Digital radiograph(DR)guided bedside IVC filter placements in patients with intracranial pressure monitors

Purpose:The purpose of this study is to report a single center experience with portable digital radiographically(DR)guided bedside IVC filters placed in intensive care unit(ICU)patients with high ICP and elevated head of bed(HOB).Materials and methods:A retrospective chart review was conducted on all bedside IVC filters placed from January 1,2010 to september 16,*** with high ICP and elevated head of bed requirements were *** were reviewed for filter type,common femoral vein(CFV)access,filter location,pre procedure imaging,pre and post filter ICPs,glascow coma scale,number of radiographs taken,and filter *** were obtained 1 h prior to procedure and 2 h post procedure and analyzed with a paired T *** were placed by reviewing prior CT scan for IVC size,caval variants,renal and iliac veins and vertebral body ***,CFV access was obtained and a Bentson wire was advanced 30-40 cm.A radiograph was used to confirm adequate position of the of the *** filter sheath was advanced and serial radiographs were used to position the filter sheath at the final predetermined position below the renal veins and above the iliac *** filter was deployed,and a radiograph was obtained to confirm filter ***:A total of 9 DR guided bedside IVC filters were placed(4 Denali,3 Option Elite,2 Celect).Indications included prophylactic placement(n=8)and acute DVT(n=1).The average patient age was 35.8 years(range:18-56 years)CT abdomen and pelvis was used to assess for the level of renal veins in all patients(n=9).No caval variants were encountered on pre-procedural *** average pre,intraprocedural,and post procedure intracranial pressure was 16 mmHg,13 mmHg,and 16 mmHg,*** of placement after final placement was available in 7 patients(4 DR,2 CT and one fluoroscopic examination).Two non-procedural related deaths *** success,defined as successful placement of IVC filter at the

The present study enumerates the diversity of corticolous lichens from Kokrajhar district of Assam *** study records 54 species of lichens belonging to 21 genera and 14 *** of the species are crustose(44 species)and 10 are *** of the species,Astrothelium alboverrucoides is a new record to India while five species viz.,Graphis sayeri,Hemithecium croceum,Malmidea piperis,Ocellularia allosporoides and Porina belanospora are new additions to the lichen biota of *** the species,nine are endemic to *** is the dominant family and Dirinaria consimilis is frequently found in all the study *** indicates its scope for future applications in other studies related to lichens in the region.

AIM:To examine whether acupuncture can prevent prolonged postoperative ileus(PPOI)after intraperitoneal surgery for colon cancer. METHODs:Ninety patients were recruited from the Fudan University Cancer Hospital,shanghai,China. After surgery,patients were randomized to receive acupuncture(once daily,starting on postoperative day 1, for up to six consecutive days)or usual *** was defined as an inability to pass flatus or have a bowel movement by 96 h after *** main outcomes were time to first flatus,time to first bowel movement, and *** outcomes were quality of life(QOL)measures,including pain, nausea,insomnia,abdominal distension/fullness,and sense of well-being. REsULTs:No significant differences in PPOI on day 4 (P=0.71)or QOL measures were found between the *** were also no group differences when the data were analyzed by examining those whose PPOI had resolved by day 5(P=0.69)or day 6(P= 0.88).No adverse events related to acupuncture were reported. CONCLUsION:Acupuncture did not prevent PPOI andwas not useful for treating PPOI once it had developed in this population.

Inflammasomes are multi-protein complexes that regulate the innate immune response by facilitating the release of inflammatory cytokines in response to pathogen exposure or cellular damage. Pro-inflammatory inflammasome signaling is vital to host defense and helps initiate the process of tissue repair following an insult to the host, but can be injurious, when excessive or chronic. As such, inflammasome activity is tightly regulated. Here we discuss one critical mechanism of inflammasome regulation, ubiquitination, that functions as a universal modulator of protein stability and trafficking. Recent studies have provided important insights into the regulation of inflammasome activation by protein ubiquitination. We review the molecular regulation of inflammasome function, specifically, as it relates to ubiquitination, and discuss the implications for the development of theraoeutics to soecificallv target aberrant inflammasome signaling.

Water transport through subnanopores in the ultimate size limit: Mechanism from molecular dynamics

Ab initio and classical molecular dynamics simulations show that water can flow through graphdiyne—an experimentally fabricated graphene-like membrane with highly dense (2.4 × 10^18 pores/m^2), uniformly ordered, subnanometer pores (incircle diameter 0.57 nm and van der Waals area 0.06 nm^2). Water transports through subnanopores via a chemical-reaction-like activated process. The activated water flow can be precisely controlled through fine adjustment of working temperature and pressure. In contrast to a linear dependence on pressure for conventional membranes, here pressure directly modulates the activation energy, leading to a nonlinear water flow as a function of pressure. Consequently, high flux (1.6 L/Day/cm^2/MPa) with 100% salt rejection efficiency is achieved at reasonable temperatures and pressures, suggesting graphdiyne can serve as an excellent membrane for water desalination. We further show that to get through subnanopores water molecule must break redundant hydrogen bonds to form a two-hydrogen-bond transient structure. Our study unveils the principles and atomistic mechanism for water transport through pores in ultimate size limit, and offers new insights on water permeation through nanochannels, design of molecule sieving and nanofluidic manipulation.

