Pelvic exenterations for primary rectal cancer:Analysis from a 10-year national prospective database

AIM To identify short-term and oncologic outcomes of pelvic exenterations(PE) for locally advanced primary rectal cancer(LAPRC) in patients included in a national prospective *** Few studies report on PE in patients with LAPRC. For this study, we included PE for LAPRC performed between 2006 and 2017, as available, from the Rectal Cancer Registry of the Spanish Association of Surgeons [Asociación Espa?ola de Cirujanos(AEC)]. Primary endpoints included procedure-associated complications, 5-year local recurrence(LR), disease-free survival(DFS) and overall survival(OS). A propensitymatched comparison with patients who underwent non-exenterative surgery for low rectal cancers was performed as a secondary *** Eight-two patients were included. The mean age was 61.8 ± 11.5 years. More than half of the patients experienced at least one complication. Surgical site infections were the most common complication(abdominal wound 18.3%, perineal closure 19.4%). Thirty-three multivisceral resections were performed, including two hepatectomies and four metastasectomies. The long-term outcomes of the 64 patients operated on before 2013 were assessed. The five-year LR was 15.6%, the distant recurrence rate was 21.9%, and OS was 67.2%, with a mean survival of 43.8 mo. R+ve resection increased LR [hazard ratio(HR) = 5.58, 95%CI: 1.04-30.07, P = 0.04]. The quality of the mesorectum was associated with DFS. Perioperative complications were independent predictors of shorter survival(HR = 3.53, 95%CI: 1.12-10.94, P = 0.03). In the propensity-matched analysis, PE was associated with better quality of the specimen and tended to achieve lower LR with similar *** PE is an extensive procedure, justified if disease-free margins can be obtained. Further studies should define indications, accreditation policy, and quality of life in LAPRC.

Stability analysis of the fluttering and autorotation of flow-induced rotation of a hinged flat plate

This work describes investigations performed on the interaction of uniform current and freely rotating plate about a fixed vertical axis. Fluttering and autorotation are two different motions that may occur during the flow induced rotation. The dimensional analysis proves that the motion in flow induced rotation motion is governed essentially by the dimensionless moment of inertia and Reynolds number. Certain combinations define the stability boundaries between fluttering and autorotation. Fluttering is oscillation of body about a vertical axis and the autorotation is a name given to the case when the body turns continuously about the vertical axis First, the loads and moment coefficients are calculated by experiments and streamline theory for different angles of attack for a fixed fiat plate. Then for dynamic case, a bifurcation diagram is presented based on experiments to classify different motion states of flow induced rotation. Finally, a dynamical model is proposed for stability analysis of flow induced rotation of a flat plate.

Aims We aimed to evaluate how climatic fluctuations influence the plasticity of anatomical vessel traits and the width of annual tree-rings of two relict-endemic Mexican Magnolia ***,few studies have assessed the drought effect on vessel traits in tropical montane cloud trees of eastern *** Through digital images of growth rings,we assessed the tree radial growth rate,age of the trees and plasticity in vessel traits regarding climatic fluctuations of the Mexican Magnolia species *** compared vessel density,hydraulic diameter and percentage of conductive area in drought years(DY)and non-drought years(NDY)in two Mexican Magnolia *** Findings For the first time,the plasticity that occurs in porous wood vessel traits to long-term climatic fluctuations was analysed for two endangered Magnolia species(Magnolia vovidesii and ***)from two tropical montane cloud forests in *** found that temperature and precipitation were strongly associated with differences in tree-ring width when DY and NDY were *** analyses revealed that a high plasticity in vessel anatomy of diffuse-porous wood was related to temperature and/or water availability for both Magnolia species *** concluded that anatomical adaptations to DY resulted in a substantial reduction in vessel traits when compared with NDY,and that the plastic adaptations played an essential role in water transport and safety for the survival of the studied species during stressful long periods.

Population dynamics of three lizard species from the genus Sceloporus:short-term changes in demographic parameters

Most demographic studies focus on numerical changes that occur within populations across ***,typically studies at an interannual scale do not provide information on the particular times of the year(particular months or seasons)when rates of survival,recruitment,or migration increase or decrease due to physiological,behavioral or ecological *** monthly or seasonal changes in demographic parameters may lead to substantial variations in population *** this study,we collected capture-mark-recapture data on 3 species of lizards of the genus Sceloporus(Sceloporus torquatus,Sceloporus grammicus and Sceloporus megalepidurus)found in ecologically similar habitats to examine potential changes in demographic rates among 3 different climatic seasons:rainy,cold-dry and warm-dry *** tested different hypotheses about the effect of these seasons on survival,recruitment of new adults,and temporary *** found that during the season with severe thermal constraints,the cold-dry season,survival of *** decreased *** also detected a considerable increase in the recruitment rate of *** during the rainy season,when these lizards are establishing territories and finding *** contrast,we found no evidence of intra-annual changes in the rate of temporary *** addition,we calculated abundance and population growth rates for each species and for each *** study represents a significant contribution to the understanding of intra-annual demographic variation in lizards.

