High mobility group box 1 in the central nervous system:regeneration hidden beneath inflammation

High-mobility group box 1 was first discovered in the calf thymus as a DNA-binding nuclear protein and has been widely studied in diverse fields,including neurology and ***-mobility group box 1 in the extracellular space functions as a pro-inflammatory damage-associated molecular pattern,which has been proven to play an important role in a wide variety of central nervous system disorders such as ischemic stroke,Alzheimer’s disease,frontotemporal dementia,Parkinson’s disease,multiple sclerosis,epilepsy,and traumatic brain *** drugs that inhibit high-mobility group box 1 as a damage-associated molecular pattern,such as glycyrrhizin,ethyl pyruvate,and neutralizing anti-high-mobility group box 1 antibodies,are commonly used to target high-mobility group box 1 activity in central nervous system *** it is commonly known for its detrimental inflammatory effect,high-mobility group box 1 has also been shown to have beneficial pro-regenerative roles in central nervous system *** this narrative review,we provide a brief summary of the history of high-mobility group box 1 research and its characterization as a damage-associated molecular pattern,its downstream receptors,and intracellular signaling pathways,how high-mobility group box 1 exerts the repair-favoring roles in general and in the central nervous system,and clues on how to differentiate the pro-regenerative from the pro-inflammatory *** targeting high-mobility group box 1 in the central nervous system may benefit from differentiating between the two functions rather than overall suppression of high-mobility group box 1.

Complication after nipple-areolar complex tattooing performed by a non-medical person:A case report

BACKGROUND Compared to earlier,there has been an increase in the tattoo procedures for cosmetic purposes;and there has also been an increase in the tattoo procedures performed by non-medical *** South Korea,only tattoos performed by a doctor are considered legal;however,there is still some debate over whether tattoo procedures performed by non-healthcare providers should be considered *** SUMMARY A 28-year-old woman visited our hospital with pain in both nipples and heat sensation over the last 4 *** had a history of a nipple tattoo performed by an unlicensed *** bleeding was noted in both areolar areas,and the exudate mixed with pus and orange color ink was *** medication and tulle with foam dressing were performed under the impression of cellulitis and allergic *** 4 wk,nipples remained dark brown in color,resulting in a color mismatch between the nipple and orange-colored *** size of the areola was also found to be distinctly asymmetrical after *** complication may have been caused by the use of illegal ink or unsanitary procedures,or a problem may have occurred in the post-tattoo management *** Doctors use approved ink,aseptic procedure and appropriate postoperative care,and appropriate management can be performed in case of complications.

Secondary craniofacial necrotizing fasciitis from a distant septic emboli: A case report

BACKGROUND Craniofacial necrotizing fasciitis(CNF)is an uncommon but fatal infection that can spread rapidly through the subfascial planes in the head and neck *** usually progress rapidly,and early management is necessary to optimize *** SUMMARY A 43-year-old man visited our hospital with left hemifacial swelling involving the buccal and submandibular *** patient had fever for approximately 10 d before visiting the hospital,but did not report any other systemic *** tomography scan demonstrated an abscess with gas *** surgical drainage of the facial abscess,the patient’s systemic condition worsened and progressed to septic *** examination revealed pulmonary and renal *** percutaneous catheter drainage was performed at the renal abscess site,which caused improvement of *** patient showed no evidence of systemic complications during the 4-mo post-operative follow-up *** As the patient did not improve with conventional CNF treatment and symptoms only resolved after controlling the infection,the final diagnosis was secondary CNF with septic *** surgical decompression is important for CNF ***,if symptoms worsen despite early diagnosis and management,such as pus drainage and surgical intervention,clinicians should consider the possibility of a secondary abscess from internal organs.

An otoscope is traditionally used to examine the eardrum and ear canal.A diagnosis of otitis media(OM)relies on the experience of *** an examiner lacks experience,the examination may be difficult and *** paper presents an ear disease classification method using middle ear images based on a convolutional neural network(CNN).Especially the segmentation and classification networks are used to classify an otoscopic image into six classes:normal,acute otitis media(AOM),otitis media with effusion(OME),chronic otitis media(COM),congenital cholesteatoma(CC)and traumatic perforations(TMPs).The Mask R-CNN is utilized for the segmentation network to extract the region of interest(ROI)from otoscopic *** extracted ROIs are used as guiding features for the *** classification is based on transfer learning with an ensemble of two CNN classifiers:EfficientNetB0 and *** proposed model was trained with a 5-fold cross-validation *** proposed method was evaluated and achieved a classification accuracy of 97.29%.

