Factors associated with early virological response to peginterferon-α-2a/ribavirin in chronic hepatitis C

AIM: To evaluate the impact of sociodemographic/clinical factors on early virological response (EVR) to pegin-terferon/ribavirin for chronic hepatitis C (CHC) in clinical practice. METHODS: We conducted a multicenter, cross-sectional, observational study in Hepatology Units of 91 Spanish hospitals. CHC patients treated with peginterferon α-2a plus ribavirin were included. EVR was defined as undetectable hepatitis C virus (HCV)-ribonucleic acid (RnA) or ≥ 2 log HCV-RnA decrease after 12 wk of treatment. A bivariate analysis of sociodemographic and clinical variables associated with EVR was carried out. Independent factors associated with an EVR were analyzed using a multiple regression analysis that included the following baseline demographic and clinical variables: age (≤ 40 years vs > 40 years), gender, race, educational level, marital status and family status, weight, alcohol and tobacco consumption, source of HCV infection, alanine aminotransferase (ALT) and aspartate aminotransferase (AST) levels, and gamma glutamyl transpeptidase (GGT) (≤ 85 IU/mL vs > 85 IU/mL), serum ferritin, serum HCV-RnA concentration (notype (1/4 vs 3/4), cirrhotic status and ribavirin dose (800/1000/1200 mg/d).RESULTS: A total of 1014 patients were included in the study. Mean age of the patients was 44.3 ± 9.8 years, 70% were male, and 97% were Caucasian. The main sources of HCV infection were intravenous drug abuse (25%) and blood transfusion (23%). Seventyeight percent were infected with HCV genotype 1/4 (68% had genotype 1) and 22% with genotypes 2/3. The HCV-RnA level was > 400 000 IU/mL in 74% of patients. The mean ALT and AST levels were 88.4 ± 69.7 IU/mL and 73.9 ± 64.4 IU/mL, respectively, and mean GGT level was 82 ± 91.6 IU/mL. The mean ferritin level was 266 ± 284.8 μg/L. Only 6.2% of patients presented with cirrhosis. All patients received 180 mg of peginterferon α-2a. The most frequently used ribavirin doses were 1000 mg/d (41%) and 1200 mg/d (41%). AIM:To analyze the prognostic factors involved in survival and cancer recurrence in patients undergoing surgical treatment for colorectal liver metastases(CLM) and to describe the effects of time-related changes on survival and recurrence in these patients.METHODS:From January 1994 to January 2006,236 patients with CLM underwent surgery with the aim of performing curative resection of neoplastic disease at our institution and 189(80%) of these patients underwent resection of CLM with curative intention.Preoperative,intraoperative and postoperative data,including primary tumor and CLM pathology results,were retrospectively reviewed.Patients were divided into two time periods:a first period from January 1994 to January 2000(n = 93),and a second period from February 2000 to January 2006(n = 143).RESULTS:Global survival at 1,3 and 5 years in patients undergoing hepatic resection was 91%,54% and 47%,respectively.Patients with preoperative extrahepatic disease,carcinoembryonic antigen(CEA) levels over 20 ng/dL,more than four nodules or extrahepatic invasion at pathological analysis had worse survival.Tumor recurrence rate at 1 year was 48.3%,being more frequent in patients with preoperative and pathological extrahepatic disease and CEA levels over 20 ng/dL.Although patients in the second time period had more adverse prognostic factors,no differences in overall survival and recurrence were observed between the two periods.COnCLUSIOn:Despite advances in surgical technique and better adjuvant treatments and preoperative imaging,careful patient staging and selection is crucial to continue offering a chance of cure to patients with CLM.

Agricultural expansion and intensiﬁcation are having a huge impact on plant and arthropod diversity and abun-dance,affecting food availability for farmland birds.Difﬁcult food access,in turn,can lead to immunosuppression and a higher incidence of parasites.In the studies designed to examine changes in the diet of birds and their par-asites,metabarcoding is proving particularly useful.This technique requires mini-barcodes capable of amplifying the DnA of target organisms from fecal environmental DnA.To help to understand the impact of agricultural expansion on biodiversity,this study sought to design and identify mini-barcodes that might simultaneously as-sess diet and intestinal parasites from the feces of farmland birds.The capacity to identify diet and parasites of 2 existing and 3 newly developed mini-barcodes was tested“in silico”in relation to the behavior of a reference eukaryotic barcode.Among the newly designed mini-barcodes,MiniB18S_81 showed the higher taxonomic cover-age of eukaryotic taxa and a greater ampliﬁcation and identiﬁcation capacity for diet and parasite taxa.Moreover,when it was tested on fecal samples from 5 different steppe bird species,MiniB18S_81 showed high taxonomic resolution of the most relevant diet and parasite phyla,Arthropoda,nematoda,Platyhelminthes,and Apicomplexa at the order level.Thus,the mini-barcode developed emerges as an excellent tool to simultaneously provide detailed information regarding the diet and parasites of birds,essential for conservation and management.

