The current study investigated a triad,which comprises of adipose tissue derived stem cells isolated from infrapatellar fat pad and gelatin/polyvinyl alcohol(PVA)-based matrix with exclusive ascorbic acid ***,the bio-mechanical properties of the gelatin–PVA blended scaffolds in wet condition are equivalent to the ECM of soft tissues in general,in this study,the triad was tested as a model for neural tissue *** from being cytocompatible and biocompatible,the porosity of the scaffold has been designed in such a manner that it facilitates the cell signalling and enables the exchange of nutrients and *** highly proliferative stem cells from Passage 2 were characterized using both,mesenchymal and embryonic stem cell *** an initial exploration the mesenchymal stem cells at Passage 4 were exposed to ascorbic acid and basic fibroblast growth factor signalling for neuronal differentiation in 2D environment *** MSCs successfully differentiated and acquired neuron specific markers related to cytoskeleton and ***,three phases of experiments have been conducted on the 3D gelatin/PVA matrix to prove their efficacy,the growth of stem cells,growth of differentiated neurons and the in situ growth and differentiation of *** scaffold was conducive and directed MSCs to neuronal lineage under specific ***,this organotypic model triad could open a new avenue in the field of soft tissue engineering as a simple and effective tissue construct.

Endoscopic management of massive rectal bleeding from a Dieulafoy’s lesion:Case report

BACKGROUND Rectal Dieulafoy's lesions (DLs) are very rare;however, they can be life threatening when presented with massive hemorrhage. CASE SUMMARY A 44-year-old female with medical history of chronic renal failure who was on renal replacement therapy presented with lower gastrointestinal hemorrhage. Physical examination revealed signs of hypovolemic shock and massive rectal bleeding. Complete blood count revealed abrupt decrease in hematocrit. After hemodynamic stabilization, an urgent colonoscopy was performed. A rectal DL was diagnosed, and it was successfully treated with two hemoclips. There were no signs of recurrent bleeding at thirty days of follow-up. CONCLUSION Rectal DLs represent an unusual cause of lower gastrointestinal bleeding. Massive hemorrhage can increase the morbidity and mortality of these patients. Endoscopic management continues to be the reference standard in the diagnosis and therapy of these lesions. Thermal, mechanical (hemoclip or band ligation), or combination therapy (adrenaline injection combined with thermal or mechanical therapy) should be considered the first choice for treatment.

Electrocardiographic changes during induced therapeutic hypothermia in comatose survivors after cardiac arrest

AIM: To assess the safety of therapeutic hypothermia(TH) concerning arrhythmias we analyzed serial electrocardiograms(ECG) during ***: All patients recovered from a cardiac arrest with Glasgow < 9 at admission were treated with induced mild TH to 32-34℃. TH was obtained with cool fluid infusion or a specific intravascular device. Twelvelead ECG before,during,and after TH,as well as ECG telemetry data was recorded in all patients. From a total of 54 patients admitted with cardiac arrest during the study period,47 patients had the 3 ECG and telemetry data available. ECG analysis was blinded and performed with manual caliper by two independent cardiologists from blinded copies of original ECG,recorded at 25 mm/s and 10 mm/m V. Coronary care unit staff analyzed ECG telemetry for rhythm disturbances. Variables measured in ECG were rhythm,RR,PR,QT and corrected QT(QTc by Bazett formula,measured in lead v2) intervals,QRS duration,presence of Osborn's J wave and U wave,as well as ST segment displacement and T wave amplitude in leads Ⅱ,v2 and ***: Heart rate went down an average of 19 bpm during hypothermia and increased again 16 bpm with rewarming(P < 0.0005,both). There was a nonsignificant prolongation of the PR interval during TH and a significant decrease with rewarming(P = 0.041). QRS duration significantly prolonged(P = 0.041) with TH and shortened back(P < 0.005) with rewarming. QTc interval presented a mean prolongation of 58 ms(P < 0.005) during TH and a significant shortening with rewarming of 22.2 ms(P = 0.017). Osborn or J wave was found in 21.3% of the patients. New arrhythmias occurred in 38.3% of the patients. Most frequent arrhythmia was non-sustained ventricular tachycardia(19.1%),followed by severe bradycardia or paced rhythm(10.6%),accelerated nodal rhythm(8.5%) and atrial fibrillation(6.4%). No life threatening arrhythmias(sustained ventricular tachycardia,polymorphic ventricular tachycardia or ventricular fibrillation) occurred during TH.

