Globally, air pollution is a significant cause of death, illness and social discomfort. The problem is particularly severe in Nigeria, the country with the highest number of premature deaths due to ambient particulate matter pollution in Sub Saharan region. It is especially worrying in Lagos, the country’s commercial capital and one of the world’s fastest growing megacities. Despite growing concerns about its deadly impacts, there is currently no reliable monetary estimate of the effects of ambient air pollution, nor a comprehensive control plan in Lagos. Using available ground-level monitored data and the most recent valuation techniques, this paper estimates that in 2018 alone, ambient fine particulate matter (PM2.5) caused about 11,200 premature deaths, and generated a health cost of US$2.1 billion in Lagos. This is equivalent to about 2.1 percent of Lagos’ GDP in the same year. These results call for an urgent plan of action to improve air quality in the city, with primary focus on the main pollution sources: road transport, industrial emissions, and power generation.

热带雨林是地球的生物多样性宝库,近年人类活动导致全球热带雨林面积不断减少,使得保护雨林、保护环境,促进可持续发展成为当今世界各个国家和人民的共同愿望。从20世纪50年代至今,西双版纳森林覆盖面积减少了近30%,大部分转变为人工橡胶林。面对经济发展与不良生态效应之间的矛盾关系,以政府为主导的有偿"退胶"是在一定程度上拯救雨林的直接途径。本研究采用土地期望值理论和复利计算法,估算出西双版纳曼旦村橡胶林的土地期望值和森林价值,进而制定出"退胶还林"经济补偿标准,并采用了以20年为年限分期付款、征收/租赁的补偿方式。这一研究结果在理论和实践两方面为"退胶还林"面临的实际问题提供了科学的解决方案和参考依据。为促进"退胶还林"的实施,本文还建议在大规模"退胶还林"计划实施之前,要采取政策和措施,尽可能缓解大面积橡胶林对西双版纳当地生态环境的负面影响。

BACKGROUND： Effective pain management among hospitalized patients is an important aspect of providing quality care and achieving optimal clinical outcomes and patient satisfaction. Common pharmacologic approaches for pain, though effective, have serious side effects and are not appropriate for all inpatients. Findings from randomized controlled trials（RCTs） support the efficacy of acupuncture for many symptoms relevant to inpatients including postoperative pain, cancer-related pain, nausea and vomiting, and withdrawal from narcotic use. However, the extent to which findings from RCTs translate to real-world implementation of acupuncture in typical hospital settings is unknown. METHODS/DESIGN： In partnership with the launch of a clinical program offering acupuncture services to inpatients at the University of California San Francisco’s Mount Zion Hospital, we are conducting a pilot study using a hybrid effectiveness-implementation design to：（1） assess the effectiveness of acupuncture to manage pain and other symptoms and improve patient satisfaction; and（2） evaluate the barriers and facilitators to implementing an on-going acupuncture service for inpatients. During a twomonth pre-randomization phase, we evaluated and adapted clinical scheduling and treatment protocols with acupuncturists and hospital providers and pretested study procedures including enrollment, consent, and data collection. During a six-month randomization phase, we used a two-tiered consent process in which inpatients were first consented into a study of symptom management, randomized to be offered acupuncture, and consented for acupuncture if they accepted. Background and Aims:In the REALM (Randomized, Obser-vational Study of Entecavir to Assess Long-Term Outcomes Associated with Nucleoside/Nucleotide Monotherapy for Pa-tients with Chronic HBV Infection) study, 12,378 patients with chronic hepatitis B virus (HBV) infection received up to 10 years of randomized therapy with entecavir or another HBV nucleos(t)ide analogue. Monitored clinical outcome events (COEs) included malignant neoplasms, HBV disease progres-sion events, and deaths. An external event adjudication com-mittee (EAC) was convened to provide real-time review of reported COEs to optimize data quality, and minimize poten-tial adverse effects of the large cohort, interdisciplinary out-come assessments, geographic scope, and long duration. Methods:The EAC comprised an international group of hep-atologists and oncologists with expertise in diagnosis of tar-geted COEs. The EAC reviewed and adjudicated COEs according to prospectively defined diagnostic criteria cap-tured in the EAC charter. Operational processes, including da-ta collection and query procedures, were implemented to optimize efficiency of data recovery to maximize capture of adjudicated COEs, the primary study outcome measure. Results: A total of 1724 COEs were reported and 1465 of these events were adjudicated by the EAC as reported by the investigators (85.0% overall concordance). Concordance by COE type varied: deaths, 99.6%;hepatocellular carcino-ma (HCC), 83.3%;non-HCC malignancies, 88.0%;non-HCC HBV disease progression, 68.2%. Reasons for lack of con-cordance were most commonly lack of adequate supporting data to support an adjudicated diagnosis or evidence that the event pre-dated the study. Conclusions: The REALM EAC performed a critical role in ensuring data quality and consis-tency;EAC performance was facilitated by well-defined diag-nostic criteria, effective data capture, and efficient operational processes. Dear Editor:Central nervous system tumors in the brain or spine are the most common solid tumors in children,which accounts for about 25%of cancers in children younger than 15 years of age,and are the most common cause of cancer deaths in children[1].The 5-year survival rate for central nervous system neoplasms has increased dramatically to 74%for patients under 18 years old(97%for benign/borderline malignant tumors)in 2022,compared to 20%in the 1970s[1].Without cure treatments and specific medications for many brain cancers,the dramatic increase of survival rate is largely due to improved hospital care and availability of clinical *** length of hospitalization of pediatric patients with brain cancer is an important indicator of prognosis as it reflects the required medical effort needed to care for these *** length of hospitalization is also critical for the healthcare system as hospital admission is a part of the care trajectory of the respective patients,related to the cost of medical care[2,3].
