Infliximab in pediatric inflammatory bowel disease rapidly decreases fecal calprotectin levels

AIm: To study the response to infliximab in pediatric inflammatory bowel disease (IBD), as reflected in fecal calprotectin levels. mEThODS: Thirty-six pediatric patients with IBD [23 Crohn's disease (CD), 13 ulcerative colitis (UC); median age 14 years] were treated with infliximab. Fecal calprotectin was measured at baseline, and 2 and 6 wk after therapy, and compared to blood inflammatory markers. maintenance medication was unaltered until the third infusion but glucocorticoids were tapered off if the patient was doing well. RESUlTS: At introduction of infliximab, median fecal calprotectin level was 1150 μg/g (range 54-6032 μg/g). By week 2, the fecal calprotectin level had declined to amedian 261 μg/g (P he patients, fecal calprotectin was normal (here was no additional improvement in the fecal calprotectin level (median 345 μg/g). In 22% of the patients, fecal calprotectin levels increased by week 6 to pretreatment levels or above, suggesting no response (or a loss of early response). Thus, in CD, the proportion of non-responsive patients by week 6 seemed lower, because only 9% showed no improvement in their fecal calprotectin level when compared to the respective figure of 46% of the UC patients (P lUSION: When treated with infliximab, fecal calprotectin levels reflecting intestinal inflammation normalized rapidly in one third of pediatric patients suggesting complete mucosal healing.

heterogeneous Networks(hetNets)and cell densification represent promising solutions for the surging data traffic demand in wireless *** dense hetNets,user traffic is steered toward the low-Power Node(lPN)when possible to enhance the user throughput and system capacity by increasing the area spectral ***,because of the transmit power differences in different tiers of hetNets and irregular service demand,a load imbalance typically exists among different serving *** offload more traffic to lPNs and coordinate the Inter-Cell Interference(ICI),Third-Generation Partnership Project(3GPP)has facilitated the development of the Cell Range Expansion(CRE),enhanced Inter-Cell Interference Coordination(eICIC)and Further enhanced ICIC(FeICIC).In this paper,we develop a cell clustering-based load-aware offsetting and an adaptive low-Power Subframe(lPS)*** solution allows the separation of User Association(UA)functions at the User Equipment(UE)and network server such that users can make a simple cell-selection decision similar to that in the maximum Received Signal Strength(max-RSS)based UA scheme,where the network server computes the load-aware offsetting and required lPS periods based on the load conditions of the *** proposed solution is evaluated using system-level simulations wherein the results correspond to performance changes in different service *** show that our method effectively solves the offloading and interference coordination problems in dense hetNets.

AIm: To examine whether the fasting levels of serum gastrin-17 (G-17) are lower in Barrett's esophagus (BE)patients than in non-Barrett ***: Nineteen patients with BE (presenting with a tubular segment ≥2 cm long in lower esophagus and intestinal metaplasia of incomplete type ('specialized columnar epithelium') in endoscopic biopsies from the tubular segment below the squamocolumnar junction were collected prospectively from outpatients referred to diagnostic gastroscopy. The controls comprised 199 prospectively collected dyspeptic outpatients without BE or any endoscopically visible lesions in the upper GI *** levels of serum G-17 (G-17fast) were assayed with an EIA test using a mab highly specific to amidated G-17. None of the patients and controls received therapy with PPIs or other antisecretory ***: The mean and median levels of G-17fast in serum were significantly lower (P = 0.001) in BE patients than in controls. The positive likelihood ratios (lR+) of low G-17fast to predict BE in the whole study population at G-17fast levels ＜0.5, ＜1, or ＜1.5 pmol/l were 3.5, 3.0,and 2.8, respectively. Among patients and controls with healthy stomach mucosa, the lR+ were 5.6, 3.8, and 2.6,respectively. In the whole study population, serum G-17 was below 2 pmol/l in 15 of 19 BE patients (79%). The corresponding prevalence was 66 of 199 (33%) in controls (P＜0.001). The G-17fast was 5 pmol/l or more in only one of the 19 BE patients (5%). In controls, 76 of the 199 patients (38%) had such high serum G-17fast levels (P＜0.01).CONClUSION: Serum levels of G-17fast tend to be lower in native patients with BE than in healthy controls.

