In this paper, the authors aim to propose the use of waste plastics as a binder in a coconut shell reinforcement for the development of an 8/6 size composite rafter to replace the natural 8/6 size backbone in construction. Following a study into the choice of the best proportions, a total of 30 size 8/6 composite rafters with different proportions of 20%, 25%, 30%, 35%, 40% and 50% plastic content were developed. All the 8/6 composite rafters were subjected to mechanical (3-point bending strength and monnin hardness) and physical (bulk density and water absorption) characterization analyses. The results show that flexural strength increases from 27.56 mPa to 33.30 mPa for proportions ranging from 20% to 35% plastic content. Above 35% plastic, the strength drops to 19.60 mPa for a 50% plastic content. Similarly, the monnin hardness drops from 9 mm to 5 mm when the plastic content varies from 20 to 50%. As for the results of the physical characterisation, the values obtained for apparent density vary from 0.89 to 1 for proportions varying from 20% to 35% plastic content and drop to 0.94 for 50% plastic content. As for water absorption, values drop from 6.82% to 2.45% when the plastic content increases from 20% to 50%. These mechanical strengths stabilise at 35% plastic content. The development of an 8/6 chevron composite material based on plastic and coconut shell could therefore be a way of recovering waste and solving the problem of deforestation.

The Reasons for Admission of Elderly Subjects in Intensive Care at the CHU Ignace Deen

Objective: To determine the reasons for admission of elderly subjects and the prognosis in general intensive care. Patients and methods: Observational descriptive and analytical study with prospective collection of data over a period of one year from January 1 to December 31, 2021. Patients aged 65 or over were included. Abstract: During the study period, 223 cases were collected out of 587 patients admitted, giving a prevalence of 37.9%. The average age was 74.127.39 ± years with extremes of 65 and 96 years and a male predominance (58.7%). The comorbidities were dominated by arterial hypertension (71.3%). The patients were: transferred from medical and surgical emergencies (75.8%). The average admission time was 48.8 ± 29.8 hours. One hundred and eight patients had a Glasgow score between 3 and 7. The reasons for admission were dominated by vascular causes (51.6%). Strokes of any type accounted for 43.9% of these reasons for admission. The average time for carrying out the biological assessments and imaging was 41.8 ± 27.3 hours with the extremes of 3 and 89 hours, 37.2% had a complete assessment within 24 hours. The average duration of hospitalization was 7.10 ± 8.87 days with extremes of 1 and 72 days. The mortality rate was 71.7%. Conclusion: This study has made it possible to take stock of the reasons for the admission of elderly subjects to intensive care. It appears that vascular causes are the main reasons for admission with heavy comorbidities which results in high mortality.

Objective: Was to present the management of a pregnant woman at 39 SA received for severe head trauma in the emergency room. Severe trauma in pregnant women can have serious maternal and especially fetal consequences in the absence of a diagnostic and therapeutic strategy. We report the case of serious trauma in a 39 WA pregnant woman, nulliparous. Admitted to the emergency room for serious head trauma following a public road accident. The clinical picture was dominated by impaired consciousness with a Glasgow score of 8/15, right otorrhagia, desaturation at 89% AA and respiratory rate at 36 cycles/min. The obstetrical examination noted a fundal height of 32 cm, a fetal heart sound (BCF) of 167 beats/min, a cephalic presentation, a cervical opening of 3 cm and uterine contractures. Imaging examinations outside the hospital because not available. The abdominopelvic ultrasound shows a monofetal pregnancy without placental abruption. The cerebral scanner shows an edemato-haemorrhagic contusion intra parenchymatous frontotemporal left. Respiratory support and an indication for caesarean section allowed the extraction of a live male child, Apgar at one minute 10/10 to five minutes 10/10, weighing 3000 g without abnormality on clinical examination. The evolution was favorable after 27 days of resuscitation. Conclusion: The care of a severely traumatized pregnant woman relies above all on good multidisciplinary coordination, based on close collaboration between emergency physicians, anesthetists-resuscitators, obstetricians, pediatricians, neonatologists and biologists.

Commercially pure Fe,Fe35mn,and(Fe35mn)5Ag alloys were prepared by uniaxial pressing of the mixture of individual powders,followed by *** influence of the alloying elements mn and Ag on the corrosion behaviour of these Fe-based alloys was investigated in Hanks’Balanced Salt Solution(HBSS).Furthermore,the role of the components in HBSS,particularly Ca^(2+)ions during alloys degradation was *** of local pH and dissolved oxygen concentration was measured 50μm above the interface of the degrading *** results revealed that 5 wt%Ag addition to Fe35mn alloy triggered micro-galvanic corrosion,while uniform corrosion dominated in pure Fe and *** precipitation of Ca-P-containing products on the surface of these Fe-based alloys buffered local pH at the metal interface,and blocked oxygen diffusion at the initial stages of *** the(Fe35mn)5Ag,the detachment or structural changes of Ca-P-containing products gradually diminished their barrier *** findings provided valuable insights into the degradation mechanism of promising biodegradable Fe-based alloys.

