Penile prosthesis: Patient satisfaction, use and preference for malleable vs inflatable

AIM: To evaluate and compare long-term patient satisfaction and use after either malleable penile prosthesis(MPP) or inflatable penile prosthesis(IPP) implantation. METHODS: we present a retrospective unicenter study of 108 patients implanted with either 2 or 3-piece American Medical System^(TM)(AMS^(TM)) or Coloplast^(TM) inflatable penile prosthesis(AMS 700CX^(TM), AMS 700CXR^(TM), AMS Ambicor^(TM) or Coloplast TITAn^(TM)) or malleable(AMS Spectra^(TM) or Coloplast Genesis^(TM)) in our Centre between 1993 and 2011. We collected data from the medical record including follow-up, age and type of prosthesis. We used a four-question telephone surveydesigned ad hoc by urologist in our Department, with three multiple choice questions and a grading answer. After verbal consent was obtained, proposed questions concerned global satisfaction regarding to the procedure, quality of sexual intercourses graded from 0 to 10, frequency of sexual intercourse and about undergoing the same procedure again. SPSS^(TM) version 20.0 was used for the descriptive analysis of the data. RESUlTS: Sixty seven(64%) patients underwent a MPP and 41(36%) an IPP. The mean age was 52.6 ± 3.6 years in the MPP group and 57.2 ± 2.8 years in the IPP group(P = 0.02). Total respond rate was 55.5%(60/108). Twenty six out of 33 MPP patients(78.9%) and 19 of the 27 IPP subjects(70.3%) were satisfied or very satisfied with the procedure. The quality of sexual intercourse was rated 7.13 ± 0.39 points in the MPP group and 6.16 ± 0.47 points in the IPP group. Frequency of sexual intercourse was 1 or more times per week in 15(46.9%) patients with MPP and in 12(46.1%) of the IPP patients. Twenty-eight(84.9%) patients who received a MPP would undergo the procedure for the same device again as well as 24(88.9%) of the IPP group. Translational pancreatic cancer research:a comparative study on patient-derived xenograft models

AIM To assess the viability of orthotopic and heterotopic patient-derived pancreatic cancer xenografts implanted into nude *** This study presents a prospective experimental analytical follow-up of the development of tumours in mice upon implantation of human pancreatic adenocarcinoma samples. Specimens were obtained surgically from patients with a pathological diagnosis of pancreatic adenocarcinoma. Tumour samples from pancreatic cancer patients were transplanted into nude mice in three different locations(intraperitoneal, subcutaneous and pancreatic). Histological analysis(haematoxylin-eosin and Masson's trichrome staining) and immunohistochemical assessment of apoptosis(TUnEl), proliferation(Ki-67), angiogenesis(CD31) and fibrogenesis(α-SMA) were performed. When a tumour xenograft reached the target size, it was reimplanted in a new nude mouse. Three sequential tumour xenograft generations were generated(F1, F2 and F3).RESUlTS The overall tumour engraftment rate was 61.1%. The subcutaneous model was most effective in terms of tissue growth(69.9%), followed by intraperitoneal(57.6%) and pancreatic(55%) models. Tumour development was faster in the subcutaneous model(17.7 ± 2.6 wk) compared with the pancreatic(23.1 ± 2.3 wk) and intraperitoneal(25.0 ± 2.7 wk) models(P = 0.064). There was a progressive increase in the tumour engraftment rate over successive generations for all three models(F1 28.1% vs F2 71.4% vs F3 80.9%, P no significant differences in tumour xenograft differentiation and cell proliferation between human samples and the three experimental models among the sequential generations of tumour xenografts. However, a progressive decrease in fibrosis, fibrogenesis, tumour vascularisation and apoptosis was observed in the three experimental models compared with the human samples. A Two-Player Coalition Cooperative Scheme for the Bodyguard Allocation Problem

We address the bodyguard allocation problem （BAP）, an optimization problem that illustrates the conflict of interest between two classes of processes with contradictory preferences within a distributed system. While a class of processes prefers to minimize its distance to a particular process called the root, the other class prefers to maximize it; at the same time, all the processes seek to build a communication spanning tree with the maximum social welfare. The two state-of-the-art algorithms for this problem always guarantee the generation of a spanning tree that satisfies a condition of nash equilibrium in the system; however, such a tree does not necessarily produce the maximum social welfare. In this paper, we propose a two-player coalition cooperative scheme for BAP, which allows some processes to perturb or break a nash equilibrium to find another one with a better social welfare. By using this cooperative scheme, we propose a new algorithm called FFC-BAPs for BAP. We present both theoretical and empirical analyses which show that this algorithm produces better quality approximate solutions than former algorithms for BAP.

