Multicenter application of a nursing workload measurement scale in adult hospitalization units

Objectives:The study aimed to the multicenter application of a nursing workload measurement scale in the internal medicine and surgery adults hospitalization ***:The study design was a multicenter,observational,and descriptive study.A multicenter application of the MIDENF®nursing workload measurement scale was carried out,which consists of 21 items,and covers the four nursing functions(patient care items,teaching,manager,and researcher),in units of hospitalization of adults of internal medicine and surgery of four different *** item contains one or more of the nursing interventions of Nursing Interventions Classification(NIC)and has an assigned time,after comparing the real time it takes to perform each intervention with the North American Nursing Diagnosis Association(NANDA)standardized *** study was carried out during nine months of the year 2020,measuring two days each month in the three work shifts(morning,evening,and night)to all patients admitted on the days of measurement in the indicated ***:The descriptive and inferential analysis of 11,756 completed scales,5,695 in general surgery and 6,061 in internal medicine,showed a greater care load for the two units during the morning shift(227,034 min in general surgery,261,835 min in internal medicine),especially in the items of“self-care”,“medication”,“common invasive procedures”,“fluid therapy”,and“patient and family support”,while the managerial function was similar during the three work shifts in the two units studied,getting values between 57,348 and 62,*** the analysis by shift and unit,statistical significance was obtained both for the total workload and the four nursing functions(Plusions:It is shown that the use of validated scales with the standardized language of nursing functions,adapted to the units,provides objective information to adjust the nursing staff to the real situation of care in any hospital and unit where it is applied,improving quality

Dear Editor,Thioredoxins （Trxs） are key components of the redox system that regulates the activity of a spectrum of target proteins through dithiol-disulfide exchange reactions. Trxs are reduced by mem- bers of the Trx reductase （TR） family （Jacquot et al., 2009）. NADP-dependent thioredoxin reductases （NTRs）, the most com-mon type, belong to the family of dimeric pyridine nucleotide disul-fide oxidoreductase flavoproteins that use NADPH as the source of reducing equivalents.

Multi-walled carbon nanotubes (CNTs) were submitted to chemical and thermal treatments in order to incorporate different heteroatoms on the surface. O-, S- and N-containing groups were successfully introduced onto the CNTs without significant changes of the textural properties. The cata-lytic activity of these heteroatom-modified CNTs was studied in two liquid phase oxidation processes: catalytic ozonation and catalytic wet air oxidation (CWAO), using oxalic acid and phenol as model compounds. In both cases, the presence of strongly acidic O-containing groups was found to decrease the catalytic activity of the CNTs. On the other hand, the introduction of S species (mainly sulfonic acids) enhanced the removal rate of the model compounds, particularly in the CWAO of phenol. Additional experiments were performed with a radical scavenger and sodium persulfate, in order to clarify the reaction mechanism. Nitrogen functionalities improve the catalytic performance of the original CNTs, regardless of the process or of the pollutant.

Physical activity in primary and secondary prevention of cardiovascular disease:Overview updated

Although the observed progress in the cardiovascular disease treatment, the incidence of new and recurrent coronary artery disease remains elevated and constitutes the leading cause of death in the developed countries. Three-quarters of deaths due to cardiovascular diseases could be prevented with adequate changes in lifestyle, including increased daily physical activity. New evidence confirms that there is an inverse dose-response relationship between physical activity and cardiovascular disease and mortality risk. However, participation in moderate to vigorous physical activity may not fully attenuate the independent effect of sedentary activities on increased risk for cardiovascular diseases. Physical activity also plays an important role in secondary prevention of cardiovascular diseases by reducing the impact of the disease, slowing its progress and preventing recurrence. Nonetheless, most of eligible cardiovascular patients still do not benefit from secondary prevention/cardiac rehabilitation programs. The present review draws attention to the importance of physical activity in the primary and secondary prevention of cardiovascular diseases. It also addresses the mechanisms by which physical activity and regular exercise can improve cardiovascular health and reduce the burden of the disease.

Disturbance and forest recovery in the Monarch Butterfly Biosphere Reserve,Mexico

Analysis of the change in forest cover is important to determine stand dynamics and the processes involved in disturbance and *** of the core zone of the Monarch Butterfly Biosphere Reserve were studied using photo interpretation techniques,considering biennial changes between 1999 and 2013 and changes over the whole period of study 1999–*** matrices were elaborated to determine the processes of change involved in both recovery and *** biennial changes for the whole period amounted to 2274 ha;343 ha accounted as degraded in more than one biennial *** total changes in forest cover between 1999 and 2013 involved 4902 ha,out of which,2912 ha were affected by disturbance,and 1990 ha were *** density and 2013 vegetation cover maps,the overall accuracy was 95.6%and 90.2%,*** combining both maps,gradual processes were revealed that were not evident in separate *** methodology is useful for the management and conservation of natural protected areas.

