Dynamic Cerebral Microdialysis during Pallidotomy and Thalamotomy in Parkinson’s Disease: A Preliminary Neurochemical Study

In Parkinson’s disease (PD), dopaminergic neurons reduce the regulation of glutamatergic (glutamate-Glu) input from the cortex to neostriatum (caudate and putamen nuclei) consequently leading to a hyperactivity of globus pallidus internae (GPi) neurons that release gamma-amino-butyric acid (GABA) into the thalamic ventrolateral (VL) nucleus. The objective of the present experiment was to measure changes in GABA and Glu in the caudate and the thalamus of 2 patients during the application of electrical stimuli following either a pallidotomy or a thalamotomy. Proper insertion of the electrode was tested by applying high frequency electrical pulses (HfEP). During these procedures, we obtained neurochemical information placing cerebral (CMD) microdialysis probes in caudate nucleus and VL nucleus of ipsi- and contra-lateral thalamus. In VL thalamus, extracellular GABA decreased during HfEP, tending to reach previous levels once HfEP was finalized. following the pallido- or thalamotomy GABA decreased again. Similarly, in the contralateral VL thalamus, extracellular GABA levels showed a similar but less pronounced profile but did not show any decrement after the lesion. Caudate Glu decreases when HfEP is applied to the GPi and recovers to previous levels after HfEP, but did not decrease again after lesion (GPi-tomy), instead it continued to rise. These results suggest that HfEP exerts a similar but reversible biochemical effect as thermopallido- or thermothalamotomy on GABA extracellular concentration in the ipsilateral VL thalamus. We also observe a distant effect of HfEP, but not of thermolesion, on contralateral thalamic GABA and ipsilateral caudate Glu.

In order to address the study of complex systems,the detection of patterns in their dynamics could play a key role in understanding their *** particular,global patterns are required to detect emergent concepts and trends,some of them of a qualitative *** concept analysis(fCA) is a theory whose goal is to discover and extract knowledge from qualitative data(organized in concept lattices).In complex environments,such as sport competitions,the large amount of information currently available turns concept lattices into complex *** authors analyze how to apply fCA reasoning in order to increase confidence in sports predictions by means of detecting regularities from data through the management of intuitive and natural attributes extracted from publicly available *** complexity of concept lattices-considered as networks with complex topological structure- is *** is applied to building a knowledge based system for confidence-based reasoning,which simulates how humans tend to avoid the complexity of concept networks by means of bounded reasoning skills.

The USLE soil erodibility nomograph revisited

Helicobacter pylori recurrence after eradication in Latin America:Implications for gastric cancer prevention

To estimate Helicobacter pylori (H. pylori) recurrence rate in Latin America, a region with a significant H. pylori prevalence and gastric cancer burden. METHODSPubMed, LILACS, SciELO, Cochrane databases and abstracts from relevant meetings were reviewed. Information collected included: Participants’ characteristics, recruitment strategy, diagnostic modality, treatment arms, follow-up and recurrence rates. Recurrence was calculated using 100-patients-year rates, and data were pooled using a random effects model. The I2 statistic assessed between study heterogeneity. Meta-regression analyses evaluated for effect modifying variables. RESULTSLiterature search yielded 163 articles. Twelve studies involving 4848 patients from 9 countries met inclusion criteria. four hundred and thirty-two reinfections were recorded in 5487 person-years of follow-up. Pooled analysis showed a recurrence rate of 7.9 cases per 100 person-years (95%CI: 5.3-10.5). Meta-regression revealed that neither the antibiotic schema, a second antibiotic course, nor the diagnostic modality had an impact on the observed risk of recurrence. The recurrence rate in the first year after treatment, predominantly recrudescence, was 11.2 (6.1-16.4) per 100 patient years. Recurrence in subsequent years, was only 6.2 (3.8-8.7). CONCLUSIONH. pylori recurrence rates in Latin America are significant, and with geographic variability, yet are acceptable based upon the current literature for consideration of large scale intervention trials. further research in Latin America is warranted to evaluate the efficacy, cost-effectiveness, and potential adverse outcomes of proposed eradication programs.

Injuries in Spanish female soccer players

Background: Epidemiologic research to learn the incidence, type, location, and severity of female soccer injuries and the risk factors for sustaining a sport injury is the first step in developing preventive policies. The aim of this study was to analyze the incidence of injuries in the population of female soccer players in ***: The injuries incurred by 25,397 female soccer players were registered by the medical staff of the Spanish football federation during 1 season. A standardized medical questionnaire was used to classify the injury according to type, severity, location, and injury mechanism. A total of 2108 injuries was reported with an incidence of 0.083 injuries per player per season. Most injuries were in the lower limbs(74.0%), mainly affecting knee(30.4%) and ankle joints(17.9%).Results: The proportion of injuries derived from contact with another player was higher during matches(33.7%) than during training(11.4%;p fied as severe more frequently than were contact injuries(51.0% vs. 42.6%; p f injury was found in adult soccer players(≥18 years) vs. their counterparts younger than 18 years(0.094 vs. 0.072 injuries per player per year, respectively; p fferences between age groups in any other injury variable(e.g., type, mechanism, location, or severity; p > 0.05).Conclusion: Most female soccer injuries were located at the knee and ankle; the injury mechanism determined the playing time lost; and the player's age did not affect injury characteristics.

Parameter Control of Genetic Algorithms by Learning and Simulation of Bayesian Networks——A Case Study for the Optimal Ordering of Tables

Parameter setting for evolutionary algorithms is still an important issue in evolutionary computation. There are two main approaches to parameter setting： parameter tuning and parameter control. In this paper, we introduce self-adaptive parameter control of a genetic algorithm based on Bayesian network learning and simulation. The nodes of this Bayesian network are genetic algorithm parameters to be controlled. Its structure captures probabilistie conditional （in）dependence relationships between the parameters. They are learned from the best individuals, i.e., the best configurations of the genetic algorithm. Individuals are evaluated by running the genetic algorithm for the respective parameter configuration. Since all these runs are time-consuming tasks, each genetic algorithm uses a small-sized population and is stopped before convergence. In this way promising individuals should not be lost. Experiments with an optimal search problem for simultaneous row and column orderings yield the same optima as state-of-the-art methods but with a sharp reduction in computational time. Moreover, our approach can cope with as yet unsolved high-dimensional problems.

Analysis of doping control test results in individual and team sports from 2003 to 2015

Intracerebral Microdialysis in Neurosurgery for Obsessive-Compulsive Disorder: Report of 2 Preliminary Cases

Neurosurgery for psychiatric disorders, notably for obsessive-compulsive disorder (OCD), was initiated in Venezuela in the decade of 1970, and consisted since that time in the classic stereotactic anterior cingulotomy. In order to know further about the physiopathology of this disorder, we performed intracerebral microdialysis in 2 patients who were operated on. The aim was to measure changes in extracellular neurotransmitters within the basal ganglia. The microdialysis probes were stereotactically placed in the right caudate nucleus and in the dorsomedial nucleus of the right thalamus. The microdialysis was done before the left cingulotomy, during the pause and after the right cingulotomy. Glutamate and gamma-aminobutyric acid (GABA) changes were similar in the caudate nucleus of both patients, whereas in the dorsomedial nucleus the changes were opposite among the 2 patients. Although this study does not bring enough data to explain such differences yet, the existence of dynamic changes in the neurochemistry of the basal ganglia during cingulotomy shows that intracerebral microdialysis can help in the understanding of the pathophysiology of OCD and eventually in the design of new surgeries with better results.
