Levamisole-Adultered Cocaine: Vasoconstrictive and Vasculitis Effects Occurring Simultaneously

This case presents a rarely seen combination of two Cocaine-induced syndromes occurring simultaneously. Levamisole-adultered cocaine leads to Levamisole-induced Vasculitis (LIV), while cocaine’s vasoconstrictive effect causes destruction of the osteo-cartilaginous structures of nose called Cocaine-induced midline destructive lesions (CIMDL). This case raises awareness of a new pattern of recognition in order to avoid misdiagnosis. We present a case of a 51-year-old male cocaine user who developed severe left-sided ear pain and yellowish secretions with circular necrotic lesions in the nose, right earlobe, and dorsum of feet bilaterally. An extensive workup to assess purpuric lesions’ etiology, including vasculitis, collagen vasculitides, anti-phospholipid syndrome, hypercoagulable state and infectious process, was performed and found negative. He developed bilateral tympanic membrane perforation and purpuric necrotic skin lesions, at the same time, due to CIMDL and LIV, respectively. These two syndromes rarely present simultaneously in a patient. This case report exhibits a new Cocaine-induced clinical presentation that will help hospitalists recognize the disease and avoid misdiagnosis leading to unnecessary tests, prolonged hospitalization, and higher healthcare costs. Diagnosis is challenging, as it consists of establishing a temporal relationship between cocaine ingestion and symptoms’ onset. This disease usually has a benign progression because symptoms frequently resolve without intervention.

Tropical Stratospheric Circulation and Monsoon Rainfall

Interannual variability of both SW monsoon (June-September) and nE monsoon (October-December) rainfall over subdivisions of Coastal Andhra Pradesh, Rayalaseema and Tamil nadu have been examined in relation to monthly zonal wind anomaly for 10 hPa, 30 hPa and 50 hPa at Balboa (9°n, 80°W) for the 29 year period (1958-1986). Correlations of zonal wind anomalies to SW monsoon rainfall (r = 0.57, significant at 1% level) is highest with the longer lead time (August of the previous year) at 10 hPa level suggesting some predictive value for Coastal Andhra Pradesh. The probabilities estimated from the contingency table reveal non-occurrence of flood during easterly wind anomalies and near non-occurrence of drought during westerly anomalies for August of the previous year at 10 hPa which provides information for forecasting of performance of SW monsoon over Coastal Andhra Pradesh. However, nE monsoon has a weak relationship with zonal wind anomalies of 10 hPa, 30 hPa and 50 hPa for Coastal Andhra Pradesh, Rayalaseema and Tamil *** of the SW monsoon storms and depressions in association with the stratospheric wind were also examined to couple with the fluctuations in SW monsoon rainfall. It is noted that easterly / westerly wind at 10 hPa, in some manner, suppresses / enhances monsoon storms and depressions activity affecting their tracks.

Malnutrition negatively impacts the quality of life of patients with cirrhosis: An observational study

AIM To verify how malnutrition is related to health-related quality of life(HRQL) impairment in patients with *** Data was retrospectively abstracted from medical records and obtained by direct interview. We included patients with cirrhosis from any etiology, evaluated at the Liver Clinic from Gastroenterology Department in a tertiary healthcare center, from June 2014 to June 2016. Child-Pugh score, data about complications, and demographic, clinical and anthropometric characteristics of patients were obtained. nutritional status was evaluated by the Subjective Global Assessment(SGA). HRQL was evaluated through the Chronic Liver Disease Questionnaire. Patients were requested to assess their global HRQL with the following code: 0 = impairment of HRQL, when it was compared with other healthy subjects; 1 = good HRQL, if it was similar to the quality of life of other healthy subjects. To compare the primary outcome between malnourished and well-nourished groups, the χ~2 test, Fisher's exact test or Student's t-test were used, based on the variable type. Associations between predictor variables and deterioration of HRQL were determined by calculating the hazard ratio and 95% confidence interval using Cox proportional hazards regression. RESULTS A total of 127 patients with cirrhosis were included, and the mean age was 54.1 ± 12.3 years-old. According to Child-Pugh scoring, 25(19.7%) were classified as A(compensated), 76(59.8%) as B, and 26(20.5%) as C(B/C = decompensated). According to SGA, 58(45.7%) patients were classified as well-nourished. Sixty-nine patients identified HRQL as good, and 76 patients(59.8%) perceived impairment of their HRQL. A gap-free genome assembly of Chlamydomonas reinhardtii anddetection of translocations induced by CRISPR-mediated mutagenesis

Genomic assemblies of the unicellular green alga Chlamydomonas reinhardtii have provided important resources for ***,assembly errors,large gaps,and unplaced scaffolds as well as strain-specific variants currently impede many types of *** combining PacBio HiFi and Oxford nanopore long-read technologies,we generated a de novo genome assembly for strain CC-5816,derived from crosses of strains CC-125 and *** methods of evaluating genome completeness and base-pair error rate suggest that the final telomere-to-telomere assembly is highly *** CC-5816 assembly enabled previously difficult analyses that include characterization of the 17 centromeres,rDnA arrays on three chromosomes,and 56 insertions of organellar DnA into the nuclear *** nanopore sequencing,we identified sites of cytosine(CpG)methylation,which are enriched at *** analyzed CRISPR-Cas9 insertional mutants in the PF23 *** of the three alleles produced progeny that displayed patterns of meiotic inviability that suggested the presence of a chromosomal *** nanopore reads from pf23-2 and pf23-3 onto the CC-5816 genome showed that these two strains each carry a translocation that was initiated at the PF23 gene locus on chromosome 11 and joined with chromosomes 5 or 3,*** translocations were verified by demonstrating linkage between loci on the two translocated chromosomes in meiotic *** three pf23 alleles display the expected short-cilia phenotype,and immunoblotting showed that pf23-2 lacks the PF23 *** CC-5816 genome assembly will undoubtedly provide an important tool for the Chlamydomonas research community.

Invertebrate Iridovirus Modulation of Apoptosis

Programmed cell death (apoptosis) is a key host response to virus infection. Viruses that can modulate host apoptotic responses are likely to gain important opportunities for transmission. Here we review recent studies that demonstrate that particles of Invertebrate iridescent virus 6 (IIV-6) (Iridoviridae, genus Iridovirus), or an IIV-6 virion protein extract, are capable of inducing apoptosis in lepidopteran and coleopteran cells, at concentrations 1000-fold lower than that required to shut-off host macromolecular synthesis. Induction of apoptosis depends on endocytosis of one or more heat-sensitive virion component(s). Studies with a JnK inhibitor (SP600125) indicated that the JnK signaling pathway is significantly involved in apoptosis in IIV-6 infections of Choristoneura fumiferana cells. The genome of IIV-6 codes for an inhibitor of apoptosis iap gene (193R) that encodes a protein of 208 aa with 15% identity and 28% similarity in its amino acid sequence to IAP-3 from Cydia pomonella ganulovirus (CpGV). Transcription of IIV-6 iap did not require prior DnA or protein synthesis, indicating that it is an immediate-early class gene. Transient expression and gene knockdown studies have confirmed the functional nature of the IIV-6 iap gene. We present a tentative model for IIV-6 induction and inhibition of apoptosis in insect cells and discuss the potential applications of these findings in insect pest control.
