Pancreatic tuberculosis is an extremely rare form of extrapulmonary disease. The diagnosis preoperatively is difficult because clinical, laboratory and radiologic findings are nonspecific. Published data indicate that these lesions mimic cystic neoplasms of the pancreas and the confirmation of clinical suspicion could only be obtained by an open surgical biopsy. Recently, fine needle aspiration cytology has been shown to be a safe, reliable and cost-effective alternative. We report a new case of a peripancreatic tuberculosis in a 52 year old woman and review the relevant literature, paying special attention to the usefulness of endoscopic ultrasound guided-fine needle aspiration in the diagnosis of abdominal tuberculosis.

The goal of efficient and robust error control, through local mesh adaptationin the computational solution of partial differential equations, is predicated on theability to identify in an a posteriori way those localized regions whose refinement willlead to the most significant reductions in the error. The development of a posteriori errorestimation schemes and of a refinement infrastructure both facilitate this goal, howeverthey are incomplete in the sense that they do not provide an answer as to where themaximal impact of refinement may be gained or what type of refinement — elementalpartitioning (h-refinement) or polynomial enrichment (p-refinement) — will best leadto that gain. In essence, one also requires knowledge of the sensitivity of the error toboth the location and the type of refinement. In this communication we propose theuse of adjoint-based sensitivity analysis to discriminate both where and how to *** present both an adjoint-based and an algebraic perspective on defining and usingsensitivities, and then demonstrate through several one-dimensional model problemexperiments the feasibility and benefits of our approach.

Spinal cord injury(SCI)can permanently impair motor and sensory function and has a devastating cost to patients and the United States healthcare *** cell transplantation for treatment of SCI is a new technique aimed at creating biological functional *** techniques in stem cell transplantation for SCI are *** review various clinical treatment paradigms,surgical techniques and technical considerations important in SCI *** NCBI PubMed database was queried for“SCI”and“stem cell”with a filter placed for“clinical trials”.Thirty-nine articles resulted from the search and 29 were included and evaluated by study authors.A total of 10 articles were excluded(9 not SCI focused or transplantation focused,1 canine model).Key considerations for stem cell transplantation include method of delivery(intravenous,intrathecal,intramedullary,or excision and engraftment),time course of treatment,number of treatments and time from injury until *** are no phase III clinical trials yet,but decreased time from injury to treatment and a greater number of stem cell injections both seem to increase the chance of functional recovery.

This paper presents the results of thermal comfort and air quality studies in naturally ventilated residential buildings in Kampala City,*** surveys were used for obtaining occupant subjective thermal sensation *** and outdoor measurements for air temperature(T_(a)),Mean Radiant Temperature(MRT),relative humidity,air speed,and air quality were done for seven buildings over fifteen days during the month of June *** software was used to develop the reference building model and to simulate strategies for improvement of thermal *** results showed that 67.8%of the respondents were comfortable with the indoor thermal *** indoor air temperature range was 22.7℃-27.9℃ which lies within the 80%acceptable limits set by ASHRAE *** measured indoor MRT range was 24.24℃-25.57℃.Measured levels of indoor PM_(2.5)were double the World Health Organization recommended limits,while the levels of PM 10 were thrice the recommended limits of ASHRAE *** CO_(2)concentrations were within the limits set by ASHRAE *** developed model predicted comfortable indoor conditions with a temperature range of 23.6℃-25.3℃ based on the 80%acceptable limits set by ASHRAE *** results show that majority of occupants preferred cooler temperatures during the day which is justified by the use of adaptive measures to obtain thermal comfort.

