Cancer cell growth is associated with immune surveillance ***,restoring the desired immune response against cancer cells remains a major therapeutic *** to the recent advances in biological knowledge,efficient therapeutic tools have been developed to support the best bio-clinical approaches for immune precision *** of the most important successes in immune therapy is represented by the applicational use of monoclonal antibodies,particularly the use of rituximab for B-cell lymphoproliferative *** recently,other monoclonal antibodies have been developed,to inhibit immune checkpoints within the tumor microenvironment that limit immune suppression,or to enhance some immune functions with immune adjuvants through different targets such as Toll-receptor *** aim is to inhibit cancer proliferation by the diminishing/elimination of cancer residual cells and clinically improving the response duration with no or few adverse *** effect is supported by enhancing the number,functions,and activity of the immune effector cells,including the natural killer(NK)lymphocytes,NKT-lymphocytes,γδT-lymphocytes,cytotoxic T-lymphocytes,directly or indirectly through vaccines particularly with neoantigens,and by lowering the functions of the immune suppressive *** these new therapeutics and their personalized usage,new considerations have to be taken into account,such as epigenetic regulation particularly from microbiota,evaluation of transversal functions,particularly cellular metabolism,and consideration to the clinical consequences at the body *** aim of this review is to discuss some practical aspects of immune therapy,giving to clinicians the concept of immune effector cells balancing between control and *** precision medicine is a combination of modern biological knowledge and clinical therapeutic decisions in a global vision of the patient.

Contribution of Geographic Information Systems in Protection Zones Delineation around a Surface Water Resource in Adzope Region (Southeast of Côte d’Ivoire)

Adzope’s Municipal Artificial Lake which serves as a drinking water supply to the population is prone to pollution due to human activities and increasing urbanization. Thus, it is essential to protect this surface water supply for sustainable use. The objective of this study is to determine the protection zones around this water reservoir in order to preserve the stored water quality. To achieve this, a methodological approach based on a Geographic Information System (GIS) was adopted. This methodological approach first consisted to assess the water reservoir vulnerability to pollution according to certain hydrological parameters (slope, land use, soil type, runoff, and drainage network density) and then to determine the protection zones according to the different vulnerability classes. After the scoring of different parameters, a weight was assigned to each, from the multi-criteria method (AHP). The combination of the weighted parameters in the GIS enabled to establish the water reservoir vulnerability indexes map. The vulnerability map established subsequently presents five (5) vulnerability classes in the watershed: very low (12%) and low (23%) in the North and South of the study area, moderate (32%) disseminated throughout the study area, high (20%) and very high (13%) particularly in the West and North of the water reservoir and in the East of study area. This water reservoir vulnerability map is potentially influenced by the land use parameter according to the sensitivity analysis test. The different vulnerability classes then allowed the delimitation of three protection zones (Zone 1, Zone 2 and Zone 3) around the water reservoir. The immediate protection zone (Zone 1) has a width between 100 and 450 m around the water reservoir with a surface of 1.16 km2. The delimitation of this immediate protection zone takes into account the high and very high vulnerability classes near the water reservoir. Cartography for the Aquifer Potential Recharge Areas of the Base of the Department of Aboisso(South-East Cote d’Ivoire)

The study highlights the potential recharge areas of the aquifers of the Department of Aboisso for an adequate and rational management of their use. The methodology used consisted in the identification of parameters (land use, soil types, drainage density, slope, induced permeability and alteration thickness) allowing infiltration and their expression as thematic maps. A weighting of these parameters was made by the multicriteria analysis method of Saaty. The integration to GIS conducted to elaborate the map of the potential recharge area of the aquifers for the Department of Aboisso. It brings to light five recharge level classes (bad: 20%;mediocre: 19%, medium: 45%;good: 15% and very good: 1%) in the study area. That map reveals the dominance of good recharge areas covering 61% and practically all over the studied area. They are generally characterized with gentle slopes coupled with mosaics of cultivations and forests, and high induced permeability allowing better infiltration. The zones of low recharge cover 39% of the department’s area, and are mainly located in urban areas inside the streams. This study exhibited that the land use is the preponderant parameter influencing the recharge in base area. This map could be considered as an excellent support for all kinds of activities especially to implement planning in the Department of Aboisso, the setting up of drilling broadbands and the choice of future sites for landfills or landfill of wastes to protect groundwater from any pollution.

