Introduction: Depression is common in HIV/AIDS. We aim to investigate the frequency and the grade of this mental disorder in partners and determine its influence on sexual dysfunctions in serodiscordant couples in Ouagadougou. Patients/Method: This cross-sectional study was monitored from 1 January to 31 June 2010 in the internal medicine department of CHUYO, Ouagadougou. HIV-infected patients and their seronegative partners who gave their consent were included. Depression was diagnosed using the Beck Depression Inventory 13 items (BDI-13). Results: Eighty heterosexual and monogamous serodiscordant couples were studied. 31 (38.7%) HIV-infected and 23 (28.7%) seronegative partners were affected by depression;p = 0.18. The score of depression was 3.5 ± 2.1 in HIV-infected and 3.3 ± 1.6 in seronegative partners;p = 0.85. Depression was diagnosed in both partners in 13 couples (16.2%) and in only one partner in 28 couples (35%). Depression was diagnosed at least for one partner in 40 (51.3%) couples where sexual dysfunctions affected partner (s) and in 1 (50%) couple where no sexual dysfunction was reported. Conclusion: Psychological assessment is needed for a better management of HIV/AIDS in serodiscordant couples.

Trends of Kalemia at Diagnosis of Acidosis versus Non-Acidosis Diabetic Ketosis Décompensations in Ouagadougou (Burkina Faso)

Introduction: Electrolyte’s profile in non-acidosis diabetic ketosis is poorly specified. We aimed to determine the nature of diabetic ketosis decompensations as well as the profile of kalemia and factors associated with its disorders at diagnosis of acidosis compared to non-acidosis diabetic ketosis. Methods: The study was retrospective from 1 January 2010 to 31 December 2011 in Yalgado Ouédraogo teaching hospital. Diabetic in-patients suffering from simple ketosis, keto-acidosis or mixed decompensation, who achieved blood electrolytes assessment before intensive insulin therapy were included. Results: Sixty two patients were studied. The sex ratio was 0.7 and the mean age was 41.7 years. Keto-acidosis, simple ketosis and mixed decompensation were diagnosed respectively in 18 (29%), 32 (51.6%) and 12 (19.4%) patients. Kalemia was normal in 42 (67.7%), while hypokalemia and hyperkalemia were reported respectively in 11 (17.8%) and 9 (14.5%) patients. Kalemia was often normal in all types of ketosis decompensation and disorders of kalemia occurred more in patients with keto-acidosis (50%) than those with simple ketosis (21.9%);p = 0.04. Renal failure was diagnosed in 10 patients (50%) with and 2 (4.8%) without kalemia’s disorders;p = 0.0001. Seven patients (35%) with and 4 (9.5%) without kalemia’s disorders suffered from unconsciousness;p = 0.02. It happens more in hyperkalemia (44.4%) than in normal kalemia condition (9.5%);p = 0.02. Conclusion: If kalemia is often normal in all types of diabetic ketosis emergencies, hypokalemia is the most initial frequent potassium disorder.

Introduction: The periodic management of diabetes improves disease’s prognosis. We aimed to evaluate the quality of diabetes annual management and the effectiveness of its treatment in Ouagadougou. Material and Method: It was a cross-sectional and descriptive study, from retrospective records in the internal medicine department, Yalgado Ouédraogo Teaching Hospital (CHU-YO). Diabetic patient’s (15 years old and more) files that were registered between January, 2012 and December, 2016 have been analyzed. Files containing less than 80% data were excluded. Data relating to the baseline characteristics of the patients, the therapeutic education dispensation, the clinical and paraclinical assessment of the initial and annual visits at the first year of patient’s follow-up have been collected. The 2007 diabetes management standards of the French “Haute Autorité de Santé” as well as the biological standards of the same instance have been used. The quantitative variables were expressed as means and standard deviations and qualitative variables were expressed as absolute and relative frequencies. Results: 317 patients, including 218 women (68.77%) were studied. The mean age of women was 51.67 ± 12.46 years, and that of men 55.71 ± 10.63. Diabetes was type 2 in 302 (95.26%) patients. The mean duration since the diagnosis of diabetes was 2.9 years. The completeness rate of therapeutic education at the annual visit was 10.46%. That of the clinical examinations was at best 38.17% and 44.23% for the measurement of weight and blood pressure;otherwise it was less than 5%. About the paraclinical data, apart from the measurement of the fasting blood glucose which completeness rate was 71.61%, that of the others exams were less than 10%. At the annual visit, the mean value of HbA1C was 7.5%, and the target for HbA1C was achieved in 47.90% patients. Conclusion: The quality of diabetes annual management is unsatisfying.

