Effects of six months losartan administration on liver fibrosis in chronic hepatitis C patients: A pilot study

AIM： TO evaluate the safety and efficacy of chronic administration of Iosartan on hepatic fibrosis in chronic hepatitis C patients.METHODS： Fourteen patients with chronic hepatitis C non-responders （n = 10）, with contraindications （n = 2） or lack of compliance （n = 2） to interferon plus ribavirin therapy and liver fibrosis were enrolled. Liver and renal function test, clinical evaluation, and liver biopsies were performed at baseline and after Iosartan administration at a dose of 50 mg/d during the 6 mo. The control group composed of nine patients with the same inclusion criteria and paired liver biopsies （interval 6-14 mo）. Histological activity index （HAI） with fibrosis stage was assessed under blind conditions by means of Ishak＇s score. Subendothelial fibrosis was evaluated by digital image analyses.RESULTS： The changes in the fibrosis stage were significantly different between Iosartan group （decrease of 0.5±1.3） and controls （increase of 0.89±1.27; P〈0.03）. In the treated patients, a decrease in fibrosis stage was observed in 7/14 patients vs 1/9 control patients （P〈0.04）. A decrease in sub-endothelial fibrosis was observed in the Iosartan group. No differences were found in HAI after Iosartan administration. Acute and chronic decreases in systolic arterial pressures （P〈0.05） were observed after the Iosartan administration, without changes in mean arterial pressure or renal function.CONCLUSION： Chronic AT-Ⅱ type 1 receptor （AT1R） blockade may reduce liver fibrosis in patients with chronic hepatitis C.

Comorbidity negatively influences prognosis in patients with extrahepatic cholangiocarcinoma

AIM:To study the outcome and prognostic factors in a series of patients with extrahepatic cholangiocarcinoma and determine the impact of comorbidity on ***:A retrospective analysis of 68 patients with extrahepatic cholangiocarcinoma(perihilar,n = 37;distal,n = 31) seen at a single tertiary-care institution during the period 1999-2003 was *** on presentation,management,and outcome were assessed by chart *** confirmation was obtained in 37 cases(54.4%) .Comorbidity was evaluated by using the Charlson comorbidity index(CCI) .RESULTS:Mean age at diagnosis was 73.4 ± 11.5 *** was the most common symptom presented(86.8%) .Median CCI score was 1(range,0 to 4) .Nineteen patients(27.9%) underwent tumor *** biliary drainage was performed in 39 patients(57.4%) ,and 6 patients(8.8%) received only best supportive ***-free margin status(R0) was achieved in 15 cases(78.9% of resection group) .Baseline serum carbohydrate antigen 19-9(CA 19-9) level was revealed to be an independent predictor of surgical treatment(P = 0.026) .Overall median survival was 3.1 ± 0.9 mo,with 1-and 2-year survival rates of 21% and 7%,*** the univariate analysis,tumor resection,CCI score,and serum CA 19-9 levels correlated significantly with *** the multivariate analysis,only resection(HR 0.10;95% CI,0.02-0.51,P = 0.005) and a CCI score ≥ 2(HR 3.36;95% CI,1.0-10.9,P = 0.045) were found to independently predict ***:Tumor resection and comorbidity emerged as significant prognostic variables in extrahepatic *** evaluation instruments should be applied in the clinical management of such patients.

New insights into Wnt signaling alterations in amyotrophic lateral sclerosis: a potential therapeutic target?

Amyotrophic lateral sclerosis is a fatal neurodegenerative disorder characterized by upper and lower motor neuron degeneration, which leads to progressive paralysis of skeletal muscles and, ultimately, respiratory failure between 2–5 years after symptom onset. Unfortunately, currently accepted treatments for amyotrophic lateral sclerosis are extremely scarce and only provide modest benefit. As a consequence, a great effort is being done by the scientific community in order to achieve a better understanding of the different molecular and cellular processes that influence the progression and/or outcome of this neuropathological condition and, therefore, unravel new potential targets for therapeutic intervention. Interestingly, a growing number of experimental evidences have recently shown that, besides its well-known physiological roles in the developing and adult central nervous system, the Wnt family of proteins is involved in different neuropathologica conditions, including amyotrophic lateral sclerosis. These proteins are able to modulate, at least, three different signaling pathways, usually known as canonical(β-catenin dependent) and non-canonical(β-catenin independent) signaling pathways. In the present review, we aim to provide a general overview of the current knowledge that supports the relationship between the Wnt family of proteins and its associated signaling pathways and amyotrophic lateral sclerosis pathology, as well as their possible mechanisms of action. Altogether, the currently available knowledge suggests that Wnt signaling modulation might be a promising therapeutic approach to ameliorate the histopathological and functional deficits associated to amyotrophic lateral sclerosis, and thus improve the progression and outcome of this neuropathology.

