New insights into the pathophysiology of achalasia and implications for future treatment

Idiopathic achalasia is an archetype esophageal motor disorder, causing significant impairment of eating ability and reducing quality of life. The pathophysiological underpinnings of this condition are loss of esophageal peristalsis and insufficient relaxation of the lower esophageal sphincter(LES). The clinical manifestations include dysphagia for both solids and liquids, regurgitation of esophageal contents, retrosternal chest pain, cough, aspiration, weight loss and heartburn. Even though idiopathic achalasia was first described more than 300 years ago, researchers are only now beginning to unravel its complex etiology and molecular pathology. The most recent findings indicate an autoimmune component, as suggested by the presence of circulating anti-myenteric plexus autoantibodies, and a genetic predisposition, as suggested by observed correlations with other well-defined genetic syndromes such as Allgrove syndrome and multiple endocrine neoplasia type 2 B syndrome. Viral agents(herpes, varicella zoster) have also been proposed as causative and promoting factors. Unfortunately, the therapeutic approaches available today do not resolve the causes of the disease, and only target the consequential changes to the involved tissues, such as destruction of the LES, rather than restoring or modifying the underlying pathology. New therapies should aim to stop the disease at early stages, thereby preventing the consequential changes from developing and inhibiting permanent damage. This review focuses on the known characteristics of idiopathic achalasia that will help promote understanding its pathogenesis and improve therapeutic management to positively impact the patient's quality of life.

Using fine electromagnetic signals to measure observables of other fields like curvature and torsion of a space, and the corresponding value of their integrals of the action of perception of curvature through electronic signals that detect curvature on a curved surface, it is designed and constructed a sensor of curvature of accelerometer type that detects and curvature measures in 2 and 3-dimensional spaces using the programming of shape operators on spheres and the value of their integrals along the curves and geodesics in their principal directions.

Trends of Cardiovascular mortality in Blacks Africans from Central African City: Hospital Based Study, Brazzaville (Congo)

Background: To determine mortality causes and the associated factors. Patients and method: This cross-sectional study was conducted from June 2014 to may 2015 (1 year) in the department of cardiology at University and teaching Hospital of Brazzaville. Patients admitted for cardiovascular diseases were included. Patients admitted for cardiovascular disease in other departments of the hospital, or without cardiovascular diseases were not included. Results: In total, 1035 patients, 605 women (5804%) were included. mean age was 57.1 ± 7 years (range: 18 - 85 years). The patients were: elderly (n = 498, 48.1%), educated (n = 809, 78.1%), low-income (n = 253, 24.2%). The medical history of patients was: hypertension (n = 440, 42.5%), diabetes (n = 316, 30.5%), reduced physical activity (n = 154, 14.9%), excessive alcohol intake (n = 56, 5.4%). Heart failure was reported in 386 cases (37.3%). The mortality rate was 8% (n = 83);the mean age of deceased was 61.9 ± 7.3 years (p s in death were dilated cardiomyopathy (n = 37, 44.6%, OR 5, 95%CI 3.1 - 8, p -3.3, p = 0.002), atrial fibrillation (n = 24, 29%, OR 3.6, 95%CI 2.2 - 6, p - 9.8, p = 0.005). The associated death comorbidities were: acute respiratory infection (n = 44, 53%, OR 10.1, 95%CI 6.2 - 16, p - 10 p - 6.8, p Conclusion: Hypertension, heart failure and cardiomyopathies are the leading causes of mortality in the department of cardiology. Prevention and patient education in a low-income environment are essential elements to reduce this morbidity.

Perception of the People of Tepoztlfin About Employment Since the Denomination of magic Town

This article aims to describe the perception of the residents of Tepoztlan morelos regarding the level of employment since their designation as magic Town. Tepoztlan is a municipality in the northern part of the State of morelos, located in the South-Central of mexico. It is a magical town situated on the slopes of the Tepozteco mountain. It is recognized as one of the most attractive places in the aforementioned state. Its relative proximity to mexico City makes this town an attractive tourist destination for visitors. The importance of this issue lies mainly in the fact that tourism is one of the largest sources of income and employment generation for the country, so it is essential to strengthen the tourism infrastructure through federal programs of social participation, which promotes the development from the bottom to top. With that objective, the mexican government created a federal program primarily with the purpose of contributing to the development of rural tourism. So, the ＂magic Towns＂ program emerged as a strategy to generate resources for certain communities and increase the tourist offer. Tepoztlan was one of the first towns to enter the program, so it is relevant to analyze if the benefits generated by this program related to employment are perceived by the population. That is to say, to know the opinion of the settlers on the increase and the conditions of the employment that tourism generates and how the denomination of magical town influences this. Regarding the method used, we can say that it is a research with a quantitative approach, a questionnaire was designed as an instrument for data collection. The sample was obtained by the sampling method called： simple random probabilistic sampling. The results presented here correspond to a sample of 119 inhabitants of Tepoztlan. In summary, the research on this subject allows us to conclude that there is an increase in the generation of jobs in Tepoztl/m since its denomination as magical Town; however,

