Energy valorization of organic waste material is nowadays an assessed practice of circular *** heat and power(CHP)technologies based on biomass gasification represent viable substitutes to traditional energy conversion units based on combustion,whose need has recently experienced a huge growth,due to the increasing concerns about the release of greenhouse gases(GHGs)emissions and the related effects on climate *** present,only a few solutions have yet achieved a level of full development for *** of them is the system developed by CMD,the CMD ECO20,made of a gasifier,a syngas cleaning system and a spark ignition internal combustion engine working as a *** the present work,a numerical model is developed to study this system into detail and search for optimal controlling *** simulation relies on a combined use of the Thermoflex?environment and a proper one-dimensional(1D)model of the engine module built within GT-suite?.An original contribution is given to the turbulent combustion model that accounts for the laminar flame speed of the specific *** numerical model,that covers the entire biomass-to-energy conversion process,is validated under real operative *** final purpose of the work is the optimization of input parameters,as the initial biomass moisture content,the equivalence ratio at the gasifier or the timing of spark advance,to maximize the system electrical energy output.

The administration of oral raloxifene in 30 postmenopausal hypertensive women was evaluated to demonstrate its effect on arterial stiffness. Casual and ambula tory blood pressure (BP)-and pulse-wave velocity(PWV) data were obtained befor e and after patients received raloxifene, estrogen-progestin replacement therap y, or placebo in a randomized crossover study. It was shown that the 2 therapies decreased BP and carotid-femo-ral PWV, and the effect of raloxifene on vascul ar compliance was independent of the effects on BP.

Pyoderma gangrenosum (PG) is a neutrophilic dermatosis of unknown etiology characterized by typical skin ulcers. It may be related to systemic disorders but its association with solid tumors is very unusual. In this setting, we describe a patient in whom PG was the first and isolated manifestation of advanced gastric adenocarcinoma.

Histopathological Features of Dental Pulp Tissue from Bleached Mandibular Incisors

Mandibular incisors scheduled for extraction were subjected or not to three 15-minute applications of a 35% hydrogen peroxide gel. Two days thereafter, bleaching effectiveness (according to a value-oriented shade guide) was evaluated, and histological analysis of pulp tissue was performed. Immediately and two days after bleaching, tooth sensitivity (Ts) experience was recorded. Bleached teeth of both groups showed significant color improvement (Mann-Whitney; p se teeth presented significant pulp alterations when compared with non-bleached teeth (Mann-Whitney; p s of superficial necrosis associated with mild inflammatory reactions. These data were achieved by recordings of Ts immediately after bleaching (Fisher"s exact test; ps reduced with time. Instead of "no significant difference" being observed between young and old teeth (Mann-Whitney; psis occurred in 60% of old bleached teeth in comparison with 100% of young teeth, associated with underlying tertiary dentin deposition. It was concluded that, regardless of the age of patients, the application of a 35%-HP bleaching gel to mandibular incisors causes pulp damage, which was related to initial Ts; however, superficial necrosis was more prevalent in young teeth.

Cytomegalovirus (CMV) and Pneumocystis jirovecii fungus are the main opportunistic microorganisms that affect transplanted individuals. Immunosuppressive drugs administered to prevent organ rejection leave the immune system vulnerable to these infections. The present report is about a kidney transplanted patient using immunosuppressants who was diagnosed with cytomegalovirus and pneumocystosis requiring admission to the intensive care unit (ICU). Female patient, 57 years old, a kidney transplanted three years ago, with comorbidities, such as systemic arterial hypertension, hypertriglyceridemia and type 2 diabetes mellitus. she was admitted to the hospital in January 2020 with a history of diarrhea, cough, malaise and weight loss of seven kg in a month. she made continuous use of the immunosuppressants tacrolimussup>®sup> and mycophenolate sodium (MFs). After five days of hospitalization, she was transferred to the ICU due to refractory diarrhea, worsening renal function and respiratory pattern, requiring mechanical ventilation. Chest tomography showed changes that led to the diagnostic hypothesis of CMV pneumonia or Pneumocystis jirovecii. Treatment with Ganciclovirsup>®sup> and Bactrimsup>®sup> was started. The bronchial lavage polymerase chain reaction test confirmed the infectious condition for CMV and Pneumocystis jirovecii. Despite the drug therapy instituted, there was no improvement in the infectious condition. The patient started to present a general and progressive worsening of the clinical picture with loss of renal graft function, respiratory failure, metabolic acidosis, hemodynamic instability and severe distributive shock, evolving to death. In the present report, it was observed that after late kidney transplantation the fragility of the immune system caused by the use of immunosuppressants contributed to the development of a severe infection with CMV and Pneumocystis jirovecii. Farmers＇ Perceived Impact of Fair Trade： The Case of costa Rica