Background:Dietary supplementation of omega-3 fatty acids can reduce the activation of the endocannabinoid system(ECS)by decreasing the availability of arachidonic acid,thus lowering endocannabinoids(e CBs)*** ECS is a modulator of energy metabolism,stress response and inflammation in mammals,yet there is little information on the roles of the ECS in transition dairy *** the periparturient period,the adipose tissue and liver are the main metabolic organs that participate in the adaptations of dairy cows to onset of lactation;however,exceeded adipose tissue lipolysis and accumulation of lipids in the liver have adverse effects on cows'*** we aimed to examine whether omega-3 supplementation during the transition period will modulate ECS activation and affect metabolic and inflammatory indices in postpartum dairy cows,by supplementing twenty-eight transition Holstein dairy cows with either saturated fat(CTL)or encapsulated flaxseed oil(FLX).Components of the ECS,metabolic and inflammatory markers were measured in blood,liver,and subcutaneous adipose ***:FLX supplementation reduced feed intake by 8.1%(Pma levels of arachidonic acid(by 44.2%;P=0.02)and anandamide(by 49.7%;P=0.03)postpartum compared to *** m RNA transcription levels of the cannabinoid receptor 1(CNR1/CB1)tended to be lower(2.5 folds)in white blood cells of FLX than in CTL(P=0.10),and protein abundance of ECS enzyme monoacylglycerol lipase was higher in peripheral blood mononuclear cells of FLX than in CTL(P=0.04).In adipose tissue,palmitoylethanolamide levels were lower in FLX than in CTL(by 61.5%;P=0.02),relative m RNA transcription of lipogenic genes were higher,and the protein abundance of cannabinoid receptor 2(P=0.08)and monoacylglycerol lipase(P=0.10)tended to be higher in FLX compared to *** 2-arachidonoylglycerol tended to be higher(by 73.1%;P=0.07),and interlukin-6 m RNA transcription level was 1.5 folds lower in liver of FLX

Periodontal disease has been recently linked to a variety of systemic conditions such as diabetes, cardiovascular disease, preterm delivery, and oral cancer. The most common bacteria associated with periodontal disease, Porphyromonas gingivalis （P. gingivalis） has not yet been studied in the malignant gingival tissues. The objective of this study was to investigate the presence of R gingivalis in specimens from squamous cell carcinoma patients. We have performed immunohistochemical staining to investigate the presence of R gingivafis and Streptococcus gordonii （S. gordonii）, a non invasive oral bacteria, in paraffin embedded samples of gingival squamous cell carcinoma （n=10） and normal gingiva （n=5）. Staining for R gingivalis revealed the presence of the bacteria in normal gingival tissues and gingival carcinoma, with higher levels （more than 33%, P〈0.05） detected in the carcinoma samples. The staining intensity was also significantly enhanced in the malignant tissue by 2 folds （P〈0.023） compared to specimens stained for the non-invasive S. gordonii. R gingivalis is abundantly present in malignant oral epithelium suggesting a potential association of the bacteria with gingival squamous cell carcinoma.

BACKGROUND Hepatic artery stenosis is a complication of orthotopic liver transplant occurring in 3.1%-7.4%of patients that can result in graft failure and need for *** therapy with angioplasty and stenting has been used with a high degree of technical success and good clinical outcomes,but tortuous hepatic arteries present a unique challenge for *** stents for this application should be maneuverable and conformable while also exerting adequate radial force to maintain a patent *** SUmmARY Herein we report our experience with a neurovascular Wingspan stent system in a challenging case of recurrent hepatic artery stenosis and discuss the literature of stenting in tortuous transplant hepatic *** Wingspan neurovascular stent is self-expanding,has good conformability,and adequate radial resistance and as such it could be added to the armamentarium of interventionalists in the setting of a tortuous and stenotic transplant hepatic artery.

The goal of efficient and robust error control, through local mesh adaptationin the computational solution of partial differential equations, is predicated on theability to identify in an a posteriori way those localized regions whose refinement willlead to the most significant reductions in the error. The development of a posteriori errorestimation schemes and of a refinement infrastructure both facilitate this goal, howeverthey are incomplete in the sense that they do not provide an answer as to where themaximal impact of refinement may be gained or what type of refinement — elementalpartitioning (h-refinement) or polynomial enrichment (p-refinement) — will best leadto that gain. In essence, one also requires knowledge of the sensitivity of the error toboth the location and the type of refinement. In this communication we propose theuse of adjoint-based sensitivity analysis to discriminate both where and how to *** present both an adjoint-based and an algebraic perspective on defining and usingsensitivities, and then demonstrate through several one-dimensional model problemexperiments the feasibility and benefits of our approach.

BACKGROUND Bowel preparation in children can be *** To describe the efficacy, safety, and tolerability of sodium picosulfate, magnesium oxide, and citric acid(SPmC) bowel preparation in *** Phase 3, randomized, assessor-blinded, multicenter study of low-volume, divided dose SPmC enrolled children 9-16 years undergoing elective colonoscopy. Participants 9-12 years were randomized 1:1:1 to SPmC ? dose × 2, SPmC 1 dose × 2, or polyethylene glycol(PEG). Participants 13-16 years were randomized 1:1 to SPmC 1 dose × 2 or PEG. PEG-based bowel preparations were administered per local protocol. Primary efficacy endpoint for quality of bowel preparation was responders(rating of ‘excellent' or ‘good') by modified Aronchick Scale. Secondary efficacy endpoint was participant's tolerability and satisfaction from a 7-item questionnaire. Safety assessments included adverse events(AEs) and laboratory *** 78 participants were randomized, 48 were 9-12 years, 30 were 13-16 years. For the primary efficacy endpoint in 9-12 years, 50.0%, 87.5%, and 81.3% were responders for SPmC ? dose × 2, SPmC 1 dose × 2, and PEG groups, respectively. Responder rates for 13-16 years were 81.3% for SPmC 1 dose × 2 and 85.7% for PEG. Overall, 43.8% of participants receiving SPmC 1 dose × 2 reported it was ‘very easy' or ‘easy' to drink, compared with 20.0% receiving PEG. Treatment-emergent AEs were reported by 45.5% of participants receiving SPmC 1 dose × 2 and 63.0% receiving *** SPmC was an efficacious and safe for bowel preparation in children 9-16 years, with comparable efficacy to PEG. Tolerability for SPmC was higher compared to PEG.
