Prehospital critical care drug-therapy and 30-day mortality in patients with acute respiratory disease

BACKGROUnD: Prehospital medication is a reality, and the role of these therapies must be explored to assess their validity, especially for acute respiratory diseases, which are usually associated with increased morbidity. The aim of this study was to examine the association of prehospital medication use with mortality in patients with acute respiratory ***: A prospective, multicenter, emergency medical service(EMS) delivery cohort study was carried out in adults with unselected respiratory diseases managed by EMS who were transferred to the emergency department. From January 1, 2019, to October 31, 2023, six advanced life support units, thirty-eight basic life support units, and four hospitals in Spain participated in the study. Demographic data, vital signs, use of mechanical ventilation, prehospital respiratory diagnosis, and prehospital medication were collected. The primary outcome was 30-day in-hospital ***: A total of 961 patients were included, with a mortality rate of 17.5%(168 patients). Age, an increasing number of comorbidities, the use of invasive mechanical ventilation(IMV), the use of major analgesics, hypnotics, and bicarbonate were risk factors. In contrast, elevated systolic blood pressure and Glasgow Coma Scale scores were found to be protective factors against mortality. The predictive capacity of the model reached an area under the curve(AUC) of 0.857(95% confidence interval [95% CI] 0.827–0.888).COnCLUSIOn: Our data revealed that IMV, major analgesics, hypnotics and bicarbonate administration were associated with elevated mortality. Adding prehospital drug therapy information to demographic variables and vital signs could improve EMS decision-making, allowing a better characterization of patients at risk of clinical worsening.

Agronomic treatments combined with embryo rescue for rapid generation advancement in tomato speed breeding

Unlike other major crops,little research has been performed on tomato to reduce the generation time for speed *** evaluated several agronomic treatments to reduce the generation time of tomato in the‘M82'(determinate)and‘Moneymaker'(indeterminate)varieties and evaluated the best combination in conjunction with embryo *** container sizes with volumes of 0.2 L(XS),0.45 L(S),0.8 L(M),1.3 L(L),and6 L(XL),were evaluated in the first experiment under the autumn *** found that plants grown in XL containers exhibited better development and required less time from sowing to anthesis(DSA)and from anthesis to fruit ripening(DAR).In the second experiment,using XL containers in the autumn-winter cycle,we evaluated the effects of cold priming at the cotyledonary stage,water stress,P supplementation,and K supplementation on generation *** to the control,we found that cold priming significantly reduced the number of leaves,plant height to first the inflorescence,and DSA(2.7 d),whereas K supplementation reduced the DAR(8.8 d).In contrast,water stress and P supplementation did not significantly affect any of the measured traits like DAR,DSA or fruit *** validate these data,in a third experiment with XL containers in the spring-summer cycle,the combination of cold priming and K supplementation was tested,confirming the significant effect of this combination on the reduction of generation time(2.9 d for DSA and 3.9 d for DAR)compared to the *** rescue during the cell expansion cycle(average of 22.0 d and 23.3 d after anthesis for‘M82'and‘Moneymaker',respectively)allowed the shortening of the generation time by 8.7 d in‘M82'and 11.6 d in‘Moneymaker'compared to the in planta fruit *** combination of agronomic treatments with embryo rescue can effectively increase the number of generations per year from three to four for speed breeding of tomato.

The biosynthesis and wall-binding of hemicelluloses in cellulose-deficient maize cells:An example of metabolic plasticity

Cell-suspension cultures（Zea mays L.,Black Mexican sweet corn） habituated to 2,6-dichlorobenzonitrile（DCB） survive with reduced cellulose owing to hemicellulose network modification. We aimed to de fine the hemicellulose metabolism modifications in DCB-habituated maize cells showing a mild reduction in cellulose at different stages in the culture cycle. Using pulse-chase radiolabeling, we fed habituated and non-habituated cultures with [3H]arabinose,and traced the distribution of 3H-pentose residues between xylans, xyloglucans and other polymers in several cellular compartments for 5 h. Habituated cells were slower taking up exogenous [3H]arabinose. Tritium was incorporated into polysaccharide-bound arabinose and xylose residues, but habituated cells diverted a higher proportion of their new [3H]xylose residues into（hetero） xylans at the expense of xyloglucan synthesis. During logarithmic growth, habituated cells showed slower vesicular traf ficking of polymers,especially xylans. Moreover, habituated cells showed a decrease in the strong wall-binding of all pentose-containing polysaccharides studied; correspondingly, especially in log phase cultures, habituation increased the proportion of 3H-hemicelluloses（[3H]xylans and [3H]xyloglucan） sloughed into the medium. These findings could be related to the cel walls’ cellulose-deficiency, and consequent reduction in binding sites for hemicelluloses; the data could also re fl ect the habituated cells’ reduced capacity to integrate arabinox ylans by extra-protoplasmic phenolic cross-linking, as well as xyloglucans, during wall assembly.