BACKGROUnD Antiviral treatment of patients with chronic hepatitis B(CHB)in the grey zone of treatment comands risk management in order to optimize the health outcome.In this sense,the identification of HBV mutants related with an increased risk of hepatocellular carcinoma(HCC)could be useful to identify subpopulations with potential indication of antiviral treatment.AIM To analyze the prevalence/persistence of hepatitis B virus(HBV)preS and basal core promoter(BCP)/precore/core variants associated to HCC development in CHB patients in the grey zone.METHODS Work was designed as a longitudinal retrospective study,including 106 plasma samples from 31 patients with CHB in the grey zone of treatment:Hepatitis B e antigen negative,HBV-DnA levels between 12-20000 IU/mL,normal or discordant transaminase levels during follow up and mild/moderate necroinflammatory activity in liver biopsy or Fibroscan(up to 9.5 kPa).Serum HBVDnA was tested using the Abbott Real Time HBV Assay and the BCP/precore/core and the hepatitis B surface antigen(HBsAg)coding regions were analyzed in positive samples by PCR/bulk-sequencing to identify the HCCrelated HBV mutants.RESULTS High-risk HCC related mutants were detected in 24(77%)patients:19(61%)in the BCP/precore/core,and 7(23%)in the HBsAg coding region(2 preS1 and 5 preS2 deletions).The prevalence of preS deletions was genotype-dependent:3/5(60%)patients with preS2 deletions and 1/2 with preS1 deletions were infected with the HBV-E genotype.Since HBV-E was the most prevalent in sub-Saharan patients,a correlation between preS deletions and ethnicity was also found:6/8(75%)sub-Saharan vs 1/19(5%)Caucasian patients had preS deletions(P=0.00016).Remarkably,this correlation was maintained in those patients infected with HBV-A,a minor genotype in sub-Saharan patients:2/2 patients infected with HBV-A from West Africa vs 0/6 of Caucasian origin had preS deletions.The HCC related variants were the major strains and persisted over time(up to 48 mo).Patients with preS deletions had a significant hig

Colorectal cancer(CRC) is the third most commonly diagnosed cancer in the world and the fourth principal cause of cancer deaths worldwide. Currently, there is a lack of low cost and noninvasive screening tests for CRC, becoming a serious health problem. In this context, a potential biomarker for the early detection of CRC has recently gained attention. Circular RnAs(circ RnA), a re-discovered, abundant RnA specie, is a type of noncoding covalent closed RnAs formed from both exonic and intronic sequences. These circular molecules are widely expressed in cells, exceeding the abundance of the traditional linear m RnA transcript. They can regulate gene expression, acting as real sponges for mi RnAs and also regulate alternative splicing or act as transcriptional factors and inclusive encoding for proteins. However, little is known about circ RnA and its relationship with CRC. In this review, we focus on the biogenesis, function and role of these circ RnAs in relation to CRC, including their potential as a new biomarker.

This article studies a posteriori error analysis of fully discrete finite element approximations for semilinear parabolic optimal control problems.Based on elliptic reconstruction approach introduced earlier by Makridakis and nochetto[25],a residual based a posteriori error estimators for the state,co-state and control variables are derived.The space discretization of the state and co-state variables is done by using the piecewise linear and continuous finite elements,whereas the piecewise constant functions are employed for the control variable.The temporal discretization is based on the backward Euler method.We derive a posteriori error estimates for the state,co-state and control variables in the L^(∞)(0,T;L^(2)(Ω))-norm.Finally,a numerical experiment is performed to illustrate the performance of the derived estimators.

Unified Parallel C （UPC） is a parallel extension of AnSI C based on the Partitioned Global Address Space （PGAS） programming model, which provides a shared memory view that simplifies code development while it can take advantage of the scalability of distributed memory architectures. Therefore, UPC allows programmers to write parallel applications on hybrid shared/distributed memory architectures, such as multi-core clusters, in a more productive way, accessing remote memory by means of different high-level language constructs, such as assignments to shared variables or collective primitives. However, the standard UPC collectives library includes a reduced set of eight basic primitives with quite limited functionality. This work presents the design and implementation of extended UPC collective functions that overcome the limitations of the standard collectives library, allowing, for example, the use of a specific source and destination thread or defining the amount of data transferred by each particular thread. This library fulfills the demands made by the UPC developers community and implements portable algorithms, independent of the specific UPC compiler/runtime being used. The use of a representative set of these extended collectives has been evaluated using two applications and four kernels as case studies. The results obtained confirm the suitability of the new library to provide easier programming without trading off performance, thus achieving high productivity in parallel programming to harness the performance of hybrid shared/distributed memory architectures in high performance computing.

The potential of legume-derived proteins in the food industry

Legume-derived proteins present an opportunity to replace existing animal-source protein in various applications. Those proteins are abundant, relatively lowcost, sustainable, not highly allergenic, and widely acceptable by consumers. In this paper, it was found that legume-derived protein’s technofunctional properties(e.g. gelation, emulsification and foaming) are being investigated, in order to assess their ability to substitute for animal-derived proteins in food systems and applications. This paper reviewed the functional attributes of legume-derived proteins, their possible applications in food systems, and their potential in the food industry. The techno-functional properties of the proteins vary among different legumes, and while some proteins properties are sufficient for industrial uses, the other may need to be modified. It was concluded that legume-derived proteins could replace some existing animal-derived protein based food systems.
