Ischaemic heart disease(IHD)is a major cause of morbidity and mortality *** there have been major advances in this field,these patients are still a higher risk *** such,strategies to mitigate risk and tailor secondary prevention measures are of the utmost *** rehabilitation(CR),encompassing several domains including exercise training,cardiovascular risk factor optimization,nutritional and psychological assessments,as well as other ancillary interventions has shown to be one of the pillars in the contemporary management of patients with ***,CR is associated with several benefits in this population,ranging from functional capacity to improvements in *** this,there are still several issues concerning the optimal application of CR which are still not fully ascertained,such as lack of referral and completion,as well as questions related to programme design(particularly among patients with multiple comorbidities).In this review,we aim at presenting a pragmatic overview on the current role of CR in the management of individuals with IHD,while also discussing some of the caveats in the current data,as well as future concepts which could help improve the uptake and personalization of this pivotal time-tested intervention.

The extraction of bioactive compounds has become one of the most interesting areas of modern *** therapeutic reasons,it´s important to obtain antimicrobial agents from natural *** objective of the present study was to determine the inhibitory effect of ethanolic extract of basil(Ocimum basilicum *** Rubin)subjected to different concentrations of potassium(K^(+))on the activity of three bacterial strains that are pathogens in *** was evaluated by inhibition surface and these results were compared to two antibiotics:Gentamicin(GE)and Ciprofloxacin(CpF)for their efficacy against each bacterial *** variables presented significant difference(p≤0.05).According to the results,basil extract evaluated showed positive antibacterial activity against the three strains of pseudomonas aeruginosa,Listeria monocytogenes and Staphylococcus aureus on Mueller Hinton *** was observed highest inhibition areas of different diameters(15.3 mm,21.3 mm and 21.6 mm respectively)when the extract used was obtained from the plants with the highest concentration of potassium(13 mmol L-1).These values were even superior to the highest values showed on the treatments with the antibiotic gentamicin.

Oil production and mainte nance are essential issues in naturally fractured reservoirs because they are the largest and most productive on ***,they present early water and gas channeling but could be remediated by using foaming agents to control these phenomena through blocking channeling *** Mexico these reservoirs have pressure up to 5,500 psi,high temperature up to 200℃,salinity up to400,000 ppm,and hardness up to 250,000 ppm;due to these thermodynamic conditions,there has been no available technology to form stable enough *** this work,a foaming supramolecular surfactant with the capability to chelate Ca^(2+)ions is *** a result,surfactant monomers are bridged by captured Ca^(2+)cations leading to the formation of high-molecular-weight oligomers,which significantly increment the viscosity of the solution improving the foam stability,and since at this manner the Ca2+cations are no longer available to precipitate as components of solid salts,the foaming supramolecular surfactant also performs as *** observations are explained through quantum theoretical *** foam is stable,effectively blocking the gas channels,whereas in presence of oil the foam is broken leading the oil to pass into the *** characteristic rheological properties of the foam allow its injection into the formation at a range of flow rates,foam qualities,and shear stress to achieve the flooding and the blocking of a variety of fractured carbonate formations,and the change of the wettability of the matrix,which is a desirable behavior in a huff and puff process,as reported in a previous publication about a successful pilot test of this foam.

Liver disease and erythropoietic protoporphyria:A concise review

The porphyries are a group of metabolic disorders characterized by deficiencies in the activity of enzymes involved in the biosynthesis of *** erythropoietic protoporphyria(Epp),in the majority of cases an autosomal dominant disease,there is a mutation of the gene that encodes ferrochelatase(FECH).FECH deficiency is associated with increased concentrations of protoporphyrin in erythrocytes,plasma,skin and *** prevalence of this inherited disorder oscillates between 1:75 000 and 1:200 *** manifestations of Epp appear in early infancy upon first exposure to the ***,approximately 5%-20% of patients with Epp develop liver *** of protoporphyrin in the liver is associated with cholestatic phenomena and oxidative stress that predisposes to hepatobiliary disease of varying degrees of severity,such as cholelithiasis,mild parenchymal liver disease,progressive hepatocellular disease with end-stage liver disease and acute liver *** damage is the major risk in Epp patients,so surveillance and frequent clinical and biochemical liver follow-up is *** diagnostic approach consists in detecting increased levels of protoporphyrin,decreased activity of FECH and genetic analysis of the FECH gene.A variety of nonsurgical therapeutic approaches have been adopted for the management of Epp associated with liver disease,but none of these has been shown to be unequivocally ***,some may have a place in preparing patients for liver *** transplantation does not correct the constitutional deficiency of ***,there is a risk of recurrence of liver disease after liver transplantation as a result of continuing overproduction of *** authors recommend that bone marrow transplantation should be considered in liver allograft recipients to prevent recurrence of hepatic disease.

