Surgical Treatment of Chronic Constrictive Pericarditis in a Developing Country

Background: Chronic constrictive pericarditis is a rare but serious clinical entity with a poor prognosis in the absence of surgical treatment. The purpose of this study was to report on our experience of subtotal anterior pericardectomy and analyze our surgical results. Results: We included 74 patients operated on for chronic constrictive pericarditis in our institution during the period from January 1985 to December 2020. There were 29 female and 45 male patients, with an average age of 28 years (range: 8 - 64 years). 36.5% of patients were in NYHA class III or IV. Physical signs were dominated by peripheral signs of right heart failure in 93.2% of cases. The surgical procedure was a subtotal anterior pericardectomy from the left to the right phrenic nerve, freeing the heart chambers and the large vessels. The surgical results were marked by a functional improvement in 82.4% of the cases. The postoperative complications were marked by a low cardiac output in 8.1% of the cases, a atrial fibrillation in 4.1% of the cases, a haemorrhage in 1.4% of the cases, a haemothorax in 2.7% of the cases. Perioperative mortality was 5.4% patients. The causes of death were low cardiac output in 50% of cases, hemorrhage in 25% of cases, and hepatocellular insufficiency in 25% of cases. We observed a mortality of 2.9% after an average follow-up of 5.17 years ± 4.76 years. All other survivors were asymptomatic and no re-intervention for recurrence was performed. Conclusions: Subtotal anterior pericardectomy was the technique we used for the treatment of chronic constrictive pericarditis. It allowed having satisfactory surgical results with a functional improvement and an acceptable morbidity.

The stem barks of Prunus africana are used in the treatment of the benign prostate. Cameroon is one of the important exporters of the barks. Despite the important measures adopted in Cameroon for sustaining its harvesting, some many chalenges still remain. The objective of this work is to refine the forest management parameters in relation to P. africana in the regions of Adamaoua and the South-West by developing a volume rate which makes it possible to estimate the production for a new stem. The work took place in two phases: in the South-West in 2010 and in Adamaoua in 2011. Data collection used the semi-direct method, while the cubing equation was deduced by the multiple linear regression method. Two models for volume estimation and three models for mass prediction were developed. The predictive parameters retained are diameter, height of the bole and thickness of the bark. Results show that the average mass of the dry bark for a given P. africana tree species is 27.55 ± 14.44 kg and this varies according to the site. The strong adjusted coefficient of determination (adjusted R2) observed illustrates the reliability of the proposed models. These models provide a reliable tool that can be adopted as a standard in Cameroon for P. africana.

Antibiotic resistance has become a major threat to human health worldwide. Environment, particularly the water environment, has long been overlooked as a player in the antibiotic resistance cycle, although its role remains unclear. These can provide an ideal setting for the acquisition and dissemination of antibiotic resistance, as they are frequently affected by anthropogenic activities. The objective of this study was to establish a diffusion map of resistance integrons used as genetic markers of resistance associated with antibiotic resistance conferring genes (ARGs). Total DNA extracts from non-cultivable bacterial communities were used for the analyses. These communities were obtained from wastewater samples from 14 sites upstream and downstream of drainage channels or effluents in the cities of Abidjan, Bouaké, and Yamoussoukro. The results obtained correspond to the number of positives among the treated samples (n = 39). Among the genetic markers of dissemination, class 1 integrons were the most evident in 94.8% of samples in Abidjan (93.3%), Bouaké (100%) and Yamoussoukro (91.6%). Class 2 integrons and class 3 integrons were found respectively in 41% and 51% of all samples. Genes coding for β-lactamases and blaTEM was identified in almost all samples at a rate of 97.4%. A co-presence of the three genes blaTEM, blaSHV and blaCTX-M is also remarkable in the sites of the city of Yamoussoukro. Among the genes coding for carbapenemases, only blaKPC 17.94%, blaNDM 30.76% and blaOXA48 38.46% were detected in the samples.

