Aims: we investigate whether insulin resistance is associated with an increased prevalence for chronic kidney disease irrespective of the concurrent presence of metabolic syndrome. Methods: 1638 patients with abdominal obesity were selected. Metabolic syndrome and abdominal obesity were defined according to the International Diabetes Federation criteria. Insulin resistance was defined by Homeostasis Model Assessment Index >P75. Chronic kidney disease was defined by the presence of a low estimated glomerular filtration rate (Results: metabolic syndrome was present in 1030 (62.9%) patients and insulin resistance in 787 (48%). Conversely 61% of those with metabolic syndrome were insulin resistant and 79% of those with insulin resistance had metabolic syndrome. Chronic kidney disease was present in 18%. In multivariate analysis, chronic kidney disease was increased in subjects with insulin resistance (odds ratio [OR] = 1.350;CI 95%: 1.021 - 1.785;p = 0.035) and in those with metabolic syndrome (OR = 1.417;CI 95%: 1.045 - 1.922;p = 0.025). Conclusions: Metabolic syndrome and insulin resistance were significant and independently associated with chronic kidney disease in nondiabetic adults with abdominal obesity.

Radical cystectomy(RC)with pelvic lymph node dissection(PLND)is the standard treatment for localized muscle-invasive bladder cancer(MIBC)and non-muscle-invasive bladder cancer(NMIBC)with recurrence or high risk of progression.Also,the robotic approach to this type of surgery is well established in the literature.Our objective is to summarize in this manuscript the most relevant articles related to the robotic-assisted radical cystectomy for prostate cancer.We performed a literature review of articles describing the robotic approach to RC in patients with bladder cancer.Also,we described the procedure since the patient selection until the bladder removal.The reconstructive techniques were not included in this review.Twenty-five articles were used to divide our manuscript into key points such as preoperative patient selection and protocols,surgical technique,pathology report,oncological outcomes,complication rates,and quality of life after the procedure.Robotic-assisted radical cystectomy is feasible and safe with satisfactory oncological outcomes.The robotic approach is related to lower blood loss and fewer transfusion rates.However,when compared to open surgery,the use of this technology increases the operative time.

Gravity data are sensitive to local vertical offsets across high-angle faults, where rocks with different densities are juxtaposed. Yet high densities in some Mesozoic sedimentary rocks just above the basement may smear out the subtle gravity signatures of basement faults. At this study the gravity data processing tends to avoid ill-described “black-box” techniques. The study area is situated in the Palomas site, Cuatrociénegas region, Coahuila, NE Mexico. The San Marcos Fault is at least 300 km long and has WNW-ESE trend from the central part of Nuevo León State through Coahuila, and finally to the eastern part of Chihuahua State. Gravimetric data shows that the lowest values of free air and Bouguer anomalies are in the southern part of the area, and the highest values are in the western and central part of the area. Between these parts exists a zone of high horizontal gravity gradient. Configuration of linear elements of gravity field (gradient zones) delimited the San Marcos Fault in the San Marcos valley below thickness of recent sedimentary cover. Two density models were carried out, which showed that the Cretaceous rocks are in discordant contact with the Paleo- zoic rocks that can be related to the San Marcos Fault. The density was determinate using to Nettleton’s method, which results highlight the presence of the San Marcos Fault. Density models showed that the Quaternary sediments are in direct contact with the San Marcos Fault.

An analysis of light–matter interactions based on symmetries can provide valuable insight,particularly because it reveals which quantities are conserved and which ones can be transformed within a physical system.In this context,helicity can be a useful addition to more commonly considered observables such as angular momentum.The question arises how to treat helicity,the projection of the total angular momentum onto the linear momentum direction,in practical experiments.In this paper,we put forward a simple but versatile experimental treatment of helicity.We then apply the proposed method to the scattering of light by isolated cylindrical nanoapertures in a gold film.This allows us to study the helicity transformation taking place during the interaction of focused light with the nanoapertures.In particular,we observe from the transmitted light that the scaling of the helicity transformed component with the aperture size is very different to the direct helicity component.

