Neuromuscular trunk activation patterns in back pain patients during one-handed lifting

AIM To analyze neuromuscular activity patterns of the trunk in healthy controls(H) and back pain patients(BPP) during one-handed lifting of light to heavy loads. METHODS After assessment of back pain(graded chronic pain scale according to von Korff) all subjects(n = 43) performed a warm-up(treadmill walking). Next, subjects were instructed to lift 3 × a 20 kg weight placed in front of them(with both hand) onto a table(height: 0.75 m). Subsequently, all subjects lifted with one hand(left-side, 3 repetitions) a weight of 1 kg(light), 10 kg(middle) and 20 kg(heavy) in random order from the ground up onto the table left of them. Trunk muscle activity was assessed with a 12-lead EMG(6 ventral/6 dorsal muscles; 4000 Hz). EMG-RMS(%) was averaged over the 3 repetitions and analyzed for the whole one-handed lifting cycle, then normalized to RMS of the two-handed lifting. Additionally, the mean(normalized) EMG-RMS of four trunk areas [right/left ventral area(VR/VL); right/left dorsal area(DR/DL)] was calculated. Data were analyzed descriptively(mean ± SD) followed by student's t-test comparing H and BPP(α = 0.05). With respect to the unequal distribution of subjects in H and BPP, a matched-group analysis was conducted. Seven healthy controls were gender- and age-matched(group H_(matched)) to the 7 BPP. In addition, task failure was calculated and compared between H/H_(matched) vs BPP using χ~2. RESULTS Seven subjects(3m/4f; 32 ± 7 years; 171 ± 7 cm; 65 ± 11 kg) were assigned to BPP(pain grade ≥ 2) and 36(13m/23f; 28 ± 8 years; 174 ± 10 cm; 71 ± 12 kg) to H(pain grade ≤ 1). H and BPP did not differ significantly in anthropometrics(P > 0.05). All subjects were able to lift the light and middle loads, but 57% of BPP and 22% of H were not able to lift the heavy load(all women). χ~2 analysis revealed statistically significant differences in task failure between H vs BPP(P = 0.03). Increased Mortality Risk in Children with Fetal Alcohol Spectrum Disorders: A Scoping Review

Objective: Fetal Alcohol Spectrum Disorders (FASDs) are common, often undiagnosed, lifelong developmental disorders that result from prenatal alcohol exposure. FASD is present at birth and typically identified around seven years of age. The most severe outcome in cases of FASD is mortality. The purpose of this scoping review is to 1) use a systematic review to provide an estimated mortality proportion for children with FASD, and 2) update a study published in 2014 by reviewing published reports of mortality in individuals diagnosed with FASD. Method: A search of PubMed, CINAHL, and Google Scholar for reports published between 2013 and 2023 on mortality in individuals with FASD. Results: Three population-based studies have reported on all-cause mortality rates, finding a combined mortality rate of 10.9%, a 2.63 fold (95% CI: 2.61 to 2.65) increase in mortality risk over the general population. Since 2016, this review identified only eight new cases meeting the study inclusion criteria. The reported causes of death were five cases of pneumonia, and one case each of failure to thrive and dehydration, intestinal dilatation and asphyxiation caused by overeating due to pica, and acute gastric volvulus. Discussion: While current research suggests a diagnosis of FASD is associated with a 2.6-fold increase in mortality risk, this is likely an underestimation, as most cases of FASD-related mortality go unreported. Globally, about 1 new case is reported every 15 months. However, in the United States alone, between 1752 to 4400 FASD related deaths occur annually. Our review suggests that FASD is rarely identified as a causal or contributing factor in deaths of children and adolescents, resulting in a substantial undercount of FASD-related deaths. Increased attention to the role of FASD in infant and child mortality case reviews, child death review committee reports, and mortality reviews is needed.

Is a Mini-Screen for Fetal Alcohol Spectrum Disorder Feasible?

Background: Fetal Alcohol Spectrum Disorders (FASDs) are a global public health concern with lifelong consequences for affected individuals. Recent prevalence studies suggest FASD prevalence rates range from 1-5% among school age children. Most people with FASD are not correctly diagnosed and inadequate screening to identify patients with increased risk may contribute to under-diagnosis. This study developed a 10-item screening tool for FASD and examined its feasibility. Methods: The sample consisted of 355 children who had been evaluated at an FASD clinic. Data from the 33-item Alcohol Related Neurodevelopmental Disorder Behavioral Checklist was used to develop a brief FASD screen by comparing the changes in Cronbach’s alpha for different combinations of items. The validity of the brief scale was then further examined using receiving operating characteristic analyses. Results: The 10-item screen demonstrated acceptable sensitivity, specificity, and accuracy to identify children at high risk for FASD. The percentage correctly classified was 91.3 and the area under the receiving operating characteristic curve was 0.971. Conclusions: This feasibility study demonstrated that a screen for FASD consisting of 10 items with yes or no responses can be completed in 3 - 4 minutes. The tool is brief, with a low administration burden and has acceptable epidemiologic performance characteristics including accuracy. Future research should examine the performance of this tool when used in larger, community-based populations where screening for FASD would be appropriate.

