Teaching Methodology for the Training of a Laparoscopic Technique: Intestinal Anastomosis

Traditionally surgeons learned surgical techniques by training them directly in the patient by imitating the actions of the surgeon with more experience. Today this methodology is difficult to apply, since there are multiple factors that hinder its use. The limited time of training systems, scarcity of resources, patient safety or the need to learn new techniques of minimally invasive surgery, make the training of surgeons require a different learning methodology. This study has designed a new methodology for the learning of surgical techniques that minimizes the impact of all the previous factors on the training of surgeons. Its effectiveness in the teaching of laparoscopic manual intestinal anastomosis is analyzed. It has been asked a.group of 24 expert surgeons teaching in intestinal anastomoses, which are the steps that must include the teaching of the technique. The phases evaluated with the highest score were included in the methodology. Once the steps of the methodology were designed, 25 general surgeons were chosen and divided into three groups according to their level of experience (experts, trained and beginners). Each participant performed five training sessions following the new methodology. The technical quality of the anastomosis, the overall development of the operation, as well as the feedback developed between the participants and the instructor was analyzed. After analyzing the results it was observed that structured feedback between the instructor and the participant increases the involvement of the participant in the learning process and reduces the time needed to acquire the competence. With this methodology the inexperienced participants perform the fifth anastomosis with quality parameters close to those obtained by the expert participants. In addition, this methodology allows to individualize the learning according to the needs of each participant.

Soil Quality Evaluation of Hydrocarbon Polluted Parcels

For hydrocarbon polluted soils that underwent bioremediation it is important to assess its condition after a period of time, but it is more useful when there is an opportunity of comparison against an unpolluted soil and an untreated polluted one. This paper provides a comparison of three adjacent parcels, being the first clean, the second polluted and bioremediated, the third polluted and left to natural attenuation. Study was conducted determining pH, electrical conductivity, carbonates, soil organic matter, chemical oxygen demand, eight anions, and twelve metals. Data were compared against those references for agricultural soils found in the Mexican NOM-021. A Pearson correlation was applied to find coincidences between the three parcels, obtained results allowed to say that bioremediated parcel allowed for most uniform pH, negligible salinity risk, and medium content of soil organic matter, but treatment has enabled heavy metal accumulation since its values are higher in respect to the other parcels. Natural attenuated parcel has some spots with lower pH, a moderately saline risk, a high content of soil organic matter, and lower content of heavy metals. The clean or unaffected parcel exhibit the higher pH values, a slightly saline condition, soil organic matter ra.ges from high to very high content, heavy metals content is medium, but no reaching da.gerous levels. An important assessment is that bioremediation has enhanced the bioavailability of soil organic matter but it is not similar to the values in the unpolluted parcel.

In their recent article in Science,Chi et al.[1]characterized a mechanism that regulates tissue/organ-specific immunity in *** findings revealed that the androgen type 2 innate lymphoid cell-dendritic cell axis played a prominent role in a microbiota-and sex hormone-dependent *** observations implicate sexual immune dimorphism as a molecular,cellular and tissue/organ-specific regulatory hub for the(im)balance between health and *** factors,including nutritional status,diet,vital light-dark rhythms(i.e.,circadian and infradian rhythms)and the microbiota,shape the development and function of the immune system differently in males and females,which can result in differences in susceptibility to autoimmune diseases,malignancies and infectious diseases and affect the outcome of vaccination[3](Fig.1).

Influence of in vitro capacitation time on structural and functional human sperm parameters

A cascade of dramatic physiological events is linked to the sperm acrosome reaction and binding to the oocyte's zona pellucida during human sperm ***,structural and functional sperm cha.ges during capacitation currently remain poorly ***,we performed a multibiomarker approach based on the utilization of sperm concentration,motility,viability,morphology,acrosome reaction,tyrosine phosphorylation,DNA fragmentation,and lectin-binding sites to analyze the impact caused by swim-up selection times(uncapacitated,1 h capacitated,and 4 h capacitated)on sperm function and structure in normozoospermic *** found that a 4 h swim-up capacitation increased sperm quality,because a la.ge number of cells with normal morphology and lower DNA fragmentation rates were ***,the long-term capacitation induced a higher percentage of cells with tyrosine phosphorylation of the principal piece as well as a redistribution of lectin-binding ***,the multivariate biomarkers analyzed showed a less variable distribution on spermatozoa recovered after 4 h capacitation than that with the shorter capacitation *** findings stress the importance of capacitation time as a relevant factor in sperm quality with potential biological reproductive implications both for basic research and in assisted reproduction techniques.

Spatial analysis and structure of a cross-timber stand in the Tallgrass Prairie Preserve (Pawhuska, Oklahoma)

I analyzed the spatial distribution and structure of trees in a cross timber forest in the Tallgrass Prairie Preserve （Pawhuska, Oklahoma, USA）. I mapped and measured diameter of all stems, saplings （〉1.5 m tall） and dead trees in a 4-ha plot. The stand was dominated by Quercus stellata and Q. marilandica. In total, I mapped 7,636 trees, consisting of 6,785 Q. stellata, 846 Q. marilandica, 2 Celtis occidentalis, 1 Fraxinus pensilvanica and 2 Prunus americana. For saplings, I mapped 54 Q. stellata and 21 Q. marilandica. The size class distribution of the two dominant species did not differ. The dominant mortality class was “standing dead”, while I only found saplings less than 2 m tall. The spatial distribution of the species indicated segregation in the use of the environment, generating a clumped univariate distribution of stems of the same species within radii of 30 m, but repulsion outside 30 m. This segregation can be explained by the different ecological requirements of each species.

The precipitation kinetics in a low carbon high strength low alloy (HSLA) steel,microalloyed with V+Nb has been studied by using the stress relaxation *** technique was carried out in a dynamic dilatometer and allows to determine the onset and finishing precipitation times in hot deformed *** evolution of the precipitates was also observed using field emission scanning electron microscopy (FESEM) on samples quenched at different times at given *** is completed with the predictions by the thermodynamic software FactSage to identify stable precipitate phases under equilibrium *** confirm the feasibility of the stress relaxation technique as an efficient method for investigating precipitation kinetics.

One of the biggest challenges in the biocompatibility of implantable metals is the prevention of the stress shielding effect,which is related to the coupling of the bone-metal mechanical *** stress shielding phenomenon provokes bone resorption and the consequent adverse effects on prosthesis ***,it can be inhibited by adapting the stiffness of the implant *** the use of titanium(Ti)porous structures is a.great alternative not only to inhibit this effect but also to improve the osteointegration of orthopedic and dental implants,a brief description of the techniques used for their manufacturing and a review of the current commercialized implants produced from porous Ti assemblies are compiled in this *** powder metallurgy(PM)with space holder(SH)is a powerful technology used to produce porous Ti structures,it is here discussed its potential for the fabrication of medical devices from the perspectives of both design and *** most important parameters of the technique such as the size and shape of the initial metallic particles,the SH and binder type of materials,the compaction pressure of the green form,and in the sintering stage,the temperature,atmosphere,and time are reviewed according to the bibliography ***,the importance of the porosity and its types together with the influence of the mentioned parameters in the final porosity and,consequently,in the ultimate mechanical properties of the structure are ***,a few examples of the PM-SH application for the manufacturing of orthopedic implants are presented.