The aim of this paper is to offer a statistically sound method to make a precise account of the speed of land degradation and regeneration processes.Most common analyses of land degradation focus instead on the extent of degraded areas,rather than on the intensity of degradation processes.The study was implemented for the Potential Extent of Desertification in China(PEDC),composed by arid,semi-arid,and dry sub-humid regions and refers to the period 2002 to 2012.The metrics were standard partial regression coefficients from stepwise regressions,fitted using Net Primary Productivity as the dependent variable,and year number and aridity as predictors.The results indicate that:①the extension of degrading lands(292896 km 2 or 9.12%of PEDC)overcomes the area that is recovering(194560 km 2 or 6.06%of PEDC);and②the intensity of degrading trends is lower than that of increasing trends in three land cover types(grassland,desert,and crops)and in two aridity levels(semi-arid and dry sub-humid).Such an outcome might pinpoint restoration policies by the Chinese government,and document a possible case of hysteresis.

在当代英国文学领域,许多小说家以战争为题材,探及当代战争背后的政治、社会、文化、心理、道德等方面的问题。这些小说家(如塞巴斯蒂安·福克斯、派特·巴克、杰克·希金斯等)通过对战争历史的深入剖析与重构,呈现了一个个生动的战争历史空间。本文在左伦空间叙事理论观照下,从地形空间、时空体空间和文本空间三个方面分别探讨当代英国战争小说中战争历史叙事的重构、战争灰色地带的书写和创作这些战争小说的小说家们的叙事伦理,力图呈现多维、立体的当代英国战争小说的叙事空间形式。

Physical Activation of Elderly and the Decrease of Falling Risk

A Physical Activation Program was designed and implemented with a particular emphasis in recovering of motor independence, and in an indirect manner;the increase of activities was carried out in a social context in such a way that it allowed isolated elderly to develop their ability to perform daily activities diminishing the burden on their caregivers. 30 people were randomly selected: 15 subjects in the experimental group and 15 in the control group;7 women and 8 men each group. Subjects aged between 60 - 84 years with a mean age of 78 years and a standard deviation of 6.5 were applied in a three-month Physical Activation Program with a 45-min daily sessions. Results obtained showed that physical activity in elderly, systematically improved their amplitude and joint mobility which impact directly and indirectly their quality of life, reducing their risk of falling.

Database belong to the Programa Nacional de Aprovechamiento del Atún y Protección de los Delfines (PNAAPD) from México. The area covers from latitude 5 to 20 N and longitude 90 to 125 W, Eastern Pacific Ocean (EPO), for 1998, 2001, 2002, 2005 and 2006 includes date, geographical position, sea surface temperature, tuna’s catch, yellowfin tuna’s size and dolphin’s number. 5 degress squares of latitude and longitude, each year were made. Yellowfin tuna number was normally distributed (KSd = 0.02, P > 0.20). The problem to solve is the relationship of the spotted dolphin and yellowfin tuna, that may analyze in the background of a non linear generalized model. Considering the array of latitude and longitude over the years that the abundance of yellowfin tuna is a dependent variable and the spotted dolphin abundance is the covariance continuous variable, the interaction and covariance, indicates a significant contribution to variability. The tuna abundance index does produce significative differences in latitude and longitude. Considering the dolphin as a dependent variable and the yellowfin tuna as a covariance continuous variable, the dolphin abundance index produced significative differences in latitude and longitude.

Conservative management of a recurrent puerperal uterine inversion with bakri<sup>®</sup>balloon tamponade

Puerperal Uterine Inversion (PUI) is a rare but potentially life-threatening delivery complication in which the uterine fundus collapses within the endometrial cavity. This “glove-finger” introflexion of uterine walls generally occurs as an immediate postpartum complication and is responsible of different degrees of vaginal bleeding, shock and hypogastric pain that can cause serious maternal complications, including death. There are few reports of recurrent postpartum uterine inversion like the one we present here, and its causes remain unclear. Early diagnosis of this complication is crucial as it is the only one measure that can allow a successful and conservative treatment: an inverse relationship between the time that uterus keeps inverted and the probability of repositioning has been firmly established. This case report describes the exceptional and innovative use of the SOS Bakri? balloon (Cook Medical Incorporated) in the management of a recurrent puerperal uterine inversion. To our knowledge it is one of the first reports in the world of this procedure, perhaps the second one after Soleymani’s et al description;and the first one in a third degree recurrent puerperal uterine inversion.