IGF-1 signaling is Essential for Differentiation of Mesenchymal stem Cells for Peak Bone Mass

survival of children with chronic medical illnesses is leading to an increase in secondary osteoporosis due to impaired peak bone mass （PBM）. Insulin-like growth factor type Ⅰ （IGF-1） levels correlate with the pattern of bone mass accrual and many chronic illnesses are associated with low IGF-1 levels. Reduced serum levels of IGF-1 minimally affect the integrity of the skeleton, whereas recent studies suggest that skeletal IGF-I regulates PBM. To determine the role of IGF-1 in postnatal bone mass accrual regardless of source, we established an inducible type 1 Igf receptor Cre/lox knockout mouse model, in which the type 1 Igf receptor was deleted inducibely in the mesenchymal stem cells （MsCs） from 3-7 weeks of age. The size of the mouse was not affected as knockout and wild type mice had similar body weights and nasoanal and femoral lengths. However, bone volume and trabecular bone thickness were decreased in the secondary spongiosa of female knockout mice relative to wild type controls, indicating that IGF-1 is critical for bone mass. IGF-1 signaling in MsCs in vitro has been implicated to be involved in both migration to the bone surface and differentiation into bone forming osteoblasts. To clarify the exact role of IGF-1 in bone, we found by immunohistochemical analysis that a similar number of Osterix-positive osteoprogenitors were on the bone perimeter, indicating migration of MsCs was not affected. Most importantly, 56% fewer osteocalcin-positive mature osteoblasts were present on the bone perimeter in the secondary spongiosa in knockout mice versus wild type littermates. These in vivo data demonstrate that the primary role of skeletal IGF-1 is for the terminal differentiation of osteoprogenitors, but refute the role of IGF-1 in MsC migration in vivo. Additionally, these findings confirm that impaired IGF-1 signaling in bone MsCs is sufficient to impair bone mass acquisition.

Prediction of influence of process parameters on tensile strength of AA6061/TiC aluminum matrix composites produced using stir casting

stir casting was used to produce AA6061/15%TiC （mass fraction） aluminum matrix composites （AMCs）. An empirical relationship was developed to predict the effect of stir casting parameters on the ultimate tensile strength （UTs） of AA6061/TiC AMCs. A central composite rotatable design consisting of four factors and five levels was used to minimize the number of experiments, i.e., castings. The factors considered were stirring speed, stirring time, blade angle and casting temperature. The effect of those factors on the UTs of AA6061/TiC AMCs was derived using the developed empirical relationship and elucidated using microstructural characterization. Each factor significantly influenced the UTs. The variation in the UTs was attributed to porosity content, cluster formation, segregation of TiC particles at the grain boundaries and homogenous distribution in the aluminum matrix.

Effect of body mass index on functional outcome in primary total knee arthroplasty——a single institution analysis of 2180 primary total knee replacements

AIM To evaluate the effect of body mass index(BMI) on short-term functional outcome and complications in primary total knee arthroplasty. METHODs All patients undergoing primary total knee arthroplasty at a single institution between 2007 and 2013 were identified from a prospective arthroplasty database. 2180 patients were included in the study. Age, gender, BMI, pre- and post-operative functional scores [Western Ontario and Mc Master University Arthritis Index(WOMAC) and sF-36], complications and revision rate were recorded. Patients were grouped according to the WHO BMI classification. The functional outcome of the normal weight cohort(BMI s compared to the overweight and obese(BMI ≥ 25) cohort. A separate sub-group analysis was performed comparing all five WHO BMI groups; Normal weight, overweight, class 1 obese, class 2 obese and class 3 *** With a mean age of 67.89(28-92), 2180 primary total knee replacements were included. 64.36%(1403) were female. The mean BMI was 31.86(18-52). Ninty-three percent of patients were either overweight or obese. Mean follow-up 19.33 mo(6-60 mo). There was no significant difference in pre or post-operative WOMAC score in the normal weight(BMI s with a BMI ≥ 25(P > 0.05). sub-group analysis revealed significantly worse WOMAC scores in class 2 obese 30.80 compared to overweight 25.80(P ss 1 obese 25.50(P similarly, there were significantly worse sF-36 scores in class 2 obese 58.16 compared to overweight 63.93(P ss 1 obese 63.65(P superficial infections, 9(0.41%) deep infections and 19(0.87%) revisions overall with no complications or revisions in the normal weight cohort(BMI sION Post-operative functional outcome was not influenced by BMI comparing normal weight individuals with BMI > 25. Patients should not be denied total knee arthroplasty based solely on weight alone.