A Comparative Study on Efficacy of Fludrocortisones versus Glucocorticoids and Vasodilators in the Treatment of Idiopathic Sensorineural Cochlear Hearing Loss

Introduction and Objectives: The idiopathic sensorineural cochlear hearing loss is one of the most frequent human sensory deficits and there is no specific drug therapy for it. The possible hearing recovery is related with the reestablishment of normal ionic homeostasis of the endolymph controlled by the mineralocorticoid as could be demonstrate experimentally. The purpose of this clinical trial was to confirm the efficacy of mineralocorticoids to the recovery of hearing level in patients suffering idiopathic sensorineural hearing loss (SNHL) against the glucocorticoids and vasodilator drugs. Material and Methods: The research lasted three months and involved 90 patients allocated into four different groups: Placebo group, consisted of 20 patients (10 men and 10 women);the group consisting of 22 patients treated with glucocorticoid therapy (12 men and 10 female);the group treated with mineralocorticoid therapy encompassed 26 patients (13 males and 13 females) and the group of vasodilators formed by 22 patients (12 men and 10 women). The level of hearing loss was estimated by the tests Liminal Tone Audiometry (LTA) and Auditory Brainstem Response (ABR). Results: The main features in this research were overall better response in improving the hearing level with the mineralocorticoid therapy. This improvement in hearing levels was greater in women than in men, and a higher response was found in the left ear regardless of patient’s gender. Conclusions: The hearing gain was significantly superior in the mineralocorticoids group followed by the glucocorticoids group whereas the response to vasodilators was lesser and with no statistical significance.

AIM: To evaluate if canine models are appropriate for teaching endoscopy fellows the techniques of endoscopic submucosal dissection (ESD). METHODS: ESD was performed in 10 canine models under general anesthesia, on artificial lesions of the esophagus or stomach marked with coagulation points. After ESD, each canine model was euthanized and surgical resection of the esophagus or stomach was carried out according to "The Principles of Humane Experimental Technique, Russel and Burch". The ESD specimens were fixed with needles on cork submerged in a formol solution with the esophagus or stomach, and delivered to the pathology department to be analyzed. RESULTS: ESD was completed without complications using the Hook-knife in five esophageal areas, with a procedural duration of 124 ± 19 min, a length of 27.4 ± 2.6 mm and a width of 21 ± 2.4 mm. ESD was also completed without complications using the IT-knife2 in five gastric areas, with a procedural duration of 92.6 ± 19 min, a length of 32 ± 2.5 mm and a width of 18 ± 3.7 mm. CONCLUSION: ESD is feasible in the normal esopha- gus and stomach of canine models, which are appropriate for teaching this technique.

Monitoring of the Corrosion on a Steel Sheet-Pile Marine Breakwater by Systematic Thickness Measurements

In Quepos, Pacific of Costa Rica, it was finished on 2010 the first phase of a marina, including two mix breakwaters, with rubble mound （rocks and concrete units）, and 25 circular steel sheet piles cofferdam cells, filled with sand and gravel. The maintenance plan, considers tracking sheet pile corrosion, comparing ＂actual＂ against expected rates, checking structural limits, and programming countermeasures if accelerated corrosion is identified. Specific control sections, along the breakwaters, both inside and outside the basin, were established. In each section, thicknesses were measured every meter from the top of the steel cell to seabed using an ultrasonic equipment, and an underwater transducer. Both land crew, and divers for submerged portions, were used. The measurements campaigns are for several years from 2011 to 2016. Sectors of the breakwater with varied corrosion attack levels could be differentiated. Also, corrosion rates and lifespans were estimated, both general for the structures, and specific for each section and level. In turn, this allowed to identify maintenance priorities, defining sites where measures of corrosion protection should initiate, as well, to have confidence in the structural capacity and safety of the breakwaters.

