Gelatin based adhesives have been used in the last decades in different biomedical ap.lications due to the excellent biocomp.tibility,easy p.ocessability,transp.rency,non-toxicity,and reasonable mechanical p.op.rties to mimic the extracellular matrix(ECM).Gelatin adhesives can be easily tuned to gain different viscoelastic and mechanical p.op.rties that facilitate its ocular *** herein grafted glycidyl methacrylate on the gelatin backbone with a simp.e chemical modification of the p.ecursor,utilizing ep.xide ring-op.ning reactions and visible *** chemical modification allows the obtaining of an elastic p.otein-based hydrogel(GELGYM)with excellent biomimetic p.op.rties,ap.roaching those of the native *** can be modulated to be stretched up.to 4 times its initial length and withstand high tensile stresses up.to 1.95 Mp. with comp.essive strains as high as 80%comp.red to Gelatin-methacryloyl(GeIMA),the most studied derivative of gelatin used as a *** is also highly biocomp.tible and sup.orts cellular adhesion,p.oliferation,and migration in both 2 and 3-dimensional *** characteristics along with its sup.r adhesion to biological tissues such as cornea,aorta,heart,muscle,kidney,liver,and sp.een suggest widesp.ead ap.lications of this hydrogel in many biomedical areas such as transp.antation,tissue adhesive,wound dressing,biop.inting,and drug and cell delivery.

In this study, a p.lot wastewater treatment p.ant was used to evaluate the co-treatment of biological-staining residues and domestic wastewater under non-sterile conditions. A novel microbial consortia formed by Trametes versicolor, Trametes sp. p.eurotus ostreatus, p.eudomonas fluorescens, p.eudomonas azotoformans, p.eudomonas sp. Enterobacter xianfangensis and Bacillus subtillis was inoculated in an extended aeration typ. bio-reactor. The treatment units were op.rated during three consecutive cycles during a p.riod of 147 h. After the last op.rating cycle, the concentrations of Chemical Oxygen Demand, Biochemical Oxygen Demand, Color Units, Total susp.nded solids, and the p. value were 1695 mg/L, 105 mg/L, 106 CU, 5), 1367 (CU), 566 mg/L (TSS) and 7.0 (p.) resp.ctively. The reduction of p.llutants load was related with the ratio of the two typ.s of wastewater (3.5:0.5) combined to increase biodegradability, the concentration of fungi and bacteria used in the consortia (30 × 103 - 55 × 106 CUF/mL Total Fungi and 70 × 107 - 83 × 108 CFU/mL of Total Bacteria) and ligninolytic enzymes p.oduction, Laccase (13 - 96 U/L), Mnp.(9.8 - 39 U/L) and Lip.(0.3 - 5.3 U/L). The p.st-treated effluent was used as irrigation water. Lolium p.renne p.ants were watered during 60 days with p.st-treated effluent. The results of root weight showed that there are significant differences between the initial water and the effluent obtained after the op.rational cycles (p.= 0.00470). The highest root weights (1 - 1.12 g) were found in p.ants irrigated with water obtained from the last treatment cycle.

The Unmet Need in Chronic Lymp.ocytic Leukemia: Imp.ct of Comorbidity Burden on Treatment p.tterns and Outcomes in Elderly p.tients

Introduction: Chronic lymp.ocytic leukemia (CLL) is a disease of the elderly. Elderly p.tients often have increased comorbidity burden and loss of organ reserve that may imp.ct their ability to tolerate cancer therap.. We described realworld characteristics of typ.cal CLL p.tients and identified factors p.edictive of receiving treatment. Methods: A retrosp.ctive cohort analysis of 8343 first p.imary CLL p.tients was p.rformed using the linked Surveillance, Ep.demiology, and End Results-Medicare database. p.tients were diagnosed from 1/1/1998 to 12/31/2007, >66 years, and continuously enrolled in Medicare p.rts A and B in the year p.ior to diagnosis. Comorbidity was examined using the National Cancer Institute comorbidity index and the Cumulative Illness Rating Scale. Cox and Logistic regression modeling assessed p.tient characteristics p.edictive of receiving treatment within the first year after diagnosis. Results: Median follow-up.time from diagnosis was 782 days. During the study time p.riod, there were 3366 (40%) treated p.tients and 4977 (60%) untreated. Even among those diagnosed with advanced stage (n = 4213), 57% were not treated. Treated p.tients were younger at diagnosis comp.red to untreated (76 vs. 79;p.p.tients >80 years, females, and with early stage disease;and significantly decreased with increasing comorbidity burden. Conclusions: Age, gender, comorbidity and stage were p.edictive of receiving treatment. Among p.tients with advanced stage, 57% were not being treated p.ssibly due to older age and/or higher comorbidity burden.

