Optimal timing of invasive intervention for high-risk non-ST-segment-elevation myocardial infarction patients

OBJECTIVE:To compare the immediate, early, and delayed percutaneous coronary intervention (PCI) strategies in non-ST-segment-elevation myocardial infarction (nSTEMI) patients with high-risk.METHODS:Medical records of patients treated at the Daping Hospital, Third Military Medical University, Chongqing, China between 2011 and 2021 were retrospectively reviewed. Only patients with complete available information were included. All patients assigned into three groups based on the timing of PCI including immediate (nd delayed (≥ 24 h) intervention. Multivariable Cox hazards regression and simpler nonlinear models were performed.RESULTS:A total of 657 patients were included in the study. The median follow-up length was 3.29 (interquartile range: 1.45-4.85) years. Early PCI strategy improved the major adverse cardiac event (MACE) outcome compared to the immediate or delayed PCI strategy. Early PCI, diabetes mellitus, and left main or/and left anterior descending or/and left circumflex stenosis or/and right coronary artery ≥ 99% were predictors for MACE outcome. The optimal timing range for PCI to reduce MACE risk is 3-14 h post-admission. For high-risk nSTEMI patients, early PCI reduced primary clinical outcomes compared to immediate or delayed PCI, and the optimal timing range was 3-14 h post-admission. Delayed PCI was superior for nSTEMI with chronic kidney injury.COnCLUSIOnS:Delayed invasive strategy was helpful to reduce the incidence of MACE for high-risk nSTEMI with chronic kidney injury. An immediate PCI strategy might increase the rate of MACE.

Mg–3nd–0.2Zn–0.4Zr(nZ30K,wt.%)alloy is a new kind of high-performance metallic *** combination of the nZ30K Magnesium(Mg)alloy and selective laser melting(SLM)process seems to be an ideal solution to produce porous Mg degradable ***,the microstructure evolution and mechanical properties of the SLMed nZ30K Mg alloy were not yet studied ***,the fabrication defects,microstructure,and mechanical properties of the SLMed nZ30K alloy under different processing parameters were *** results show that there are two types of fabrication defects in the SLMed nZ30K alloy,gas pores and unfused *** the increase of the laser energy density,the porosity sharply decreases to the minimum first and then slightly *** minimum porosity is 0.49±0.18%.While the microstructure varies from the large grains with lamellar structure inside under low laser energy density,to the large grains with lamellar structure inside&the equiaxed grains&the columnar grains under middle laser energy density,and further to the fine equiaxed grains&the columnar grains under high laser energy *** lamellar structure in the large grain is a newly observed microstructure for the nZ30K Mg *** laser energy density leads to finer grains,which enhance all the yield strength(YS),ultimate tensile strength(UTS)and elongation,and the best comprehensive mechanical properties obtained are YS of 266±2.1 MPa,UTS of 296±5.2 MPa,with an elongation of 4.9±0.68%.The SLMed nZ30K Mg alloy with a bimodal-grained structure consisting of fine equiaxed grains and coarser columnar grains has better elongation and a yield drop phenomenon.

Gender Pay Gap in the Gig Economy

The rapid ascent of digital platforms and the gig economy has prompted concerns about the gender pay *** results show that in the gig economy,gender continues to be a crucial determinant of workers'earnings,with women earning 85%of what men earn on a monthly ***,in comparison to the traditional waged employment during the same period,the gender pay gap in the gig economy has *** some gig jobs(e.g.,ride-hailing services,delivery services,online education)exhibit certain occupational segregation,women in gig economy work are no longer concentrated in low-paying roles,thereby challenging the occupational crowding hypothesis prevalent in traditional *** the gig economy,the vast majority of the gender pay gaps arise from factors within occupations,and occupational segregation only has a very limited impact on the earnings ***,the gender pay gap among platform gig workers can be mostly explained by observable factors,which implies that compared to traditional employment,the gig economy exhibits a lower level of implicit gender discrimination in ***,we investigate new factors that determine the gender pay gap in the gig *** exhibit a stronger aversion to algorithmic control,a preference for job flexibility,and a tendency toward more isolated and less socially interactive work environmentsall of which serve to widen the gender pay gap that might have otherwise *** results of this research suggest that despite the criticisms surrounding the gig economy,it continues to exert a positive influence on China's labor market。

In this editorial,we comment on the article published by Agatsuma et al in a recent issue of the World J Gastroenterol(2024;30:1368-1376).We firmly concur with Agatsuma et al regarding the vital significance of colorectal cancer(CRC)screening as a public health strategy to diminish disease *** exposed to risk factors for CRC,those with comorbid conditions,and those with limited health literacy should undergo ***,we believe that more regular screenings should be accompanied by a greater focus on primary prevention(PP)of *** remains a significant global health challenge,and its incidence is strongly linked to age,lifestyle,and socioeconomic *** is particularly noteworthy that the majority of CRC patients are diagnosed outside of established screening pathways and frequently at an advanced stage of the disease,and the majority of patients possess inadequate or even nonexistent knowledge regarding CRC,which significantly impacts the prognosis and imposes a substantial economic *** study revealed that CRC identified during hospital visits for comorbid conditions was typically diagnosed at an earlier stage than detected via symptomatic ***,early incidental detection of CRC aligns closely with the timing of discovery through routine cancer *** suggests that by adopting more inclusive screening protocols that combine opportunistic testing with traditional screening methods,health care systems can create a more comprehensive safety net for individuals at risk of ***,before maximizing the health benefits of screening programs,it is essential to make additional efforts prior to screening,such as raising awareness via public education,risk assessment,and personalized recommendations,enhancing the knowledge and skills of health care professionals,optimizing the accessibility and convenience of screening processes,ensuring the quality and safety of screening services,strengthening follow-up and su