Vacuum promotes metabolic shifts and increases biogenic hydrogen production in dark fermentation systems

The successful operation of any type of hydrogen-producing bioreactor depends on the performance of the microorganisms present in the system. Both substrate and partial gas pressures are crucial factors affecting dark fermentation metabolic pathways. The main objective of this study was to evaluate the impact of both factors on hydrogen production using anaerobic granular sludge as inoculum and, secondly, to study the metabolic shifts of an anaerobic community subjected to low partial gas pressures. With this goal in mind, seven different wastewater （four synthetic media, two industrial waste- water, and one domestic effluent） and the effect of applying vacuum on the systems were analyzed. The application of vacuum promoted an increase in the diversity of hydrogen-producing bacteria, such as Clostridium, and promoted the dominance of acetoclasticover hydrogenotrophic methanogens. The application of different media promoted a wide variety of metabolic pathways. nevertheless, reduction of the hydrogen partial pressure by application of vacuum lead to further oxidation of reaction intermediates irrespective of the medium used, which resulted in higher hydrogen and methane production, and improved the COD removal. Interestingly, vacuum greatly promoted biogenic hydrogen production from a real wastewater, which opens possibilities for future application of dark fermentation systems to enhance biohydrogen yields.

Drought and Spatiotemporal Variability of Forest Fires across Mexico

Understanding the spatiotemporal links between drought and forest fire occurrence is crucial for improving decision-making in fire management under current and future climatic conditions. We quantified forest fire activity in Mexico using georeferenced fire records for the period of 2005–2015 and examined its spatial and temporal relationships with a multiscalar drought index, the Standardized Precipitation-Evapotranspiration Index(SPEI). A total of 47975 fire counts were recorded in the 11-year long study period, with the peak in fire frequency occurring in 2011. We identified four fire clusters, i.e., regions where there is a high density of fire records in Mexico using the Getis-Ord G spatial statistic. Then, we examined fire frequency data in the clustered regions and assessed how fire activity related to the SPEI for the entire study period and also for the year 2011. Associations between the SPEI and fire frequency varied across Mexico and fire-SPEI relationships also varied across the months of major fire occurrence and related SPEI temporal scales. In particular, in the two fire clusters located in northern Mexico(Chihuahua, northern Baja California), drier conditions over the previous 5 months triggered fire occurrence. In contrast, we did not observe a significant relationship between drought severity and fire frequency in the central Mexico cluster, which exhibited the highest fire frequency. We also found moderate fire-drought associations in the cluster situated in the tropical southern Chiapas where agriculture activities are the main causes of forest fire occurrence. These results are useful for improving our understanding of the spatiotemporal patterns of fire occurrence as related to drought severity in megadiverse countries hosting many forest types as Mexico.

A Comparative Study on Efficacy of Fludrocortisones versus Glucocorticoids and Vasodilators in the Treatment of Idiopathic Sensorineural Cochlear Hearing loss

Introduction and Objectives: The idiopathic sensorineural cochlear hearing loss is one of the most frequent human sensory deficits and there is no specific drug therapy for it. The possible hearing recovery is related with the reestablishment of normal ionic homeostasis of the endolymph controlled by the mineralocorticoid as could be demonstrate experimentally. The purpose of this clinical trial was to confirm the efficacy of mineralocorticoids to the recovery of hearing level in patients suffering idiopathic sensorineural hearing loss (SnHl) against the glucocorticoids and vasodilator drugs. Material and Methods: The research lasted three months and involved 90 patients allocated into four different groups: Placebo group, consisted of 20 patients (10 men and 10 women);the group consisting of 22 patients treated with glucocorticoid therapy (12 men and 10 female);the group treated with mineralocorticoid therapy encompassed 26 patients (13 males and 13 females) and the group of vasodilators formed by 22 patients (12 men and 10 women). The level of hearing loss was estimated by the tests liminal Tone Audiometry (lTA) and Auditory Brainstem Response (ABR). Results: The main features in this research were overall better response in improving the hearing level with the mineralocorticoid therapy. This improvement in hearing levels was greater in women than in men, and a higher response was found in the left ear regardless of patient’s gender. Conclusions: The hearing gain was significantly superior in the mineralocorticoids group followed by the glucocorticoids group whereas the response to vasodilators was lesser and with no statistical significance.