Introduction: Hypertriglyceridemia is a frequent dyslipidemia in type 2 diabetes. We aimed to determine its prevalence, its typology, and its associated factors in newly type 2 diabetes patients in Ouagadougou. Material and Method: This cross-sectional, descriptive and analytic study has been performed from May 2015 to June 2016 in the Internal medicine department in Yalgado Ouédraogo teaching hospital, Ouagadougou. An accidental sample of newly type 2 diabetes outpatients, na?ve of antidiabetics therapy was studied. A 12-hour fasting triglyceridemia was performed;hypertriglyceridemia was defined for triglyceridemia > 1.5 g/L (1.7 mmol/L). Data were analyzed with Epi info 7.***.5.0. Proportions and means were compared respectively with Khi2 or Fisher’s test and Student’s test with a significance of p Results: One hundred and three patients, i.e. 35 (34%) men and 68 (66%) women were included. The sex ratio was 0.51 and the mean age 49.3 ± 10.1 years [limits: 24 and 70]. The mean value of triglyceridemia was 1.8 ± 1.7 mmol/L [limits: 0.5 and 14.9]. Hypertriglyceridemia was reported in 32 patients (31.1%): minor (11.30 mmol/L) in one (3.1%) patients. Hypertriglyceridemia’s phenotype was commonly integrated into a mixed dyslipidemia;it was often combined with a total cholesterolemia increase and a HDL cholesterolemia decrease. In bivariate analysis, hyperglycemia was associated with severe hyperglycemia (p = 0.006) and renal failure (p = 0.03). Conclusion: Hypertriglyceridemia in type 2 diabetes is frequent and often combined with other lipids disorders. It may need an optimal care.

Septic arthritis is a medical emergency. The causative organisms are primarily staphylococcus and streptococcus. Aeromonas hydrophila septic arthritis seems to be rare. We report one case observed in a 58-year-old patient infected with HIV serotype 1 and treated by zidovudine, lamivudine and nevirapine. CD4 count was 413 cells per microliter and viral load undetectable. Two blood cultures were A. hydrophila positive. The evolution was favorable after treatment by ceftriaxone and gentamicin.

Objective: To study the quality of life of patients with knee OA with the questionnaire OAKHQOL (OsteoArthritis of Knee Hip Quality Of Life). Patients and Methods: This was a cross-sectional study over a period of six months, in patients with symptomatic knee osteoarthritis. All patients met the clinical and radiological criteria of the ACR. The OAKHQOL questionnaire was used to investigate the quality of life of patients. Results: One hundred and six patients were included. There were 94 (88.7%) women and 12 (11.3%) men with a sex ratio of 0.12. The average age of patients was 55 ± 10.4 years. The average pain intensity was 55.1 ± 22.4. The majority of patients (69.8%) had stage 3 of Kellgren and Lawrence classification. The dimension “pain” had the lowest normalized score (60.6). There was a significant correlation between the age range of 30 to 40 years and declining mental health (49.5) . The patients with over 60 years of age had a decrease in pain tolerance and reduced social activities. Lequesne score was associated with severe all dimensions except “social support” of the patient. Conclusion: The knee has an impact on quality of life of patients followed in rheumatology by OAKHQOL questionnaire.

AIM: To design a medical cost calculator and show that diabetes care is beyond reach of the majority particularlypatients with ***: Out-of-pocket expenditures of patients for medical treatment of type-2 diabetes were estimated based on price data collected in Benin,Burkina Faso,Guinea and Mali. A detailed protocol for realistic medical care of diabetes and its complications in the African context was defined. Care components were based on existing guidelines,published data and clinical experience. Prices were obtained in public and private health facilities. The cost calculator used Excel. The cost for basic management of uncomplicated diabetes was calculated per person and per year. Incremental costs were also computed per annum for chronic complications and per episode for acute complications. RESULTS: Wide variations of estimated care costs were observed among countries and between the public and private healthcare system. The minimum estimated cost for the treatment of uncomplicated diabetes(in the public sector) would amount to 21%-34% of the country's gross national income per capita,26%-47% in the presence of retinopathy,and above 70% for nephropathy,the most expensive complication. CONCLUSION: The study provided objective evidence for the exorbitant medical cost of diabetes considering that no medical insurance is available in the study countries. Although the calculator only estimates the cost of inaction,it is innovative and of interest for several stakeholders.

