Importance-performance analysis: Revisiting a tool for the evaluation of clinical services

The importance-performance analysis method (IPA) is used in market research in order to measure the level of customer satisfaction. The aim of this study was to evaluate and highlight the use of IPA as a management tool to measure quality of dental services. We suggest that this method can be easily implemented in a dental educational setting, as a performance outcome measure that includes patient input. The study was conducted in a dental service through a valid questionnaire, SERVQUAL. This instrument explores levels of service quality perceived by patients. It was conducted in four companies in Brazil in 2011 (derived from different industry segments;two from textiles, one from hospital care and the other from the manufacturing industry) in two States (S?o Paulo and Minas Gerais). These companies were covered by the same dental managed care organization which contracted with independent dentists to provide care. The study was divided into two stages: the first, in which beneficiaries answered the questionnaire prior to receiving dental treatment, and the second when the same beneficiaries answered after completing their dental treatment. Data obtained from SERVQUAL generated graphs that were used to characterize the IPA matrix using several dimensions of care. The Assurance Dimension had the highest average in both expectation and perception. The Reliability dimension showed the value of the most negative GAP among the dimensions, and the best value occurred in the Responsiveness dimension. The IPA tool may be effective in Dental Medicine since it highlights the key points to be improved in the delivery of dental services in a clinical setting.

The aim of the present work was to present the outcomes of the patients with Y-chromosome microdeletions treated by intracytoplasmic sperm injection （ICSI）, either using fresh （TESE） or frozen-thawed （TESE-C） testicular sperm and ejaculated sperm （EJAC）. The originality of this work resides in the comparisons between the different types of Y-microdeletions （AZFa, AZFb, and AZFc） and treatments, with detailed demographic, stimulation, embryological, clinical, and newborn （NB） outcomes. Of 125 patients with Y-microdeletions, 33 patients presented severe oligozoospermia （18 performed ICSI with ejaculated sperm） and 92 secretory azoospermia （65 went for TESE with 40 having successful sperm retrieval and performed ICSI）. There were 51 TESE treatment cycles and 43 TESE-C treatment cycles, with a birth of 19 NB （2 in AZFa/TESE-C, 12 in AZFc/TESE, and 5 in AZFc/TESE-C）. Of the 29 EJAC cycles, there was a birth of 8 NB （in AZFc）. In TESE and EJAC cycles, there were no significant differences in embryological and clinical parameters. In TESE-C cycles, there was a significant lower oocyte maturity rate, embryo cleavage rate and mean number of embryos transferred in AZFb, and a higher mean number of oocytes and lower fertilization rate in AZFc. In conclusion, although patients with AZFc microdeletions presented a high testicular sperm recovery rate and acceptable clinical outcomes, cases with AZFa and AZFb microdeletions presented a poor prognosis. Due to the reported heredity of microdeletions, patients should be informed about the infertile consequences on NB and the possibility of using preimplantation genetic diagnosis for female sex selection.

Contribution of the Incorporation of Poultry Litter and Limestone for the Fertility of Sandy Soil in the Brazilian Amazon

Sandy soils have limitations for growing plants. In general, they have low fertility, high toxic aluminum content and low organic matter. The organic matter is responsible for most of the cation exchange capacity of these soils. Added to this, the low clay content and the structure of these soils with large volumes of macropores, determine their low water retention. An alternative is the incorporation of organic waste combined with the practice of liming. Thus, the aim of this study was to evaluate the contribution of the incorporation of poultry litter and limestone in the fertility of sandy soils in the Brazilian Amazon. The experiment was conducted in Colorado do Oeste, RO, on a Quartzipsamment in a completely randomized block design in a 4 × 4 × 2 factorial scheme, corresponding to four doses of poultry litter (0, 5, 10 and 15 mg·ha-1), its incorporation for four times (0, 75, 110 and 145 days) in the presence and absence of 2 mg·ha-1 of limestone, with six replications. The incorporation of 15 mg·ha-1 of poultry litter combined with 2 mg·ha-1 of limestone for 75 days after the merger raises several components of Quartzipsamment fertility, mainly being the availability of phosphorus.