Weathering of clay minerals is a source of nutrients to plants. Palygorskite is a mg-rich fibrous clay mineral that commonly occurs in the soils and sediments of arid regions. Although many studies have examined the environmental conditions required for the formation and stability of palygorskite, information on the transformation of this mineral in the root zone （or rhizosphere） of agricultural crops is limited. This study explored the possibility of palygorskite transformation in the rhizosphere of wheat and oat and compared the ability of these crops to extract structural mg from palygorskite. The crops were cultivated in pots consisting of a mixture of sand from hamadan region, Iran and Florida palygorskite, irrigated with distilled water, and treated with either complete or mg-free nutrient solutions. After 100 d, mg uptake by the crops was measured. Clay-sized particles in each pot were also separated from the sand and were analyzed through X-ray diffraction （XRD）. The X-ray pattern of the particles was compared with that of the pure palygorskite particles （before cultivation）. The results showed that palygorskite could provide sufficient mg for the growth of wheat in the pots supplied with mg-free nutrient solution. In spite of the magnesium uptake by both plants from the palygorskite structure, no detectable peaks indicating the transformation of pmygorskite were recognized by XRD. A decrease of 1.05 nm in peak intensity was more obvious for the pots containing palygorskite and irrigated with mg-free nutrient solution than for the pots irrigated with complete nutrient solution. Although the decrease in peak intensity was greater for oat than wheat, indicating greater palygorskite weathering by oat, mg uptake by oat was low, as evidenced by the deficiency symptoms observed.

mg-based amorphous alloys are one of the potential hydrogen storage materials but suffer from sluggish dehydrogenation/hydrogenation(de/hydrogenation)*** this work,as a new strategy,a hydrogen pump is built on the surface of amorphous alloys to solve this *** milling crystalline YFe_(2-x)Al_(x) hydrogen storage alloy with mg_(60)la_(10)Ni_(20)Cu_(10) amorphous alloy,fine crystalline particles were seeded on amorphous alloy powder to form a“strawberry”*** to the TEm observation,a metallurgical bonding boundary formed between the mg-based amorphous matrix and the Y-Fe-Al crystalline *** microstructure and de/hydrogenation kinetics investigation,the“hydrogen pump”effect of the seeded crystalline alloy was confirmed,which makes it much easier for the hydrogen to dissociate on and diffuse through the surface of the mg-based amorphous *** such effect,the h absorption rate of mg_(60)la_(10)Ni_(20)Cu_(10) amorphous alloy became almost eight times faster and it absorbs ~2.8 wt.% in 1 h at 130℃ under 4.5 mPa-h_(2).Further,fast hydrogenation can even achieve at 70℃ and the low-temperature dehydrogenation kinetics of the amorphous hydride can be also greatly *** present work proves that surface modification is of great importance for obtaining mg-based amorphous alloy with ideal hydrogen storage performance.

Analytical studies on electromagnetoelastic behaviors are presented for the functionally graded piezoelectric material （FGPm） solid cylinder and sphere placed in a uniform magnetic field and subjected to the external pressure and electric loading. When the mechanical, electric and magnetic properties of the material obey an identical power law in the radial direction, the exact displacements, stresses, electric potentials and perturbations of magnetic field vector in the FGPm solid cylinder and sphere are obtained by using the infinitesimal theory of electromagnetoelasticity. Numerical examples also show the significant influence of material inhomogeneity. It is interesting to note that selecting a specific value of inhomogeneity parameter β can optimize the electromagnetoelastic responses, which will be of particular importance in modern engineering designs.

In the framework of the Thomas–Fermi(TF) approach, a model for the p-type double-δ-doped(DDD) system in Ga As is presented. This model, unlike other works in the literature, takes into account that the Poisson equation associated with the system is nonlinear. The electronic structure is calculated for heavy and light holes. The changes in the electronic structure result of the distance d between the doped layers are studied. In particular, the relative absorption coefficient as well as the relative refractive index change is calculated as a function of the incident photon energy for heavy holes. The effect of the interlayer distance exhibits, in the absorption coefficient, a red shift of the peak position and a decrease in amplitude when the distance increases. In addition, the relative refractive index change node has a red shift as well as the interlayer distance increases. The calculations show that the effect of the separation between layers has a greater influence on the linear terms. These results are very important for theoretical calculations and engineering of optical and electronic devices based in δ-doped Ga As.
