BACKGROUND Portal vein arterialization(PVA)has been used in liver transplantation(LT)to maximize oxygen delivery when arterial circulation is compromised or has been used as an alternative reperfusion technique for complex portal vein thrombosis(PVT).The effect of PVA on portal perfusion and primary graft dysfunction(PGD)has not been assessed.All patients receiving PVA and LT at the Fundacion Santa Fe de Bogota between 2011 and 2022 were analyzed.To account for the time-sensitive effects of graft perfusion,patients were classified into two groups:prereperfusion(pre-PVA),if the arterioportal anastomosis was performed before graft revascularization,and postreperfusion(post-PVA),if PVA was performed afterward.The pre-PVA rationale contemplated poor portal hemodynamics,severe vascular steal,or PVT.Post-PVA was considered if graft hypoperfusion became evident.Conservative interventions were attempted before PVA.

The aim of thisstudy was to conduct a randomized, double-blind and placebo-controlled study to investigate the effects of D-004, a lipid extract of the Roystonea regia fruit that prevents testosterone- and phenylepinephrine- induced prostate hyperplasia in rodents, on plasma oxidative markers in healthy men. We enrolled male volunteers （20-55 years） in good health and without lower urinary tract symptoms. Thirty-four eligible participants were randomized to placebo or D-004 （320 mg） capsules administered daily for 6 weeks. An interim check-up and a final visit were conducted after 3 and 6 weeks of therapy, respectively. Physical examinations were performed at each visit, and laboratory tests were performed at baseline and at treatment completion. Oxidative variables included plasma malondialdehyde （MDA）, total hydroxyperoxides （TOH）, sulphydryl （SH） groups and total antioxidant status （TAS）. We assessed treatment compliance and addressed adverse experiences （AEs） at weeks 3 and 6. At week 6, with D-004, the mean reductions of plasma MDA （26.7%）, TOH （18.8%） and SH groups （31.6%）, and the mean increase of TAS （35.3%） were significantly different from those of placebo （P 〈 0.001 for plasma TAS, P 〈 0.0001 for all other comparisons）. D-004 did not differ from the placebo in safety indicators. There were two withdrawals （both in the D-004 group）, with one due to dyspepsia （the only AE during the trial）. In conclusion, D-004 displayed antioxidant effects on plasma oxidative markers in healthy men, which was consistent with findings from laboratory experimental studies.

Complications of hip fractures: A review

Nowadays, fracture surgery represents a big part of the orthopedic surgeon workload, and usually has associated major clinical and social cost implications. These fractures have several complications. Some of these are medical, and other related to the surgical treatment itself. Medical complications may affect around 20% of patients with hip fracture. Cognitive and neurological alterations, cardiopulmonary affections(alone or combined), venous thromboembolism, gastrointestinal tract bleeding, urinary tract complications, perioperative anemia, electrolytic and metabolic disorders, and pressure scars are the most important medical complications after hip surgery in terms of frequency, increase of length of stay and perioperative mortality. Complications arising from hip fracture surgery are fairly common, and vary depending on whether the fracture is intracapsular or extracapsular. The main problems in intracapsular fractures are biological: vascularization of the femoral head, and lack of periosteum-a major contributor to fracture healing- in the femoral neck. In extracapsularfractures, by contrast, the problem is mechanical, and relates to load-bearing. Early surgical fixation, the role of anti-thromboembolic and anti-infective prophylaxis, good pain control at the perioperative, detection and management of delirium, correct urinary tract management, avoidance of malnutrition, vitamin D supplementation, osteoporosis treatment and advancement of early mobilization to improve functional recovery and falls prevention are basic recommendations for an optimal maintenance of hip fractured patients.

Biology of the aphid Therioaphis maculata was studied on alfalfa （Medicago sativa L.）, including four resistant （mesa-Sirsa, CUF101, Baker and Lahontan） and two susceptible （ARC and Caliverde） alfalfa cultivars, and one of the most cropped Brazilian cultivars, Crioula. Under controlled conditions, antibiosis （i.e., reduced longevity, fecundity and increased mortality of the aphid） was observed mainly on the resistant alfalfa cultivars, except on Lahontan. Crioula seemed to be tolerant to aphids. Present data support geographic limitation usage of cultivars, and we suggest Baker and mesa-Sirsa as sources of antibiosis, and provide biological information of a tropical T. maculata biotype on alfalfa.

