Introduction: Hepatic diseases comprise inflammations of the liver, which can originate from drug-induced, toxic, autoimmune sources and particularly hepatitis B and C virus infection. The outcome of the disease is linked to interactions between the immune system and the virus, and also depends on the age and immune status of the patient. The aim of this study was to evaluate the association of a MAP3K14 (rs2074292), CD40 (rs1883832) polymorphism and chronic hepatitis B virus carriage in a population from Burkina Faso. Methods: In this case-control analysis, 223 and 173 samples, consisting of 90 and 53 controls and 133 and 120 cases, were examined for MAP3K14 and CD40 respectively. The cases included patients with Chronic Hepatitis B (CHB), cirrhosis or hepatocellular carcinoma (HCC). Genomic DNA extraction was executed using INVITROGEN and FAVORGEN kits. Genotyping of MAP3K14 (rs2074292) and CD40 (rs1883832) gene polymorphisms was accomplished via real-time PCR on the QuantStudioTM 5 Real-Time instrument, followed by allelic discrimination using TaqMan Genotyper Software. Data was interpreted using SPSS version 20 and Epi info version 7.***.2.0. Odds ratios (OR), confidence intervals (CI), and p-values were derived for risk and significance evaluation. Results: This study showed that the heterozygous CT genotype and the mutated T allele of the CD40 (rs1883832) gene are involved in the progression of chronic hepatitis to cirrhosis and hepatocellular carcinoma in HBV-infected patients. However, no association was found between polymorphisms in the MAP3K14 gene (rs2074292) and the progression of HBV infection. By combining the two polymorphisms, we observed either high risk or protection, depending on the genotypes of the MAP3K14 and CD40 genes simultaneously carried by the patient. Conclusion: Polymorphisms of the MAP3K14 and CD40 genes are associated with the evolution of HBV infection.

Detection of Low-Risk and High-Risk Oncogenic Human Papillomavirus in Archived Tissues from ENT Tumors in Burkina Faso

Human papillomavirus (HPV) is classified into high-risk HPV (HR-HPV) and HPV (LR-HPV) according to their oncogenic potential. These viruses can be found in the cervix, vagina, vulva, anus and in the ENT sphere. HPV ENT infections can lead to benign or malignant tumors in which we could find both LR-HPV and HR-HPV genotypes. The objective of this study was to investigate the genotypes of HR-HPV and LR-HPV in archived tissue samples derived from both benign and malignant tumors of the ear, nose, and throat (ENT) in Ouagadougou, Burkina Faso. One hundred and twenty formalin-fixed, paraffin-embedded archived tissues of the ENT sphere from 26 benign tumors and 94 malignant tumors were included. The tissues were first deparaffinized with xylem. The extracted DNA was used to test for high-risk and low-risk HPV by Real-Time Multiplex PCR. HPV DNA was found in 57.7% (15/26) of benign tumors and 43.61% (41/94) of malignant tumors. The prevalence of HPV infection was 46.67% (56/120) in all tumors combined. The most common HPV genotypes found were HPV 11 (34.28%), HPV 6 (30%), HPV56 (14.28%) and HPV 33 (8.57%). There were 21.43% (12/56) cases of genotypes co-infections with 10 cases of double infection and 2 cases of triple infection. Both low-risk and high-risk HPV are found in ENT tumors with relatively high HPV prevalence.

