Risk of Surgical Site Infection in Patients Undergoing Total Knee Arthroplasty

The aim of this study was to determine the participation of risk factors in the infection in patients who underwent total knee arthroplasty. This is a prospective longitudinal study, descriptive and analytical, that was carried out between January 2013 and December 2015. Eighty-one procedures were performed in 78 individuals, 16 males and 62 females. Three subjects underwent the procedure on both knees. The following predictive factors of infection were identified: Age, gender, comorbidities, evaluation of functional physical status using the American Society of Anesthesiologists Physical Status Classification System (ASAPS), length of hospital stay, use of prophylactic antibiotics and duration of surgery. The data was tabulated in a database using a Microsoft Excel spreadsheet. For statistical analysis, the chi-square test, chi-square test for independence and Fischer’s exact test were applied, all at a significance level of 5% (0.05). Surgical site infection was more prevalent in males (23.5%), age over 70 years (25.0%), for left gonarthrosis (17.9%), in the presence of comorbidities (17.9%), when hospital stay was 6 or more days (27.8%), in total arthroplasties of left knee (17,9%), in surgical times of up to 2 hours (20%), in the use of prophylactic antibiotics (16.7%) and in level II classification in the ASAPS (18.8%). The independence test was not significant in any of the evaluated factors (p-value greater than 0.05). The most prevalent bacteria identified in cases of infection was Staphylococcus aureus (33.2%). Measures of ionizing radiation were observed in a tower which is 20 meters above the ground relative height. They show a distinct period of 24 hours oscillation during this Februaryl 1 to April 11, 2015. It is suggested that these oscillations originate from radon gas （222Rn） vented from the ground floor. One Russian specific Geiger tube with associated electronics developed at ITA was used, Monitoring of ionizing radiation from 30 keV to l0 MeV shows that the X and gamma rays up to 3.0 MeV is prevalent in the region comes from the Radon gas 222Rn that decay in 214Bi and 214pb in the energies of the gamma rays 0.269, 0.609 and 1.05 MeV respectively. Then, with this simple, portable instrument was possible to monitoring the dynamics of radon gas in the region every interval minutes.

Helical anchors are commonly used in Brazil for guyed transmission towers subjected to static and cyclic wind *** most cases,these anchors are installed in tropical residual soil,a micro-structured material in which the shear strength is provided by soil *** installation of a helical anchor,as the helical plate moves downward into the ground,the soil penetrated is sheared and ***,in this type of soil,anchor installation affects the soil shear strength significantly associated with a bonded ***,the cyclic responses of helical anchors in this type of structured soils are rarely *** address this problem,tests were conducted in a Brazilian residual soil to investigate the monotonic,cyclic and post-cyclic performances of single-helix *** tests used two instrumented single-helix anchors installed in this typical residual soil of sandstone,which is frequently observed in large areas in the southern *** testing results indicate that the disturbance caused by the anchor installation affected the monotonic uplift performance *** results of cyclic loading tests also show no significant degradation of helix bearing resistance and reduced displacement accumulation with increasing load *** is perhaps due to the soil improvement caused by previous loading,which then increases the stiffness response of the anchor.

MM： To investigate whether, under the influence of pol ypectomy, the incidence of adenoma decreases with age.METHODS： Consecutive patients with colonic adenomas identified at index colonoscopy were retrospectively selected if they had undergone three or more complete colonoscopies, at least 24 mo apart. Patients who had any first-degree relative with colorectal cancer were excluded. Data regarding number of adenomas at each colonoscopy, their location, size and histological classification were recorded. The monthly incidence density of adenomas after the index examination was estimated for the study population, by using the person-years method. Baseline adenomas were excluded from incidence calculations but their characteristics were correlated with recurrence at follow-up, using the X^42 test.RESULTS： One hundred and fifty-six patients were included （109 male, mean age at index colonoscopy 56.8 ± 10.3 years）, with follow-up that ranged from 48 to 232 mo. No significant correlations were observed between the number, the presence of villous component, or the size of adenomas at index colonoscopy and the presence of adenomas at subsequent colonoscopies （P = 0.49, 0.12 and 0.78, respectively）. The incidence of colonic adenomas was observed to decay from 1.4% person-months at the beginning of the study to values close to 0%, at 12 years after index colonoscopy.CONCLUSION： Our results suggest the sporadic formation of adenomas occurs within a discrete period and that, when these adenomas are removed, all neoplasia-prone clones may be extinguished.