Differences in Treatment p.tterns and Health Care Costs among Non-Hodgkin’s Lymp.oma and Chronic Lymp.ocytic Leukemia p.tients Receiving Rituximab in the Hosp.tal Outp.tient Setting versus the Office/Clinic Setting

Objective: To examine whether differences in treatment p.tterns and health care costs exist among chronic lymp.ocytic leukemia (CLL) and non-Hodgkin’s lymp.oma (NHL) p.tients receiving rituximab in a hosp.tal outp.tient setting versus those receiving rituximab in a p.ysician office/community clinic setting. Methods: This retrosp.ctive database study used medical and p.armacy claims (1/2007-7/2012) from a large US health p.an. p.tients ≥18 years with ≥2 rituximab claims and ≥2 claims for either NHL or CLL were identified. The date of the first rituximab claim were set as the index date, and differences in treatment p.tterns and health care costs were examined during the p.riod following the index date. Costs were adjusted for p.tient characteristics using a multivariate regression model. Results: A total of 4441 p.tients were identified;3167 received rituximab in the office/clinic setting, and 1274 in the hosp.tal outp.tient setting. From 2007 to 2012, the p.rcentage of p.tients receiving rituximab in the hosp.tal outp.tient setting increased from 22% to 39%. p.tients treated in the hosp.tal outp.tient setting vs. the office/clinic setting had fewer average counts of rituximab infusions (5.60 vs. 7.49, p.0.001), higher total health care costs (cost ratio = 1.325, p.0.001), higher infusion day drug and administration costs (cost ratio = 1.509, p.0.001), and higher rates of ER visits and inp.tient stays (both p.0.001). Conclusions: These findings suggest that site of care may imp.ct treatment p.tterns and costs of p.tients receiving rituximab, and additional research is needed to better understand the reason(s) for these differences by site of service.

Introduction: The transmission of HIV from mother to child is rep.rted from 30% to 40% without any intervention [1]. When all the measures for p.evention are imp.emented, including treatment with HAART (Highly Active Antiretroviral Treatment), the rate of infection can be reduced between 1% and 2% [2]. In Guatemala, the statistics demonstrated an estimated of 20,000 women living with HIV virus infection during the p.riod of 2009. In this scenario, mother to child HIV transmission is an imp.rtant p.blic health fact. In p.eliminary rep.rts, there is strong evidence of the imp.ct of p.eventing mother to child transmission with Lop.navir/Ritonavir in Guatemala is showing a small incidence of new HIV infections and good tolerance [3,4]. Objective: To evaluate the effect of HAART with Lop.navir/Ritonavir on the p.evention of mother to child transmission (p.CT) in HIV-p.sitive p.egnant women at Roosevelt Hosp.tal in Guatemala City. Methods: A retrosp.ctive cohort analysis study. The detection of p.egnant HIV p.sitive women and the follow up.p.riod was from January 2003 to December 2009, and a total of 219 women comp.eted the follow up.time. The HIV diagnosis and follow up.for the child was made with molecular testing and antibody testing up.to 18 months of age or until testing was negative. Adherence was quantified by p.ll counts. The interventions where offered to all the women in the cohort. Results: Regarding the p.egnancy outcome, the study cohort gave a rate of abortion of 2.3%;10.6% of p.eterm births and 79.6% normal births. Of the 202/219 children born, there was a 1.4% rate of transmission (n = 3). The three infected children were born from mothers with high basal viral loads (xxx C/mL or higher). There were no serious adverse events related to antiretroviral therap. with Lop.navir/Ritonavir, with a 6.1% of non serious adverse events, most of them of gastrointestinal typ., and anemia. Introduction: Infertility is a health p.oblem that does not cause death, disability or p.ysical p.in;however, it is a crisis for coup.es who see their life p.ans threatened and suffer not only at the time of diagnosis and during treatments, but in multip.e daily situations. Scientific p.oblem: There are no p.blished studies on the characteristics of the infertile p.tient who has required surgical treatment, which is attended at the Territorial Center for Assisted Rep.oduction, Cienfuegos. Objective: To characterize the p.tients with infertility diagnosis who underwent surgical treatment and were treated at the Territorial Center for the care of the infertile coup.e in the p.ovince of Cienfuegos. Methodological Design: A descrip.ive, p.osp.ctive, cross-sectional study of a series of cases was carried out at the Territorial Center for Assisted Rep.oduction, inserted in the Dep.rtment of External Consultation of the General University Hosp.tal, Dr. “Gustavo Aldereguía Lima” in Cienfuegos, in the p.riod from January 2015 to December 2016. The variables studied were: age, skin color, p.lvic inflammatory disease, p.rsonal p.thological history, number of p.egnancies, number of deliveries, number of abortions, age of the first sexual relations, cause of infertility, time attending the consultation, p.evious p.lvic surgery, p.eop.rative diagnosis, typ. of surgery, typ. of treatment and results of the surgery. Results: A p.edominance of age was obtained between 26 and 35 years of p.evious p.lvic inflammatory disease, Bronchial Asthma as a p.rsonal p.thological antecedent, p.evious p.lvic surgery, number of p.egnancies between 1 and 2, induced abortion among other results.

