Influence of indoor airflow on airborne disease transmission in a classroom

It has been widely accepted that the most effective way to mitigate airborne disease transmission in an indoor space is to increase the ventilation airflow,measured in air change per hour(ACH).However,increasing ACH did not effectively prevent the spread of *** better understand the role of ACH and airflow large-scale patterns,a comprehensive fully transient computational fluid dynamics(CFd)simulation of two-phase flows based on a discrete phase model(dPM)was performed in a university classroom setting with people *** investigations encompass various particle sizes,ventilation layouts,and flow *** findings demonstrated that the particle size threshold at which particles are deemed airborne is highly influenced by the background flow strength and large-scale flow pattern,ranging from 5µm to 10µm in the cases *** effects of occupants are significant and must be precisely accounted for in respiratory particle transport *** enhanced ventilation design(UFAd-CdR)for university classrooms is introduced that places a premium on mitigating airborne disease *** to the baseline design at the same ACH,this design successfully reduced the maximum number density of respiratory particles by up to 85%.A novel airflow-related parameter,Horizontality,is introduced to quantify and connect the large-scale airflow pattern with indoor aerosol *** underscores that ACH alone cannot ensure or regulate air *** addition to the necessary ACH for air exchange,minimizing horizontal bulk motion is essential for reducing aerosol transmissibility within the room.

Bile acids,once considered mere dietary surfactants,now emerge as critical modulators of macronutrient(lipid,carbohydrate,protein)metabolism and the systemic pro-inflammatory/anti-inflammatory *** acid metabolism and signaling pathways play a crucial role in protecting against,or if aberrant,inducing cardiometabolic,inflammatory,and neoplastic conditions,strongly influencing health and *** curative treatment exists for any bile acid influenced disease,while the most promising and well-developed bile acid therapeutic was recently rejected by the ***,we provide a bottom-up approach on bile acids,mechanistically explaining their biochemistry,physiology,and pharmacology at canonical and non-canonical *** this mechanistic model of bile acids,we explain how abnormal bile acid physiology drives disease pathogenesis,emphasizing how ceramide synthesis may serve as a unifying pathogenic feature for cardiometabolic *** provide an in-depth summary on pre-existing bile acid receptor modulators,explain their shortcomings,and propose solutions for how they may be ***,we rationalize novel targets for further translational drug discovery and provide future *** than dismissing bile acid therapeutics due to recent setbacks,we believe that there is immense clinical potential and a high likelihood for the future success of bile acid therapeutics.

Over the past two decades,the USA has witnessed a steep rise in the incidence of acute cholecystitis(AC),[1]even among children,[2]with over 200,000 cases annually.[3]Patients with AC often present to the emergency department(Ed)experiencing severe distress,with most describing their pain level as“intolerable.”[4]Furthermore,there is usually a considerable delay before surgery,with a median time from diagnosis of AC to surgery of 28.5 h,[5]and a substantial percentage of patients wait as long as 10 d.[6]Prolonged delays before surgery lead to increased opioid use,which is associated with extended hospital stays and higher rates of readmission after cholecystectomy.[7]Furthermore,opioids can lead to vomiting,respiratory depression,delirium,and ultimately addiction.[8]We propose a novel regional anesthesia technique for managing AC:right phrenic nerve *** method could offer improved pain control and a more favorable risk profile in selected patients when compared to current Ed practices.