Clinical assessment and management of liver fibrosis in nonalcoholic fatty liver disease

Non-alcoholic fatty liver disease(NAFLD)is among the most frequent etiologies of cirrhosis worldwide,and it is associated with features of metabolic syndrome;the key factor influencing its prognosis is the progression of liver *** review aimed to propose a practical and stepwise approach to the evaluation and management of liver fibrosis in patients with NAFLD,analyzing the currently available *** the assessment of NAFLD patients,it is important to identify clinical,genetic,and environmental determinants of fibrosis development and its *** properly detect fibrosis,it is important to take into account the available methods and their supporting scientific evidence to guide the approach and the sequential selection of the best available biochemical scores,followed by a complementary imaging study(transient elastography,magnetic resonance elastography or acoustic radiation force impulse)and finally a liver biopsy,when *** help with the selection of the most appropriate method a Fagan′s nomogram analysis is provided in this review,describing the diagnostic yield of each method and their post-test probability of detecting liver ***,treatment should always include diet and exercise,as well as controlling the components of the metabolic syndrome,+/-vitamin E,considering the presence of sleep apnea,and when available,allocate those patients with advanced fibrosis or high risk of progression into clinical *** final end of this approach should be to establish an opportune diagnosis and treatment of liver fibrosis in patients with NAFLD,aiming to decrease/stop its progression and improve their prognosis.

Is interferon-beta an alternative treatment for chronic hepatitis C?

The treatment of chronic hepatitis C （CHC） is still far from optimal, particularly for those subpopulations that do not respond to the standard combination therapy with Interferon-α （IFNα） plus ribavirin. Although in some cases the use of higher doses or longer treatment periods may be effective, these approaches are generally associated with a higher incidence of adverse events, which may either lead to a reduction in patient compliance or require drug withdrawal. IFNβ could represent an interesting alternative for treating CHC patients. Controversial data about IFNβ efficacy in CHC exist, the main reason being that many results stem from pilot studies with small cohorts of patients. However, promising results have been obtained in some subgroups of patients that fail to respond to IFNα. Additionally, the good tolerability of IFNβ represents an important advantage of the drug. The rates of dropouts in controlled clinical trials, as well as the need for dose reductions or treatment discontinuation are very low. It might be worth assessing the value of IFNβ plus ribavirin in randomized studies with a larger cohort of patients, not eligible or not tolerating standard therapy, and for non-responders.

Energy Conversion Factor for Gasoline Engines in Real-World Driving Emission Cycle

A precise energy conversion factor is required to define the impact of greenhouse gas emissions by gasoline-powered vehi-cles and policies that will guide the application of future *** current energy conversion factor adopted by many countries is based on the Willans line approach,initially proposed in 1888 for steam engines,later adapted for internal combustion *** actual energy conversion factor,which defines the energy conversion for drivers in real traffic,is *** this article,eight world-class engines are tested in an engine bench for the acquisition of specific fuel consumption 3D ***,their energy conversion factors,calculated by dividing the energy output by the energy input,are simulated in real and urban traffic,acquired according to the real driving emissions(RDE)*** addition,a reference vehicle is instrumented to measure the energy input(fuel flow)and the energy output(mechanical energy in the half axles)under the same RDE cycle *** results of both procedures are very similar,respectively,0.405±0.04 L/kWh for the simulation based on eight benchmark engines,and 0.392±0.04 L/kWh for the reference vehicle driven in RDE traffic conditions,with a 95%confidence *** turbocharged engines,the factor attained by the simulation is 0.395±0.04 L/*** values of the energy conversion factor for gasoline engines got in this research are higher than those obtained through the Willans line approach,suggesting a new standard value of 0.405 L/kWh,replacing the current 0.264 L/*** could substantially change the greenhouse gas emissions in a tank-to-wheel approach for the entire vehicle and add-on eco-innovations.

Does an association exist between chronic pancreatitis and liver cirrhosis in alcoholic subjects?

AIM: To study the possible association between chronic pancreatitis (Cp) and liver cirrhosis (LC) of alcoholic etiology,after excluding any other ***: One hundred and forty consecutive alcoholic patients were subdivided into three groups: Cp (n = 53),LC (n = 57),and asymptomatic alcoholic (n = 30).Clinical,biochemical and morphological characteristics,Child-pugh index,indocyanine green test,and fecal pancreatic elastase-1 test were ***: In patients with cirrhosis,major clinical manifestations of Cp such as pancreatic pain and steatorrhea,as well as imaging alterations of Cp such as calcifications,duct dilation and pseudocysts were absent; insulin-dependent diabetes was present in 5.3% of cases,and elastase-1 test was altered in only 7%,and severely altered in *** patients with Cp,clinical characteristics of cirrhosis such as ascites,encephalopathy and gastrointestinal hemorrhage were present in one case,Child-pugh grade > A in 5.7%,and altered indocyanine green test in 1.9% *** asymptomatic alcoholism,there was only a non-coincident alteration of elastase-1 test and indocyanine test in 14.8% and 10%,respectively,but other characteristics of cirrhosis or Cp were *** inverse correlation (r = -0.746) between elastase-1 test and indocyanine test was found in alcoholic ***: There is a scarce coincidence in clinical and morphological alterations among patients with Cp or LC of alcoholic etiology,but an inverse correlation between pancreatic and liver function *** findings support that these alcoholic diseases evolve in a different manner and have different etiopathogenesis.