BACKGROUND This report describes and discusses recurrent intramuscular lipoma(IML)of the extensor pollicis brevis(EPB).An IML usually occurs in a large muscle of the limb or *** of IML is *** IMLs,especially those with unclear boundaries,necessitate complete *** cases of IML in the hand have been ***,recurrent IML appearing along the muscle and tendon of EPB on wrist and forearm has not been reported *** SUMMARY In this report,the authors describe clinical and histopathological features of recurrent IML at EPB.A 42-year-old Asian woman presented with a slowgrowing lump in her right forearm and wrist area six months *** patient had a history of surgery for a lipoma of the right forearm one year ago with a scar of 6cm on the right *** resonance imaging confirmed that the lipomatous mass,which had attenuation similar to subcutaneous fat,had invaded the muscle layer of *** and biopsy were performed under general *** histological examination,it was identified as an IML showing mature adipocytes and skeletal muscle ***,surgery was terminated without further *** recurrence occurred during a follow-up of five years after *** Recurrent IML in the wrist must be examined to differentiate it from *** to surrounding tissues should be minimized during excision.

Low-intensity light detection necessitates high-responsivity *** achieve this,we report In_(0.53)Ga_(0.47)As∕In As∕In_(0.53)Ga_(0.47)As quantum well(InAs QW)photo-field-effect-transistors(photo-FETs)integrated on a Si substrate using direct wafer *** of the In As QW channel was carefully designed to achieve higher effective mobility and a narrower bandgap compared with a bulk In_(0.53)Ga_(0.47)As,while suppressing the generation of defects due to lattice ***-performance 2.6 nm In As QW photo-FETs were successfully demonstrated with a high on/off ratio of 10~5 and a high effective mobility of 2370 cm^(2)∕(V·s).The outstanding transport characteristics in the InAs QW channel result in an optical responsivity 1.8 times greater than InGaAs photo-FETs and the fast rising/falling ***,we experimentally confirmed that the InAs QW photo-FET can detect light in the short-wavelength infrared(SWIR;1.0–2.5μm)near 2μm thanks to bandgap engineering through In As QW *** result suggests that the InAs QW photo-FET is promising for high-responsivity and extended-range SWIR photodetector applications.

BACKGROUND Transcutaneous oxygen pressure(TcpO2)is a precise method for determining oxygen perfusion in wounded *** device uses either electrochemical or optical *** To evaluate the usefulness of TcpO2 measurements on free flaps(FFs)in diabetic foot ulcers(DFUs).METHODS TcpO2 was measured in 17 patients with DFUs who underwent anterolateral thigh(ALT)-FF surgery and compared with 30 patients with DFU without FF *** Significant differences were observed in the ankle-brachial index;duration of diabetes;and haemoglobin,creatinine,and C-reactive protein levels between the two ***2 values were similar between two groups except on postoperative days 30 and 60 when the values in the ALT-FF group remained50 *** Even if the flap is clinically stable,sympathectomy due to adventitia stripping during anastomosis and arteriovenous shunt progression due to diabetic polyneuropathy could lead to low TcpO2 values in the ALT-FF owing to its thick fat tissues,which is supported by the slow recovery of the sympathetic tone following ***,TcpO2 measurements in patients with DFU who underwent FF reconstruction may be less accurate than in those who did not.

Finger compartment syndrome due to a high-pressure washer injury:A case report

BACKGROUND Although the finger compartment syndrome is not common,it compresses the neurovascular bundles in a limited space and blocks blood flow to the fingers,causing necrosis of the *** fasciotomy through unilateral or bilateral midline release of the finger can achieve decompression of the finger ***,we report a case of the compartment syndrome in a finger injury caused by a high-pressure water flow which is commonly used in car washing *** SUMMARY A 60-year-old man injured his right middle finger while using a high-pressure washer at a car washing *** patient complained of severe pain in his middle finger and a 0.2 cm punctured open wound on the volar side of the distal phalangeal joint of the middle *** fingertip was pale,numb,and characterized by severe swelling and a limited range of *** radiography showed that there was no fracture in the *** decompression was performed through finger fasciotomy by bilateral midline *** the second day after surgery,the color of the fingertip returned to pink,swelling was resolved,and the range of motion returned to *** sensation of the fingertip was completely restored,and the capillary refill test and pinprick test were *** The fingertip compartment syndrome can be caused by a high-pressure water flow damage to the fingers when using high-pressure washers at a car washing *** avoid finger necrosis,rapid diagnosis of the finger compartment syndrome and appropriate digital decompression are essential to better outcome.
