpose:To identify the potential target genes of blast lung injury(BLI)for the diagnosis and treatment.Methods:This is an experimental study.The BLI models in rats and goats were established by conducting a fuel-air explosive power test in an unobstructed environment,which was subsequently validated through hematoxylin-eosin staining.Transcriptome sequencing was performed on lung tissues from both goats and rats.Differentially expressed genes were identified using the criteria ofq≤0.05 and|log2 fold change|≥1.Following that,enrichment analyses were conducted for gene ontology and the Kyoto Encyclopedia of Genes and Genomes pathways.The potential target genes were further confirmed through quantitative real-time polymerase chain reaction and enzyme linked immunosorbent assay.Results:Observations through microscopy unveiled the presence of reddish edema fluid,erythrocytes,and instances of focal or patchy bleeding within the alveolar cavity.Transcriptome sequencing analysis identified a total of 83 differentially expressed genes in both rats and goats.Notably,49 genes exhibited a consistent expression pattern,with 38 genes displaying up-regulation and 11 genes demonstrating down-regulation.Enrichment analysis highlighted the potential involvement of the interleukin-17 signaling pathway and vascular smooth muscle contraction pathway in the underlying mechanism of BLI.Furthermore,the experimental findings in both goats and rats demonstrated a strong association between BLI and several key genes,including anterior gradient 2,ankyrin repeat domain 65,bactericidal/permeability-increasing fold containing family A member 1,bactericidal/permeability-increasing fold containing family B member 1,and keratin 4,which exhibited up-regulation.Conclusions:Anterior gradient 2,ankyrin repeat domain 65,bactericidal/permeability-increasing fold containing family A member 1,bactericidal/permeability-increasing fold containing family B member 1,and keratin 4 hold potential as target genes for the prognosis,diagnosis,and treatment of BL

We introduce notions of continuous orbit equivalence and its one-sided version for countable left Ore semigroup actions on compact spaces by surjective local homeomorphisms,and characterize them in terms of the corresponding transformation groupoids and their operator algebras.In particular,we show that two essentially free semigroup actions on totally disconnected compact spaces are continuously orbit equivalent if and only if there is a canonical abelian subalgebra preserving C^(∗)-isomorphism between the associated transformation groupoid C^(∗)-algebras.We also give some examples of orbit equivalence,consider the special case of semigroup actions by homeomorphisms and relate continuous orbit equivalence of semigroup actions to that of the associated group actions.

Phyllosphere microorganisms are a crucial component of environmental microorganisms,highly influenced by host characteristics,and play a significant role in plant health and productivity.Nonetheless,the impact of host characteristics on shaping phyllosphere microbial communities of plants with different life forms remains ambiguous.Utilizing high-throughput sequencing technology,this study analyzed the diversity and community composition of phyllosphere epiphytic microorganisms(e.g.,bacteria and fungi)of various plant life forms in the hinterland of the Gurbantunggut Desert,Northwest China.Functional annotation of prokaryotic taxa(FAPROTAX)and fungi function guild(FUNGuild)were employed to assess the ecological functions of microorganisms and to investigate the role of stochastic and deterministic processes in shaping phyllosphere microbial communities.Result showed a diverse array of phyllosphere epiphytic microorganisms in the desert plants,with Proteobacteria,Cyanobacteria,and Actinobacteriota dominating bacterial community,while Ascomycota and Basidiomycota were prevalent in fungal community.Comparison across different plant life forms highlighted distinct microbial communities,indicating strong filtering effects by plant characteristics.FAPROTAX prediction identified intracellular parasites(accounting for 27.44%of bacterial community abundance),chemoheterotrophy(10.12%),and phototrophy(17.41%)as the main functions of epiphytic bacteria on leaves of different life form plants.FUNGuild prediction indicated that phyllosphere epiphytic fungi primarily served as Saprotrophs(81.77%),Pathotrophs(17.41%),and Symbiotrophs(0.82%).Co-occurrence network analysis demonstrated a predominance of positive correlations among different microbial taxa.Raup-Crick dissimilarity index analysis revealed that deterministic processes predominantly influenced phyllosphere bacterial and fungal community assembly.Variance partitioning analysis and random forest modeling suggested that plant leaf functional traits significantly impacted

To restore dam-blocked natural fish migratory passages,a growing number of artificial fishways have been built in water conservancy and hydropower projects in China.The Angu hydropower station involved diverse important fish habitats in the lower reaches of the Daduhe River in Southwest China.Therefore,a vertical slot fishway(VSF)and a nature-like fishway(NLF)were built near the backwater area of the reservoir to connect the upstream and downstream habitats.Hydrodynamic and aquatic ecological surveys were conducted after the completion of the project to estimate the fish passing effect of the two fishways.The results indicated that both fishways were in effective operation and could maintain the desired hydrodynamic conditions and be used by several local fish species.During the survey,149 fish from 15 species and 111 fish from 17 species were captured by the traps in the VSF and NLF,respectively,while 1263 fish from 27 species were found in the downstream area.Some species captured in the VSF were not found in the NLF,and vice versa,which implied the different preferences of fish.Meanwhile,3789 signals including 2099 upward ones and 1690 downward ones were monitored with an ultrasonic fish detector at the inlet of the VSF.These findings revealed the characteristics of fish species observed in and near the fishways and provided valuable insights into the different fish passing capabilities of VSFandNLF.

Upregulation of vascular endothelial growth factor A/basic fibroblast growth factor(VEGFA/b FGF)expression in the penumbra of cerebral ischemia can increase vascular volume,reduce lesion volume,and enhance neural cell proliferation and differentiation,thereby exerting neuroprotective effects.However,the beneficial effects of endogenous VEGFA/b FGF are limited as their expression is only transiently increased.In this study,we generated multilayered nanofiber membranes loaded with VEGFA/b FGF using layer-by-layer self-assembly and electrospinning techniques.We found that a membrane containing 10 layers had an ideal ultrastructure and could efficiently and stably release growth factors for more than 1 month.This 10-layered nanofiber membrane promoted brain microvascular endothelial cell tube formation and proliferation,inhibited neuronal apoptosis,upregulated the expression of tight junction proteins,and improved the viability of various cellular components of neurovascular units under conditions of oxygen/glucose deprivation.Furthermore,this nanofiber membrane decreased the expression of Janus kinase-2/signal transducer and activator of transcription-3(JAK2/STAT3),Bax/Bcl-2,and cleaved caspase-3.Therefore,this nanofiber membrane exhibits a neuroprotective effect on oxygen/glucose-deprived neurovascular units by inhibiting the JAK2/STAT3 pathway.
