Can tranexamic acid change preoperative anemia management during total joint arthroplasty?

AIM: To investigate the postoperative transfusion and complication rates of anemic and nonanemic total joint arthroplasty patients given tranexamic acid(TXA).METHODS: A cross-sectional prospective study was conducted of primary hip and knee arthroplasty cases performed from 11/2012 to 6/2014. Exclusion criteria included revision arthroplasty, bilateral arthroplasty, acute arthroplasty after fracture, and contraindication to TXA. Patients were screened prior to surgery, with anemia was defined as hemoglobin of less than 12 g/d L for females and of less than 13 g/d L for males. Patients were divided into four different groups, based on the type of arthroplasty(total hip or total knee) and hemoglobin status(anemic or nonanemic). Intraoperatively, all patients received 2 g of intravenous TXA during surgery. Postoperatively, allogeneic blood transfusion(AbT) was directed by both clinical symptomsand relative hemoglobin change. Complications were recorded within the first two weeks after surgery and included thromboembolism, infection, and wound breakdown. The differences in transfusion and complication rates, as well as the relative hemoglobin change, were compared between anemic and nonanemic ***: A total of 232 patients undergoing primary joint arthroplasty were included in the study. For the total hip arthroplasty cohort, 21%(18/84) of patients presented with preoperative anemia. Two patients in the anemic group and two patients in the nonanemic group needed AbTs; this was not significantly different(P = 0.20). One patient in the anemic group presented with a deep venous thromboembolism while no patients in the nonanemic group had an acute complication; this was not significantly different(P = 0.21). For nonanemic patients, the average change in hemoglobin was 2.73 ± 1.17 g/d L. AIM:To determine the impact of different characteristics on postoperative outcomes for patients in a joint arthroplasty Perioperative Surgical Home(PSH) ***:A retrospective review was performed for patients enrolled in a joint arthroplasty PSH program who had undergone primary total hip arthroplasty(THA) and total knee arthroplasty(TKA).Patients were preoperatively stratified based on specific procedure performed,age,gender,body mass index(bMI),American Society of Anesthesiologists Physical Classification System(ASA) score,and Charleston Comorbidity Index(CCI) *** primary outcome criterion was hospital length of stay(LOS).Secondary criteria including operative room(OR) duration,trans-fusion rate,Post-Anesthesia Care Unit(PACU) stay,readmission rate,post-operative complications,and discharge *** each outcome,the predictor variables were entered into a generalized linear model with appropriate response and assessed for predictive relationship to the dependent *** level was set to ***:A total of 337 patients,200 in the TKA cohort and 137 in the THA cohort,were eligible for the *** two-third of patients were *** age averaged 64 years and preoperative bMI averaged 29 kg/*** majority of patients were ASA score Ⅲ and CCI score *** analysis,ASA score was the only variable predictive for LOS(P = 0.0011) and each increase in ASA score above 2 increased LOS by approximately 0.5 *** score was also the only variable predictive for readmission rate(P = 0.0332).bMI was the only variable predictive for PACU duration(P = 0.0136).Specific procedure performed,age,gender,and CCI score were not predictive for any of the outcome *** duration,transfusion rate,postoperative complications or discharge disposition were not significantly associated with any of the predictor ***:The joint arthroplasty PSH model reduces postoperative outcome variability for patients with different preo

Physiological Mechanism of Salicylic Acid for Alleviation of Salt Stress in Rice

Soil salinity is one of the most important problems of crop production in estuarine and coastal zones. Improvement in salt tolerance of major food crops is an important way for the economic utilization of coastal zones. This study proved that the application of salicylic acid(SA) improved the growth and yield under salt stress conditions and investigated its physiological mechanisms for salt tolerance. The investigation on the effect of SA for salt tolerance during germination showed that the decreased rates of germination and growth(in terms of shoot and root lengths) by the salt stress were significantly increased by the SA application(SA + NaCl). The treatment of SA to the high and low saline soils enhanced the growth, yield and nutrient values of rice. The effects of SA on Na^+, K^+ and Cl~– ionic accumulation were traced under salt stress condition by inductively coupled plasma optical emission spectrometry and ion chromatography. It was revealed that the increased accumulation of Na^+ and Clˉ ions by the salt stress were reduced by SA application. An increased concentration of endogenous SA level was detected from the SA-treated rice varieties(ASD16 and bR26) by liquid chromatography electrospray Ionization-tandem mass spectrometry. The activities of antioxidant enzymes such as superoxide dismutase, catalase and peroxidase were increased by salt stress whereas decreased by the SA application. The study proved that the application of SA could alleviate the adverse effects of salt stress by the regulation of physiological mechanism in rice plants. In spite of salt stress, it can be applied to the coastal and estuarine regions to increase the rice production.