Preparation of the Rutin-SBA-16 Drug Delivery System

This study sets out a scheme for a controlled release delivery system using SBA-16 as a carrier matrix and Rutin as a drug (Rutin-SBA-16). Physicochemical characterizations were performed to confirm the structure of the SBA-16 for post-synthesis by scanning electron microscopy (SEM), Transmission Electron Microscopy (TEM) and X-ray Diffraction (XRD). The presence of Rutin-SBA-16 was confirmed by Fourier-transform infrared spectroscopy (FTIR) and Nitrogen adsorption-desorption isotherms at 77 K. The dissolution kinetics was evaluated by the Zero Order, First Order and Higuchi models, and Rutin quantification was carried out by High Performance Liquid Chromatography (HPLC). The best impregnation time, which was 8 hours, adsorbing 284 μg Rutin per mg of silica, and the maximum degree of dissolution occurred in a period of 20 - 25 h. The release kinetics of the Rutin was called Higuchi, and showed high linearity, with a correlation coefficient (R2) of 0.999 compared with 0.905 and 0.980 of the zero order and first order models respectively. The study shows the benefits of Rutin-SBA-16 as a drug delivery system.

Quantification of Thiamethoxam in Rhizomes and Leaves of the Hedychium coronarium and Water and Soil by High-Pressure Liquid Chromatography

The linearity, stability, accuracy and inter-day precisions of the assay method were evaluated in water, soil and rhizomes and leaves of Hedychium coronarium of the Zingiberaceae family. Mato Grosso do Sul is a predominantly agricultural state in Brazil and has many rivers and permanent and seasonal lakes, in which the Hedychium coronarium, an aquatic plant, is found the swampy environments. The prepared samples were analyzed quantitatively by High Performance Liquid Chromatography with PDA and UV detection for the presence of thiamethoxam. The thiamethoxam was recovered from these samples at rates ranging from 81.16% - 99.93%. The coefficient of variation in the quantitative analysis of the thiamethoxam was under 5%. The linearity of the method was determined by linear regression. The analysis of the samples spiked with known amounts of analyte demonstrated that the response was proportional to the concentrations of the samples with determination coefficients of r2 = 0.9992 (water and soil) and r2 = 0.9990 (leaves and rhizomes) for the linear range of the analytical calibration curves of the samples. The detection limit was 0.36 μg.L-1 and quantification limit was 1.2 μg.L-1 for thiamethoxam. The method was considered sensitive for quantification of the thiamethoxam in water, soil and rhizomes and leaves of Hedychium coronarium.

Multimodal Stratified Approach for Prevention of Thromboembolism in Patients Undergoing Total Hip Arthroplasty

Objective: To demonstrate the efficacy and safety of a multimodal stratified approach for prevention of thromboembolism in patients undergoing primary total hip arthroplasty. Method: A longitudinal retrospective clinical study of a cohort of 341 individuals undergoing total hip arthroplasty between March 2008 and July 2016. Of these, 242 patients met the criteria for inclusion in the study. Individuals with a history of deep vein thrombosis or pulmonary thromboembolism, thrombophilia, hypercoagulation conditions, and chronic users of anticoagulants before surgery received enoxaparin at a dose of 40 mg/day for 21 days. For the other group, acetylsalicylic acid was prescribed at a dose of 200 mg/day for 30 days. All patients had surgical risk assessed by the American Anesthesiology Association classified as I or II, and underwent regional anesthesia, effective hydration, the same protocol of early postoperative rehabilitation, and the use of compression stockings. Drug cost analysis was performed based on the Brasíndice. To date, there are no publications in PubMed and Scielo on this subject regarding the Brazilian population. Results: The need for blood transfusion in the enoxaparin group was higher and the overall complication and hemorrhagic rates were also significantly higher (p < 0.010) in this group. Aspirin prescription costs 39 times less than enoxaparin. Conclusion: In patients without predisposing factors to thromboembolism and undergoing primary total hip arthroplasty, the multimodal approach with acetylsalicylic acid seems to be effective, with low cost and a low rate of complications.

