BACKGROUND The prevalence of heart failure(HF)increases with age,and it is one of the leading causes of hospitalization and death in older patients.However,there are little data on in-hospital mortality in patients with HF≥75 years in spain.METHODs A retrospective analysis of the spanish Minimum Basic Data set was performed,including all HF episodes discharged from public hospitals in spain between 2016 and 2019.Coding was performed using the International Classification of Diseases,10th Revision.Patients≥75 years with HF as the principal diagnosis were selected.We calculated:(1)the crude in-hospital mortality rate and its distribution according to age and sex;(2)the risk-standardized in-hospital mortality ratio;and(3)the association between in-hospital mortality and the availability of an intensive cardiac care unit(ICCU)in the hospital.REsULTs We included 354,792 HF episodes of patients over 75 years.The mean age was 85.2±5.5 years,and 59.2%of patients were women.The most frequent comorbidities were renal failure(46.1%),diabetes mellitus(35.5%),valvular disease(33.9%),cardiorespiratory failure(29.8%),and hypertension(26.9%).In-hospital mortality was 12.7%,and increased with age[odds ratio(OR)=1.07,95%CI:1.07–1.07,Ps lower in women(OR=0.96,95%CI:0.92–0.97,Ps of mortality were the presence of cardiogenic shock(OR=19.5,95%CI:16.8–22.7,Pstroke(OR=3.5,95%CI:3.0–4.0,Pspitals with ICCU,the in-hospital risk-adjusted mortality tended to be lower(OR=0.85,95%CI:0.72–1.00,P=0.053).CONCLUsIONs In-hospital mortality in patients with HF≥75 years between 2016 and 2019 was 12.7%,higher in males and elderly patients.The main predictors of mortality were cardiogenic shock,stroke,and advanced cancer.There was a trend toward lower mortality in centers with an ICCU.

The Intergovernmental Panel on Climate Change(IPCC)recognises the pivotal role of renewable energies in the future energy system and the achievement of the zero-emission target.The implementation of renewables should provide major opportunities and enable a more secure and decentralised energy supply system.Renewable fuels provide long-term solutions for the transport sector,particularly for applications where fuels with high energy density are required.In addition,it helps reducing the carbon footprint of these sectors in the long-term.Information on biomass characteristics feedstock is essential for scaling-up gasification from the laboratory to industrial-scale.This review deals with the transformation biogenic residues into a valuable bioenergy carrier like biomethanol as the liquid sunshine based on the combination of modified mature technologies such as gasification with other innovative solutions such as membranes and microchannel reactors.Tar abatement is a critical process in product gas upgrading since tars compromise downstream processes and equipment,for this,membrane technology for upgrading syngas quality is discussed in this paper.Microchannel reactor technology with the design of state-of-the-art multifunctional catalysts provides a path to develop decentralised biomethanol synthesis from biogenic residues.Finally,the development of a process chain for the production of(i)methanol as an intermediate energy carrier,(ii)electricity and(iii)heat for decentralised applications based on biomass feedstock flexible gasification,gas upgrading and methanol synthesis is analysed.

Twenty-four-hour movement guidelines during middle adolescence and their association with glucose outcomes and type 2 diabetes mellitus in adulthood

Purpose:The aim of the present study was to determine the association between adherence to the 24-h movement guidelines during middle adolescence and glucose outcomes(glycated hemoglobin and fasting glucose)and type 2 diabetes mellitus(T2DM)in adulthood,14 and 22 years later.Methods:We analyzed data from apparently healthy adolescents aged 12-18 years who participated in WavesⅠandⅡ(1994-1996,n=14,738),WaveⅣ(2008-2009,n=8913),and Wave V(2016-2018,n=3457)of the National Longitudinal study of Adolescent to Adult Health(Add Health)in the United states.Physical activity,screen time,and sleep duration were measured using questionnaires,and the 24-h guidelines were defined as:5 or more times moderate-to-vigorous physical activity per week,≤2 h per day of screen time,and 9-11 h of sleep for 12-13 years and 8-10 h for 14-17 years.Capillary and venous whole blood was collected and analyzed to determine glycated hemoglobin and fasting glucose for WavesⅣandⅤ,respectively.Results:Only 2.1%of the adolescents met all the 3 guidelines,and 37.8%met none of them.In both wavesⅣandⅤ,adolescents who met physical activity and screen time guidelines had lower odds of T2DM in adulthood than those who did not meet any of these guidelines(WaveⅣ;prevalence ratio(PR)=0.57,95%confidence interval(95%CI):0.21-0.89;Wave V:PR=0.43,95%CI:0.32-0.74).Only for Wave V did adolescents who met all 3 guidelines have lower odds of T2DM at follow-up compared with those who did not meet any of these guidelines(PR=0.47,95%CI:0.24-0.91).Also,for each increase in meeting one of the 24-h recommendations,the odds of T2DM decreased by 18%(PR=0.82,95%CI:0.61-0.99)and 15%(PR=0.85,95%CI:0.65-0.98)in adulthood for WavesⅣandⅤ,respectively.Conclusion:Promoting all 24-h movement guidelines in adolescence,especially physical activity and screen time,is important for lowering the potential risk of T2DM in adulthood.