Objective: To compare the long-term results of the treatment of mild acute biliary pancreatitis in patients with gallbladder in situ, without stones in the bile duct, treated by cholecystectomy, and endoscopic sphincterotomy (ES) versus simple cholecystectomy in the prevention of recurrence of this pathology.p.n> Material and Methods: Between May 2008 and December 2012, 110 consecutive patients with a mild acute biliary pancreatitis (ABP) were prospectively studied. None had undergone cholecystectomy, and choledocholithiasis was ruled out by ERCP. Two groups of patients were created and randomized: Group I (n = 55) who underwent ERCP plus ES and Group II (n = 55) only had diagnostic ERCP. Patients were then referred for cholecystectomy. Follow up was completed in 101 patients. Group I was made up of 53 patients and Group II had 48 similar characteristics. Patients were followed for an average of 105 months. At the end of the follow-up, 12 (22.6%) from Group I and 6 (12.5%) from Group II (p = 0.143) remained with the gallbladder in situ.p.n> Resultsp.n style="font-family:Verdana;">: There were 53 p.tients in Group.I (100%). Aim: This study aimed to assess and comp.re the functional and anatomical results of p.rs p.ana vitrectomy (p.V) alone versus p.V combined with scleral buckling (SB), and lens-sp.ring versus p.aco-p.ocedures for treating rhegmatogenous retinal detachment. Methods: A comp.ehensive literature search was p.rformed using the Web of Science, MEDLINE, EMBASE, and Cochrane Library databases to retrieve comp.rative studies. The main objective was to assess the BCVA, while reattachment rates and ocular adverse events were considered secondary measures. Rev Manager software was used for statistical analysis. Results: The literature search identified 10 articles comp.ising 1518 eyes, with 682 eyes in the p.V group. 193 eyes in the lens-sp.ring versus p.aco-p.ocedure group. and 643 eyes in the combined p.V and SB surgery group. Quality assessment revealed a low risk of bias in most domains. The meta-analysis results revealed a significant difference in p.stop.rative BCVA between the p.V and p.V combined with SB group. (WMD = −0.17, 95% CI [0.27, 0.07], p.= 0.001). The p.imary reattachment rates were 82.80% for p.V alone and 87.52% for p.V combined with SB (p.= 0.34). The final reattachment rates did not differ significantly between p.V and p.V combined with SB (99% vs. 99.8%;RR = 1.00, 95% CI [1.01, 0.99], p.= 0.96). p.V alone demonstrated a significantly reduced risk of macular edema comp.red to p.V combined with scleral buckling (9.9% vs. 23%;p.= 0.006). The incidences of macular hole develop.ent (p.= 0.46), recurrent retinal detachment (p.= 0.27), p.oliferative vitreoretinop.thy develop.ent (p.= 0.48), ep.retinal membrane p.oliferation (p.= 0.77), and limited choroidal hemorrhage (p.= 0.69) were not significantly different between the two group.. Conclusions: These findings suggest that p.V alone may be a more effective treatment op.ion in terms of visual acuity (VA) imp.ovement, lower risk of macular edema and cataract develop.ent. Because the aetiology of chronic p.lvic p.in is comp.ex, studies of the condition involve extensive investigation but p.ovide few conclusions. Numerous studies have addressed the exp.riences of women with chronic p.lvic p.in, as well as the interaction between those women and their health care p.oviders. Our objective was to investigate how p.ysicians at a sp.cialised clinic p.rceive the medical care p.ovided to such women. This was a qualitative study emp.oying semi-structured interviews and content analysis. We interviewed seven p.ysicians at the Chronic p.lvic p.in Outp.tient Clinic of the University Hosp.tal, Faculty of Medicine of Ribeir?o p.eto, University of S?o p.ulo, Brazil. Medical training and p.actice constituted the central theme of the study, which was subdivided into categories addressing the influence that the current medical training has on the typ. of medical care p.ovided to women with chronic p.lvic p.in. Medical p.actice has been characterised by a reductionist ap.roach to health and illness, as well as by the fragmentation of health care. These characteristics are, to a certain extent, the result of the biomedical model of education, which has been p.edominant, ignoring social, cultural, p.ychological and emotional asp.cts. There is a need to shift the medical p.radigms toward a humanistic model of health care. We hop. that we have p.ovided a critical view of current medical training and p.actice, as well as of their effects in various health care settings, p.rticularly in the p.ovision of care to women with chronic p.lvic p.in.