Primary Liver Large B-Cell Lymphoma: A Rare Diagnosis

Primary liver lymphoma is extremely rare, in most of cases it is a B cell lymphoma. Usually the diagnosis is made in middle-aged individuals and most of them have a relatively short life expectancy. In this article, the authors present a case report of a 75-year-old woman with symptoms of three weeks of evolution of diffuse abdominal pain, asthenia and anorexia. The analysis showed cyto cholestasis and the radiological image with lush hepatomegaly because of a large hepatic mass. The patient started chemotherapy and actually is well twenty months after the end of the chemotherapy.

Objective: To report atypical morphological features of Trichophyton(T.) tonsurans strains associated with tinea capitis. Methods: Eighty-two T. tonsurans strains isolated in Ceará, Brazil, were analyzed regarding macro and micromorphological features and nutritional patterns. Results: Fifty-two samples presented abundant chlamydoconidia, which were produced in chains. Macroscopically, these strains developed small glabrous colonies that were firmly attached to the surface of the culture medium, with few or no aerial mycelia and intense rusty yellow pigmentation. Seven strains did not grow with stimulus from thiamine. Samples were heterogeneous regarding urease production and none presented in vitro hair perforation. Conclusions: The observation of T. tonsurans strains with distinct phenotypic features indicates the need to revise the taxonomic criteria for routine identification of this dermatophyte.

BACKGROUND Inflammatory bowel diseases(IBD)have been associated with a low quality of life(QoL)and a negative impact on work productivity compared to the general *** about disease control,patient-reported outcomes(PROs),treatment patterns and use of healthcare resources is relevant to optimizing IBD *** To describe QoL and work productivity and activity impairment(WPAI),treatment patterns and use of healthcare resources among IBD patients in *** A multicenter cross-sectional study included adult outpatients who were previously diagnosed with moderate to severe Crohn’s disease(CD)or ulcerative colitis(UC).At enrolment,active CD and UC were defined as having a Harvey Bradshaw Index≥8 or a CD Activity Index≥220 or calprotectin>200μg/g or previous colonoscopy results suggestive of inadequate control(per investigator criteria)and a 9-point partial Mayo score≥5,*** PRO assessment included the QoL questionnaires SF-36 and EQ-5D-5L,the Inflammatory Bowel Disease Questionnaire(IBDQ),and the WPAI *** about healthcare resources and treatment during the previous 3 years was collected from medical ***-square,Fisher’s exact and Student’s t-/Mann-Whitney U tests were used to compare PROs,treatment patterns and the use of healthcare resources by disease activity(α=0.05).RESULTS Of the 407 patients in this study(CD/UC:64.9%/35.1%,mean age 42.9/45.9 years,54.2%/56.6%female,38.3%/37.1%employed),44.7%/25.2%presented moderate-to-severe CD/UC activity,respectively,at *** in median values for CD/UC,respectively,the SF-36 physical component was 46.6/44.7 and the mental component was 45.2/44.2,the EQ-visual analog scale score was 80.0/70.0,and the IBDQ overall score was 164.0/*** to severe activity,female gender,being unemployed,a lower educational level and lower income were associated with lower QoL(P<0.05).Median work productivity impairment was 20%and 5%for CD and UC patients,re

The cultivation of the marine species of shrimp Litopenaeus vannamei has emerged as one of the segments that best develop in the Brazilian aquaculture, representing the agribusiness that grew the most in recent years. Regarding the economic importance of the farming of this species in Brazil, further studies on genetic improvement were conducted. A F2 segregating population consisting of 192 samples for each G1 and G2 families, from crossing inbred lines was used for the studies performed in the present work. The genetic linkage analysis was based on polymorphic markers derived from nine AFLP (Amplified Fragment Length Polymorphism) primers. Fourteen genetic linkage groups including 103 segregating polymorphic markers were constructed covering350 cMfor G1 and four genetic linkage groups including 59 markers were constructed covering300 cMfor G2. Simple marker analyses were performed among individuals evaluated phenotypically, finding markers linked to genes that may be potentially important and useful to assess characteristics of economic importance for traits related to weight and the disease IMN (idiopathic muscle necrosis). The statistical model including markers explained major proportion of phenotypic characteristic weight in relation to disease incidence IMN.
