A Pregnant Patient with Aortic Regurgitation and Symptoms of Acute Heart Failure Caused by Peripartum Cardiomyopathy: A Case Report

Introduction: Knowledge of the risks of pregnancy with heart disease is important because the maternal mortality is much higher than the average. Peripartum cardiomyopathy (PPCM) is rare but it is one of major causes of maternal death. We experienced a pregnant patient with severe aortic regurgitation (AR) presented symptoms of acute heart failure. Her heart failure was not better after an emergency cesarean section and aortic valve replacement (AVR) therefore we think that PPCM caused her heart failure. Case presentation: A 35-year-old woman diagnosed as having severe AR became pregnant. No changes in the AR were apparent during pregnancy. However, the patient developed symptoms of acute heart failure at 37 weeks of gestation, and an emergency cesarean section was performed under general anesthesia. Her hemodynamic status worsened after the cesarean section, and AVR was performed. She was supported with percutaneous cardiopulmonary support (PCPS) after the operation. As recovery seemed to take longer than usual, we decided to implant a ventricular assist device (VAD). Her condition improved after VAD placement, but then she died from a cerebral infarction. In this case, the heart failure was an acute-onset even though AR was stable before and after the pregnancy, and the heart failure did not improve after AVR. Therefore, we concluded that PPCM, rather than AR caused her heart failure. Conclusions: We encountered a case of a pregnant patient with severe AR who presented with symptoms of acute heart failure caused by PPCM. The effect of AR to her heart failure could not be easily denied. This delayed the diagnosis of PPCM, which in turn delayed our decision to use a VAD. Therefore, PPCM should be considered when pregnant patients with heart disease present symptoms of heart failure.

根据荷兰文献及口述资料考察17—18世纪哆啰美远人的历史,发现台湾东海岸的各个族群之间有着千丝万缕的关系。各族群同属南岛语族的文化系统,且互动关系密切,但是各族群之间也存在着巨大的文化差异。哆啰美远人与周边族群的关系包括:与拥有相同语言系统的巴塞人的贸易关系,与宜兰平原的哆啰美远社同族人的关系,与被总称为Takilis的Pabanangh人和Dadangh人的关系,与被称为Parrougearon人的山地住民的关系,以及同Mk-qaulin及太鲁阁人的关系。这些族群关系体现了从17世纪末到18世纪中叶,各族群之间围绕社会生活领域所展开的争夺历史,以及汉人开拓移民剧增等因素引起的原住民社会经济生活的变化。

The objective of this study is to investigate the effect of the ultrasound-microbubble technique in nuclear factor kappa B(NF-κB) decoy oligodeoxynucleotide(ODN) transfection in the gingival tissue in mice. The 6-FAM-labeled scrambled decoy ODN with microbubbles was applied to the periodontal tissue in 8-week-old male C57BL/6J mice by ultrasound radiation at low(LUM-Sc) and high(HUM-Sc) intensities to optimize the transfection condition of the ultrasound-microbubble *** inspections were performed two hours after transfection to compare the expression with that in the sham-operated group without ultrasound radiation(A-Sc). Then, an NF-κB decoy was transfected into the periodontal tissue using the highintensity ultrasound-microbubble(HUM-NF) technique to examine the anti-inflammatory effects of the decoy ODN. Western blot analysis was performed to investigate the expression of interleukin(IL)-1β, IL-6 and intercellular adhesion molecule-1(ICAM-1)in the gingival tissues in the HUM-Sc, the HUM-NF and control groups. The fluorescence microscopy results showed that the fluorescent intensity in the periodontal tissues in the LUM-Sc and HUM-Sc groups was significantly higher than that in the A-Sc and the control groups. The fluorescent intensity in the HUM-Sc group, especially in the gingival connective tissue,was the highest of all groups. Western blot analysis indicated that the protein expression levels of IL-1β, IL-6 and ICAM-1 in the HUM-NF group were significantly lower than those in the HUM-Sc and the control groups. These findings suggest that the high-intensity ultrasound-microbubble technique is an effective tool for decoy transfection into the periodontal tissue.

土壤生物，尤其是土壤微生物多样性与活性的保持是农业生态系统健康稳定的基础。农业活动尤其是农作物植被类型的改变对土壤生物的群落结构和活性具有显著的影响。释放后的转基因作物作为生态系统的一种新的生物组分，被引入农田生态系统之后所引发的农田生物群落（包括土壤微生物群落）的变化及其对农业生态系统的健康与稳定产生的影响，已成为研究热点。本文对转抗虫Bt基因作物、转T4－溶菌酶基因作物、转蛋白酶抑制剂1基因作物的基因产物、作物残体在土壤中的行为（如降解产物的存留形态与生物活性）及其对根际或残体周围土壤中各类生物，尤其是微生物群落结构与功能的影响进行了简要综合评述，指出基因表达产物的后效肯定是存在的且长远的，由其引发的土壤生物群落结构的变化是复杂的，因而有必要对不同类型的转基因作物释放后的生态效应做长期的跟踪研究。建议未来的研究工作应集中在以下3方面：（1）不同的转基因表达产物在环境中的迁移、结构变化、消长动态及其对生物保持毒杀性的时间；（2）不同类型转基因植物对土壤生物群落结构的影响趋势；（3）在实验室条件下，研究分离纯化的各种转基因表达产物对土壤各生物功能类群的影响。

Influence of Pressure on Germination of Garden Cress, Leaf Mustard, and Radish Seeds at Various Temperatures

The effects of hydrostatic pressure (0.1 - 400 MPa) and temperature (4°C, 25°C, and 35°C) on the germination of three types of seeds (garden cress, leaf mustard, and radish) were studied. The normal germination rate of the three types of seeds was decreased at high hydrostatic pressure, and germination time tended to be delayed. Pressure and temperature had two types of effects on seed germination. Germination of garden cress and leaf mustard seeds was more resistant to pressure at lower temperature. Conversely, germination of radish seeds was most pressure-sensitive at low temperature, and germination drastically decreased with treatment at 50 MPa and 4°C. Generally, pressure and temperature effects on protein structure and enzyme activity have been classified into two types, “hillside”-like (pressurization decreases the stable temperature range) and “tongue”-like (stabilizing effect of moderate pressure against heat denaturation). Therefore, the type of temperature-pressure effects on germination of garden cress and leaf mustard seeds is classified as “hillside”-like and that of radish seeds is classified as “tongue”-like, similarly to the generally observed effects on protein denaturation.
