AIM： To study the efficacy, safety, and feasibility of a granulocyte adsorptive type apheresis system for the treatment of patients with chronically active ulcerative colitis despite standard therapy. METHODS： An open label multicenter study was carried out in 39 patients with active ulcerative colitis （CAI 6-8） despite continuous use of steroids （a minimum total dose of 400 mg prednisone within the last 4 wk）. Patients received a total of five aphereses using a granulocyte adsorptive technique （Adacolumn, Otsuka Pharmaceutical Europe, UK）. Assessments at wk 6 and during follow-up until 4 mo comprised clinical （CAI） and endoscopic （EI） activity index, histology, quality of life （IBDQ）, and laboratory tests. rESULTS： Thirty-five out of thirty-nine patients were qualified for intent-to-treat analysis. After the apheresis treatment at wk 6, 13/35 （37.1%） patients achieved clinical remission and 10/35 （28.6%） patients had endoscopic remission （CAI〈4, EI〈4）. Quality of life （IBDQ） increased significantly （24 points, P〈0.01） at wk 6. Apheresis could be performed in all but one patient. Aphereses were well tolerated, only one patient experienced anemia. CONCLUSION： In patients with steroid refractory ulcerative colitis, five aphereses with a granulocyte/ monocyte depleting filter show potential short-term efficacy. Tolerability and technical feasibility of the procedure are excellent.

Background:Pediatric cardiology patients often experience decreased quality of life(QoL)and higher rates of mental illness,particularly with severe disease,but the relationship between them and comparisons across diagnostic groups are *** mixed-methods cross-sectional study assessed the association between QoL anxiety and behavior problems among children with structural heart disease,arrhythmia,or other cardiac ***:Children(6–14 years,n=76,50%female)and their parents completed measures of QoL(PedsQL),behavior(BASC-2,subset of 19 children)and anxiety(MASC-2,children 8+years).Pearson correlations/regression models examined associations between QoL,behavior and anxiety,controlling for age,sex,diagnosis and condition *** t-tests compared results to published normative ***-structured interviews(15 children,13 parents)examined self-and parent-reported perceptions of the patient’s emotional ***:Child reported social QoL(95%CI difference:−11.0,−2.1),emotional well-being(95%CI difference:−12.5,−3.8)and adjustment skills(95%CI difference:−13.9,−3.0)were significantly below published norms(pr age,sex,nor diagnosis were significantly related to questionnaire *** to children with mild conditions,child and parent-reported QoL was significantly lower in children with complex conditions(np2≥0.14,pr QoL was strongly associated with more internalizing problems based on both child(r=−0.78,prent(r=−0.67,presponses suggested emotional distress was attributed to“normal”child problems rather than the child’s cardiac ***:Children with complex cardiac conditions,regardless of diagnosis,had diminished *** the PedsQL in clinical settings may help identify pediatric cardiology patients experiencing emotional distress(i.e.,anxiety and depression).

AIM： To test the hypothesis that calcium sensing receptor （CASr） polymorphisms are associated with chronic pancreatitis （CP）, and to determine whether serine protease inhibitor Kazal 1type （SPINK1） N34S or alcohol are necessary co-factors in its etiology. METHODS： Initially, 115 subjects with pancreatitis and 66 controls were evaluated, of whom 57 patients and 21 controls were predetermined to carry the high-risk SPINK1 N34S polymorphism. We sequenced CASr gene exons 2, 3, 4, 5 and 7, areas containing the majority of reported polymorphisms and novel mutations. Based on the initial results, we added 223 patients and 239 controls to analyze three common nonsynonymous single nucleotide polymorphisms （SNPs） in exon 7 （A986S, r990G, and Q1011E）. rESULTS： The CASr exon 7 r990G polyrnorphism was significantly associated with CP （Or, 2.01; 95% CI, 1.12-3.59; P = 0.015）. The association between CASr r990G and CP was stronger in subjects who reported moderate or heavy alcohol consumption （Or, 3.12; 95% CI, 1.14-9.13; P = 0.018）. There was no association between the various CASr genotypes and SPINK1 N34S in pancreatitis. None of the novel CASr polymorphisms reported from Germany and India was detected. CONCLUSION： Our United States-based study confirmed an association of CASr and CP and for the first time demonstrated that CASr r990G is a significant risk factor for CP. We also conclude that the risk of CP with CASr r990G is increased in subjects with moderate to heavy alcohol consumption.

Flowers are required for the Darwinian fitness of flowering plants, but flowers＇ advertisements for pollination services can attract florivores. Previous glasshouse work with Nicotiana attenuata revealed the role of jasmonate （JA） signaling in flower development, advertisement and defense. However, whether JA signaling mediates flowers＇ filtering of floral visitors in nature remained unknown. This field study revealed that silencing JA signaling resulted in flowers that produce less nectar and benzyl acetone, two pollinatorattractive traits. Meanwhile, flowers of defenseless plants were highly attacked by a suite of native herbivores, and damage to buds in native plants correlated negatively with their JA-lle levels.

Study Design: Between January 2003 and June 2009, we collected fresh tumor and extracted high-quality rNA from the omental/peritoneal metastases of 47 patients with stage IIB-IV ovarian cancer. Clinical data were abstracted from the patients’ medical records. Expression of Six1 level by quantitative rT-PCr was compared with preoperative factors and intraoperative findings using the χ2 test and the Fisher exact test. The effect of Six1 elevation on survival was assessed with the Kaplan/Meier method. results: The mean age of patients enrolled was 60 (range 33 - 84). The histological subtypes were 77% serous (36/47), 11% endometrioid (5/47), 4% mucinous (2/47), and 4% clear cell (2/47). Eighty-one percent were optimally cytoreduced. Median Six1 expression for the samples was 114 fg/ng 18S rrNA and Six1 overexpression, defined as >300 fg/ng 18S rrNA, was observed in 19% of tumors. Six1 expression above sample median was associated with peritoneal disease (p = 0.049) and inability to optimally cytoreduce (p = 0.02). Six1 overexpression was associated with worsened survival in the high grade serous subgroup (43 months versus 71 months, p = 0.039 Log rank test). Conclusions: Elevated levels of Six1 predict peritoneal disease and larger residual tumor after maximal cytoreductive effort. Prospective prediction of surgical cytoreduction using a combination of Six1 expression, included with other factors, is currently being evaluated.