Carbonates and organic matter in soils characterized by reflected energy from 350-25000 nm wavelength

The soil carbon pool which is the sum of soil organic carbon(SOC)and soil inorganic carbon(SIC)is the second largest active store of carbon after the oceans and it is an important component of the global carbon ***,accurate estimation of SOC and SIC as important carbon reservoirs in terrestrial ecosystems using fast,inexpensive and non-destructive methods is crucial for planning different climate change *** aim of the current research was to examine the effectiveness of Vis-NIR(visible and near-infrared spectroscopy:350-2500 nm)and MIR(mid-infrared spectroscopy:4000-400 cm-1)to characterize and estimate soil organic matter(SOM)and carbonates as main components of soil carbon stocks in Juneqan,Charmahal va Bakhtiari,*** do so,a total of 548 soil samples from this area were collected(October 2015)and analyzed in laboratory(August 2017).In order to develop models capable of predicting SOM and carbonates content,seven spectral preprocessing methods comprising Absorbance(Abs),De-trending(Det),Continuum removal(CR),Savitzky-Golay derivatives(SGD),standard normal variate transformation(SNV),multiplicative scatter correction(MSC)and Normalization by range(NBR)were conducted along with five multivariate methods including Random Forest(RF),Partial Least-Squares Regression(PLSR),Artificial Neural Network(ANN),Support Vector Machine(SVM)and Gaussian Process Regression(GPR).The content of carbonates caused spectral reflectance intensity to augment on several ranges of spectrum and strong absorption feature at 2338 nm in the Vis-NIR and 714,850,870,1796,2150 and 2510 cm-1 in the MIR spectra *** absorbed energy in several ranges,but also showed specific peaks in *** facts are associated with the structure of carbonates and SOM and its interaction with *** best combination of preprocessing and calibration models for carbonates quantification in Vis-NIR spectra was Det/PLSR(R2=0.74,RPD=2.19,RMSE=6.45).For SOM,it was Det/PLSR(R2=0.82,RPD=2.41,RM

This work describes the synthesis, antinociceptive and anti-inflammatory effect of O-prenyl derivatives: O-prenyl derivatives 2-(3-methylbut-2-enyloxy)acetophenone (L1) and 3-methoxy-4-(3-methylbut-2-enyloxy)benzaldehyde (V1). Treatment with L1- or V1-produced antinociceptive effect on classical pain models: acetic acid abdominal contortions and formalin test (first and second phases), and in hot plate or tale flick models in mice without changing the locomotors performance, and anti-oedematogenic or anti-inflammatory effect in vivo: carrageenan-induced paw oedema and peritonitis in mice. In addition, L1 and V1 derivatives reduced nitric oxide production on RAW 264.7 cells stimulated with lipopolysaccharide without changing the cell viability. Taken together, our results show for the first time that L1 and V1 can produce antinociception and modulate inflammatory response when administered in mice.

Rural economic development can differ intensely among municipalities within the same region. The economic activity disparity among them makes public policy actions difficult. It is possible to find highly efficient and globally competitive producers, as well as those producing for subsistence, in the same area. This disparity stands out the total productivity importance of the factors of production in the agricultural sector, especially the productivity of the land. The way the land is occupied in the rural area, namely crops, pastures, reforestation and other areas, can be indicative of the productivity of the land factor and the value of agricultural production. The products that compose the value of the agricultural production present different land occupation through their own productive characteristic. The main objective of this work was to measure the association between the production value of groups of agricultural products and the diversified uses of the rural area in the production of the municipalities in the state of Sao paulo. In this research, 52 agricultural products produced in 2008 were used, grouped in five production value variables and other nine variables of the land use in production of the municipalities in Sao paulo. The multivariate statistical technique of canonical correlation was used to measure the association between the product variables group of the production value with the land use group in agricultural activities. It was concluded that there is a strong correlation （94.3%） in the first pair of canonical variables, representing the production value and the land use, allowing groups of municipalities to be formed at different stages of development in agricultural production. It can be verified that 61.8% of the municipalities in the state were below the average in the production group and land use and that only 4.8% were above average for the production variables group and with values below the average in land use.
