Background Minimally invasive surgery(MIS)for spontaneous supratentorial intracerebral haemorrhage(ICH)is controversial but may be beneficial if end-of-treatment(EOT)haematoma volume is reduced to≤15 *** explored whether MRI findings of cerebral small vessel disease(CSVD)modify the effect of MIS on long-term *** Prespecified blinded subgroup analysis of 288 subjects with qualified imaging sequences from the phase 3 Minimally Invasive Surgery Plus Alteplase for Intracerebral haemorrhage Evacuation(MISTIE)*** tested for heterogeneity in the effects of MIS and MIS+EOT volume≤15 ml on the trial’s primary outcome of good versus poor function at 1 year by the presence of single CSVD features and CSVD scores using multivariable *** Of 499 patients enrolled in MISTIE III,288 patients had MRI,149(51.7%)randomised to MIS and 139(48.3%)to standard medical care(SMC).Median(IQR)ICH volume was 42(30–53)*** the full MRI cohort,there was no statistically significant heterogeneity in the effects of MIS versus SMC on 1-year outcomes by any specific CSVD feature or by CSVD scores(all P_(interaction)>0.05).In 94 MIS patients with EOT ICH volume≤15 ml,significant reduction in odds of poor outcome was found with cerebral amyloid angiopathy scorelacunes(OR,0.37(0.18–0.80);P_(interaction)=0.02)and absence of severe white matter hyperintensities(WMHs)(OR,0.22(0.08–0.58);P_(interaction)=0.03).Conclusions Following successful haematoma reduction by MIS,we found significantly lower odds of poor functional outcome with lower total burden of CSVD in addition to absence of lacunes and severe *** features may have utility for prognostication and patient selection in clinical trials of MIS.

Viruses are obligate intracellular parasites that subvert cellular metabolism and pathways to mediate their own replication—normally at the expense of the host cell. Polyomaviruses are a group of small DNA viruses, which have long been studied as a model for eukaryotic DNA replication. Polyomaviruses manipulate host replication proteins, as well as proteins involved in DNA maintenance and repair, to serve as essential cofactors for productive infection. Moreover, evidence suggests that polyomavirus infection poses a unique genotoxic threat to the host cell. In response to any source of DNA damage, cells must initiate an effective DNA damage response(DDR) to maintain genomic integrity, wherein two protein kinases, ataxia telangiectasia mutated(ATM) and ATM- and Rad3-related(ATR), are major regulators of DNA damage recognition and repair. Recent investigation suggests that these essential DDR proteins are required for productive polyomavirus infection. This review will focus on polyomaviruses and their interaction with ATMand ATR-mediated DNA damage responses and the effect of this interaction on host genomic stability.

l-Arginine Supplementation Mitigates Dichlorvos-Induced haematocardiotoxicity, and Oxidative Stress in Male Wistar Rats

Due to its toxicity, dichlorvos—a common organophosphate pesticide—poses significant risks to human health. This study utilized male Wistar rats to explore the potential protective effects of l-arginine supplementation against dichlorvos-induced toxicity, focusing on cardiotoxicity, haematotoxicity and oxidative stress. The rats were divided into four groups: Control, l-arginine (l), Dichlorvos (D), and l-arginine + Dichlorvos (l + D). Dichlorvos was administered to the D group, l-arginine (100 mg/kg) to the l group, and both l-arginine and dichlorvos to the l + D group. The study evaluated various parameters, including cardiovascular, oxidative stress markers, and haematological indices. Significant changes in haematological parameters such as haemoglobin (Hb), haematocrit (HCT), and red blood cell count (RBC) indicated haematotoxicity after dichlorvos administration. Additionally, elevated cardiac markers, including lactate dehydrogenase (lDH) and creatine kinase-MB (CK-MB), suggested cardiotoxic effects. Exposure to dichlorvos also resulted in decreased antioxidant enzyme levels and increased oxidative stress indicators like malondialdehyde (MDA). Remarkably, l-arginine supplementation mitigated the damage caused by dichlorvos. It normalized the altered haematological parameters, demonstrating its protective effect against haematotoxicity. The rise in cardiac markers was reduced with l-arginine supplementation, indicating protection against cardiotoxicity. Moreover, l-arginine significantly decreased oxidative stress, as evidenced by lower MDA levels and restored antioxidant enzyme activity. In conclusion, l-arginine supplementation in male Wistar rats showed promising protective effects against dichlorvos-induced cardiotoxicity, haematotoxicity and oxidative stress. This suggests that l-arginine may offer a beneficial intervention to mitigate the adverse effects of dichlorvos on blood and heart health, paving the way for potential treatments for pesticide poison
