AIM:To examine the epidemiology and the morphology of the proximal sessile serrated adenomas(SSAs).METHODS:We conducted a retrospective study to identify patients with SSAs using a university-based hospital pathology database query from January 2007to April *** collected included:age,gender,ethnicity,body mass index,diabetes,smoking,family history of colorectal cancer,aspirin,and statin *** collected data on morphology of SSAs including site(proximal or distal),size,and endoscopic appearance(flat or protuberant).We also compared proximal SSAs to proximal tubular adenomas detected during same time ***:One hundred and twenty patients with SSAs were identified:61%were distal and 39%were proximal *** SSAs were more likely to be flat than distal(100%vs 78%respectively;P=0.0001).Proximal SSAs were more likely to occur in smokers(OR=2.63;95%cI:1.17-5.90;P=0.02)and in patients with family history of colorectal cancer(OR=4.72;95%cI:1.43-15.55;P=0.01)compared to *** SSAs were statistically more likely to be≥6 mm in size(OR=2.94;P=0.008),and also more likely to be large(≥1 cm)(OR=4.55;P=0.0005)compared to the distal *** were more likely to have proximal(P=0.02),flat(P=0.01)and large(P=0.007)SSAs compared to *** to proximal tubular adenomas,proximal SSAs were more likely to be large and occur in ***:Proximal SSAs which accounted for two-fifths of all SSAs were more likely to present as flat lesions,larger SSAs,and were more likely to occur in smokers and in patients with family history of colorectal *** data has implications for colorectal cancer screening.

MET receptor tyrosine kinase promotes the generation of functional synapses in adult cortical circuits

Loss of synapse and functional connectivity in brain circuits is associated with aging and neurodegeneration,however,few molecular mechanisms are known to intrinsically promote synaptogenesis or enhance synapse *** have previously shown that MET receptor tyrosine kinase in the developing cortical circuits promotes dendritic growth and dendritic spine *** investigate whether enhancing MET in adult cortex has synapse regenerating potential,we created a knockin mouse line,in which the human MET gene expression and signaling can be turned on in adult(10–12 months)cortical neurons through doxycycline-containing *** found that similar to the developing brain,turning on MET signaling in the adult cortex activates small GTPases and increases spine density in prefrontal projection *** findings are further corroborated by increased synaptic activity and transient generation of immature silent *** MET signaling resulted in an increasedα-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid/N-methyl-Daspartate(AMPA/NMDA)receptor current ratio,indicative of enhanced synaptic function and *** data reveal that enhancing MET signaling could be an interventional approach to promote synaptogenesis and preserve functional connectivity in the adult *** findings may have implications for regenerative therapy in aging and neurodegeneration conditions.

The emerging role and targetability of the TcA cycle in cancer metabolism

The tricarboxylic acid （TcA） cycle is a central route for oxidative phosphorylation in cells, and fulfills their bioenergetic, biosynthetic, and redox balance require- ments. Despite early dogma that cancer cells bypass the TcA cycle and primarily utilize aerobic glycolysis, emerging evidence demonstrates that certain cancer cells, especially those with deregulated oncogene and tumor suppressor expression, rely heavily on the TcA cycle for energy production and macromolecule synthesis. As the field progresses, the importance of aberrant TcA cycle function in tumorigenesis and the potentials of applying small molecule inhibitors to perturb the enhanced cycle function for cancer treatment start to evolve. In this review, we summarize current knowledge about the fuels feeding the cycle, effects of oncogenes and tumor suppressors on fuel and cycle usage, common genetic alterations and deregulation of cycle enzymes, and potential therapeutic opportunities for targeting the TcA cycle in cancer cells. With the application of advanced technology and in vivo model organism studies, it is our hope that studies of this previously overlooked biochemical hub will provide fresh insights into cancer metabolism and tumorigenesis, subsequently revealing vulnerabilities for thera- peutic interventions in various cancer types.