Sex-dependent resilience to extreme drought events: implications for climate change adaptation of a South American endangered tree species

Background Recent changes in climatic trends are resulting in an increased frequency and intensity of extreme events, with unknown effect on ecosystem dynamics in the near future. Extreme drought episodes are recognized as disturbance factors capable of modifying forest dynamics and tree growth. Within this context, dioecious tree species may be impacted by climatic extremes, affecting male/female proportions and, consequently, reproductive processes and species persistence. Therefore, there is an urgent need for species-specific assessments of growth tolerance to extreme dry spells in dioecious tree species, to establish effective conservation strategies for these particular natural resources. Araucaria araucana(araucaria), an endangered dioecious Patagonian tree species, has recently undergone decay and mortality episodes in response to increasing dry climatic conditions. While sex-dependent tolerance to extreme drought episodes has been assessed in the species' humid distribution range, there is still a lack of information on the gender-based resilience of trees growing in the drier environments of the species' *** We reconstructed, through dendrochronological methods, the sex-dependent response of 105 araucaria individuals(55 female and 50 male trees) to five regional extreme dry spells employing a set of different indices. Resistance, recovery period, and average growth reduction of standardized tree-ring growth were examined, analysing the effect of biotic(sex, pre-drought stem tree growth) and abiotic(local climatic conditions before, during, and after extreme climatic episodes) factors on tree *** Sex influences only the species resistance to climatic disturbance, with male individuals showing lower tolerance to extreme drought events. CD271 as a marker to identify mesenchymal stem cells from diverse sources before culture

Mesenchymal stem cells, due to their characteristics are ideal candidates for cellular therapy. Currently, in culture these cells are defined by their adherence to plastic, specific surface antigen expression and multipotent differentiation potential. However, the in vivo identification of mesenchymal stem cells, before culture, is not so well established. Pre-culture identification markers would ensure higher purity than that obtained with selection based on adherence to plastic. Up until now, CD271 has been described as the most specific marker for the characterization andpurification of human bone marrow mesenchymal stem cells. This marker has been shown to be specifically expressed by these cells. Thus, CD271 has been proposed as a versatile marker to selectively isolated and expand multipotent mesenchymal stem cells with both immunosuppressive and lymphohematopoietic engraftment-promoting properties. This review focuses on this marker, specifically on identification of mesenchymal stem cells from different tissues. Literature revision suggests that CD271 should not be defined as a universal marker to identify mesenchymal stem cells before culture from different sources. In the case of bone marrow or adipose tissue, CD271 could be considered a quite suitable marker; however this marker seems to be inadequate for the isolation of mesenchymal stem cells from other tissues such as umbilical cord blood or wharton's jelly among others.

Background:The characterization of surface and canopy fuel loadings in fire-prone pine ecosystems is critical for understanding fire behavior and anticipating the most harmful ecological effects of ***,the joint consideration of both overstory and understory strata in burn severity assessments is often *** aim of this work was to assess the role of total,overstory and understory pre-fire aboveground biomass(AGB),estimated by means of airborne Light Detection and Ranging(LiDAR)and Landsat data,as drivers of burn severity in a megafire occurred in a pine ecosystem dominated by Pinus pinaster *** the western Mediterranean ***:Total and overstory AGB were more accurately estimated(R^(2) equal to 0.72 and 0.68,respectively)from LiDAR and spectral data than understory AGB(R^(2)=0.26).Density and height percentile LiDAR metrics for several strata were found to be important predictors of *** severity responded markedly and non-linearly to total(R^(2)=0.60)and overstory(R ^(2)=0.53)AGB,whereas the relationship with understory AGB was weaker(R ^(2)=0.21).Nevertheless,the overstory plus understory AGB contribution led to the highest ability to predict burn severity(RMSE=122.46 in dNBR scale),instead of the joint consideration as total AGB(RMSE=158.41).Conclusions:This study novelty evaluated the potential of pre-fire AGB,as a vegetation biophysical property derived from LiDAR,spectral and field plot inventory data,for predicting burn severity,separating the contri-bution of the fuel loads in the understory and overstory strata in Pinus pinaster *** evidenced relationships between burn severity and pre-fire AGB distribution in Pinus pinaster stands would allow the implementation of threshold criteria to support decision making in fuel treatments designed to minimize crown fire hazard.