AIM: To study a cohort of consecutive patients under-going transcatheter aortic valve implantation (TAVI) and compare the outcomes of atrial fibrillation (AF) patients vs patients in sinus rhythm (SR). METHODS: All consecutive patients undergoing TAVI in our hospital were included. The AF group comprised patients in AF at the time of TAVI or with history of AF, and were compared with the SR group. Procedural, echocardiographic and follow-up variables were compared. Likewise, the CHA 2 DS 2-VASC stroke risk score and HAS-BLED bleeding risk score and antithrombotic treatment at discharge in AF patients were compared with that in SR patients. RESULTS: From a total of 34 patients undergoing TAVI, 17 (50%) were allocated to the AF group, of whom 15 (88%) were under chronic oral anticoagulation. Patients in the AF group were similar to those in the SR group except for a trend (P = 0.07) for a higher logistic EuroSCORE (28% vs 19%), and a higher prevalence of hypertension (82% vs 53%) and chronic renal failure (17% vs 0%). Risk of both stroke and bleeding was high in the AF group (mean CHA 2 DS 2-VASC 4.3, mean HAS-BLED 2.9). In the AF group, treatment at discharge included chronic oral anticoagulation in all except one case, and in association with an antiplatelet drug in 57% of patients. During a mean follow-up of 11 mo (maximum 32), there were only two strokes, none of them during the peri-procedural period: one in the AF group at 30 mo and one in the SR group at 3 mo. There were no statistical differences in procedural success, and clinical outcome (survival at 1 year 81% vs 74% in AF and SR groups, respectively, P = NS). CONCLUSION: Patients in AF undergoing TAVI show a trend to a higher surgical risk. However, in our cohort, patients in AF did not have a higher stroke rate compared to the SR group, and the prognosis was similar in both groups.

Objective:To identify the prevalence of soil-transmitted helminthiasis(STH)among school-age children in the Cagayan Valley,the Philippines,assess their level of awareness on the disease,and determine predisposing factors of the disease.Methods:A total of 478 GradesⅢ-Ⅴschool-age children in Pamplona and Sanchez-Mira School Districts in the Cagayan Valley answered the questionnaire assessing their knowledge,attitude,and practices on STH,subjected to anthropometric measurements,and provided faecal samples for parasitologic assessment(direct smear,Kato-Katz,and formol-ether concentration techniques).Results:The participants of the study,with 55.86%females,were 8 to 14 years old.Their nutritional status was assessed‘normal’(84.31%),‘severely wasted’(6.49%),‘wasted’(5.23%),‘overweight’(2.72%),and‘obese’(1.26%).The prevalence of infection with at least 1 STH species was 25.99%in Pamplona and 19.40%in SanchezMira.Overall,the prevalence of heavy intensity was 7.11%for Ascaris lumbricoides and 1.67%for Trichuris trichiura.All hookworm infections had light intensities.The majority of the school-age children had a low score in the KAP test.In knowledge of STH,‘stunted growth as a symptom of infection’was associated with a lower risk of Ascaris lumbricoides infection(OR 0.448;95%CI 0.212,0.945;P=0.035)while‘playing with soil as a mode of transmission’was associated with an increased risk of Ascaris lumbricoides infection(OR 2.067;95%CI 1.014,4.212;P=0.046).In attitude towards STH,‘I think I have intestinal worm now’was associated with a higher risk of Ascaris lumbricoides infection(OR 1.681;95%CI 1.061,2.662;P=0.027).Conclusions:The prevalence rate of Ascaris lumbricoides among the school-age children in the Cagayan Valley shows the need to further intensify intervention in the area to meet the threshold set by the World Health Organization.The identified predictors of infection,which concerns the school-age children’s knowledge and attitude toward STH,can be used in augmenting intervention programs in the future.

Background During the COVID-19 pandemic,decreased volumes of stroke admissions and mechanical thrombectomy were reported.The study’s objective was to examine whether subarachnoid haemorrhage(SAH)hospitalisations and ruptured aneurysm coiling interventions demonstrated similar declines.Methods We conducted a cross-sectional,retrospective,observational study across 6 continents,37 countries and 140 comprehensive stroke centres.Patients with the diagnosis of SAH,aneurysmal SAH,ruptured aneurysm coiling interventions and COVID-19 were identified by prospective aneurysm databases or by International Classification of Diseases,10th Revision,codes.The 3-month cumulative volume,monthly volumes for SAH hospitalisations and ruptured aneurysm coiling procedures were compared for the period before(1 year and immediately before)and during the pandemic,defined as 1 March-31 May 2020.The prior 1-year control period(1 March-31 May 2019)was obtained to account for seasonal variation.Findings There was a significant decline in SAH hospitalisations,with 2044 admissions in the 3 months immediately before and 1585 admissions during the pandemic,representing a relative decline of 22.5%(95%CI−24.3%to−20.7%,p<0.0001).Embolisation of ruptured aneurysms declined with 1170-1035 procedures,respectively,representing an 11.5%(95%CI−13.5%to−9.8%,p=0.002)relative drop.Subgroup analysis was noted for aneurysmal SAH hospitalisation decline from 834 to 626 hospitalisations,a 24.9%relative decline(95%CI−28.0%to−22.1%,p<0.0001).A relative increase in ruptured aneurysm coiling was noted in low coiling volume hospitals of 41.1%(95%CI 32.3%to 50.6%,p=0.008)despite a decrease in SAH admissions in this tertile.Interpretation There was a relative decrease in the volume of SAH hospitalisations,aneurysmal SAH hospitalisations and ruptured aneurysm embolisations during the COVID-19 pandemic.These findings in SAH are consistent with a decrease in other emergencies,such as stroke and myocardial infarction.