Testosterone Levels and Development of the Penile Spines and Testicular Tissue during the Postnatal Growth in Wistar Rats

Aim of Study: Gonadal hormones exert a profound influence on the development, structure and function of the sexual organs. The testosterone is one of the androgens that plays an essential role in the development of sexual organs in male mammals. Therefore, the present study was undertaken to evaluate the testosterone levels and developmental pattern of the penile spines and seminiferous tubules during early postnatal life of Wistar rats. Methods and Materials: At 7, 14, 21, 28, 35, 42, 49, 56, 63 and 70 days after birth, penile and testicular tissues of male rats were dissected out and fixed for histological study and plasma testosterone levels were determined using high resolution chromatography. Results: An increase in the number of penile follicles, primarily in the distal region of the penis, was observed from postnatal days 14 to 42, followed by a gradual decrease. Penile spines were absent from birth until the first growth peak, which was observed at 42 postnatal days. Both testicular weight and the area of seminiferous tubules showed gradual increases before achieving their highest values at 42 postnatal days. Similarly, a gradual increase in testosterone levels was detected from day 28, with a peak at 42 postnatal days. Conclusions: These data show a temporal association between the development of the penile spines and testicular tissue with gradual increases in testosterone levels. These results may contribute to a better understanding of the behavioral, hormonal and morphological changes underlying sexual maturation in male rats.

Dynamic Analysis of Steel Platforms When Subjected to Mechanical Equipment-Induced Vibrations

The competitive trends of the world market have long been forcing structural engineers to develop minimum weight and labour cost solutions. A direct consequence of this new design trend has been a considerable increase in problems related to undesired floor vibrations. For this reason, structural floor systems can become vulnerable to excessive vibrations that are produced by, for example, impacts due to mechanical equipment （e.g., rotating machinery）. This study investigates the dynamic behaviour of a production platform constructed of steel and located in the Santos Basin （Merluza field）, Sao Paulo/SP, Brazil, when subjected to impacts produced by mechanical equipment （rotating machinery）. The structural model consists of two steel decks with a total area of 1,915 m^2 （upper deck： 445 m^2, lower deck： 1,470 m^2） and supported by piles. A numerical analysis is performed to assess the dynamic impacts on the deck structure originating from the electrical generators and compressors. Based on the peak acceleration values obtained for the structure steady-state response, it is possible to evaluate the structural model performance in terms of human comfort, the maximum tolerances of the mechanical equipment and the vibration serviceability limit states of the structure.

New Records for GASTROPODA Class of Species Found in the Rocky Intertidal Zone of the Marine Priority Region 32,Guerrero,Mexico

The GASTROPODA class, one of the best known marine environments, is formed by a large number of species and the shape of their shell structure varies greatly. These mollusks are distributed from the intertidal zone all the way to the abyssal zone, and there are also swimming and floating species. This research was conducted at seven sites, located in the Marina Priority Region 32 (MPR 32), located in the State of Guerrero, Mexico. The aim of this report is to document the scope of the geographic distribution of the 34 species of GASTROPODA class. 11,263 total specimens were analyzed. 108 species were identified, of which four species are new records for Mexico, 15 for the Transitional Mexican Pacific, 11 for the State of Guerrero and four for the MPR 32. Here we report a significant number of new records of species of class GASTROPODA found on the rocky intertidal zone of the MPR 32. The reporting of these new records, demonstrates the need to focus research efforts on the study of marine diversity, since knowledge in this respect is quite incomplete, especially regarding marine mollusks, a fact that has been reported by the National Commission for the Management and Use of Biodiversity.
