AIM:To investigate usefulness of adherence to gastroesophageal reflux disease(GERD) guideline established by the Spanish Association of ***:Prospective,observational and multicentre study of 301 patients with typical symptoms of GERD who should be managed in accordance with guidelines and were attended by gastroenterologists in daily ***(aged > 18 years) were eligible for inclusion if they had typical symptoms of GERD(heartburn and/or acid regurgitation) as the major complaint in the presence or absence of accompanying atypical symptoms,such as dyspeptic symptoms and/or supraesophageal *** and therapeutic decisions should be made based on specific recommendations of the Spanish clinical practice guideline for GERD which is a widely disseminated and well known instrument among Spanish in digestive disease ***:Endoscopy was indicated in 123(41%) patients:50 with alarm symptoms,32 with age > 50 years without alarm ***-two patients(58.5%) had esophagitis(grade A,23,grade B,28,grade c,18,grade D,3).In the presence of alarm symptoms,endoscopy was indicated consistently with recommendations in 98% of ***,in the absence of alarm symptoms,endoscopy was indicated in 33% of patients > 50 years(not recommended by the guideline).Adherence for proton pump inhibitors(PPIs) therapy was 80%,but doses prescribed were lower(half) in 5% of cases and higher(double) in 15%.Adherence regarding duration of PPI therapy was 69%;duration was shorter than recommended in 1%(4 wk in esophagitis grades c-D) or longer in 30%(8 wk in esophagitis grades A-B or in patients without endoscopy).Treatment response was higher when PPI doses were consistent with guidelines,although differences were not significant(95% vs 85%).cOncLUSIOn:GERD guideline compliance was quite good although endoscopy was over indicated in patients > 50 years without alarm symptoms;PPIs were prescribed at higher doses and longer duration.

Obesity is a major worldwide health problem that is related to most chronic diseases,including male *** to its wide impact on health,mechanisms underlying obesity-related infertility remain *** this study,we report that mice fed a high-fat diet(HFD)for over 2 months showed reduced fertility rates and increased germ cell apoptosis,seminiferous tubule degeneration,and decreased intratesticular estradiol(E2)and E2-to-testosterone ***,we also detected a decrease in testicular fatty acid levels,behenic acid(c22:0),and docosahexaenoic acid(DHA,22:6n-3),which may be related to the production of dysfunctional ***,we did not detect any changes in the frequency of seminiferous tubule stages,sperm count,or rate of in vitro ***,there was an increase in spontaneous and progesterone-induced acrosomal exocytosis(acrosome reaction)in spermatozoa from HFD-fed *** data suggest that a decrease in E2 and fatty acid levels influences spermatogenesis and some steps of acrosome biogenesis that will have consequences for ***,our results add new evidence about the adverse effect of obesity in male reproduction and suggest that the acrosomal reaction can also be affected under this condition.

Amyotrophic lateral sclerosis(ALS) is a progressive neurodegenerative disorder that almost exclusively involves motor neurons although autonomic dysfunction has also been reported. We present an 84-year-old female with no documented history of heart disease, who was admitted with negative T waves in the electrocardiogram precordial leads mimicking myocardial ischaemia. no other abnormalities were shown in the rest of the cardiologic evaluation, suggesting autonomic nervous system dysfunction. A neurophysiological study demonstrated acute and chronic denervation in multiple muscles with normal nerve conduction studies, confirming ALS diagnosis. Previous studies have shown that subclinical sympathetic hyperfunction and parasympathetic hypofunction might result in cardiovascular dysfunction in ALS patients. It is important to detect disturbances of autonomic cardiac control because this dysfunction may influence survival and quality of life, leading to a decrease in life expectancy in ALS *** case Report may support the impairment of cardiac autonomic control in patients with ALS.

miR-21, miR-221 and miR-222 expression and prostate cancer recurrence among obese and non-obese cases

Recent evidence shows that certain microRnAs （miRnAs） play a role in both obesity and prostate cancer recurrence, but the association between the expression of these miRnAs and obesity in prostate cancer recurrence is unknown. In this study, we examined the effect of the interaction between obesity and miR-21, miR-221 or miR-222 expression on prostate cancer recurrence among 28 recurrent and 37 non-recurrent prostate cancer cases, miRnA expression was determined using quantitative real-time polymerase chain reaction. cox proportional hazard models adjusting for age at diagnosis, clinical stage and Gleason score were used to estimate hazard ratios （HR） and 95% confidence intervals （95% cI） for recurrence free survival. A significantly （P=0.014） higher proportion of recurrent cases （78,6%） than non-recurrent cases （48.6%） had a low expression of miR-21 and the difference was more prominent in obese than non-obese patients. Multivariate analysis showed that the expression of miR-21 was an independent risk factor for recurrence in obese （HR=6.15, 95% cI= 1.04-36.48, P=-0.045）, but not in non-obese （HR= 1.28, 95% c1=0.30-5.49, P=0.74） cases. A significant association with recurrence was not observed for the expression of miR-221 and miR-222. In summary, our findings show that miR-21 is associated with prostate cancer recurrence after radical prostatectomy and suggest that the differential expression of miR-21 is more prominent in obese than in non-obese cases. Future larger studies are warranted to confirm these initial findings and to elucidate the mechanisms involved.
