BACKGROUND paracoccidioidomycosis(pCm)may involve the hepatic pedicle and peripan creatic lymph nodes,cause damage to the bile duct and manifest,exceptionally,in combination with extrahepatic cholestasis(EHC),making investigation and treatment *** To investigate the management of patients with visceral pCm admitted with *** All patients diagnosed with pCm treated in a public,tertiary teaching hospital between 1982 and 2020 were retrospectively *** also identified with EHC were allocated to two groups according to the treatment approach for the purpose of comparing clinical,laboratory,and imaging findings,resources used for etiological diagnosis,treatment results,and *** analyses were performed using the linear mixed-effects model(random and fixed effects),which was adjusted using the pROC mIXED procedure of the SAS®9.0 software,and Fisher’s exact *** Of 1645 patients diagnosed with pCm,40(2.4%)had *** these,20(50.0%)lived in the rural area and 29(72.5%)were men,with a mean age of 27.1 years(3-65 years).Jaundice as first symptom and weight loss of at least 10 kg were observed in 16 patients(40.0%),and a mass in the head of the pancreas was observed in 8(20.0%).The etiological diagnosis was made by tissue collection during surgery in 4 cases(10.0%)and by endoscopic methods in 3 cases(7.5%).Twenty-seven patients(67.5%)received drug treatment alone(Group 1),whereas 13(32.5%)underwent endoscopic and/or surgical procedures in combination with drug treatment(Group 2).EHC was significantly reduced in both groups(40.7% in Group 1,with a mean time of 3 months;and 38.4% in Group 2,with a mean time of 7.5 months),with no statistically significant difference between *** recurrence rates,associated mainly with treatment nonadherence,were similar in both groups:37% in Group 1 and 15.4% in Group *** mortality rate was 18.5% in Group 1 and 23% in Group 2,with survival estimates of 71.3% and 72.5%,respectively,with

Hepatocellular carcinoma(HCC)is one of the most common causes of cancerrelated *** particular type of cancer has the distinctive characteristic of mostly happening in individuals with an underlying liver *** makes the management of patients more challenging,since physicians must take into consideration two different conditions,the chronic liver disease and the *** underlying liver disease has several implications in clinical practice,because different kinds of chronic liver disease can lead to varying degrees of risk of developing HCC,obstacles in surveillance,and differences in the efficacy of the treatment against HCC.A shift in the prevalence of liver diseases has been evident over the last few years,with viral hepatitis gradually losing the leading position as cause of HCC and metabolic dysfunction-associated steatotic liver disease gaining ***,in an era of personalized medicine,it is imperative that physicians are aware of the underlying liver disease of individuals with HCC and its impact in the management of their tumors.

Correcting distortions of thin-walled machined parts by machine hammer peening

Thin-walled aerostructural components frequently get distorted after the machining *** to correct distortions or eventually rejecting parts significantly increases the *** paper proposes a new approach to correct distortions in thin-walled components by strategically applying hammer peening on target surfaces of a machined *** alloy 7475-T7351 was chosen for this *** study was divided in two ***,the residual stresses(RS)induced by four different pneumatic hammer peening conditions(modifying the stepover distance and initial offset)were characterised in a test coupon,and one of the conditions was selected for the next *** the second stage,a FEm model was used to predict distortions caused by machining in a representative ***,the RS induced by hammer peening were included in an FEm model to define two hammer peening strategies(varying the coverage area)to analyse the capability to reduce *** workpieces were machined and then treated with the simulated hammer peening strategies for experimental *** in the test coupon showed that pneumatic hammer peening can generate high compressive RS(-50 to350 mpa)up to 800 lm depth,with their magnitude increasing with a reduced stepover *** of hammer peening over 4% of the surface of the representative workpiece reduced the machininginduced distortions by 37%,and a coverage area of 100% led to and overcorrection by a factor of *** confirms that hammer peening can be strategically applied(in target areas and changing the percentage of coverage)to correct low or severe distortions.

We report the first high-repetition-rate generation and simultaneous characterization of nanosecond-scale return currents of kA-magnitude issued by the polarization of a target irradiated with a pW-class high-repetition-rate titanium:sapphire laser system at relativistic *** present experimental results obtained with the VEGA-3 laser at intensities from5×10^(18)to 1.3×10^(20)W cm^(-2).A non-invasive inductive return-current monitor is adopted to measure the derivative of return currents of the order of kA ns-1and analysis methodology is developed to derive return *** compare the current for copper,aluminium and Kapton targets at different laser *** data show the stable production of current peaks and clear prospects for the tailoring of the pulse shape,which is promising for future applications in highenergy-density science,for example,electromagnetic interference stress tests,high-voltage pulse response measurements and charged particle beam *** compare the target discharge of the order of hundreds of nC with theoretical predictions and a good agreement is found.

Alagille syndrome(ALGS;OmIm#118450)is a highlyvariable,multi-systemic,autosomal dominant disorder,caused by mutations in the Jagged Canonical Notch Ligand 1(JAG1)gene(20p12.2)or in the Neurogenic locus notch homolog protein 2(NOTCH2)gene(1p13).^(1)It primarily affects the liver,heart,eyes,face,kidney,skin,vertebrae,and skeleton.^(1,2)The JAG1 gene has 26 exons and about 90%of the ALGS cases result from pathogenic variants in the JAG1 and around 7%of cases have deletions in chromosome 20 that include this gene.
