End Organ Recovery and Survival with the QuadroxD Oxygenator in Adults on Extracorporeal Membran Oxygenation

Introduction: Extracorporeal Membrane Oxygenation (ECMO) is used in selected patient with cardiogenic and/or re- spiratory shock. We report our experience with standardized management protocols and the application of the Qua- droxD oxygenator with a centrifugal pump to maximize end-organ recovery and improve survival. Methods: This is an Internal Review Board (IRB) approved, single institution retrospective study of end-organ recovery and survival in pa- tients who required ECMO for cardiogenic and/or respiratory shock.between July 2010 and June 2011. Results: Sixteen patients (median age: 46 years) were initiated on either Veno-Arterial (VA) or Veno-Venous (VV) ECMO. Cardiogenic shock. acute respiratory distress syndrome (ARDS) and a combined respiratory and cardiogenic compromise were the primary indications for ECMO in 8 (50%), 5 (31%) and 3 (19%) patients respectively. The median time on ECMO was 8 days (range: 4 - 26 days). Twelve patients (75%) were successfully weaned off ECMO, of which four (25%) were bridged to a ventricular assist device (VAD) and eight (50%) were weaned to recovery. All eight patients (100%) that were weaned to recovery and two patients (50%) that were bridged to a VAD were successfully discharged from the hospital, resulting in a discharge rate of 63%. There was an improvement in pre- vs. post-ECMO AST (449 IU/L vs. 63 IU/L, p Conclusion: ECMO using the QuadroxD oxygenator and a centrifugal pump, coupled with standardized management protocols is beneficial in carefully selected patients. Improvement or main- tenance of end-organ function is associated with successful bridge to device therapy and/or increased survival.

We report in this study the identification of a natural product-lik. antagonist(1a) of Vps34 as a potent autophagy modulator via structure-based virtual screening. Aurone derivative 1a strongly inhibited Vps34 activity in cell-free and cell-based assays. Significantly, 1a prevents autophagy in human cells induced either by starvation or by an mT OR inhibitor. In silico modeling and k.netic data revealed that 1a could function as an ATP-competitive inhibitor of Vps34. Moreover, it suppressed autophagy in vivo and without inducing heart or liver damage in mice. 1a could be utilized as a new motif for more selective and efficacious antagonists of Vps34 for the potential treatment of autophagy-related human diseases.

NaH doped TiO_(2)as a high-performance catalyst for Mg/MgH_(2)cycling stability and room temperature absorption

This paper presents the catalytic effect of NaH doped nanocrystalline TiO_(2)(designated as NaTiOxH)in the improvement of MgH_(2)hydrogen storage *** catalyst preparation involves ball milling NaH with TiO_(2)for 3 *** addition of 5 wt%NaTiOxH powder into MgH_(2)reduces its operating temperature to∼185℃,which is∼110℃lower than the additive-free as-milled MgH_(2).The composite remark.bly desorbs∼7.2 wt%H_(2)within 15 min at∼290℃and reabsorbs∼4.5 wt%H_(2)in 45 min at room temperature under 50 bar H_(2).MgH_(2)dehydrogenation is activated at 57 k./mol by the *** importantly,the addition of 2.5 wt%NaTiOxH catalyst aids MgH_(2)to reversibly produce∼6.1 wt%H_(2)upon 100 cycles within 475 hr at 300℃.Microstructural investigation into the catalyzed MgH_(2)composite reveals a firm contact existing between NaTiOxH and MgH_(2)***,the NaTiOxH catalyst consists of catalytically active Ti_(3)O_(5),and“rod-lik.”Na_(2)Ti_(3)O_(7)species liberated in-situ during preparation;these active species could provide multiple hydrogen diffusion pathways for an improved MgH_(2)sorption ***,the elemental characterization identifies the reduced valence states of titanium(Ti<4+)which show some sort of reversibility consistent with H_(2)insertion and *** phenomenon is believed to enhance the mobility of Mg/MgH_(2)electrons by the creation and elimination of oxygen vacancies in the defective(TiO_(2-x))*** findings have therefore moved MgH_(2)closer to practical applications.

Work.with cereals （barley and wheat） and a legume （Medicago truncatula） has established thioredoxin h （Trx h） as a central regulatory protein of seeds. Trx h acts by reducing disulfide （S-S） groups of diverse seed proteins （storage proteins, enzymes, and enzyme inhibitors）, thereby facilitating germination. Early in vitro protein studies were complemented with experiments in which barley seeds with Trx h overexpressed in the endosperm showed accelerated germination and early or enhanced expression of associated enzymes （α-amylase and pullulanase）. The current study extends the transgenic work.to wheat. Two approaches were followed to alter the expression of Trx h genes in the endosperm：（1） a hordein promoter and its protein body targeting sequence led to overexpression of Trx hS, and （2） an antisense construct of Trx h9 resulted in cytosolic underexpression of that gene （Arabidopsis designation）. Underexpression of Trx h9 led to effects opposite to those observed for overexpression Trx h5 in barley--retardation of germination and delayed or reduced expression of associated enzymes. Similar enzyme changes were observed in developing seeds. The wheat lines with underexpressed Trx showed delayed preharvest sprouting when grown in the greenhouse or field without a decrease in final yield. Wheat with overexpressed Trx h5 showed changes commensurate with earlier in vitro work. increased solubility of disulfide proteins and lower aUergenicity of the gliadin fraction. The results are further evidence that the level of Trx h in cereal endosperm determines fundamental properties as well as potential applications of the seed.

乳腺癌是一种高度异质性肿瘤，这种异质性体现在不同肿瘤类型之间以及同一肿瘤内部之间等多层次的差异，亦表现在传统的组织病理分型／免疫组织化学（ immunohistochemical ， IHC ）特征的分型和近期依据基因表达谱（ gene expression profiling ， GEP）的分子分型上［1］。根据最新版WHO分类，基于不同的生长方式、组织学结构和细胞学形态，乳腺癌可分为21种不同的组织学类型［2］。不同类型的肿瘤具有不同的组织学形态、基因表达及预后特征。使用分子分型可将乳腺癌分为不同的亚型，包括表达雌激素受体（ ER）相关因子的亚型（ luminal型）、表达人类表皮生长因子受体2（ HER2）相关通路因子的亚型（ HER2过表达亚型）和表达基底因子但不表达激素受体通路的亚型（ basal lik. breast cancer ， BLBC ）。在日常应用中，依据GEP分析的分子分型常常被IHC分型所替代。通过IHC分型方法［3］，乳腺癌被划分为激素受体（ ER、PR）阳性组和阴性组，后者根据HER2表达情况进一步分为HER2过表达乳腺癌和三阴性（ ER、PR、HER2阴性）乳腺癌（ triple negative breast carcinoma ， TNBC ）。2013年， St Gallen国际乳腺癌会议专家组再次确认IHC分类法，以乳腺癌IHC分型替代GEP分子分型：luminal A型（ ER ＋， PR≥20％， HER2－， k.-67＜20％）；luminal B型（ ER＋， PR＜20％和／或HER2＋和／或k.-67≥20％）；HER2过表达型（ ER －， PR －， HER2＋）；基底样型（ER－，PR－，HER2－）［4］。尽管这种IHC分类方法便于使用，但与GEP分型并不完全一致［4-5］。