Background: Breast cancer mortality remains high in the majority of developing countries. The Ministry of Health has established two population-based cancer registries in Benin: the first one in Cotonou in 2014 and the second one in Parakou in 2017. However, there is a scarcity of data on breast cancer survival and prognosis in Benin Republic. Objective: This study sought to investigate epidemiological, diagnostic, and survival aspects of breast cancer in Parakou, based on data from its population-based cancer registry from 2017 to 2021. Method: For descriptive and analytical purposes, we used a retrospective cohort design. From January 24, 2022 to August 31, 2022, data were collected in all health facilities covered by the Parakou population-based cancer registry using an individual questionnaire. Survival and prognosis analysis were performed using KAPLAN MEIER method and David COX proportional hazard model respectively. Result: A total of 81 patients have been included in this study. The incidence rate of breast cancer in Parakou was 17.5 per 100,000 person-years with a mortality rate of 2.76 per 100,000 person-years. The median age at diagnosis was 44.50 years with extremes ranging from 19 to 76 years and a predominance of 40 - 50 years age group. The median survival time was estimated at 30 months with an overall 5-year survival of 47%. Young age at diagnosis (p-value = 0.002) and advanced stage at diagnosis (p-value = 0.000) had a negative impact on survival in women. The combination of surgery and chemotherapy improved survival (p-value = 0.018). Conclusion: Breast cancer is still a public health issue in Parakou. It comes out mandatory that resources be made available to make screening, early diagnosis and appropriate treatment of breast cancer affordable.

Diagnostic Input of Specialized Radiographic Examinations in Pediatric Setting in Ouagadougou (Burkina Faso)

Context: In spite of the development of cross-sectional medical imaging and endoscopy means, the prescription of specialized radiographic examinations is still relevant in the pediatric environment in our country. Objective: Show the contribution of specialized radiographic examinations performed in pediatric settings. Methodology: This was a retrospective descriptive study implemented over a five year-period, from June 10th, 2014 to June 10th, 2019 within the radiology department of Charles De Gaulle Pediatric Teaching Hospital in Ouagadougou. This study included the results of 192 specialized radiographic examinations collected within the concerned department. Results: The average age of the patients was 4.21 years with extremes of 8 days and 15 years. Infants (29 days to 24 months) were the predominant age group with a rate of 53.13% (n = 102 patients). Boys represented 76.51% of the sample with a sex ratio estimated at 3.25. Specialized radiographic examinations represented 0.46% of the overall imaging examinations and 0.72% of radiographic examinations performed during the study period. 67.19% of pathological examinations were reported. Colonic enema and retrograde urethrocystography (RUC) were the most performed examinations respectively accounting for 33.34% and 27.6%. The radiographic findings were consistent with the diagnostic hypotheses in 41.6% of cases. Conclusion: Specialized radiographic examinations keep their place in the exploration of several pathologies in pediatric settings.

Microbiological Quality of Fresh and Grilled Mutton Sold in Ouagadougou, Burkina Faso

In Ouagadougou, grilled meats in the form of pieces and brochettes are very popular and well-known to consumers. The aim of this study was to assess the microbial quality of mutton meat sold in Ouagadougou, Burkina Faso. A total of 120 samples were collected from 20 meat grillers 60 samples of fresh meat and 60 samples of grilled meat. The sampling was done between the month of August 2018 and the month of February 2019. The samples were analyzed according to standard methods. The test performed were counts of Aerobic Mesophilic Bacteria (AMB), yeasts and molds, enterobacteria, Campylobacter spp, Staphylococcus aureus, Bacillus cereus, Brucella spp, Pseudomonas aeruginosa, the search for salmonella, and the detection of antibiotics residues. Results showed a high count of AMB (8.77 and 6.78 log UFC/g);enterobacteria (6.58 and 3.05 log UFC/g), Staphylococcus aureus (6.45 and 4.35 log UFC/g), Bacillus cereus (6.98 and 4.52 log UFC/g), Campylobacter (6.03 and 3.86 log UFC/g), yeasts and molds (4.80 and 3.26 log UFC/g) and Pseudomonas aeruginosa (0.45 and 0.15 log UFC/g), respectively in fresh meat and grilled meat. Presumptive Salmonella was found in 95% of fresh meat samples and in 75% of grilled meat samples. In the tested samples, no Brucella spp were detected. However, residues of antibiotics were found in 5% of fresh meat samples and 5% of grilled meat samples. Means of moisture and pH were respectively 74.91% and 6.05% for fresh meat and 53.21% and 6.06% for grilled meat. The average microbial counts recorded in fresh and grilled meat are significantly high and indicate poor hygiene in the raw material and ready-to-eat meat. Good practices of hygiene and processing guides should be developed for the meat grilling value chain actors to reduce contamination risks.

