Technical note: Anterior cruciate ligament reconstruction in the presence of an intramedullary femoral nail using anteromedial drilling

AIM To describe an approach to anterior cruciate ligament(ACL) reconstruction using autologous hamstring by drilling via the anteromedial portal in the presence of an intramedullary(IM) femoral *** Once preoperative imagining has characterized the proposed location of the femoral tunnel preparations are made to remove all of the hardware(locking bolts and IM nail). A diagnostic arthroscopy is performed in the usual fashion addressing all intra-articular pathology. The ACL remnant and lateral wall soft tissues are removed from the intercondylar, to provide adequate visualization of the ACL footprint. Femoral tunnel placement is performed using a transportal ACL guide with desired offset and the knee flexed to 2.09 rad. The Beath pin is placed through the guide starting at the ACL's anatomic footprint using arthroscopic visualization and/or fluoroscopic guidance. If resistance is met while placing the Beath pin, the arthroscopy should be discontinued and the obstructing hardware should be removed under fluoroscopic guidance. When the Beath pin is successfully placed through the lateral femur, it is overdrilled with a 4.5 mm Endobutton drill. If the Endobutton drill is obstructed, the obstructing hardware should be removed under fluoroscopic guidance. In this case, the obstruction is more likely during Endobutton drilling due to its larger diameter and increased rigidity compared to the Beath pin. The femoral tunnel is then drilled using a best approximation of the graft's outer diameter. We recommend at least 7 mm diameter to minimize the risk of graft failure. Autologous hamstring grafts are generally between 6.8 and 8.6 mm in diameter. After reaming, the knee is flexed to 1.57 rad, the arthroscope placed through the anteromedial portal to confirm the femoral tunnel position, referencing the posterior wall and lateral cortex. For a quadrupled hamstring graft, the gracilis and semitendinosus tendons are then harvested in the standard fashion. The tendons are

Elucidating the degradation pathways of Human Insulin in the Solid State

While there have been significant advances in the development of peptide oral dosage forms in recent years, highlighted by the clinical and commercial success of approved peptides such as Rybelsus?, there remain several barriers in the way of broad range applicability of this approach to peptide delivery. One such barrier includes the poor physical and chemical stability inherent to their structures, which persists in the solid state although degradation typically occurs at different rates and via different pathways in comparison to the solution state. Using insulin as a model peptide, this work sought to contribute to the development of analytical techniques for investigating common insulin degradation pathways. Chemically denatured,deamidated and aggregated samples were prepared and used to benchmark circular dichroism spectroscopy, reverse phase HpLC and size exclusion chromatography methods for the investigation of unfolding, chemical modifications and covalent aggregation of the insulin molecule respectively. Solid state degraded samples were prepared by heating insulin powder at 60 °C and 75% relative humidity for 1, 3, 5 and 7 d, and the degradation profiles of the samples were evaluated and compared with those observed in solution. While no unfolding was observed to occur, significant deamidation and covalent aggregation were detected. Reductive disulfide bond cleavage using dithiothreitol allowed for separation of the insulin A-and B-chains,offering a facile yet novel means of assessing the mechanisms of deamidation and covalent aggregation occurring in the solid state.

Satellite Multi-Temporal Data and Cropping pattern Approach for Green Gram Crop Management in the Lower Midland Zone IV and V in Kenya

Creation of a spectral signature reflectance data, which aids in the identification of the crops is important in determining size and location crop fields. Therefore, we developed a spectral signature reflectance for the vegetative stage of the green gram (Vigna. radiata L.) over 5 years (2020, 2018, 2017, 2015, and 2013) for agroecological zone IV and V in Kenya. The years chosen were those whose satellite resolution data was available for the vegetative stage of crop growth in the short rain season (October, November, december (OND)). We used Landsat 8 OLI satellite imagery in this study. Cropping pattern data for the study area were evaluated by calculating the Top of Atmosphere reflectance. Farms geo-referencing, along with field data collection, was undertaken to extract Top of Atmosphere reflectance for bands 2, 3, 4 and 7. We also carried a spectral similarity assessment on the various cropping patterns. The spectral reflectance ranged from 0.07696 - 0.09632, 0.07466 - 0.09467, 0.0704047 - 0.12188,0.19822 - 0.24387, 0.19269 - 0.26900, and 0.11354 - 0.20815 for bands 2, 3, 4, 5, 6, and 7 for green gram, respectively. The results showed a dissimilarity among the various cropping patterns. The lowest dissimilarity index was 0.027 for the maize (Zea mays L.) bean (phaseolus vulgaris) versus the maize-pigeon pea (Cajanus cajan) crop, while the highest dissimilarity index was 0.443 for the maize bean versus the maize bean and cowpea cropping patterns. High crop dissimilarities experienced across the cropping pattern through these spectral reflectance values confirm that the green gram was potentially identifiable. The results can be used in crop type identification in agroecological lower midland zone IV and V for mung bean management. This study therefore suggests that use of reflectance data in remote sensing of agricultural ecosystems would aid in planning, management, and crop allocation to different ecozones.

defining the association between the prolonged operative time and 90-day complications in patients undergoing radical cystectomy

Objective:Radical cystectomy is a complex lengthy procedure associated with postoperative *** aimed to assess the operative time(OT)in patients undergoing radical cystectomy and its impact on 90-day postoperative complications and readmission ***:The retrospective cohort study included 296 patients undergoing radical cystectomy and urinary diversion from May 2010 to december 2018 in our *** OT of 369 min was set as a cutoff value between short and long OT *** primary outcome was 90-day postoperative complication *** outcomes were gastrointestinal recovery time,length of hospital stay,and 90-day readmission ***:The overall incidence of 90-day postoperative complications was 79.7%where 43.2%representing low-grade complications according to the ClavieneDindo classification(Grade 1 and Grade 2),and 36.5%representing high-grade complications(Grade3).Gastrointestinal tract and infectious complications are the most common complications in our data set(45.9%and 45.6%,respectively).On multivariable analysis,prolonged OT was significantly associated with odds of high-grade complications(odds ratio 2.340,95%confidence interval 1.288e4.250,p=0.005).After propensity score-matched analysis,a higher incidence of major complications was identified in the long OT group 55(51.4%)compared to 35(32.7%)in the short OT group(p=0.006).A shorter gastrointestinal tract recovery time was noticed in the short OT group(p=0.009).prolonged OT was associated with a higher 90-day readmission rate on univariate and multivariate analyses(pp=0.001,respectively).