Objective: In the context of increasing cases despite vaccination campaigns, a survey was conducted in the Bangui population from January 17 to 26, 2022, to evaluate the strains of Severe Acute Respiratory Infection Coronavirus 2 (SARS-Cov-2) circulating in a healthy population. Materials and methods: This study was conducted by taking nasopharyngeal samples from randomly selected volunteers. Antigen detection was performed systematically, and RT-PCR was done on the positive samples. Results and discussion: We collected 2,554 samples. Thirty were found RT-PCR positive (1.2%) and sent for viral genome sequencing. Twenty-eight SARS-Cov-2 strains belonged to the Omicron type, and only 2 to the Delta type. Conclusion: Thus, infections were uncommon in the tested population, but the presence of Omicron and Delta types raises concerns that vaccination may not be effective in fighting the virus, and newly designed vaccines should be implemented to better protect the population at risk of infection and reinfection by these variants.

Patient education improves adherence to peg-interferon and ribavirin in chronic genotype 2 or 3 hepatitis C virus infection:A prospective,real-life,observational study

AIM： To evaluate the impact of therapeutic education on adherence to antiviral treatment and sustained virological response （SVR） in a real-life setting in genotype 2/3 hepatitis C, as there are few adherence data in genotype 2/3 infection, even from randomized trials. METHODS： This prospective survey included genotype 2/3 patients who received peg-interferon alfa-2b and ribavirin. There was no intervention. Adherence wasself-reported over the past 4 wk （peg-interferon） or 7 d （ribavirin）. Adherence to bitherapy was defined as adherence to the two drugs for ≥ 20 wk. SVR was defined as undetectable RNA ≥ 12 wk after the end of treatment. RESULTS： 370/674 patients received education during the first 3 mo of treatment. After 6 too, adherence to bitherapy was higher in educated patients （61% vs 47%, P = 0.01）. Adherence to peg-interferon was 78% vs 69% （P=0.06）. Adherence to ribavirin was 70% vs 56% （P = 0.006）. The SVR （77% vs 70%, P = 0.05） and relapse （10% vs 16%, P = 0.09） rates tended to be improved. After adjustment for baseline differences, education improved adherence [Odds ratio （OR） 1.58, P = 0.04] but not the SVR （OR 1.54, P = 0.06）. CONCLUSION： In genotype 2/3 patients, therapeutic education helped maintain real-life adherence to bitherapy.

The nuclear factor kappa B (NF-κB) transcriptionfactor coordinates several aspects of innate and adaptiveimmunity, inflammation, cell survival and proliferation [1,2]. Dysregulation of the NF-κB pathway has been associ-ated with cancer development and progression as well aswith other human diseases including viral infections anda number of inflammatory diseases [1]. NF-κB is formedthrough the dimerization of 5 subunits, including membersof the Rel protein family, namely RelA (p65), c-Rel, RelB,NF-κB1 (p50 and its precursor p105) and NF-κB2 (p52and its precursor p 100). Two pathways are involved in theactivation of NF-κB. In both these pathways, kinase activityand proteasome activity play a major role in freeing NF-κBfrom cytoplasmic inhibitors to promote its nuclear translo-cation and activation. The classical or canonical pathwayis activated by a large range of stimuli (microbial and viralinfections, proinflammatory cytokines). In this pathway,NF-κB dimers, mostly p50/RelA and p50/c-Rel, are usuallyretained in the cytoplasm by their interaction with specificinhibitors, called IκBs. Intraocular complications of IFN-α and ribavirin therapy in patients with chronic viral hepatitis C

We report a panel of severe inflammatory and vascular intraocular disorders occurring during interferon-alpha （IFN-α） treatment in eight hepatitis C virus （HCV）- infected patients. These events include three cases of Vogt-Koyanagi-Harada like （VKH） disease （an association of panuveitis, retinal detachment, ear and meningeal detachment and skin and hair changes）, two cases of central retinal vein occlusion, one case of central retinal artery occlusion, one case of severe hypertensive retinopathy and one case of bilateral ischemic optic neuropathy with severe visual impairment. Rare as they are, such severe ophthalmological complications require a close follow-up of HCV-infected patients under IFN-α breabnent with ophthalmological monitoring if any ocular manifestation occurs.

