AiM： To evaluate the diagnostic accuracy of histological evaluation of pancreatic tissue samples obtained by a modified method for recovering and processing the endoscopic ultrasound （EUS）-guided fine needle aspiration （FNA） material in the differential diagnosis of pancreatic solid masses. METHODS： Sixty-two consecutive patients with pancreatic masses were prospectively studied. EUS was performed by the linear scanning Pentax FG-38UX echoendoscope. Three FNAs （22G needle） were carried out during each procedure. The materials obtained with first and second punctures were processed for cytological study. Haterials of the third puncture were recovered into 10% formol solution by careful injection of saline solution through the needle, and processed for histological study. RESULTS： Length of the core specimen obtained for histological analysis was 6.5±5.3 mm （range 1-22 mm）. Cytological and histological samples were considered as adequate in 51 （82.3%） and 52 cases （83.9%）, respectively. Overall sensitivity of both pancreatic cytology and histology for diagnosis of malignancy was 68.4%. Conbary to cytology, histology was able to diagnose tumours other than adenocarcinomas, and all cases of inflammatory masses. Combination of cytology and histology allowed obtaining an adequate sample in 56 cases （90.3%）, with a global sensitivity of 84.21%, specificity of 100% and an overall accuracy of 90.32%. The complication rate was 1.6%.CONCLUSiON： Adequate pancreatic core specimens for histological examination can be obtained by EUS-guided FNA. This technique is mainly useful for the diagnosis of different types of pancreatic tumours and evaluation of benign diseases.

Endoscopic ultrasound (EUS) has become an essential tool for the study of pancreatic diseases. Specifically, EUS plays a pivotal role evaluating patients with a known or suspected pancreatic mass. in this setting, differential diagnosis remains a clinical challenge. EUS-guided fine-needle aspiration (FNA) and fine-needle biopsy (FNB) have been proven to be safe and useful tools in this setting. EUS-guided FNA and FNB, by obtaining cytological and/or histological samples, are able to diagnose pancreatic lesions with high sensitivity and specificity. in this context, several methodological features, trying to increase the diagnostic yield of EUS-guided FNA and FNB, have been evaluated. in this review, we focus on the role of rapid on-site evaluation (ROSE). From data reported in the literature, ROSE may increase diagnostic yield of EUS-FNA specimens by 10%-30%, and thus, diagnostic accuracy. However, we should point out that many recent studies have reported adequacy rates of > 90% without ROSE, indicating that, perhaps, at high-volume centers, ROSE may not be indispensable to achieve excellent results. The use of ROSE can be considered important during the learning curve of EUS-FNA, and also in hospital with diagnostic accuracy rates < 90%.

Littorina fabalis is an intertidal snail commonly living on the brown algae Fucus vesiculosus and showing frequent shell-color polymorphisms in the wild. The evolutionary mechanism underlying this polymorphism is currently unknown. Shell color variation was studied in mated and non-mated specimens of this species from different microareas in one locality from NW Spain, in order to estimate sexual selection and assortative mating that may （still） be operating in this population. The analyses across microareas allowed us to investigate frequency-dependent selection and assortative mating components, mechanisms that could maintain the polymorphism. The presence of shell scars caused by crab attacks, an environmental variable not related with sexual selection or assortative mating, was used as experimental control. This study provides new evidence of significant disas- sortative mating and some degree of sexual selection against some shell colors, supporting the results found 21 years ago in a similar study, i.e. in the same species and locality. The similarity of these estimates during the studied period suggests that this experimental approach is consistent and valid to be extended to other populations and organisms. in addition, sexual selection and assortative mating estimates did not change across microareas differing in shell color frequencies, suggesting than the polymor- phism can not be maintained by a frequency-dependent （sexual selection-based） mechanism. Our main hypothesis is that negative assortative mating could contribute to the maintenance of the polymorphism, perhaps by males showing distinct female color preferences when searching for mates [Current Zoology 58 （3）： 463-474, 2012].

Large scavengers are strongly dependent on environmental conditions and carrion distribution and abundance,so season and breeding-related factors may influence the spatial ecology of species such as the Cinereous Vulture(Aegypius monachus),the largest European *** holds one of the biggest populations worldwide,but some aspects of the spatial ecology of the species in this region remain *** this study,17 adult Cinereous Vultures were GPS-tracked in order to study their spatial ecology during the adult *** average monthly home ranges(95%Kernel Density Estimation,KDE)and core areas(50%KDE)were 6543±19,935 km^(2)and 1174±4004 km^(2),*** average monthly home range fidelity ranged between 50 and 73%.Differences in movement-related variables between the seasonal periods(incubation,chick-rearing and non-breeding)were *** the chick-rearing period,the monthly accumulated distance was higher than during the other periods:3316±1108(chick-rearing)vs.1621±622(incubation)vs.1726±1159 km per month(nonbreeding).Additionally,large home range sizes were more frequent during the chick-rearing *** are two likely causes for these seasonal ***,chick-rearing entails a higher energetic expenditure by the parental individuals in foraging activities,so larger movements and foraging areas are expected during this ***,the flight is favoured during spring and summer due to environmental *** chick-rearing and warm months is a great evolutionary advantage for soaring-gliding raptors,as it allows them to cover larger areas with low energy ***,six individuals tagged as nestlings highlight the philopatric behaviour of the species:vultures settle their breeding areas 54±51 km from their natal nest(range=9–138 km).