Background: Diabetes mellitus is a real public health problem worldwide and is growing in developing countries. Our aim was to determine the prevalence of undiagnosed type 2 diabetes mellitus (T2DM) and to evaluate the performance of the Finnish Diabetes Risk Score (FINDRISC) questionnaire to identify type 2 diabetics in the urban population of Ouagadougou, Burkina Faso. Methodology: This was a cross-sectional study among volunteers aged 18 years and older recruited systematically from November 11 to 16, 2019, in the city of Ouagadougou, Burkina Faso, FINDRISC score was used. Analysis of the Receiver Operating Characteristic (ROC) curve was used to study the diagnostic performance of FINDRISC for the identification of type 2 diabetics. The optimal threshold was determined by the sum of the highest sensitivity and specificity. Multivariate logistic regression was used to analyze the association of each variable used in the calculation of the FINDRISC score. Results: A total of 1276 individuals were included in the analyses, of which 667 (52.27%) were women. The average age was 34.16 years (SD: ±12.42). The prevalence of T2DM was 10.74%. The mean FINDRISC score was 5.85 (SD: ±4.31). The majority (58.54%) of individuals had a low risk of diabetes according to the FINDRISC score 0 - 7, while 3.61% had a score ≥ 15. The FINDRISC score showed good performance (AUC = 0.70) in identifying undiagnosed type 2 diabetics. Background: Diabetes mellitus and physical inactivity are modifiable risk factors for stroke. Physical activity is one of the pillars of type 2 diabetes management. The aim of this study was to evaluate the physical fitness of a sample of type 2 diabetes patients at the CHU of Martinique (UHCM). Patients and Methods: This was a descriptive cross-sectional study, from 1st June to 30th September 2018. It consisted of the exhaustive prospective collection of data on the usual physical activity and physical fitness of type 2 diabetic patients hospitalized in the Endocrinology department of the UHCM for a short stay. The physical fitness tests of the French National Olympic and Sports Committee, validated on 13 April 2013, were used. Results: Thirty patients were included in the study. The mean age was 51.9 ± 11 years. The sex ratio was 1.5, and hypertension in 53% (n = 16) of patients was the most associated risk factor. The median duration of diabetes progression was 4 years with Q1 and Q2 quantiles of 2 and 10 years, respectively. The physical fitness evaluation noted a balance disorder with a risk of falling in 56.7% (n = 17) of patients, muscle stiffness in 20% (n = 6) of them, while 56.7% (n = 17) and 80% (n = 24) had a decrease in the muscular strength of the upper and lower limbs, respectively. The distance covered during the standard 6 min walking test was normal for 70% (n = 21) of the patients. Conclusion: The evaluation of the patient’s physical fitness allows us to assess their deconditioning to physical activity and other obstacles to physical activity to plan appropriate interventions.

Several studies have shown the importance of therapeutic education in the management of diabetes. Its implementation requires a prior assessment of the level of patients’ knowledge. The objective of this study was to assess the level of diabetics’ knowledge about their disease. Methods: This was a cross-sectional study conducted between 2013 and 2015;consenting diabetics, followed for at least one year in medicine department of Souro Sanou Hospital, have been surveyed. The data were collected using a 51-item questionnaire, adapted from “Audit of Diabetes Knowledge” and administered by an interviewer. The level of knowledge was satisfactory/good if at least 80% of the set items or a topic were known. Results: A total of 268 diabetics were surveyed. Their mean age was 54.3 ± 1.4 years and the sex ratio 0.8. The majority (80.9%) lived in urban areas and half (50.4%) were uneducated. Type 2 diabetics accounted for 93.3% of the population;the average seniority was 6.0 ± 0.7 years. The level of knowledge was generally insufficient, with an average total score of 25.4 ± 1.0/51. The topics which were not well mastered related to diet (0.8%), glycated haemoglobin (4.5%), and signs of hypoglycemia (7.8%). The main relatively well-known topics related to the effect of physical exercise on glycemia = 37.7%, the effect of food on glycemia = 36.9% and glycemia target = 32.8%. Conclusion: This study revealed a lack of knowledge from diabetic patients who are followed in the medicine department of the Teaching Hospital Souro SANON (CHUSS). The implementation of a therapeutic education program for diabetics is necessary and urgent.
