BACkGROUND As a consequence of the economic crisis,the sociopolitical instability and the advent of the coronavirus disease-19 pandemic,nested challenges faced the Lebanese healthcare *** have resulted in critical shortages of essential resources,including medications vital for oncologic *** To assess the ramifications of the ongoing economic crisis on oncology patient care focusing on our outpatient oncology *** A questionnaire was distributed during the month of February 2022 to oncology patients in Hôtel Dieu de France University Hospital in Beirut during their outpatient *** primary objective was to assess the far-reaching impact of the economic crisis on patient care and the resulting psychological *** Among 182 interviewed patients,31.87%experienced treatment interruption mainly due to acute drug *** 87.91%of the patients benefiting from third-party coverage,69.60%had to self-pay for their medications leading to 69.78%of patients perceiving that healthcare was more difficult to access after ***,one-third of the patients exhibited symptoms of anxiety and/or depression,with 7 patients reporting suicidal ***,37.93%of patients who interrupted cancer treatment reported a history of comorbidities,and 89.66%who altered their treatment cited financial *** Lebanese cancer patients face complex challenges spanning economic,healthcare,and psychological *** inequalities exacerbated by the economic crisis hindered healthcare access.

Spontaneous Tumor Lysis Syndrome (STLS) is a rare oncologic condition caused by the breakdown of neoplastic tissue in the absence of traditional anti-tumor therapy. It is postulated that cancers with rapidly dividing cells lead to increased cell turnover which exceeds the kidneys’ ability to adequately filtrate by-products of cellular breakdown (i.e., phosphate, potassium, anduric acid), leading to end organ damage. It has been reported in the past that kidney failure is a sequelae of Tumor Lysis Syndrome (TLS), but there have been no reports that demonstrate acute kidney injury (AkI) preceding TLS. The case presented here demonstrates TLS in a patient with no formal cancer diagnosis, who had received no chemotherapy or radiation that was precipitated by an iatrogenic AkI with chlorthalidone and ibuprofen. This unusual pattern of AkI preceding STLS may provide insight into the pathophysiology of the condition and could possibly lead to greater understanding of this phenomenon.

The quest to increase the performance of production systems that have become complex leads to the transfer to the maintenance function of the responsibility of guaranteeing the availability of such systems. Also, we will never stop saying that maintenance must integrate into all of the company’s initiatives, to affirm its role, which is to ensure greater availability and sustainability of the means of production. The objective of this paper is to evaluate the reliability and availability of a system without knowing the distribution law of the operating times. Among the methods for evaluating dependability criteria (Fault Trees, Petri Nets, etc.), we are interested in queues that have the advantage of taking into account functional dependencies, thus allowing a quantified optimization of maintenance. Indeed, queues make it possible to model parallel or sequential processes, implementing operations taking place at the same time or one after the other, meeting the needs of modeling production systems. The main result of this paper is the study of the influence of availability on the reliability of a multi-state production system.

The fruits of Dacryodes edulis are rich in biologically active substances, which makes them of great interest in terms of validation. In this study, we targeted the primary metabolites in the epicarp, the mesocarp and the seed of the fruit of Dacryodes edulis at taste maturity, which were selected for their nutritional quality and its appreciation throughout the Gulf of Guinea area, which is very popular because of its large size, texture and special taste. The evaluation of total carbohydrates, total lipids and soluble proteins in the epicarp, mesocarp and seed of the fruit at taste maturity was made from spectrophotometer measurements. The overall analysis of the results of the present study shows that total carbohydrates, total lipids and proteins accumulate more in the seed with respectively 251.33 ± 1.15 mg/g DM;9.92 ± 0.201 mg/g DM and 55.075 ± 0.024 mg/g DM. Likewise, the results indicate low concentrations of total carbohydrates and total lipids in the epicarp with respectively 245 ± 1 mg/g DM and 4.77 ± 0.047 mg/g DM, on the other hand, it is the mesocarp which presents the lowest content of soluble proteins: 28.075 ± 3.231 mg/g DM. This variation could be linked to the nature of the compartment, more particularly to the storage location. This comparative study could lead to the valorization of the seed of the fruit of Dacryodes edulis for its richness in metabolites and arouse significant interest in nutrition.

Anesthetic Management of a Patient with Spinal Muscular Atrophy Type III Undergoing Emergent Caesarean Section: A Case Report

In this case report, we describe the anesthetic management for a 36-year-old G2P0010 at 36 weeks gestation with Spinal Muscular Atrophy Type III who underwent an emergent caesarean section due to fetal footling breech position. The patient is a wheelchair-bound quadriplegic with kyphoscoliosis and a lack of cough reflex who required nasal continuous noninvasive ventilatory support (CNVS) for chronic hypercapnic respiratory failure. Surgery was done under general anesthesia due to its emergent nature, and the patient was successfully extubated and transitioned to nasal CNVS in the operating room at the end of the case. Postoperative care was provided in the medical intensive care unit for three days without complication and the patient was discharged home uneventfully.

Geohydraulic Investigation of Aquifer Parameters in Abak, Southern Nigeria

A geophysical investigation was carried out to characterize aquifer parameters and assess the groundwater condition in Abak, southern Nigeria. Vertical electrical sounding (VES) was carried out using Schlumberger configuration in 10 locations within the area. Both manual and computer interpretation of the resistivity data reveals three to four geoelectric units (laterite topsoil, medium-grained sand, coarse-grained sand, and sandy clay sand) which agrees with the lithologic log from existing boreholes. The aquifer layer was identified along the third formation with resistivity values ranging from 1239 - 5719 Ωm and aquifer depth ranging from 30.2 - 54.8 m. The aquifer thickness ranged from 24.2 - 43.7 m. Hydraulic conductivity ranged from 0.1206 m/day - 0.5026 m/day with an average value of 0.29403 m/day. Formation factor ranged from 14.55 - 16.64. Porosity ranged from 15.98% - 22.40%, with an average value of 19.64%. The aquiferous zone falls within the medium/coarse-grained sand. The aquifer is shallow, unconfined, and prolific with a little overburden formation of 13.5 m. The area shows good prospects for groundwater development.

Introduction: Frontal sinus fractures are potentially serious. They are defined as a solution of continuity, open or closed, of one or both bone tables of the frontal sinus. This study aims to report on the management of them at the Yalgado OUEDRAOGO University Hospital Centre. Methodology: It is a descriptive cross-sectional study with retrospective collection from January 01, 2016 to December 31, 2018. Patients with frontal sinus fractures were managed at the Yalgado OUEDRAOGO University Hospital Centre through CT-scan proof. Results: Over three years, a total of 102 cases of frontal sinus fractures were collected with 29.9 years as average age. There were 96 men. Workers in the informal sector and pupils/students represented 58.90% of patients. The residence of the patients was urban in 68.80% of cases and rural in 31.40%. Road traffic accidents (RTAs) happened in 90.20%, and involved 2-wheelers in 98.20%. None of these drivers was wearing a helmet. The type III frontal fracture of Ioannides et al. represented 51.9% of cases. In 89.21% of cases, other facial and/or cranioencephalic injuries were compounded to frontal sinus fractures. No surgical management was observed in 82 (80.39%) patients and surgical management in 20 (19.61%) patients. The outcome was favourable, but sequelae and/or complications were noted in 10 patients who had surgery and 30 patients who did not. Conclusion: These results enforce helmet wearing for all riders of two-wheeled machines. In addition, vaccinations to prevent meningitis in frontal sinus fractures with dural breach should be systematic.