Genomic insights into hybridization in a localized region of sympatry between pewee sister species(Contopus sordidulus × C. virens) and their chromosomal patterns of differentiation

background: The Great Plains of the United States includes a large number of hybrid and contact zones between bird species. The amount of gene flow between sister species in these zones ranges from very rare hybridization events to widespread and prevalent introgression. Some of these avian systems have been studied extensively, while others have been indeterminate of whether hybridization exists in areas of sympatry. Using genomic-level approaches allows investigation of genomic patterns of hybridization and gene flow between species—or lack ***: We investigated a narrow zone of sympatry in Nebraska, USA between pewee species(Contopus sordidulus and C. virens), for which no hybridization has been confirmed. We used thousands of single nucleotide polymorphisms to identify potential hybridization and investigate genomic patterns of differentiation between these two ***: We found evidence of multiple hybrid individuals in the contact zone. Little genomic variation was fixed between species, but a large proportion had differentiated allele frequencies between species. There was a positive relationship between genetic differentiation and chromosome ***: We provided the first conclusive evidence of hybridization between C. sordidulus and C. virens, in a region where secondary contact likely occurred due to human disturbance and habitat modification. The genomic patterns of differentiation affirm that these species split in the relatively recent past. Finally, the relationship of chromosome size and genetic differentiation may have resulted from differential rates of chromosomal recombination in songbirds and genetic differentiation between species largely due to genetic drift(possibly in concert with selection).

Effects of stable aqueous fullerene nanocrystal (nC_(60)) on Daphnia magna:Evaluation of hop frequency and accumulations under different conditions

We investigated the effects of environmental factors and properties of water-stable crystal fullerene （nC60） on the uptake of nC60 by Daphnia magna based on known accumulation in our laboratory. This study was performed for seven days using different environmental factors including temperature, pH, water hardness, concentration （density of particle）, and particle size. Results demonstrated that body burden of C60 increased with time in all experiments. body burden of C60 increased with increasing concentration and particle size, and uptake of particles 〉100 nm reached their maximums more quickly than those 〈100 nm. Under high hardness in aqueous systems with lower pH and high temperature, uptake was higher than those under opposite conditions. Uptake in all batch tests reached balance within five days. both nC60 properties and environmental factors influenced uptake of nC60 by D. magna in an aqueous system. Additionally, environmental factors may have affected accumulation by changing nC60 properties, which are critical to understand the accumulation of fullerenes in aqueous systems.

Improved optical enhancement using double-width plasmonic gratings with nanogaps

Plasmonic grating structures have been shown effective at increasing near-field optical enhancement. A doublewidth plasmonic grating design is introduced, where each period has two alternating metal widths separated by a nanogap. With this new design, analysis has shown that plasmonic resonances couple between each metal section,resulting in even greater optical enhancement compared with single-width gratings. The geometry that gives the greatest optical enhancement has been determined with a computational model. This work demonstrates that the increased enhancement is due to hybridized modes that couple between the two grating segments.

Climate Change in the Subtropical Jetstream during 1950–2009

A study of six decades（1950–2009） of reanalysis data reveals that the subtropical jetstream（STJ） of the Southern（Northern） Hemisphere between longitudes 0°E and 180°E has weakened（strengthened） during both the boreal winter（January,February） and summer（July, August） seasons. The temperature of the upper troposphere of the midlatitudes has a warming trend in the Southern Hemisphere and a cooling trend in the Northern Hemisphere. Correspondingly, the north–south temperature gradient in the upper troposphere has a decreasing trend in the Southern Hemisphere and an increasing trend in the Northern Hemisphere, which affects the strength of the STJ through the thermal wind relation. We devised a method of isotach analysis in intervals of 0.1 m s-1in vertical sections of hemispheric mean winds to study the climate change in the STJ core wind speed, and also core height and latitude. We found that the upper tropospheric cooling of the Asian mid-latitudes has a role in the strengthening of the STJ over Asia, while throughout the rest of the globe the upper troposphere has a warming trend that weakens the STJ. Available studies show that the mid-latitude cooling of the upper troposphere over Asia is caused by anthropogenic aerosols（particularly sulphate aerosols） and the warming over the rest of the global mid-latitude upper troposphere is due to increased greenhouse gases in the atmosphere.