Sulfonic acid-functionalized heterogeneous catalysts have been evaluated in the catalytic dehydration of C(,monosaccharides into 5-hydroxymethylfurfural(HMF) using dimethyl sulfoxide(DMSO)as *** commercial resin Amberlyst-70 was the most active catalyst,which was ascribed to its higher concentration of sulfonic acid sites as compared with the other catalysts,and it gave 93 mol%yield of HMF from fructose in 1 *** glucose as the starting material,which is a much more difficult reaction,the reaction conditions(time,temperature,and catalyst loading) were optimized for Amberlyst-70 by a response surface methodology,which gave a maximum HMF yield of 33 mol%at 147 °C with 23 wt%catalyst loading based on glucose and 24 h reaction *** promotes the dehydration of glucose into anhydroglucose,which acts as a reservoir of the substrate to facilitate the production of HMF by reducing side *** reuse without a regeneration treatment showed a gradual but not very significant decay in catalytic activity.

Accuracy of endoscopic ultrasound-guided tissue acquisition in the evaluation of lymph nodes enlargement in the absence of on-site pathologist

To evaluate factors that influence the diagnostic accuracy of endoscopic ultrasound (EUS)-guided tissue acquisition for lymph node enlargement in the absence of an on-site pathologist. METHODSA retrospective analysis of patients who underwent EUS-guided tissue acquisition for the pathological diagnosis of lymph node enlargement between April 2012 and June 2015 is reported. Tissue acquisition was performed with both cytology and biopsy needles of different calibers. The variables evaluated were lymph node location and size, number of passes and type of needle used. Final diagnosis was based on surgical histopathology or, in non-operated cases, on EUS-guided tissue acquisition and imaging assessment with a minimum clinical follow-up of 6 mo. RESULTSDuring the study period, 168 lymph nodes with a median size of 20.3 mm (range 12.5-27) were sampled from 152 patients. Ninety lymph nodes (53.6%) were located at mediastinum, and 105 (62.5%) were acquired with biopsy needles. The final diagnosis was benign/reactive origin in 87 cases (51.8%), malignant in 65 cases (38.7%), and lymphoma in 16 cases (9.5%). The sensitivity, specificity, positive predictive value and negative predictive value for the detection of malignancy were 74.1%, 100%, 100% and 80.6%, respectively. The overall accuracy was 87.5% (95%Ci: 81.7-91.7). No variables were independently associated with a correct final diagnosis according to the multivariate analysis. CONCLUSiONEUS-guided tissue acquisition is a highly accurate technique for assessing lymph node enlargement. None of the variables evaluated were associated with diagnostic accuracy.

BACKGROUND The predictive value of bleeding risk scores for atrial fibrillation in older patients is not as well *** goal of this study was to evaluate the predictive value of HASBLED,ORBiT and ATRiA for major bleeding(MB)and intracranial hemorrhage(iCH)in patients≥75 years with atrial fibrillation and oral anticoagulation(OAC).METHODS A retrospective unicenter study including patients≥75 years with atrial fibrillation(AF)and OAC.A total of 7613 patients≥75 years with AF and OAC included between 2014 and 2018(registry:NCT04364516).We analyzed the discriminative value of HASBLED,ATRiA and ORBiT scores for bleeding endpoints(major bleeding as primary endpoint and intracerebral hemorrhage as secondary).Cox regression was used to predict major bleeding with each scale and also for searching other variables potentially predictor of major *** discrimination was assessed using Harrell’s *** was assessed with goodness-of-fit test proposed by Gronnesby and *** During a mean follow up of 4.0 years(iQR:2.4–5.7 years),729 patients developed MB(2.61 per 100 patients/year)and 243 patients developed iCH(0.85 per 100 patients/year).Three scores showed a low discrimination for major bleeding,being ORBiT the best(HASBLED C statistic=0.557;ATRiA C statistic=0.568;ORBiT C statistic=0.595)and also a low discrimination for iCH(HASBLED C statistic=0.509;ATRiA C statistic=0.522;ORBiT C statistic=0.526).Among the variables that are part of the scores and other baseline characteristics,after multivariable adjustment only sex(male),dementia,prior admission for bleeding,anemia and liver disease were found as a predictors of *** in older patients under oral anticoagulation with atrial fibrillation,the risk scores HASBLED,ATRiA and ORBiT showed a weak discrimination for major bleeding and intracranial ***,other better alternatives should be evaluated for this purpose.