The perceived usefulness of Fair Trade influences both its effectiveness and farmers＇ long-term participation. The aim of this paper is to measure the perceived economic, social and environmental impact of Fair Trade by farmers in costa Rica. One hundred farmers were interviewed, and their perceived change in living and working conditions due to Fair Trade participation was measured through a t-test analysis. The sample characters＇ influence on the perceived change was also measured, adopting a regression model and a t-test. The results showed a positive perception of the impact of Fair Trade, with a particularly strong perceived improvement in the farmers＇ technical, economic and managerial skills. There was relatively less perceived change in the environmental, educational and sanitary conditions. The results showed the need for Fair Trade to better adjust its strategy to the expectations of the farmers＇ communities.

Helicobacter pylori (H. pylori) is bacteria considered to be present in half of the population and it is a public health problem worldwide. Most patients infected with H. pylori show no clinical symptoms;nonetheless, approximately 10% to 20% of these patients will develop peptic ulcers and 1% will develop gastric cancer. The International Agency for Research on Cancer has classified H. pylori as a Group 1 carcinogen, recognized as the only bacteria capable of producing cancer. samples of drinking water (n = 44) from aqueducts with chlorination treatment in selected areas with high prevalence of gastric cancer were analyzed in costa Rica. samples of drinking water from Panamá (n = 44) from aqueducts supplying untreated water for human consumption in the province of Chiriquí were also analyzed. The molecular marker of H. pylori, glmM, was used, and to optimize the Real Time PCR (qPCR) technique, annealing temperature, concentration of primers and probe were standardized;also, by analyzing different standard curves, the best reaction conditions that allowed detecting and quantifying the gene were determined. The LightCycler® 480 II (LC480II) equipment from Roche Diagnostics GmbH was used, as well as the Absolute Quantification Analysis by means of the second Derivative Maximum Method. In the case of the samples from costa Rica, it was determined that 79.5% were positive for H. pylori;removing outlier high average, quantification of bacteria was determined in 3.6 × 103 copies/100 mL. For Panamá it was determined that 86% of the samples were found positive for the presence of H. pylori;removing outlier high average quantification of bacteria was determined at 3.3 × 102 copies/100 mL. The difference in values between the aqueducts in both countries revealed an environmental distribution of the bacteria of epidemiological interest in each case.

Atrial fibrillation is a cardiac arrhythmia of high prevalence in the population, especially in the elderly. Its main electrical characteristics are the interval between two successive irregular R waves, absence of P waves and presence of f waves between QRs complexes. The most common symptoms of atrial fibrillation are irregular palpitations associated with dyspnea, dizziness, feeling tired, fatigue and general malaise, but not all patients have any symptoms. The present report presents the history of an elderly patient who arrived at the hospital’s emergency department with irregular heart rhythm and palpitations. The patient’s symptoms, associated with the electrocardiogram results, indicated paroxysmal atrial fibrillation. Electrical cardioversion was performed, and after, cardiac ablation via the femoral vein at the hospital’s cardiology service. There were no complications during the procedure. As a routine imaging exam after ablation, control esophagogastroduodenoscopy was requested to verify that there was no formation of atrio-esophageal fistula developed by the invasive ablation procedure and electrocardiogram, which showed normal sinus rhythm. The patient remained in the cardiac intensive care unit for observation for 24 hours. After the electrical cardioversion and catheter ablation procedures, the patient improved his clinical picture of atrial fibrillation and was discharged after 24 hours of hospitalization. He received treatment to perform at home, to reduce acid reflux into the esophagus and to prevent thrombosis. He did not present pulmonary thromboembolism after hospital discharge. It is believed, therefore, that this form of treatment and management of paroxysmal atrial fibrillation is effective for the solution of the proposed problem and can also serve as a reference for other professionals within the cardiology service.