Fast synthesis of gold nanoparticles by cold atmospheric pressure plasma jet in the presence of Au^(+) ions and a capping agent

Homogeneous gold nanoparticles were synthesized under atmospheric pressure using a nonthermal helium plasma jet in a single-step process.A current power supply was used to generate the plasma discharge rich in diverse reactive *** species induce rapid chemical reactions responsible for the reduction of the gold salts upon contact with the liquid *** this study,spherical and monodispersed gold nanoparticles were obtained within 5 min of plasma exposure using a solution containing gold(Ⅲ)chloride hydrate(HAuCl_(4))as a precursor and polyvinylpyrrolidone(PVP)as a capping agent to inhibit *** formation of these metal nanoparticles was initially perceptible through a visible change in the sample's color,transitioning from light yellow to a red/pink *** was subsequently corroborated by UVvis spectroscopy,which revealed an optical absorption in the 520-550 nm range for Au NPs,corresponding to the surface plasmon resonance(SPR)*** investigation into the impact of various parameters,including plasma discharge duration,precursor and capping agent concentrations,was carried out to optimize conditions for the formation of well-separated,spherical gold *** light scattering(DLS)was used to measure the size of these nanoparticles,transmission electron microscopy(TEM)was used to observe their morphology and X-ray diffraction(XRD)was also employed to determine their crystallographic *** results confirm that homogeneous spherical gold nanoparticles with an average diameter of 13 nm can be easily synthesized through a rapid,straightforward,and environmentally friendly approach utilizing a helium atmospheric pressure plasma.

Surgical risk for patients with Chagasic achalasia and its correlation with the degree of esophageai dilation

AIM： To analyze the risk of cardiovascular complications in patients with indication for surgical treatment of Chagasic esophageal achalasia and to correlate the surgical risks with the degree of esophageal dilation, thereby proposing a risk scale index. METHODS： One hundred and twenty-four patients with Chagasic esophageal achalasia, who received surgical treatment at the Hospital das Clinicas of the Federal University of Goiás, were included in this study. The patients were mostly related to the postoperative complications due to the cardiovascular system. All the patients were submitted to：（1） clinical history to define the cardiac functional class （New York Heart Association）; （2） conventional 12-lead electrocardiogram at rest; and （3） contrast imaging of the esophagus to determine esophageal dilatation according to Rezende＇s classification of Chagasic megaesophagus. RESULTS： An assessment of the functional classification （FC） of heart failure during the preoperative period determined that 67 patients （54.03%） were assigned functional class Ⅰ （FC Ⅰ）, 46 patients （37.09%） were assigned functional class Ⅱ （FC Ⅱ）, and 11 patients （8.87%） were assigned functional class Ⅲ （FC Ⅲ）. None of the patients were assigned to functional class Ⅳ （FC Ⅳ）. There was a positive correlation between the functional class and the postoperative complications （FC Ⅰ×FC Ⅱ： P〈0.001; FC Ⅰ×FC Ⅲ： P〈0.001）. The ECG was normal in 44 patients （35.48%） and presented abnormalities in 80 patients （64.52%）. There was a significant statistical correlation between abnormal ECG （arrhythmias and primary change in ventricular repolarization） and postoperative complications （P〈0.001）. With regard to the classification of the Chagasic esophageal achalasia, the following distribution was observed： group Ⅱ, 53 patients （42.74%）; group Ⅲ, 37 patients （29.83%）; and group Ⅳ, 34 patients （27.41%）. In 2000,the small bowel capsule revolutionized the management of patients with small bowel ***,the technological development achieved by the new models of double-headed endoscopic capsules,as miniaturized devices to evaluate the small bowel and colon[pan-intestinal capsule endoscopy(PCE)],makes this non-invasive procedure a disruptive concept for the management of patients with digestive *** technology is expected to identify which patients will require conventional invasive endoscopic procedures(colonoscopy or balloon-assisted enteroscopy),based on the lesions detected by the capsule,i.e.,those with an indication for biopsies or endoscopic *** use of PCE in patients with inflammatory bowel diseases,namely Crohn’s disease,as well as in patients with iron deficiency anaemia and/or overt gastrointestinal(GI)bleeding,after a non-diagnostic upper endoscopy(esophagogastroduodenoscopy),enables an effective,safe and comfortable way to identify patients with relevant lesions,who should undergo subsequent invasive endoscopic *** recent development of magnetically controlled capsule endoscopy to evaluate the upper GI tract,is a further step towards the possibility of an entirely non-invasive assessment of all the segments of the digestive tract,from mouth-to-anus,meeting the expectations of the early developers of capsule endoscopy.

本文报告了采用掺硼金刚石阳极(BDD)电氧化和电芬顿工艺处理真实水体中注入含有少量药物残留物的单组分和多组分溶液(即1 mg.L-1对乙酰氨基酚和(或)1 mg.L-1水杨酸,pH=3)的研究结果.以恒定电流密度方式在BDD/Pt电解池中进行电氧化,而在BDD/空气扩散电解池中进行电芬顿,从而在阴极电生H2O2.结果表明,由于乙酰氨基酚和水杨酸均与溶液中氯离子氧化所产生的活性氯物种发生反应,因此,电氧化处理真实水体中两种药物的降解要比超纯水中添加0.05 mol.L-1Na2SO4快.这种含氯氧化剂的反应活性甚至超过了阳极形成的有限的羟基活性基,提高电流密度大大加速了两种污染物的消除.在真实水体自然碱性pH下得到了类似的结果.当电氧化处理两种药物的混合物时,两种药物上面产生的氧化剂的竞争作用,导致药物的降解变慢,只有电芬顿处理真实水体时药物的降解才稍微加快,这是由于H2O2与Cl-的反应,生成了反应活性弱得多的含氯活性基,从而抑制了电生的H2O2和添加的Fe2+之间发生的芬顿反应所形成的同相羟基活性基的累积.对于添加了药物的真实水体,在较高的电流密度下电氧化可得到较好的天然有机物成分(NOM)矿化度,且添加0.05 mol.L-1Na2SO4效果会更好.虽然在药物溶液的电氧化中检测出微量的氧化副产物,如对苯醌、NO3-和NH4+离子,但在本研究条件下无法去除真实水体中所含有的氮基化合物.