Background Minimally invasive surgery(MIS)for spontaneous supratentorial intracerebral haemorrhage(ICH)is controversial but may be beneficial if end-of-treatment(EOT)haematoma volume is reduced to≤15 *** explored whether MRI findings of cerebral small vessel disease(CSVd)modify the effect of MIS on long-term *** Prespecified blinded subgroup analysis of 288 subjects with qualified imaging sequences from the phase 3 Minimally Invasive Surgery Plus Alteplase for Intracerebral Haemorrhage Evacuation(MISTIE)*** tested for heterogeneity in the effects of MIS and MIS+EOT volume≤15 mL on the trial’s primary outcome of good versus poor function at 1 year by the presence of single CSVd features and CSVd scores using multivariable *** Of 499 patients enrolled in MISTIE III,288 patients had MRI,149(51.7%)randomised to MIS and 139(48.3%)to standard medical care(SMC).Median(IQR)ICH volume was 42(30–53)*** the full MRI cohort,there was no statistically significant heterogeneity in the effects of MIS versus SMC on 1-year outcomes by any specific CSVd feature or by CSVd scores(all P_(interaction)>0.05).In 94 MIS patients with EOT ICH volume≤15 mL,significant reduction in odds of poor outcome was found with cerebral amyloid angiopathy scored absence of severe white matter hyperintensities(WMHs)(OR,0.22(0.08–0.58);P_(interaction)=0.03).Conclusions Following successful haematoma reduction by MIS,we found significantly lower odds of poor functional outcome with lower total burden of CSVd in addition to absence of lacunes and severe *** features may have utility for prognostication and patient selection in clinical trials of MIS.

decreased mechanical loading after orthopaedic surgery predisposes patients to develop muscle *** purpose of this review was to assess whether the evidence supports oral protein supplementation can help decrease postoperative muscle atrophy and/or improve patient outcomes following orthopaedic surgery.A systematic review was conducted according to the Preferred Reporting Items for Systematic Reviews and MetaAnalysis(PRISMA).PubMed(MEdLINE),Embase,Scopus,and Web of Science were searched for randomized controlled trials that assessed protein or amino acid supplementation in patients undergoing orthopaedic *** investigators independently conducted the search using relevant Boolean *** outcomes included functional or physiologic measures of muscle atrophy or *** studies including 611 patients(224 males,387 females)were *** studies evaluated protein supplementation after ACL reconstruction(ACLR),3 after total hip arthroplasty(THA),5 after total knee arthroplasty(TKA),and 3 after surgical treatment of hip *** supplementation showed beneficial effects across all types of *** primary benefit was a decrease in muscle atrophy compared to placebo as measured by muscle cross sectional *** authors also demonstrated improved functional measures and quicker achievement of rehabilitation *** supplementation has beneficial effects on mitigating muscle atrophy in the postoperative period following ACLR,THA,TKA,and surgical treatment of hip *** effects often correlate with improved functional measures and quicker achievement of rehabilitation *** research is needed to evaluate long-term effects of protein supplementation and to establish standardized population-specific regimens that maximize treatment efficacy in the postoperative period.

Nature provides a wealth of bio-inspiration for advanced material *** various nanomaterials into biomimetic microtextures with bioinspired functionalities has spurred increasing research interests and facilitated technological advances in various *** recent years,two-dimensional materials(2dMs)have emerged as important building block units in the biomimicry field due to their distinct chemical,physical,electrical,electrochemical,and catalytic *** this review article,various mechanically driven assembly approaches are summarized to fabricate various genealogies of biomimetic 2dM microtextures with bio-inspired ***,sequential deformation strategies are discussed to programmably construct higher dimensional 2dM microtextures,ranging from wrinkles/crumples(one-time deformation)to multiscale hierarchies(multiple deformations).Next,the current progress using higher dimensional 2dM microtextures to imitate different biological structures and/or induce bio-inspired multifunctionality is systematically *** showcases of bio-inspiration and biomimicry using different 2dM nanosheets are highlighted:(1)wrinkle patterns of an earthworm that spur the design of strain sensors with programmable working ranges and sensitivities,(2)wrinkle appearance of a Shar-Pei dog that motivates the fabrication of stretchable energy storage devices,(3)hierarchical scale textures of a desert lizard that inspire cation-induced gelation platforms for 2dM aerogels,and(4)wrinkle skin of an elephant that influences the development of 2dM protective skin for soft ***,challenges and future opportunities of adopting 2dM nanosheets to assemble biomimetic microstructures with synergistic functionalities are discussed.