Efficient and selective upcycling of waste polylactic acid into acetate using nickel selenide

The conversion of waste polylactic acid（PLA） plastics into high-value-added chemicals through electrochemical methods is a promising and sustainable ***,developing efficient and highly selective catalysts for lactic acid oxidation reaction（LAOR） and understanding the reaction process are ***,we report the electrooxidation of waste PLA to acetate at a high current density of 100 mA cm-2 with high Faraday efficiency（～95%） and excellent stability（>100 h） over a nickel selenide nanosheet *** addition,a total Faraday efficiency of up to 190% was achieved for carboxylic acids,including acetic acid and formic acid,by coupling with the cathodic CO2reduction *** situ experimental results and theoretical simulations revealed that the catalytic activity center of LAOR was dynamically formed niOOH species,and the surface-adsorbed SeOxspecies accelerated the formation of ni3+species,thus promoting catalytic *** mechanism of lactic acid electrooxidation was further *** acid was dehydrogenated to produce pyruvate first and then formed CH3CO due to preferential C-C bond cleavage,resulting in the presence of *** work demonstrated a sustainable method for recycling waste PLA and CO2into high-value-added products.

BACKGROUnD The Alcian blue(AB)and periodic acid Schiff(PAS)stains are representative mucus markers in gastric signet ring cell carcinoma(SRCC).They are low-cost special staining methods used to detect acidic mucus and neutral mucus,***,the clinical importance of the special combined AB and PAS stain is *** To investigate AB expression,PAS expression and the AB-to-PAS(A/P)ratio in gastric SRCC patients and to assess patient *** Paraffin-embedded sections from 83 patients with gastric SRCC were stained with AB and PAS,and signet ring cell positivity was assessed ***-histochemical staining for Ki67,protein 53(P53)and human epidermal growth factor receptor 2(HER2)was performed *** cancer-specific survival(CSS)rate was estimated via Kaplan-Meier *** proportional hazards models were used for univariate and multivariate survival *** Kaplan-Meier survival analysis revealed that the 3-year CSS rate was significantly greater in the high-PAS-expression subgroup than in the low-PAS-expression subgroup(Pn the A/P≤0.5 group was significantly greater than that in the A/P>0.5 group(P=0.042).Univariate Cox regression analysis revealed that the factors affecting prognosis included tumor diameter,lymph node metastasis,vessel carcinoma embolus,tumor stage,the A/P ratio and the expression of Ki67,P53 and the *** multivariate regression analysis confirmed that low PAS expression[hazard ratio(HR)=3.809,95%confidence interval(CI):1.563-9.283,P=0.003]and large tumor diameter(HR=2.761,95%CI:1.086-7.020,P=0.033)were independent risk factors for poor *** A/P>0.5 is potentially a risk factor for prognosis,and low PAS expression is an independent risk factor in the prognosis of gastric *** expression and the A/P ratio could help in predicting the clinical prognosis of patients with SRCC.

Polychlorinated dibenzo-p-dioxins and polychlorinated dibenzofurans(PCDD/Fs)have attracted widespread concern due to their high toxicity,and their difficult manipulation in laboratories has made the research process ***,in our work,furan is selected as the model compound owing to the same structure of a central oxygenate *** catalytic oxidation is regarded as an effective and applicable method for the abatement of PCDD/Fs,the synthesis of low-temperature catalysts is still a challenging problem in practical *** this situation,we prepared a novel V_(2)O_(5)/TiO_(2)catalyst modified with n-doped hierarchical porous carbon(nHPC)via a wet impregnation *** V/T-1%nHPC catalyst could achieve expectant low-temperature performances with 50%furan conversion at 150℃and a complete conversion at 200℃,which decreased 23℃and 40℃compared to the V/T catalyst ***,the addition of nHPC presented lifting chemical stability during long-time *** addition of nHPC in V/T catalysts decreased the formation of crystalline V_(2)O_(5) and increased the percentages of V^(5+)and O_(lat),which improved the utilization of vanadium ions and the catalytic ***,the higher binding energy shift of O_(lat) implied more reaction possibility with other oxidise ***,this work proved the lifting catalytic activity by the interaction between catalysts and nHPC,and proposed the promoting effects of the n *** results showed that the content of the pyridinic n and graphitic n in nHPC changed after combining with V/T catalyst,which played crucial roles in the excellent catalytic ***,this work provides comprehensive research of the V/T-1%nHPC catalyst toward furan oxidation at low temperature and explain the effects of n-doped biomass carbon in catalytic activity clearly,which gave a new thought to design low-temperature catalysts in PCDD/Fs ***,the internal functional mech