Nystatin-Neomycin-Polymyxin B Combination:Efficacy and Tolerance as 1st-Line Local Treatment of Infectious Vaginitis

Objective: To evaluate the efficacy and tolerance of a local treatment combining two antibacterials and one antifungal in patients with a clinical presentation suggesting infectious vaginitis. Patients and methods: 169 patients presenting with clinical criteria for vaginitis were included in an open, multicenter trial. Vaginal samples were taken for microbiological analyses and a triple-combination product of nystatin, neomycin and polymyxin B was then started as local treatment, without waiting for the test results. The treatment was continued with the usual dosage (1 vaginal capsule at bedtime for 12 days) for vaginal infections in the scope of the combination product with approved labeling. A second vaginal sample was performed at the end of the treatment. The main efficacy criterion was the clinical success rate (cure or improvement of the clinical signs and symptoms) according to the ***: 93 patients were included in the efficacy population. Non-exclusively fungal vaginitis (strictly bacterial or bacterial + fungal) represented 31.2% of the cases. The clinical success rate was 97.8% according to the investigator and 95.7% according to the patients. The microbiological success rate was 81.3%, with no differences between etiologies (Candida spp., bacteria or both). The combination product was well-tolerated, despite the local inflammation before treatment. Discussion and conclusion: Given the etiological diversity of vaginitis, this trial supports the efficacy of a triple-combination product (nystatin, neomycin, polymyxin B) as a first-line local treatment of Candida, bacterial or mixed vaginitis.

AIM To compare the performances of the Barcelona clinic liver cancer(BCLC) nomogram and others systems(BCLC, HKLC, CLIP, NIACE) for survival prediction in a large hepatocellular carcinoma(HCC) French *** Data were collected retrospectively from 01/2007 to 12/2013 in five French centers. Newly diagnosed HCC patients were analyzed. The discriminatory ability, homogeneity ability, prognostic stratification ability Akaike information criterion(AIC) and C-index were compared among scoring systems. RESULTS The cohort included 1102 patients, mostly men, median age 68 [60-74] years with cirrhosis(81%), child-Pugh A(73%), alcohol-related(41%), HCV-related(27%). HCC were multinodular(59%) and vascular invasion was present in 41% of cases. At time of HCC diagnosis BCLC stages were A(17%), B(16%), C(60%) and D(7%). First line HCC treatment was curative in 23.5%, palliative in 59.5%, BSC in 17% of our population. Median OS was 10.8 mo [4.9-28.0]. Each system distinguished different survival prognosis groups(P < 0.0001). The nomogram had the highest discriminatory ability, the highest C-index value. NIACE score had the lowest AIC value. The nomogram distinguished sixteen different prognosis groups. By classifying unifocal large HCC into tumor burden 1, the nomogram was less powerful. CONCLUSION In this French cohort, the BCLC nomogram and the NIACE score provided the best prognostic information, but the NIACE could even help treatment strategies.

Revisiting Akaike’s Final Prediction Error and the Generalized Cross Validation Criteria in Regression from the Same Perspective: From Least Squares to Ridge Regression and Smoothing Splines

In regression, despite being both aimed at estimating the Mean Squared Prediction Error (MSPE), Akaike’s Final Prediction Error (FPE) and the Generalized Cross Validation (GCV) selection criteria are usually derived from two quite different perspectives. Here, settling on the most commonly accepted definition of the MSPE as the expectation of the squared prediction error loss, we provide theoretical expressions for it, valid for any linear model (LM) fitter, be it under random or non random designs. Specializing these MSPE expressions for each of them, we are able to derive closed formulas of the MSPE for some of the most popular LM fitters: Ordinary Least Squares (OLS), with or without a full column rank design matrix;Ordinary and Generalized Ridge regression, the latter embedding smoothing splines fitting. For each of these LM fitters, we then deduce a computable estimate of the MSPE which turns out to coincide with Akaike’s FPE. Using a slight variation, we similarly get a class of MSPE estimates coinciding with the classical GCV formula for those same LM fitters.