Introduction: Despite the advances of interventional catheterization, surgery remains the treatment of choice for some coronary lesions. Objective: To report the indications and results of surgical revascularization of the myocardium at the Abidjan Heart Institute. Patient and Methods: This is a retrospective study of patients with coronary insufficiency who underwent surgical myocardial revascularization between March 2014 and May 2020 in the Cardiovascular Surgery Department of the Abidjan Heart Institute. There were 17 patients, 11 of whom were men (64.7%) and 6 women (35.3%), The mean age of the patients was 57.5 years ± 8.8. All patients were symptomatic with disabling angina in class III of the Canadian Cardiac Society (CCS). This sym- ptomatology had been evolving on average for 5 years and 6 patients had a history of acute coronary syndrome, 2 of whom had undergone prior angioplasty. Coronary angiography revealed mono-truncated (17.6%), bi-truncated (23.5%) and tri-truncated (58.8%) lesions. Results: The patients were operated under cardiopulmonary bypass (CPB) 15 cases (88.2%) and off pump in 2 cases (11.8%). They underwent a single bypass in 23.5% of cases, a double bypass in 47.1% of cases and a triple bypass in 29.4% of cases. We observed 3 cases of complications (17.6%), namely transient acute renal failure, mediastinitis and postoperative bleeding. The operative and hospital mortality was nil. Introduction: Jaw osteitis is an inflammatory disease of bone tissue causing aseptic necrosis of bone. This study aims to clarify the etiologies of osteitis of the jawbones and to describe the clinical and radiographic aspects of extensive forms. Cases Presentation: Our study concerns the clinical and radiographic examination of 4 cases of voluminous bone sequesters. The causes of osteitis are mainly infectious and due to late consultations of our populations. The development of these maxillary osteitis leads to the formation of large bone sequesters. Conclusion: The evolution towards voluminous sequesters is a combination of dental infection, poor oral hygiene, late consultation and traditional treatment.

Epidemiological Aspects of Cervicofacial Cellulitis Due to Dental Origin in the City of Ouagadougou (Burkina Faso)

Background: Head and neck cellulitis of dental origin are polymicrobial bacterial infections involving the cellulo-adipose spaces of the face and neck. The objective of this work was to describe the epidemiological and clinical profile of patients with head and neck cellulitis. Methods: This was a cross-sectional study conducted from 1 January to 30 June 2020. All patients consulting for cervico-facial cellulitis of dental origin in 2 university hospitals in Ouagadougou were included in the study. The usual parameters of descriptive statistics were estimated for each variable. Results: Two hundred and ninety-two cases (184 men and 108 women), aged 3 to 85 years were collected. The 25 - 35 years old were the most affected (33%). Decay was the leading dental cause (95.6%). The delay in consultation was included within 7 days in 74.66% of cases. Ninety-one-point forty-four percent of patients had poor oral hygiene. Diffuse cellulitis was the most common (64.04%) and peri-mandibular regions were the most invaded (42.81%). Conclusion: The frequencies observed in our study allow us to conclude that cervicofacial cellulitis of dental origin is still frequent and affects a young and disadvantaged population.

Haematological and Biochemical Variations in Cervico-Facial Cellulitis of Dental Origin in the City of Ouagadougou (Burkina Faso)

Background: Cervicofacial cellulitis is a severe infection and is responsible for a major disruption of host homeostasis. The aim of this work was to describe the haematological and biochemical variations of cervico-facial cellulitis. Methods: This was a cross-sectional study conducted from 1 January to 31 December 2020. All patients hospitalized for cervico-facial cellulitis of dental origin were included in the study. The usual parameters of descriptive statistics were estimated for each variable. Results: Our sample consisted of 166 patients. The average age of patients with cervicofacial cellulitis in our study was 39.9 ± 15.39 years. Red blood cell count was low in 55.5% of patients and hyperleukocytosis was noted in 89.16% of patients. Platelets were normal in 43.14% of patients and 78.43% of patients had a low haematocrit (haemodilution). Anemia was noted in 64.7% of patients. In addition, 61.0% of patients had elevated uricemia and 31.17% had elevated creatinemia. Conclusion: Cellulitis is still a common condition in less privileged environments. Its diagnosis can be guided by clinical and biological findings. Consistent education of the population on oral health, and the control of biological disorders that result from it, are sufficient to greatly reduce its prevalence and the appearance of complicated forms.