Peri-Annular and Septal Abscesses in Infectious Endocarditis. About 3 Cases Observed at the University Hospital of Brazzaville

Infectious endocarditis (IE) is a relatively rare but serious condition. We report 3 cases of infectious endocarditis complicated by peri-annular and/or septal abscess. The patients aged 24, 25 and 28 were all male. Global heart failure was observed in 2 cases. Atrioventricular block was noted in 2 cases (complete syncopation: 1 time;2nd degree mobitz 2 1 time). Transthoracic echocardiography made it possible to diagnose abscess in all cases (aortic and septal peri-annulus: once, septal only: twice). The aortic insufficiency was grade 2 in all cases, mitral insufficiency grade 2 in 1. The front door was found once, with a dental location. Blood cultures were negative in all cases. A bi-probabilistic antibiotic therapy made it possible to control the infectious process in all the patients. Cardiac surgery, indicated in all cases, was performed in 1 case, consisting of flattening the abscesses and then placing a pacemaker in the patient with complete syncopal atrioventricular block. The outcome of all patients was favorable. Return to permanent sinus rhythm, associated with complete left bundle branch block, was noted at the 4th year of development in the pacemaker wearer. The other 2 patients are still awaiting medical evacuation.

An important part of the fishing activity in mexico is carried out in the coastal strip and has a strong social, economic and ecological importance. In the state of Guerrero, mollusks fishing is coastal and craftwork activity focused on a small number of species and without information on biology and reproductive or ecological aspects for most species caught. This research was carried out in the Costa Chica region, Guerrero, mexico and the aims were: 1) To analyze the socioeconomic features of cooperative societies of fishing production;and 2) To know the diversity of mollusks caught by coastal fishing and the species that have the greatest importance in fishing. From February to April 2013-2015, seven fish coop units, four piers and places where seafood is sold were visited. For the socio-economic analysis and the determination of the species that have more importance in the coastal fishing, a total of 67 surveys were applied. In order to determine the cast of species, the specimens captured by the fishermen were used, which were identified and counted. A total of 67 interviews were analyzed and an average of $1806.00 was estimated as weekly economic income just as a product of fishing, it was also estimated that on average the fishermen work 4.3 days a week. Fisheries cooperatives market most of their produce directly to restaurateurs and locals on the piers. Replacement of older adults by young fishermen from the same region was observed, most are adults between 22 and 39 years old. 25 species of mollusks of economic importance were registered for the coastal fishermen of the Costa Chica. Six species were identified as the main target for catching fishermen. The fishing of mollusks in Costa Chica is not subject to any type of control, which has led to overfishing reflected in the decline sizes and banks in populations of commercial importance.

Objectives: Contribute to improving the management of cardiac tamponade in Congo. Background: Cardiac tamponade is an acute or subacute compression of the heart chambers by pericardial effusion responsible for uncertain prognosis for patients. The objective of this study is to help improve the management of patients with cardiac tamponade. Patients and Methods: We conducted a retrospective and descriptive study at the University Hospital of Brazzaville, from January 2015 to December 31, 2019. Included were all patients hospitalized for cardiac tamponade. Epidemioclinical, therapeutic and evolutionary data were analyzed. Results: An overall of 14 patients were divided into 9 men (64.2%) and 5 women (35.7%), imal;">.e.mal;font-size:10pt;font-family:;" "=""> a sex ratio of 1.8. The mean age of patients was 34.2 ± 15.1 years old (18 years to 64). The most represented comorbidity was hypertension (n = 2;14.3%). The most frequent clinical sign was dyspnea (n = 14;100%). The ECG showed diffuse and concordant repolarization disturbances (n = 14;100%), sinus tachycardia (n = 13;92.8%), microvoltage (n = 12;85.7%). Cardiomegaly was constant (n = 14;100%). In terms of transthoracic ultrasound, we found: diastolic compression of the right ventricle (RV) (n = 12;85.7%), dilation of the inferior vena cava (IVC) (n = 12;85.7%).