Hatching asynchrony in birds is considered an adaptation to facilitate brood reduction because under conditions of food scarcity, the smallest nestling usually dies soon after hatching, thereby minimizing parental effort. However, in species with extreme hatching asynchrony, the last hatchlings paradoxically experience a very low probability of survival and death can take so long that it can hardly be considered an adaptation. Here, we propose and experimentally tested a new adaptive hypothesis explaining the brood reduction paradox, namely the“Male Manipulation Hypothesis”. Our hypothesis suggests that by inducing asynchronous hatching,females increase the feeding requirements of the brood, which will induce males to increase provisioning effort. In addition, females may extend the period of male manipulation by feeding the smallest nestling just enough to sustain life. Our study showed that male common blackbirds(Turdus merula) increased their effort(i.e., number of food items per hour) in experimental asynchronous broods compared to synchronous broods, while females reduced their contribution, as predicted by the hypothesis.

The extraction of bioactive compounds has become one of the most interesting areas of modern chemistry.For therapeutic reasons,it´s important to obtain antimicrobial agents from natural origin.The objective of the present study was to determine the inhibitory effect of ethanolic extract of basil(Ocimum basilicum L.var.Red Rubin)subjected to different concentrations of potassium(K^(+))on the activity of three bacterial strains that are pathogens in humans.susceptibility was evaluated by inhibition surface and these results were compared to two antibiotics:Gentamicin(GE)and Ciprofloxacin(CPF)for their efficacy against each bacterial strain.Analyzed variables presented significant difference(p≤0.05).According to the results,basil extract evaluated showed positive antibacterial activity against the three strains of Pseudomonas aeruginosa,Listeria monocytogenes and staphylococcus aureus on Mueller Hinton agar.It was observed highest inhibition areas of different diameters(15.3 mm,21.3 mm and 21.6 mm respectively)when the extract used was obtained from the plants with the highest concentration of potassium(13 mmol L-1).These values were even superior to the highest values showed on the treatments with the antibiotic gentamicin.

salivary Gland Choristoma of the Middle Ear and Review of the Literature

Conductive hearing loss due to middle ear masses is uncommon and usually diagnosed after biopsy. We present a case of a permanent facial palsy occurred following an uneventful biopsy during an exploratory tympanotomy in a salivary gland choristoma of the middle ear. Most salivary gland choristomas have been found in the head and neck. Its location in the ear is extremely rare, and thus we present the 38th case in English and non-English literature from the first publication by Taylor in 1961. Complete surgical removal of salivary gland choristomas of the middle ear is indicated when may not result in permanent damage to the facial nerve. Only biopsy and observation are recommended when the mass is intimately associated with the facial nerve or there are unsafe facial nerve abnormalities. Although the facial nerve is involved in 40% of cases, transient or even permanent facial palsies are exceptional. The reactivation of latent herpes virus in the facial canal may be involved in facial palsy’ etiology following minimal and uneventful middle ear surgery like a biopsy rather than nerve injury related to facial canal malformations.

Negative thermal diffusivity enhancement in semiconductor nanofluids

Colloidal suspensions of semiconductor InP@Zns nanoparticles were prepared using single-step procedure without precursor injection. Thermal properties of toluene containing InP@Zns semiconductor with different sizes (3.1, 4.2, and 4.6 nm) were measured by mode mismatched dualbeam thermal lens technique. This was done in order to measure the effect of the presence of semiconductor nanoparticles and size on the nanofluids thermal diffusivity. The characteristic time constant of the transient thermal lens was estimated by fitting the experimental data to the theoretical expression for transient thermal lens. The thermal diffusivity of the nanofluids (toluene, containing InP@Zns semiconductor nanoparticles) it seems to be strongly dependent on the presence of semiconductor nanoparticles and particles size. For the case of nanofluids consisting of InP@Zns nanoparticles dispersed in toluene, it was observed a decrease in the thermal diffusivity. such behavior differs from other nanofluids, in the sense that they had shown positive thermal diffusivity enhancement. The minimum diffusivity was achieved for the nanoparticles with lowest size. Plausible explanation for such nanofluids low thermal diffusivity with semiconductor nanoparticles is given. UV-Vis spectroscopy, TEM and high-resolution electron microscopy (HRTEM), and energy dispersive spectroscopy (EDs) techniques were used to characterize the InP@Zns nanoparticles.