Experimental Interactions Between Clay Minerals and Bacteria: A Review

Interactions between microbes and minerals have the potential to contribute significantly to global cycles of various processes and serve as a link between the geosphere and life. Clays and clay minerals occur commonly in agriculturally utilized soils, are naturally grown underground （soil and rock） and are used in construction material. Clay minerals serve as natural, geological and technical barriers in geotechnics and environmental geotechnics. Bacteria in turn are ubiquitous in natural soils, subsoils and rocks and are in permanent contact with clay minerals. There are numerous ways in which bacteria can interact with clay minerals and alter them： dissolution, refinement and transformation, reduction of trace elements incorporated in the clay minerals and uptake of trace elements from these minerals, e.g., by the production of siderophores and chelators and enhancement or reduction of azisorbance of trace elements on clay minerals. In addition, bacteria can influence layer charge, cation exchange capacity （CEC）, exchangeable cations, Brunauer-Emmett-Teller surface, swelling and the rheological properties of clay minerals. The field of clay mineral-microorganism interaction is still wide open because of the large potential that the interactions of bacteria with clay minerals in soils and sediments may result in changes in clay mineral properties and behaviors. Fhrther detailed studies on all these tentative changes and underlying mechanisms as well as broad surveys of quantifications of extents and rates of clay mineral-microorganism interactions, especially in mimicking natural systems, are highly required. This review summarizes the influences of various bacteria on the properties of different clay minerals as determined experimentally using viable bacteria.

Yield Improvement and Advanced Defect Control——Driving Forces for Modeling of Bulk Crystal Growth

Yield improvement and advanced defect control can be identified as the driving forces for modeling of industrial bulk crystal growth. Yield improvement is mainly achieved by upscaling of the whole crystal growth apparatus and increased processing windows with more tolerances for parameter variations. Advanced defect control means on one hand a reduction of the number of deficient crystal defects and on the other hand the formation of beneficial crystal defects with a uniform distribution and well defined concentrations in the whole crystal. This "defect engineering" relates to the whole crystal growth process as well as the following cooling and optional annealing processes, respectively. These topics were illustrated in the paper by examples of modeling and experimental results of bulk growth of silicon (Si), gallium arsenide (GaAs), indium phosphide (InP) and calcium fluoride (CaF2). These examples also involve the state of the art of modeling of the most important melt growth techniques, crystal pulling (Czochralski methods) and vertical gradient freeze (Bridgman-type methods).

Does pressure cause liver cirrhosis? The sinusoidal pressure hypothesis

Independent of their etiology, all chronic liver diseases ultimately lead to liver cirrhosis, which is a majorhealth problem worldwide. The underlying molecular mechanisms are still poorly understood and no efficient treatment strategies are available. This paper introduces the sinusoidal pressure hypothesis(SPH), which identifies an elevated sinusoidal pressure(SP) as cause of fibrosis. SPH has been mainly derived from recent studies on liver stiffness. So far, pressure changes have been exclusively seen as a consequ-ence of cirrhosis. According to the SPH, however, an elevated SP is the major upstream event that initiates fibrosis via biomechanic signaling by stretching of perisinusoidal cells such as hepatic stellate cells or fibroblasts(SPH part?Ⅰ: initiation). Fibrosis progression is determined by the degree and time of elevated SP. The SPH predicts that the degree of extracellular matrix eventually matches SP with critical thresholds > 12 mmH g and > 4 wk. Elevated arterial flow and final arterialization of the cirrhotic liver represents the self-perpetuating key event exposing the low-pressure-organ to pathologically high pressures(SPH part?Ⅱ: perpetuation). It also defines the "point of no return" where fibrosis progression becomes irreversible. The SPH is able to explain the macroscopic changes of cirrhotic livers and the uniform fibrotic response to various etiologies. It also opens up new views on the role of fat and disease mechanisms in other organs. The novel concept will hopefully stimulate the search for new treatment strategies.

Pharmacological blockage of CYP2E1 and alcohol-mediated liver cancer: is the time ready?

Chronic alcohol consumption is a major risk factor worldwide affecting significantly both mortality and years of life lost （YLL）（1）. Ca. 5% of the western world show risky alcohol consumption and in some countries such as China a regional yearly increase of alcohol consumption of over 400% has been observed recently （2,3）. The liver is the major target organ of alcohol. According to the recently published ＇Global Burden of Disease Study 2010, liver cirrhosis and liver cancer are ranked at position 12 and 16 in the global deaths statistics （1）. Thus, in 2010, ca. 1 million people died from liver cirrhosis with one third directly attributable to alcohol. This is a considerable number when comparing with coronary heart disease with 7 million deaths and the leading cause of mortality. In central Europe, liver cirrhosis even ranks at the fourth position in YLL. Hepatocellular cancer （HCC） is now the most common fatal complication of patients with alcoholic cirrhosis. Moreover, it shows the second fastest increase of all tumors worldwide after kidney tumors and alcohol-associated HCC ranks on third position after HCCs caused by viral hepatitis B and C.
