BACKg.OUND Sacubitril-valsartan has been shown to reduce hospitalizations and mortality in patients with heart failure(HF)and reduced ejection *** PIONEER-HF trial demonstrated that initiation of the drug.during.acute HF hospitalization reduced NT-proBNP levels and a post-hoc analysis of the trial found a reduction in HF hospitalizations and ***-life studies in the elderly population are *** aim of our study was to assess the effectiveness of sacubitril-valsartan versus ang.otensin converting.enzyme inhibitors(ACEI)in elderly patients who initiate this treatment during.hospitalization for acute *** We conducted a retrospective cohort study using.the Spanish acute heart failure reg.stry(RICA)comparing.rehospitalizations and deaths at 3 months and 1 year among.patients ag.d 70 years or older who had initiated treatment with sacubitrilvalsartan during.hospitalization for acute HF versus those treated with *** One hundred and ninety-nine patients hospitalized between October 2016 and November 2020 were included,with a median ag. of 82 years and hig. rate of *** these,107 were treated with sacubitril-valsartan and 92 with *** adjusted OR for readmission for HF at 3 months was 0.906(95%CI:0.241-3.404)and for the combined variable readmission for HF or death at 3 months was 0.696(95%CI:0.224-2.167).The adjusted OR for HF readmission at one year was 0.696(95%CI:0.224-2.167).and for the combined variable HF readmission or death at one year 0.724(95%CI:0.325-1.612).CONCLUSION Treatment with sacubitril-valsartan initiated early in hospitalization for HF in elderly patients with hig. comorbidity was associated with a trend towards a reduction in readmissions and death due to HF compared to treatment with ACEI,which did not reach statistical sig.ificance either at 3 months or 1 year of follow-up.

Mushroom Production as an Alternative for Rural Development in a Forested Mountainous Area

Wild mushrooms are recog.ized as important non-wood forest products in mountainous ecosystems, but their real potential for g.nerating.rural economies has not been fully evaluated due to the difficulties in obtaining.reliable productivity data, minimizing.their true potential as contributor to rural economies. Mushroom yield models based on larg. data series from Pinus forest ecosystems in the reg.on of Catalonia(Spain), combined with data from the Spanish National Forest Inventory allow us to estimate the potential mushroom productivity by forest ecosystems. The results of 24,500 tons/yr of mushrooms of which 16,300 tons are classified as edible and 7,900 tons are commonly marketed demonstrate the importance of mushroom productions in Catalonian pine forests, mostly located in mountainous areas where the development of ag.icultural activities is limited. Economic mushroom value is estimated at 48 million € for the edible mushroom and 32 million € for those corresponding.to marketable yields, confirming.the potential of this non-wood forest product. These production results and corresponding.economic values provide a basis for the incorporation of wild mushrooms as sig.ificant non-wood forest products in the development of forest policies in mountainous areas.

Areas of research and clinical approaches to the study of liver abscess

AIMTo analyze the research activity on liver abscess (LA) and identify the main topic clusters in the *** identified all documents using.the medical subject heading.“LA” indexed in the MEDLINE database between 2001 and 2015. We performed a descriptive bibliometric analysis, characterizing.the evolution of scientific activity, the publication types of the documents, the document categ.ries of clinical interest (case reports, clinical trials, evaluation studies, meta-analysis, observational studies, practice g.idelines and validation studies) and the g.og.aphic distribution of the research. We also carried out an analysis of networks and research clusters in order to identify the main topic areas of *** search yielded a total of 1278 documents, showing.a stable scientific production over the study period and a marked multidisciplinary nature. The research was dominated by case reports (65.9% of the documents analyzed). In terms of g.og.aphic distribution, researchers from the United States led in the number of sig.atures (n = 229), followed by those from Taiwan (n = 185), India (n = 145), Japan (n = 144), South Korea (n = 100), and China (n = 84). With reg.rd to amebic LA, the top-producing.countries were India and Mexico (n = 69 each), followed by the United States (n = 29). In the case of pyog.nic LA, Taiwan Residents researchers led scientific production (n = 71), followed by the United States (n = 39) and China (n = 29). The most active areas of research in the field are diag.osis via computerized tomog.aphy scan, differential diag.osis with reg.rd to liver cancer, treatment with antimicrobial ag.nts, and Klebsiella infections (including.bacteremia).CONCLUSIONClinical case reports associated with diag.osis and treatment are the main topic of study, hig.lig.ting.the importance of this document type in advancing.knowledg..

Immunomodulating.effects of antibiotics used in the prophylaxis of bacterial infections in advanced cirrhosis

The use of norfloxacin either as primary or secondary prophylaxis of bacterial infections in advanced cirrhosis has improved patient's survival. This may be explained not only due to a sig.ificant decrease in the number of infections, but also because of a direct immunomodulatory effect. Selective intestinal decontamination with norfloxacin reduces translocation of either viable bacteria or bacteria-driven products from the intestinal lumen. In addition, norfloxacin directly modulates the systemic inflammatory response. The proinflammatory cytokine profile secreted by neutrophils from these patients shows a close, sig.ificant, and inverse correlation with serum norfloxacin levels. Similar effects have been described with other quinolones in different clinical conditions. Althoug. the underlying.mechanisms are not well defined for most of the antibiotics, the pathways trig.ered for norfloxacin to induce such immunomodulatory effects involve the down-reg.lation of pro-inflammatory inducible nitric oxide synthase, cyclooxyg.nase-2, and NF-κB and the up-reg.lation of heme-oxyg.nase 1 and IL-10 expression. The knowledg. of these immunomodulatory effects, additional to their bactericidal role, improves our comprehension of the interaction between antibiotics and the cellular host response and offer new possibilities for the development of new therapeutic strateg.es to manag. and prevent bacterial infections in cirrhosis.

Because of the g.owing.worldwide demand for maize g.ain,new alternatives have been soug.t for breeding.of this cereal,e.g.,development of polyembryonic varieties,which ag.onomic performance could positively impact the g.ain yield per unit area,and nutritional *** objectives of this study were to(1)determine the phytochemicals present in the embryo and endosperm of g.ain from maize families with hig.,low,and null polyembryony frequency,which were planted at different locations,and(2)state the relationship between these compounds and seed *** extracted phytochemicals from corn were identified by *** results showed that the g.notype with the hig.est presence of phytochemicals was the brachytic population with hig. polyembryony called“BAP”,which also required less water during.the g.rmination *** number of phytochemicals in both embryo and endosperm tissues was not related to the sowing.location where they came from or the type of *** number of different phytochemicals depended on the g.ain tissue from where they were *** chemical compounds found in the different maize tissues were related to the development of the plant,either in roots or nibs because these are mainly associated with the lig.in synthesis.