Oil production and mainte nance are essential issues in naturally fractured reservoirs because they are the largest and most productive on earth.However,they present early water and gas channeling but could be remediated by using foaming agents to control these phenomena through blocking channeling areas.In Mexico these reservoirs have pressure up to 5,500 psi,high temperature up to 200℃,salinity up to400,000 ppm,and hardness up to 250,000 ppm;due to these thermodynamic conditions,there has been no available technology to form stable enough foams.In this work,a foaming supramolecular surfactant with the capability to chelate Ca^(2+)ions is examined.As a result,surfactant monomers are bridged by captured Ca^(2+)cations leading to the formation of high-molecular-weight oligomers,which significantly increment the viscosity of the solution improving the foam stability,and since at this manner the Ca2+cations are no longer available to precipitate as components of solid salts,the foaming supramolecular surfactant also performs as antiscalant.These observations are explained through quantum theoretical modeling.The foam is stable,effectively blocking the gas channels,whereas in presence of oil the foam is broken leading the oil to pass into the wellbore.The characteristic rheological properties of the foam allow its injection into the formation at a range of flow rates,foam qualities,and shear stress to achieve the flooding and the blocking of a variety of fractured carbonate formations,and the change of the wettability of the matrix,which is a desirable behavior in a huff and puff process,as reported in a previous publication about a successful pilot test of this foam.

In this paper we present the nonlinear optical characterization of Au/Pd nanoparticles in order to obtain the nonlinear refractive indices using the Z-scan technique. The experiments were performed using a 514 nm laser beam Ar+, with 14 Hz of modulation frequency, as excitation source. By using a lens the excitation beam was focused to a small spot and the sample was moved across the focal region along the z-axis by a motorized translation stage. seven samples with different concentration ratio of Au/Pd nanoparticles were prepared by simultaneous reduction of gold and palladium ions in presence of poly (N-vinyl-2-pirrolidone) (PVP) using ethanol as a reducing agent. In this work, we report the application of the Z-scan technique, to generate optical transmission of laser light as a function of the z position for solutions containing bimetallic nanoparticles of Au (core)/Pd (shell) with average sizes ranging from 3 to 5 nm. The magnitude of the obtained nonlinear refractive index was in the order of 10?8 cm2/W. Our results show that the nonlinear refractive index has a nonlinear behavior when the (Au/Pd) ratio was increased.

Animal models used to study direct peripheral nerve repair: a systematic review

Objective:Peripheral nerve repair is required after traumatic injury.This common condition represents a major public health problem worldwide.Recovery after nerve repair depends on several factors,including the severity of the injury,the nerve involved,and the surgeon’s technical skills.Despite the precise microsurgical repair of nerve lesions,adequate functional recovery is not always achieved and,therefore,the regeneration process and surgical techniques are still being studied.Pre-clinical animal models are essential for this research and,for this reason,the focus of the present systematic review(according to the PRIsMA statement)was to analyze the different animal models used in pre-clinical peripheral nerve repair studies.Data sources:Original articles,published in English from 2000 to 2018,were collected using the Web of science,scopus,and PubMed databases.Data selection:Only preclinical trials on direct nerve repair were included in this review.The articles were evaluated by the first two authors,in accordance with predefined data fields.Outcome measures:The primary outcomes included functional motor abilities,daily activity and regeneration rate.secondary outcomes included coaptation technique and animal model.Results:This review yielded 267 articles,of which,after completion of the screening,49 studies were analyzed.There were 1425 animals in those 49 studies,being rats,mice,guinea pigs,rabbits,cats and dogs the different pre-clinical models.The nerves used were classified into three groups:head and neck(11),forelimb(8)and hindlimb(30).The techniques used to perform the coaptation were:microsuture(46),glue(12),laser(8)and mechanical(2).The follow-up examinations were histology(43),electrophysiological analysis(24)and behavioral observation(22).Conclusion:The most widely used animal model in the study of peripheral nerve repair is the rat.Other animal models are also used but the cost-benefit of the rat model provides several strengths over the others.suture techniques are currently the first option for n