Subsidence over room and pillar retreat mining in a low coal seam

The objective of this paper is to study the behavior of a low thick and low depth coal seam and the overburden rock mass. The mining method is room and pillar in retreat and partial pillar recovery. The excavation method is conventional drill and blast because of the small production. The partial pillar recovery is about 30% of the previous pillar size, 7 m × 7 m. The roof displacement was monitored during retreat operation; the surface movement was also monitored. The effect of the blasting vibration on the final pillar strength had been considered. Due to blasting, the pillar reduced about 20%. The consequence is more pillar deformation and roof vertical displacement. The pillar retreat and ground movement were simulated in a three-dimensional numerical model. This model was created to predict the surface subsidence and compare to the subsidence measured. This study showed that the remaining pillar and low seam reduce the subsidence that was predicted with conventional methods.

Morbidity and mortality of coagulase-negative staphylococcal sepsis in very-low-birth-weight infants

Background coagulase-negative staphylococci (coNS) are the most common cause of late-onset sepsis in the neonatal intensive care unit (NIcU) and usually require vancomycin treatment. Our objective was to determine whether coNS are associated with neonatal morbidity and mortality. Methods This was a retrospective cohort study of very-low-birth-weight (VLBW, ≤ 1500 g) infants from 1989 to 2015. Exclusion criteria were major congenital anomaly or death within 72 h. coNS was considered a pathogen if recovered from ≥ 2 cultures, or 1 culture if treated for ≥ 5 days and signs of sepsis were present. Logistic regression was used to exam-ine factors associated with morbidity and mortality. Results Of 2242 VLBW infants, 285 (12.7%) had late-onset sepsis. coNS (125, 44%),Staphylococcus aureus (52, 18%), and Escherichia coli (36, 13%) were the most commonly recovered organisms. In multivariate analysis, coNS sepsis was not associated with mortality (OR 0.6 (95% cI 0.2-2.6)), but sepsis with other organisms was (OR 4.5 (95% cI 2.6-8.0))coNS sepsis was associated with longer hospitalization but not risk for bronchopulmonary dysplasia, intraventricular hem-orrhage, or retinopathy of prematurity. conclusion coNS sepsis was not associated with mortality or morbidities other than length of stay. These findings support vancomycin-reduction strategies in the NIcU.

A numerical study on the influence of grain boundary oxides on dwell fatigue crack growth of a nickel-based superalloy

Pre-hospital assessment with ultrasound in emergencies: implementation in the field

BAcKGROUND: Point-of-care ultrasound(US) is a proven diagnostic imaging tool in the emergency department(ED). Modern US devices are now more compact, affordable and portable, which has led to increased usage in austere environments. However, studies supporting the use of US in the prehospital setting are limited. The primary outcome of this pilot study was to determine if paramedics could perform cardiac ultrasound in the fi eld and obtain images that were adequate for interpretation. A secondary outcome was whether paramedics could correctly identify cardiac activity or the lack thereof in cardiac arrest ***: We performed a prospective educational study using a convenience sample of professional paramedics without ultrasound experience. Eligible paramedics participated in a 3-hour session on point-of-care US. The paramedics then used US during emergency calls and saved the scans for possible cardiac complaints including: chest pain, dyspnea, loss of consciousness, trauma, or cardiac ***: Four paramedics from two distinct fire stations enrolled a total of 19 unique patients, of whom 17 were deemed adequate for clinical decision making(89%, 95%cI 67%–99%). Paramedics accurately recorded 17 cases of cardiac activity(100%, 95%cI 84%–100%) and 2 cases of cardiac standstill(100%, 95%cI 22%–100%).cONcLUSION: Our pilot study suggests that with minimal training, paramedics can use US to obtain cardiac images that are adequate for interpretation and diagnose cardiac standstill. Further large-scale clinical trials are needed to determine if prehospital US can be used to guide care for patients with cardiac complaints.
