AIM： To analyze the relationship between perisinusoidal stellate cell （PSC） activation and the dietary fat quantity and composition in the treatment of hepatic steatosis. METHODS： Using an experimental rat model of steatosis based on the intake of a hyperlipidic diet （14% fat as olive oil or sunflower oil, HL-O and HL-S, respectively）, we analyzed the liver＇s capability of recovery after the treatment with a normal-lipidic diet （5% fat as olive oil or sunflower oil, nL-O and nL-S, respectively） by immunoo/tochemical and Western blot analysis of glial fibrillary acidic protein （GFAP） expression in PSCs, collagen quantification and serum aminotransferase determination. RESULTS： The fatty infiltration in the steatotic livers decreased after the treatment with both nL diets, indicating liver recovery. This decrease was accompanied with a lower collagen deposition and aminotransferase level as well as changes in the PSC population that increased the GFAP expression. The above-mentioned effects were more pronounced in animals fed on nL-O based diet. COnCLUSIOn： Treatment with a balanced diet enriched in olive oil contributes to the liver recovery from a steatotic process. The PSC phenotype is a marker of this hepatic-recovery model.

An energetic analysis of two power plants with six and seven heaters was realized in this work and also a feedwater heater was taking into consideration. This analysis considered the temperature and pressure ranges under which power generating plants work. The unit thermal consumption (UTC), the specific fuel consumption (SFC) and the specific steam consumption (SSC) were obtained for power generation. The energetic analysis to the cycles of steam with regeneration with six and seven heaters was realized. There was a difference of 0.5% in the values of UTC, SFC and SSC when the pressures of condensation changed. Also an analysis of the behavior of the thermal efficiency in relation to the rise of numbers of heater from two to seven is presented.

The management of patients with advanced non-small cell lung carcinoma(nSCLC)has undergone major changes in recent *** the one hand,improved sensitivity of diagnostic tests,both radiological and endoscopic,has altered the way patients are *** the other hand,the arrival of new drugs with antitumoral activity,such as targeted therapies or immunotherapy,has changed the prognosis of patients,improving disease control and prolonging ***,the development of radiotherapy and surgical and interventional radiology techniques means that radical ablative treatments can be performed on metastases in any location in the *** of these advances have impacted the treatment of patients with advanced lung cancer,especially in a subgroup of these patients in which all of these treatment modalities *** poses a challenge for physicians who must decide upon the best treatment strategy for each patient,without solid evidence for one optimal mode of treatment in this patient *** aim of this article is to review,from a practical and multidisciplinary perspective,published evidence on the management of oligometastatic nSCLC *** evaluate the different alternatives for radical ablative treatments,the role of primary tumor resection or radiation,the impact of systemic treatments,and the therapeutic *** short,the present document aims to provide clinicians with a practical guide for the treatment of oligometastatic patients in routine clinical practice.

AIM To demonstrate the non-inferiority(15% non-inferiority limit) of monotherapy with tenofovir disoproxil fumarate(TDF) vs the combination of lamivudine(LAM) plus adefovir dipivoxil(ADV) in the maintenance of virologic response in patients with chronic hepatitis B(CHB) and prior failure with *** This study was a Phase IV prospective, randomized, open, controlled study with 2 parallel groups(TDF and LAM+ADV) of adult patients with hepatitis B e antigen(HBe Ag)-negative CHB, prior failure with LAM, on treatment with LAM+ADV for at least 6 mo, without prior resistance to ADV and with an undetectable viral load at the start of the study, in 14 Spanish hospitals. The follow-up time for each patient was 48 wk after randomization, with quarterly visits in which the viral load, biochemical and serological parameters, adverse effects, adherence to treatment and consumption of hospital resources were *** Forty-six patients were evaluated [median age: 55.4 years(30.2-75.2); 84.8% male], including 22 patients with TDF and 24 with LAM+ADV. During study development, hepatitis B virus DnA(HBV-DnA) remained undetectable, all patients remained HBe Ag negative, and hepatitis B surface antigen(HBs Ag) positive. Alanine aminotransferase(ALT) values at the end of the study were similar in the 2 groups(25.1± 7.65, TDF vs 24.22 ± 8.38, LAM+ADV, P = 0.646). no significant changes were observed in creatinine or serum phosphorus values in either group. no significant differences between the 2 groups were noted in the identification of adverse effects(AEs)(53.8%, TDF vs 37.5%, LAM+ADV, P = 0.170), and none of the AEs which occurred were serious. Treatment adherence was 95.5% and 83.3% in the TDF and the LAM+ADV groups, respectively(P = 0.488). The costs associated with hospital resource consumption were significantly lower with the TDF treatment than the LAM+ADV treatment(€4943 ± 1059 vs €5811 ± 1538, respectively, P nCLUSIOn TDF monotherapy proved to be safe