New-onset diabetes after kidney transplantation:Incidence and associated factors

AIM To determine the incidence and associated factors of new-onset diabetes after transplantation(NODAT) in a Portuguese central hospital. METHODS This single-center retrospective study involved consecutive adult nondiabetic transplant recipients, who had undergone kidney transplantation between January 2012 and March 2016. NODAT was diagnosed according to the criteria of the American Diabetes Association. Data were collected from an institutional database of the Nephrology and Kidney Transplantation Department(Santa Maria Hospital, lisbon, Portugal) and augmented with data of laboratorial parameters collected from the corresponding patient electronic medical records. Exclusion criteria were preexisting diabetes mellitus, missing information and follow-up period of less than 12 mo. Data on demographic and clinical characteristics as well as anthropometric and laboratorial parameters were also collected. Patients were divided into two groups: With and without NODAT-for statistical *** A total of 156 patients received kidney transplantduring the study period, 125 of who were included in our analysis. NODAT was identified in 27.2% of the patients(n = 34; 53% female; mean age: 49.5 ± 10.8 years; median follow-up: 36.4 ± 2.5 mo). The incidence in the first year was 24.8%. The median time to diagnosis was 3.68 ± 5.7 mo after transplantation, and 76.5% of the patients developed NODAT in the first 3 mo. In the group that did not develop NODAT(n = 91), 47% were female, with mean age of 46.4 ± 13.5 years and median follow-up of 35.5 ± 1.6 mo. In the NODAT group, the pretransplant fasting plasma glucose(FPG) levels were significantly higher [101(96.1-105.7) mg/d l vs 92(91.4-95.8) mg/d l, P = 0.007] and pretransplant impaired fasting glucose(IFG) was significantly more frequent(51.5% vs 27.7%, P = 0.01). Higher pretransplant FPG levels and pretransplant IFG were found to be predictive risk factors for NODAT development [odds ratio(OR): 1.059, P = 0.003; OR: 2.77

Phosphorus fractions and sorption dynamics in the sediments of two Ca-SO4 water reservoirs in the central Argentine Andes

Phosphorus（P） fractionation and sorption behavior were studied in the sediments of two calcium-sulfate(Ca-SO4: Ca2+170 mg l-1; SO42-400 mg l-1) water reservoirs in the southern central Argentine Andes-El Carrizal Reservoir（ECR）,which stratifies in summer and El Nihuil Reservoir（ENR）,which is vertically mixed throughout the year. Sediment size classes reflected the lithology of both basins and the reservoirs relative location: ECR（downstream from a valley adjacent to a mountain environment） had higher clay proportions while the sand fraction was more important in ENR（just next to a mountain environment）.In both reservoirs, the chemical composition revealed low alteration and calcium enrichment. Total P content was relatively high(1.1-1.6 mg P kg-1dry weight) as expected from the contribution of marine formations. P fractionation was dominated by Ca-P（ECR, 69% and ENR, 63%） followed by organic-P（23%）in summer-anoxic ECR and by iron/aluminum（Fe/Al）-P（27%） in oxic ENR. Batch experiments showed that sorption behavior in ECR had a typical langmuir isotherm plot while in ENR that model fitted only at low dissolved P values(l-1) and co-precipitation evidence was observed at higher P concentrations. It is proposed that lower Fe/Al-P content in anoxic（ECR） vs. oxic（ENR） sediments results from the P released from iron-bound phosphorus due to sulfate reduction-ferrous sulfide formation in an anoxic environment. This condition should enable ECR sediments to adsorb P into fresh oxidized iron,and ENR sediments to co-precipitate P with calcium due to Fe/Al-P saturation, as observed in batch experiments. This paper constitutes the first description of phosphorus content and dynamics in reservoir sediments in arid central western Argentina, where water sulfate concentrations are naturally high due to basin lithology.

AIM： Although increased insulin-like growth factor-I receptor （IGF-IR） gene expression has been reported in hepatocellular carcinoma, studies assessing IGF-IR in chronic hepatitis C （CHC） and cirrhosis are scarce. We therefore aimed to evaluate IGF-IR and IGF-I rnRNA expression in liver from patient with CHC. METHODS： IGF-IR and IGF-I rnRNA content were determined by semi-quantitative RT-PCR and IGF-IR protein expression was determined by immunohistochemistry in hepatic tissue obtained from patients with CHC before （34 patients） and after （10 patients） therapy with interferon-α and ribavirin. RESUlTS： An increase of IGF-IR rnRNA content was observed in hepatic tissue obtained from all CHC patients as well as from 6 cadaveric liver donors following orthopic transplantation （an attempt to evaluate normal livers） in comparison to normal liver, while no relevant modifications were detected in IGF-I mRNA content. The irnrnunohistochemical results showed that the raise in IGF-IR rnRNA content was related both to ductular reaction and to increased IGF-IR expression in hepatocytes. A decrease in IGF-IR rnRNA content was observed in patients who achieved sustained virological response after therapy, suggesting an improvement in hepatic damage. CONClUSION： The up-regulation of IGF-IR expression in hepatocytes of patients with CHC could constitute an attempt to stimulate hepatocyte regeneration. Considering that liver is the organ with the highest levels of IGF-I, our finding of increased IGF-IR expression after both acute and chronic hepatic damage highlights the need for additional studies to elucidate the role of IGF-I in liver regeneration.