Impact of Urban Anthropogenic Pressures on the Mangrove Forest of Sainte marie (East Madagascar)

This article demonstrates the urban anthropogenic threats and their impacts on Sainte marie (Eastern Madagascar) mangrove ecosystem. In addition to conducting interview with 158 residents, floristic inventories were carried out in order to compare the mangrove structure between a control site and another one that was impacted by Belle Vue dyke (south of the District downtown) rehabilitation. From 2003 to 2014, the island of Sainte marie lost 0.47% per year of its mangrove area. 87% of the harvested resources from this ecosystem are fish products and are related to human nutrition. The mangrove is used as a dumping ground for liquid and solid waste in the lots of Saint joseph and La Pointe. Defecation in the open air is topical under the island’s mangrove forest. A very weak resilience of the mangrove impacted by the rehabilitation of Belle Vue dyke was noted. Indeed, regenerations of Bruguiera gymnorrhiza and Ceriops tagal have struggled to rebuild. The total height and diameter of impacted trees decreased by 37.2% and 44.3%, respectively. The development of an updated urban master plan, including mangroves in the related zoning process is an imperative measurement for the conservation and/or valuation of existing lots.

Assessment of Groundwater for Heavy Metals in Selected Areas of North Lebanon

Heavy metals are ubiquitous in the environment and their consumption can create a potential hazard to human health, Lebanon is one of the richest countries in the Middle East. This study was undertaken to assess some of the heavy metals in drinking ground water of north Lebanon. Forty five ground water samples were collected from three different areas, the region of Bcharreh, a high mountain area, the region of Bessebaal, a semi industrial area and the region of Chekka, a sea level highly industrial area. Lead, chromium, zinc and copper were estimated using atomic absorption spectrometer. This study revealed high concentration of lead in the Bessebaal and Chekka areas (0.1328 mg/L, 0.3751 mg/L, respectively); whereas chromium was estimated to be high in all three regions, 0.2429 mg/L, 0.2249 mg/L and 0.2443 mg/L in Bcharreh, Bessebaal and Chekka, respectively. Alternatively zinc showed a high concentration only in Bessebaal and Chekka (0.3715 mg/L, 0.3216 mg/L, respectively), while, a low level of zinc (0.0393 mg/ L) was detected in Bcharreh. As for copper, it was only detected in Chekka (0.0019 mg/L). This study shined the light on the importance of heavy metals screening in our drinking water hence suggesting the developing of a successful programs for testing the quality of water sources before using it for drinking purpose.

Background: The assessment of liver fibrosis is an essential part of the fol-low-up of patients with chronic HBV. Ultrasound and fibroscan are two com-monly used non-invasive examinations and the purpose of this study is to as-sess the correlation between the results of these two examinations in the as-sessment of liver fibrosis. Methodology: This was a descriptive and analytical cross-sectional study with retrospective collection over a period of 30 months from January 01, 2018 to June 30, 2020 on the correlation between the ultra-sound aspects and the results of the fibroscan of patients with chronic carriers of Hepatitis B virus at the Teaching Hospital of Bogodogo and at the faith-based health center CANDAF. Results: A total of 85 patients with fibrosis were collected. The 30 - 40 age group was the most represented (41.18%), with a male predominance of 52.94%. The patients with a married life were the most represented (77.64%), the social stratum the most represented were the middle managers (32.94%). University level was predominant among educa-tion levels (45.64%). The personal history was traditional circumcision (34.12%), excision (21.18%), manicure (16.47%). The circumstances of the findings were blood donation (34.12%), screening (21.18%), fortuitous findings (16.47%). The association between echostructure and fibrosis on the fibroscan scan was significant (homogeneous hyperechoic echostructure p = 0.0028 and granular echostructure p = 0.0001). Fibrosis on scans had a significant associa-tion with liver contour (Regular contours p = 0.0009 and p = 0.0002). Fibrosis on CT scan had a significant association for the diagnosis of fibrosis on ultra-sound (p = 0.0002 for hepatic steatosis and cirrhosis and p = 0.0013 for hepat-ic dysmorphia), on the other hand, no significant association for hepatomegaly (p = 0.8883).