Hollow, cylindrical, prismatic light guides （CPLGs） are optical components that, using total internal reflection （TIR）, are able to transmit high-diameter light beams in daylight and artificial lighting applications without relevant losses. It is necessary to study the prism defects of their surfaces to quantify the behavior of these optical components. In this Letter, we analyze a CPLG made of a transparent dielectric material. Scanning electron microscopy （SEM） and the topographic optical profilometry by absorption in fluids （TOPAF） imaging tech- nique are conducted to determine if there are defects in the corners of the prisms. A model for light guide trans- mittance that is dependent on prism defects is proposed. Finally, a simulation and an experimental study are carried out to check the validity of the proposed model.

AIM: To assess the usefulness of urinary trypsinogen-2 test strip, urinary trypsinogen activation peptide (TAP),and serum and urine concentrations of the activation peptide of carboxypeptidase B (CAPAP) in the diagnosisof acute ***: Patients with acute abdominal pain and hospitalized within 24 h after the onset of symptoms were prospectively studied. Urinary trypsinogen-2 was considered positive when a clear blue line was observed (detection limit 50 μg/L). Urinary TAP was measured using a quantitative solid-phase elISA, and serum and urinary CAPAP by a radioimmunoassay ***: Acute abdominal pain was due to acute pancreatitis in 50 patients and turned out to be extrapancreatic in origin in 22 patients. Patients with acute pancreatitis showed significantly higher median levels of serum and urinary CAPAP levels, as well as amylase and lipase than extrapancreatic controls. Median TAP levels were similar in both groups. The urinary trypsinogen-2 test strip was positive in 68% of patients with acute pancreatitis and 13.6% in extrapancreatic controls (Pnary CAPAP was the most reliable test for the diagnosis of acute pancreatitis (sensitivity 66.7%, specificity 95.5%, positive and negative predictive values 96.6% and 56.7%, respectively), with a 14.6 positive likelihood ratio for a cut-off value of 2.32 nmol/***: In patients with acute abdominal pain,hospitalized within 24 h of symptom onset, CAPAP in serum and urine was a reliable diagnostic marker of acute pancreatitis. Urinary trypsinogen-2 test strip showed a clinical value similar to amylase and *** TAP was not a useful screening test for the diagnosis of acute pancreatitis.

BACKGROUnD From February 2020 onwards,our country has been hit by the coronavirus severe acute respiratory syndrome-2(SARS-CoV-2)*** a glance,hospitals became overrun and had to reformulate all the assistance guidelines,focusing on the coronavirus disease *** year after the start of the pandemic,we present the results of a morbimortality *** To analyze how our department was affected by the outbreak in terms of morbimortality,and to analyze demographic data,admission to hospital-related data,and subgroups analyses for patients with hip fractures and *** We designed a study comparing data from patients who were admitted to our unit due to a lower limb fracture or a high energy trauma during the pandemic(from March to April 2020)to those admitted during the same period in 2019 before the *** the pandemic *** cohorts completed a minimum of 6 mo of *** The number of patients admitted to hospital in 2020 was nearly half of those in *** fractures in the elderly represented 52 out of 73 of the admitted *** patients had a positive test result for SARS-CoV-2 *** with SARS-CoV-2 infection were admitted to the hospital for a longer time than the non-infected(Pnd had a higher mortality rate during hospitalization and follow-up(P=0.02).Patients with a hip fracture associated with a severe respiratory syndrome were mostly selected for conservative treatment(P=0.03).COnCLUSIOn Mortality and readmission rates were higher in the 2020 cohort and during follow-up,in comparison with the cohort in 2019.

Using certain models of twistor surfaces for fields of force and the mathematical relationships that lie among fields, lines, surfaces and flows of energy, it has been designed and developed a flight electromagnetic type system based on the synergic study of their electromagnetic field geodesics to generate vehicle levitation, suspension and movement without being in contact with the surface. The idea of such work is to obtain a new flight and impulse patent of an electromagnetic vehicle by principles of super-conduction and some laws of the current like Eddy currents and principles which are very similar to mechanics of sidereal objects like galaxies or stars under models of twistor surfaces. This vehicle will be controlled by one microchip that will be programmed by conscience operators algebra of electromagnetic type that leads to the flow of Eddy currents, the iso-rotations and suspension of the special geometrical characteristics vehicle, generating also on the vehicle structure certain “magnetic conscience” that provokes all movements like succeeding to the sidereal objects in the universe.