The Influence of Meteorology and Atmospheric Transport Patterns on the Sulfate Levels in Raiwater in Orizaba Valley, Veracruz, Mexico

This work assessed the relationship between synoptic circulation patterns, meteorological conditions and sulfate levels in rainwater in Orizaba Valley, Veracruz, Mexico during 2015. Samples were collected by using an automatic deposition sampler considering three climatic periods: Cold fronts or nortes, Dry and Rainy seasons. Sulfate in rainwater was determined by turbidimetry (nMX-AA-SCFI-074-1981). A backward trajectory model (HYSPLIT) was used to characterize the potential source regions and transport pathways for air parcels arriving to Orizaba Valley;trajectories extended five days during 2015 were calculated. Mean concentrations of sulfate were 105.33, 37.5, and 52.5 μEq·l-1, for Cold Fronts, Dry and Rainy seasons, respectively. An evident dilution pattern was observed with lower concentration levels when rainfall increased. Mean values for sulfate concentration exceeded the background hemispheric value reported by Galloway et al. (1982) for remote sites (10 μEq·l-1), exceeding almost 10, 3.7 and 5.2 times, during the nortes, Rainy and Dry seasons, respectively. From the obtained results, it was concluded that the high sulfate levels in the rainwater measured in Orizaba Valley had their origin in different upwind regional sources as a result of long-range transport during Rainy and nortes seasons.

alcohol drinking and mammary cancer: pathogenesis and potential dietary preventive alternatives

Alcohol consumption is associated with an increased risk of breast cancer, increasing linearly even with a moderate consumption and irrespectively of the type of alcoholic beverage. It shows no dependency from other risk factors like menopausal status, oral contraceptives, hormone replacement therapy, or genetic history of breast cancer. The precise mechanism for the effect of drinking alcohol in mammary cancer promotion is still far from being established. Studies by our laboratory suggest that acetaldehyde produced in situ and accumulated in mammary tissue because of poor detoxicating mechanisms might play a role in mutational and promotional events. Additional studies indicated the production of reactive oxygen species accompanied of decreases in vitamin E and GSH contents and of glutathione transferase activity. The resulting oxidative stress might also play a relevant role in several stages of the carcinogenic process. There are reported in literature studies showing that plasmatic levels ofestrogens significantly increased after alcohol drinking and that the breast cancer risk is higher in receptor ERpositive individuals. Estrogens are known that they may produce breast cancer by actions on ER and also as chemical carcinogens, as a consequence of their oxidation leading to reactive metabolites. In this review we introduce our working hypothesis integrating the acetaldehyde and the oxidative stress effects with those involving increased estrogen levels. We also analyze potential preventive actions that might be accessible. There remains the fact that alcohol drinking is just one of the avoidable causes of breast cancer and that, at present, the suggested acceptable dose for prevention of this risk is of one drink per day.

Phenolic metabolism and molecular mass distribution of polysaccharides in cellulose-deficient maize cells

As a consequence of the habituation to low levels of dichlobenil （DCB）, cultured maize cells presented an altered hemicellulose cell fate with a lower proportion of strongly wall-bound hemicelluloses and an increase in soluble extracellular polymers released into the culture medium. The aim of this study was to investigate the relative molecular mass distributions of polysaccharides as well as phenolic metabolism in cells habituated to low levels of DCB （1.5 μM）. Generally, cell wall bound hemicelluloses and sloughed polymers from habituated cells were more homogeneously sized and had a lower weight-average relative molecular mass. In addition, polysaccharides underwent massive cross-linking after being secreted into the cell wall, but this cross-linking was less pronounced in habituated cells than in non-habituated ones. However, when relativized, ferulic acid and p-coumaric acid contents were higher in this habituated cell line. Feasibly, cells habituated to low levels of DCB synthesized molecules with a lower weight-average relative molecular mass, although cross-linked, as a part of their strategy to compensate for the lack of cellulose.

Singular spectrum analysis （SSA） is a technique that decomposes a time series into a set of components, such as, trend, harmonics, and residuals. Leaving out the residual components and adding up the others, the time series can be smoothed. This procedure has been used to model Brazilian electricity consumption and flow series. The PAR（p）, periodic autoregressive models, has been broadly used in modelling energy series in Brazil. This paper presents an approach of this decomposition method, by fitting the PAR（p）, considering its multivariate version known as multivariate SSA （MSSA）. The method was applied to a vector of two wind speed series recorded at two locations in the Brazilian northeast region. The obtained results, when compared to the univariate decomposition of each series, were far superior, showing that the spatial correlation between the two series were considered by MSSA decomposition stage.

Virtual Conductance Based Cascade Voltage Controller for VSCs in Islanded Operation Mode

Voltage source converters have become the main enabler for the integration of distributed energy resources in *** the case of islanded operation,these devices normally set the amplitude and frequency of the network voltage by means of a cascade controller composed of an outer voltage control loop and an inner current control *** strategies to compute the gains of both control loops have been proposed in the literature in order to obtain a fast and decoupled response of the voltages at the point of common *** paper proposes an alternative and simple methodology based on the introduction of a virtual conductance in the classic cascade *** strategy allows to design each control loop independently,obtaining a closed-loop response of a first-order *** this way,the gains of each control loop are easily derived from the parameters of the LC coupling filter and the desired closed-loop time ***,a state observer is included in the controller to estimate the inductor current of the LC filter in order to reduce the number of required measurements.A laboratory testbed is used to validate and compare the proposed *** experimental results demonstrate the effectiveness of the proposal both in steady-state and transient regimes.
