Platelet count as a screening tool for compensated cirrhosis in chronic viral hepatitis

BACKGROUND Simple tools for clinicians to identify cirrhosis in patients with chronic viral hepatitis are medically necessary for treatment initiation,hepatocellular cancer screening and additional medical *** To determine whether platelets or other laboratory markers can be used as a simple method to identify the development of *** Clinical,biochemical and histologic laboratory data from treatment naive chronic viral hepatitis B(HBV),C(HCV),and D(HDV)patients at the NIH Clinical Center from 1985-2019 were collected and subjects were randomly divided into training and validation *** markers were tested for their ability to identify cirrhosis(Ishak≥5)using receiver operating characteristic curves and an optimal cut-off was calculated within the training *** final cut-off was tested within the validation *** Overall,1027 subjects(HCV=701,HBV=240 and HDV=86),66%male,with mean(standard deviation)age of 45(11)years were *** the training cohort(n=715),platelets performed the best at identifying cirrhosis compared to other laboratory markers[Area Under the Receiver Operating Characteristics curve(AUROC)=0.86(0.82-0.90)]and sensitivity 77%,specificity 83%,positive predictive value 44%,and negative predictive value 95%.All other tested markers had AUROCs≤*** optimal platelet cut-off for detecting cirrhosis in the training cohort was 143×109/L and it performed equally well in the validation cohort(n=312)[AUROC=0.85(0.76-0.94)].CONCLUSION The use of platelet counts should be considered to identify cirrhosis and ensure optimal care and management of patients with chronic viral hepatitis.

The Effects of Mild-Hyperbaric Oxygen Therapy on Cognitive Function and Symptom Relief in a 35-Year-Old Male with Post-Concussive Symptoms Following a Motor Vehicle Accident: A Case Report

This case report examines the impact of mild hyperbaric oxygen therapy (mHBOT) on cognitive function and symptom relief in a 35-year-old male presenting with concussive symptoms (CS) following a motor vehicle accident (MVA). The patient underwent 10 mHBOT sessions over five weeks (40 minutes per session at 1.5 ATA with 32% oxygen). Post-treatment assessments revealed significant improvements, including an increase in P300 voltage from 4.2 µV to 9.2 µV, aligning with the normative range of 8 - 21 µV. Electroencephalogram (EEG) analysis demonstrated enhanced alpha and theta band activity, reflecting improved cognitive processing and attentional regulation. Clinically, the patient reported reduced headache severity, improved sleep quality, and decreased pain intensity. These findings suggest that mHBOT may support neuroplasticity, mitigate inflammation, and restore cognitive function in patients with CS. Further research, including randomized controlled trials (RCTs), is warranted to validate mHBOT’s efficacy and explore its long-term benefits in traumatic brain injury (TBI) rehabilitation.

Seasonal Fruit Quality Profile of Three Orange Varieties in Teso Subregion—Eastern Uganda

Teso subregion in Eastern Uganda is a leading production hub for citrus in Uganda. However, information on fruit quality profile of the three major varieties (Hamlin, Valencia and Washington navel) grown in this region was limited and/or unreported, partly resulting in their limited use for industrial processing. The current study sought to understand the seasonal quality profile of these varieties during the wet and dry seasons for the years 2019, 2022 and 2023. The study was conducted with farmers in the districts of Kumi, Ngora, Soroti and Kalaki. Mature green oranges were periodically harvested and analysed for physical and chemical properties. The results showed that the quality of oranges varied depending on the season at harvest and the variety with significant differences (p ≤ 0.05). Mean weight per fruit was highest in wet season Washington navel (277.5 g) and lowest in dry season Hamlin (149.3 g). Similarly, fruit size varied in the wet season where Washington navel (81.2 mm) had the biggest fruits while dry season Hamlin were smallest (66.6 mm). Dry season Washington navel had had the thickest peel (7.9 mm). Valencia from the wet season had the highest juice content (36.9%) compared to Washington navel (26.5%) from the dry season with the lowest. Titratable acidity was higher in the dry season Valencia (0.9%) compared to the wet season Washington navel and Hamlin (0.3%). Similarly, Total soluble solids (TSS) was higher in the dry season (Washington navel, 10.1 ˚Brix) in contrast to the wet season (Hamlin and Valencia, 7.7 ˚Brix). Valencia had the lowest pH (3.1) however, pH did not vary significantly between season for each variety except for Hamlin. Vitamin C content was higher during the dry season led by Valencia (57.6 mg/100g) and Hamlin (57.4 mg/100g). Sugar to acid ratio was highest during the wet season (Washington navel, 41.5) and lowest in the dry season (Valencia, 17.1). Overall, dry season fruits and more especially Valencia showed versatility

Humanity is in the grip of another pandemic beyond coronavirus disease(COVID).Although it is not spreading as rapidly as the viral pandemic,it is insidious and probably presents a greater threat in the long *** the large-scale use of antibiotics,we have driven the emergence of antimicrobial resistance(AMR)in bacteria,and these resistant bacteria are increasing in number and prevalence,threatening our ability to treat common infections and reducing our opportunities to use other life-saving treatments such as chemotherapy and surgery.A recent study showed that bacterial AMR was associated with nearly 5 million deaths in2019,with about 1.3 million being directly attributable to resistance[1].These numbers are likely to increase.

Bio-inspired vibrational wireless underground communication system

The internet of the underground things(IoUT)is an emerging field that concerns connected underground sensing nodes and can find applications in various fields such as geotechnical engineering,precision agriculture,and search and rescue *** complex underground environment and multiphase nature of the soil pose challenges to wireless underground *** existing studies on wireless underground communication focus on the use of electromagnetic ***,as a highly lossy material for electromagnetic waves,soil can limit the range and reliability of data *** by subterranean animals that rely on vibrations or seismic waves for underground communication,the prototype system developed in this study is based on *** system includes a bioinspired vibrating source,a micro-electromechanical system(MEMS)accelerometer,a microcontroller,and a set of algorithms for encoding and decoding ***,the mole ratsinspired source is small in size,low in cost,and *** on-off-keying decoding algorithm enhanced with an error-correction algorithm is found to be robust in transmitting textual and imaginary *** the current design,a maximum transmission bit rate of 16e17 bits per second and a transmission distance of 80 cm can be *** bit error ratio is as low as 0.1%,demonstrating the robustness of the *** performance of the developed system shows that seismic waves produced by vibration can be used as an information carrier and can potentially be implemented in the IoUT.

Advantages of a Biomimetic Stiffness Profile in Pitching Flexible Fin Propulsion

The use of oscillating flexible fins in propulsion has been the subject of several studies in recent years, but attention israrely paid to the specific role of stiffness profile in thrust *** profile is defined as the variation in localchordwise bending stiffness (EI) of a fin, from leading to trailing *** this study, flexible fins with a standard NACA0012shape were tested alongside fins with a stiffness profile mimicking that of a Pumpkinseed Sunfish (Lepomis gibbosus).The finswere oscillated with a pitching sinusoidal motion over a range of frequencies and amplitudes, while torque, lateral force andstatic thrust were *** the range of oscillation parameters tested, it was shown that the fin with a biomimetic stiffness profile offered a significantimprovement in static thrust, compared to a fin of similar dimensions with a standard NACA0012 aerofoil *** fin also produced thrust more consistently over each oscillation cycle.A comparison of fin materials of different stiffness showed that the improvement was due to the stiffness profile itself, andwas not simply an effect of altering the overall stiffness of the *** of the same stiffness profile were observed to follow thesame thrust-power curve, independent of the stiffness of the moulding *** fins were shown to produce up to26% greater thrust per watt of input power, within the experimental range.
