Acute necrotizing pancreatitis: Surgical indications and technical procedures

Necrosis of pancreatic parenchyma or extrapancreatic tissues is present in 10%-20% of patients with acute pancreatitis, defining the necrotizing presentation frequently associated with high morbidity and mortality rates. During the initial phase of acute necrotizing pancreatitis the most important pillars of medical treatment are fluid resuscitation, early enteral nutrition, endoscopic retrograde colangiopancreatography if associated cholangitis and intensive care unit support. When infection of pancreatic or extrapancreatic necrosis occurs, surgical approach constitutes the most accepted therapeutic option. In this context, we have recently assited to changes in time for surgery(delaying the indication if possible to around 4 wk to deal with "walledoff" necrosis) and type of access for necrosectomy: from a classical open approach(with closure over large-bore drains for continued postoperative lavage or semiopen techniques with scheduled relaparotomies), trends have changed to a "step-up" philosophy with initial percutaneous drainage and posterior minimally invasive or endoscopic access to the retroperitoneal cavity for necrosectomy if no improvement has been previously achieved. These approaches are progressively gaining popularity and morbidity and mortality rates have decreased significantly. Therefore, a staged, multidisciplinary, step-up approach with minimally invasive or endoscopic access for necrosectomy is widely accepted nowadays for management of pancreatic necrosis.

Encroachment drives facilitation at alpine shrublines

Ongoing encroachment is driving recent alpine shrubline dynamics globally,but the role of shrub-shrub interactions in shaping shrublines and their relationships with stem density changes remain poorly ***,the size and age of shrubs from 26 Salix shrubline populations along a 900-km latitudinal gradient(30°-38°N)were measured and mapped across the eastern Tibetan *** pattern analyses were used to quantify the spatial distribution patterns of juveniles and adults,and to assess spatial associations between *** intensity of univariate and bivariate spatial patterns was related to biotic and abiotic *** mark correlation functions with a quantitative mark(shrub height,basal stem diameter,crown width)were also employed to investigate the spatial relationships between shrub traits of juveniles and *** equation models were used to explore the relationships among conspecific interactions,patterns,shrub traits and recruitment dynamics under climate *** shrublines showed clustered patterns,suggesting the existence of conspecific *** patterns of juveniles and conspecific interactions(potentially facilitation)tended to intensify with increasing soil moisture *** warming before 2010 triggered positive effects on population interactions and spatial patterns via increased shrub ***,summer warming after2010 triggered negative effects on interactions through reduced shrub ***,shrub recruitment shifts under rapid climate change could impact spatial patterns,alter conspecific interactions and modify the direction and degree of shrublines responses to *** changes would have profound implications for the stability of alpine woody ecosystems.

Interpenetrating polymers supported on microporous polypropylene membranes for the transport of chromium ions

Modifying polypropylene membranes with interpenetrating polymer networks(IPNs) through the incorporation of poly(glycidyl methacrylate-N-methyl-D-glucamine)(P(GMA-NMG)) was performed by in situ synthesis via radical polymerization. The surface of the polypropylene membrane was activated by hydrophilic grafted polyelectrolyte, and then, pressure injection was used for the impregnation of the reactive solution in the *** types of pore-filled membranes were synthesized, chelating interpenetrating homopolymer networks of P(GMA-NMG), and chelating-ion exchange interpenetrating polymer networks(e.g., P(GMA-NMG)/P(AA),P(GMA-NMG)/P(AMPSA), and P(GMA-NMG)/P(Cl VBTA)). After their synthesis, the modified polypropylene membranes were characterized using techniques such as the electrokinetic potential, SEM, FT-IR, and Donnan dialysis to corroborate the chromium ion transport. The P(GMA-NMG) and complex network membranes exhibited a hydrophilic character with a water-uptake capacity between 20% and 35% and a percentage of modification between 4.0% and 7.0% in comparison with the behavior of the unmodified polypropylene *** chromium ions were efficiently transported from the food chamber at p H 9.0 when the 65.2%MTA1 P(Cl VBTA) homopolymer IPN membrane and 48.5% MTAG P(GMA-NMG)/P(Cl VBTA) IPN membrane were used. Similarly, hexavalent chromium ions were removed from the food chamber at pH 3.0 when MTAG(63.30%) and MTA1(35.68%) were used in 1 mol·L^(-1)Na Cl solution as the extraction reagent.

Clinical heterogeneity in autoimmune acute liver failure

AIM: To describe the outcome and prognosis in a cohort of patients with acute liver failure due to autoimmune hepatitis without liver transplantation. METHODS: A retrospective trial was conducted in 11 patients with acute liver failure due to autoimmune hepatitis who attended the Instituto Nacional de Ciencias Médicas y Nutrición Salvador Zubiran. Demographic, biochemical and severity indexes, and treatment and outcome were assessed. RESULTS: Among the 11 patients, with a median age of 31 years, 72% had inflammatory response syndrome, and six patients received corticosteroids. The mortality rate within four weeks was 56%, and the one-year survival was 27%. In the survivors, severity indexes were lower and 83% received corticosteroids. CONCLUSION: We observed a relatively high survival rate in patients with acute liver failure due to autoimmune hepatitis. This survival rate could be influenced by severity of the disease and/or use of corticosteroids.