Objective: To describe the epidemiological, clinical and paraclinical characteristics of osteoarthritis of the lower limbs in obese patients at the Cocody University Teaching Hospital. Methodology: This was an analytical cross-sectional study carried out in the rheumatology department of the Cocody UTH in Abidjan (Ivory Coast) from March 1 to April 30 2023. Patients who came for rheumatology consultation presenting with mechanical arthralgia of the lower limbs, who were obese, had radiographic images were included. All patients without radiographic images were excluded. Obesity was defined as a body mass index (BMI) greater than or equal to 30 kg/m. The Chi2 test was used to compare proportions and determine the existence of associations between osteoarthristis and obesity, obesity severity and radiographic stage of osteoarthritis as well as the functional impact. A p-value below a predefined threshold (p = 0,05) indicates a significant relationship between the variables. Result: Out of 185 patients received for osteoarthritis of the lower limbs during the study period, 136 were obese (74%). There were 115 women (84.6%) with an average age of 56.03 with a standard deviation of 12.72 years (extremes: 22 and 84 years). The main socio-professional category was the informal sector (30%). The majority of patients had a low socio-economic level (80.2%) and lived in urban areas (92.6%). The most common past medical history was hypertension (33.08%) followed by peptic ulcer disease (16.91%). Patients had a body mass index class 1 (81.6%), class 2 (15.40%) and class 3 (2.90%). The average duration of symptom progression until diagnosis was 11 months. Genu varum was the main static disorder (56.10%) and the knee joint was the dominant topography (90.4%) with a bilateral localization (80%). The average Lequesne index was greater than 8 (59.5%). The Kellgren and Lawrence radiographic stages were stage 1 (9.20%), stage 2 (46.90%), stage 3 (29.20%) and stage 4 (6.90%).

To the Editor:Choledochal cysts,now generally termed biliary cysts to include intrahepatic cysts,are a rare lesion with unknown incidence[1].These cysts more often affect females than males(4:1)[2].Often associated with the pediatric population of congenital etiology,some series have found adults equally affected[3].Classification of biliary cysts follows the Todani classification with six groups[4].

This paper is concerned with the dispersion of particles on the fluid-liquid interface. In a previous study we have shown that when small particles, e.g., flour, pollen, glass beads, etc., contact an air-liquid interface, they disperse rapidly as if they were in an explosion. The rapid dispersion is due to the fact that the capillary force pulls particles into the interface causing them to accelerate to a large velocity. In this paper we show that motion of particles normal to the interface is inertia dominated; they oscillate vertically about their equilibrium position before coming to rest under viscous drag. This vertical motion of a particle causes a radially-outward lateral （secondary） flow on the interface that causes nearby particles to move away. The dispersion on a liquid-liquid interface, which is the primary focus of this study, was relatively weaker than on an air-liquid interface, and occurred over a longer period of time. When falling through an upper liquid the particles have a slower velocity than when falling through air because the liquid has a greater viscosity. Another difference for the liquid-liquid interface is that the separation of particles begins in the upper liquid before the particles reach the interface. The rate of dispersion depended on the size of the particles, the densities of the particle and liquids, the viscosities of the liquids involved, and the contact angle. For small particles, partial pinning and hysteresis of the three-phase contact line on the surface of the particle during adsorption on liquid-liquid interfaces was also important. The frequency of oscillation of particles about their floating equilibrium increased with decreasing particle size on both air-water and liquid-liquid interfaces, and the time to reach equilibrium decreased with decreasing particle size. These results are in agreement with our analysis.

The Impact of US Embargo on Cuban Government and People

The US embargo on Cuba has been and continues to be a very debatable topic that generates much discussions and debate in United States of America, Cuba and worldwide. My research question is why was the embargo imposed and how it affected the foreign policy and the people of Cuba and my argument is to show how the government and the people of Cuba were able to survive despite the embargo. This dissertation paper aims at analyzing the impact of US Embargo on Cuba and the Cuban people. This dissertation is divided into four chapters. Chapter1looks at Cuba’s history, all the events that took place which influenced the imposition of the embargo. How the political decision to choose a socialist system taken by the revolution was influential in the US decision to establish an embargo on Cuba. In the second chapter, it examines the Cuba relations with other countries like the Soviet Union, Venezuela and their relationship with other organizations how these factors were instrumental in helping Cuba survive the US Embargo. Also it directly looks at the US Embargo on Cuba and factors that influenced the US policy on Cuba. The third Chapter addresses the Economy of Cuba;Education and the Health Systems of Cuba, in terms of how they were affected by the US Embargo and how the Cuban government, their leaders and the people was able to keep the economy and more so their education and health system afloat despite the US Embargo. The forth chapter looked at the developments under Raul Castro including his major decisions, President Obama policy towards Cuba and the Embargo presently today. Following a comparatively aspect and an extensive study of the US Embargo on Cuba, it can be concluded that the United States of America motivation for establishing an Embargo and it objectives of having an Embargo on Cuba, did not succeed in the way that the US Government was hoping for.
