Incidental diagnosis of intestinal spirochetosis in a patient with chronic hepatitis B:A case report

BACKGROUND Intestinal spirochetosis(IS)is caused by Brachyspira colonization of the gastrointestinal *** patients are asymptomatic,while others present with gastrointestinal complaints such as abdominal pain,diarrhea,or gastrointestinal ***,the clinical significance of asymptomatic IS is unclear,and guidelines are lacking regarding decision to *** SUMMARY A 73-year-old male with peptic ulcer disease and gastroesophageal reflux was evaluated for elevated liver *** was diagnosed with chronic hepatitis B virus and prescribed ***,he was leukopenic and had stage 4 liver fibrosis on transient *** 5 mo,the patient returned for esophagogastroduodenoscopy and screening *** denied any gastrointestinal symptoms at that *** included grade I distal esophageal varices,mild portal hypertensive gastropathy,and patchy nodular gastric antral *** colonoscopy,several polyps were *** and eosin stain of mucosa adjacent to the polyps revealed a“false brush border,”and Steiner stain identified spirochetes adherent to the *** pathology findings confirmed the diagnosis of *** was managed conservatively with careful observation and without antibiotic therapy via a multidisciplinary approach between gastroenterology and infectious *** remained asymptomatic at the 7-wk *** This case reports the finding of incidental,asymptomatic IS in a leukopenic patient with hepatitis B virus. Conservative management was appropriate.

Rubber plunger surface texturing for friction reduction in medical syringes

Friction is a genuine issue in the use of many medical devices involving rubbery materials such as plungers in medical syringes. This paper presents a new direction for the reduction of friction in medical syringes based on surface texturing of the rubber plunger. The specimens were prepared by casting poly(vinylsiloxane)(PVS) rubber into a pre-fabricated negative template obtained by 3D printing. Friction tests were performed on a home-made test-rig. It was clearly shown that friction resistance can be considerably manipulated when using textured plungers.

Carbides Formation in MarM247 Directional Solidified during Stress-Rupture Test at High Temperature

In our study, stress-rupture tests were conducted at elevated temperatures to examine the impact of high temperature on MarM247 LC (low carbon). Our main objective was to investigate the alterations in the microstructure, particularly the carbon precipitation, during long-term stress-rupture tests. It was observed that cracks developed near the sample neck, following the path of the carbides and the gamma matrix, rather than occurring in the gamma-gamma prime eutectic. This occurred despite the formation of carbides because of prolonged exposure to high temperature and load, and the crack propagation did not follow that path. Based on these findings, we suggest that a reduction in the carbon content of Mar-M247 LC can enhance the sample's lifespan when subjected to temperatures below 760˚C and a stress of 690 MPa.

Multi-institutional retrospective analysis of FOLFIRI in patients with advanced biliary tract cancers

Hepatitis C infection and lymphoproliferative disease:Accidental comorbidities?

Chronic hepatitis C virus(HCV)infection has been associated with liver cancer and cirrhosis,autoimmune disorders such as thyroiditis and mixed cryoglobulinema,and alterations in immune function and chronic inflammation,both implicated in B cell lymphoproliferative diseases that may progress to non-Hodgkin lymphoma(NHL).HCV bound to B cell surface receptors can induce lymphoproliferation,leading to DNA mutations and/or lower antigen response *** findings and epidemiological reports suggest an association between HCV infection and *** performed a systematic review of the literature to clarify this potential *** searched the English-language literature utilizing Medline,Embase,Paper First,Web ofScience,Google Scholar,and the Cochrane Database of Systematic Reviews,with search terms broadly defined to capture discussions of HCV and its relationship with NHL and/or lymphoproliferative *** were screened to further identify relevant studies and literature in the basic sciences.A total of 62 reports discussing the relationship between HCV,NHL,and lymphoproliferative diseases were *** studies suggest that at least a portion of NHL may be etiologically attributable to HCV,particularly in areas with high HCV *** that showed a lack of association between HCV infection and lymphoma may have been influenced by small sample size,short follow-up periods,and database *** association appears strongest with the B-cell lymphomas relative to other lymphoproliferative *** by which chronic HCV infection promotes lymphoproliferative disease remains *** is a multifactorial process involving genetic,environmental,and infectious *** most probably have a role in the lymphomagenesis but further study to clarify the association and underlying mechanisms is warranted.

Azathioprine-induced fever in autoimmune hepatitis

Underdiagnosis of drug-induced fever leads to extensive investigation and prolongation of hospitalization, and may lead to multiple unnecessary invasive procedures and a wrong diagnosis. Azathioprine is a widely used immunosuppressive drug. We report a case of a 53-year-old female patient diagnosed with autoimmune hepatitis treated with azathioprine, who presented to the emergency room with a 6-wk history of fever and chills without other associated symptoms. Since the patient's fever was of unknown origin, she was hospitalized. All treatment was stopped and an extensive workup to explore the source of fever and chills was performed. Results of chest X-ray, viral, urine, and blood cultures, autoimmune serology, transthoracic and transesophageal echocardiography, and abdominal ultrasound revealed no source of infection. A rechallenge test of azathioprine was performed and the fever and chills returned within a few hours. Azathioprine was established as the definite cause following rechallenge. Fever as an adverse drug reaction is often unrecognized. Azathioprine has been reported to cause druginduced fever in patients with inflammatory bowel disease, rheumatoid arthritis, and sarcoidosis. To the bestof our knowledge there have been no previous reports documenting azathioprine-induced fever in patients with autoimmune hepatitis. The occurrence of fever following the readministration of azathioprine suggests the diagnosis of drug-induced fever, particularly after the exclusion of other causes. A careful rechallenge is recommended to confirm the diagnosis.