Brassinosteroids （BRs） are steroid plant hormones with important roles in plant growth and development through the regulation of processes such as cell division and expansion. In addition to their role as growth promoters, BRs can also protect plants from a va- riety of abiotic stresses, including drought, salt, heat, and cold stress. Nevertheless, the molecular determinants underlying this protective effect remain largely uncharacterized.

Uncontrolled hypertension in older patients： markers and associated factors to masked and white-coat effect

Background Hypertension is the main risk factor for cardiovascular diseases, affecting more than half the elderly population. It is essential to know if they have proper control of hypertension. The aim of this study was to identify the associated factors to masked uncon- trolled hypertension and false uncontrolled hypertension in older patients. Methods Two-hundred seventy-three individuals （70.1±6.7 years-old） had blood pressure （BP） measured at the office and by ambulatory BP monitoring （ABPM）, with the definition of controlled group （C）, individuals with high office BP and adequate ABPM, called white-coat effect group （WCE）, uncontrolled （UC）, and subjects with ap- propriate office BP and elevated ABPM denominated masked effect group （ME）. Age, body mass index, diabetes, pulse pressure （PP） and BP dipping during sleep were evaluated （Kruskal-Wallis test and logistic regression models）. Results Age was higher in UC than in C and ME （P 〈 0.01）, and 24-h ABPM PP was lower in C （48± 7 mmHg） and WCE （51±6 mmHg） than in UC （67±12 mmHg） and ME （59±8 mmHg）（P 〈 0.01）. Sleep systolic BP dipping was lower in ME than in C （P = 0.03）. Female gender was associated with a greater chance of being of ME group, which showed a higher PP and lower BP dipping during sleep. Conclusions In older individuals, office BP measure- ments did not allow the detection of associated factors that would permit to differentiate WCE from UC group and C from ME group. ABPM favored the identification of a higher PP and a lower BP dipping during sleep in the masked effect and uncontrolled groups.

The effect of rippling on the mechanical properties of graphene

Graphene is the stiffest material known so far but, due to its one-atom thickness, it is also very ***, free-standing graphene exhibit ripples that has major effects on its elastic properties. Here we will summarize three experiments where the influence of rippling is essential to address the results. Firstly, we observed that atomic vacancies lessen the negative thermal expansion coefficient(TEC) of free-standing *** also observed an increase of the Young's modulus with global applied strain and with the introduction of small density defects that we attributed to the decrease of rippling. Here, we will focus on a surprising feature observed in the data: the experiments consistently indicate that only the rippling with wavelengths between 5 and 10 nm influences the mechanics of graphene. The rippling responsible of the negative TEC and anomalous elasticity is thought to be dynamic, i.e. flexural phonons. However, flexural phonons with these wavelengths should have minor effects on the mechanics of graphene, therefore other mechanisms must be considered to address our observations. We propose static ripples as one of the key elements to correctly understand the thermomechanics of graphene and suggest that rippling arises naturally due to a competition of symmetry breaking and anharmonic fluctuations.

Group theory(GT) provides a rigorous framework for studying symmetries in various disciplines in physics ranging from quantum field theories and the standard model to fluid mechanics and chaos theory. To date, the application of such a powerful tool in optical physics remains limited. Over the past few years however, several quantum-inspired symmetry principles(such as parity-time invariance and supersymmetry) have been introduced in optics and photonics for the first time. Despite the intense activities in these new research directions, only few works utilized the power of group theory. Motivated by this status quo, here we present a brief overview of the application of GT in optics, deliberately choosing examples that illustrate the power of this tool in both continuous and discrete setups. We hope that this review will stimulate further research that exploits the full potential of GT for investigating various symmetry paradigms in optics, eventually leading to new photonic devices.

Silk fibroin-based biopolymer composite binders with gradient binding energy and strong adhesion force for high-performance micro-sized silicon anodes

Micro-sized silicon anodes have shown much promise in large-scale industrial production of high-energy lithium ***,large volume change(>300%)of silicon anodes causes severe particle pulverization and the formation of unstable solid electrolyte interphases during cycling,leading to rapid capacity decay and short cycle life of lithium-ion *** addressing such issues,binder plays key roles in obtaining good structural integrity of silicon ***,we report a biopolymer composite binder composed of rigid poly(acrylic acid)(PAA)and flexible silk fibroin(SF)tailored for micro-sized silicon *** PAA/SF binder shows robust gradient binding energy via chemical interactions between carboxyl and amide groups,which can effectively accommodate large volume change of *** PAA/SF binder also shows much stronger adhesion force and improved binding towards high-surface/defective carbon additives,resulting in better electrochemical stability and higher coulombic efficiency,than conventional PAA *** such,micro-sized silicon/carbon anodes fabricated with novel PAA/SF binder exhibit much better cyclability(up to 500 cycles at 0.5 C)and enhanced rate capability compared with conventional PAA-based *** work provides new insights into the design of functional binders for high-capacity electrodes suffering from large volume change for the development of nextgeneration lithium batteries.
