Is it possible to adopt the same oncological approach in urgent surgery for colon cancer?

BACKGROUND Locoregional complications may occur in up to 30%of patients with colon *** they are frequent events in the natural history of this disease,there should be a concern in offering an oncologically adequate surgical treatment to these *** To compare the oncological radicality of surgery for colon cancer between urgent and elective *** One-hundred and eighty-nine consecutive patients with non-metastatic colon adenocarcinoma were studied over two years in a single institution,who underwent surgical resection as the first therapeutic approach,with 123 elective and 66 urgent *** assessment of oncological radicality was performed by analyzing the extension of the longitudinal margins of resection,the number of resected lymph nodes,and the percentage of surgeries with 12 or more resected lymph *** clinicopathological variables were compared between the two groups in terms of sex,age,tumor location,type of urgency,surgical access,staging,compromised lymph nodes rate,differentiation grade,angiolymphatic and perineural invasion,and early *** There was no difference between the elective and urgency group concerning the longitudinal margin of resection(average of 6.1 in elective vs 7.3 cm in urgency,P=0.144),number of resected lymph nodes(average of 17.7 in elective vs 16.6 in urgency,P=0.355)and percentage of surgeries with 12 or more resected lymph nodes(75.6%in elective vs 77.3%in urgency,P=0.798).It was observed that the percentage of patients aged 80 and over was higher in the urgency group(13.0%in elective vs 25.8%in urgency,P=0.028),and the early mortality was 4.9%in elective vs 15.2%in urgency(P=0.016,OR:3.48,95%CI:1.21–10.06).Tumor location(P=0.004),surgery performed(P=0.016)and surgical access(Pmay be achieved in both emergency and

Slip-model Performance for Underexpanded micro-scale Rocket Nozzle Flows

In aerospace micro-Electro mechanical Systems （mEmS）, the characteristic length scale of the flow approaches the molecular mean free path, thus invalidating the continuum description and enforcing the use of particle methods, like the Direct Simulation monte Carlo （DSmC）, to deal with the non-equilibrium regions. Within the slip-regime （0.01〈Kn〈-0.1） both approaches, continuum and particle-based, seem to behave well in terms of accuracy. The present study summarizes the implementation and results obtained with a 2nO-order slip boundary condition in a Navier-Stokes solver to address the rarefaction near the nozzle walls. Its assessment and application to a cold-gas micro-scale conical nozzle of 300μm throat diameter, discharging into the low-pressure freestream, constitutes the major aim of the work. The slip-model incorporates the velocity slip with thermal creep and temperature jump, thus permitting to deal with non-isothermal flows as well. Results show that the gas experiences an intense rarefaction in the lip vicinity, pointing to the limits of model validity. Furthermore, a strong mach deceleration is observed, attributed to the rather thick subsonic boundary layer and supersonic bulk heating caused by the viscous dissipation, in contrast with the expansion to occur in large rocket nozzles during underexpanded operation.

Plant peroxisomes are subcellular compartments involved in many biochemical pathways during the life cycle of a plant but also in the mechanism of response against adverse environmental *** organelles have an active nitro-oxidative metabolism under physiological conditions but this could be exacerbated under stress ***,peroxisomes have the capacity to proliferateand also undergo biochemical adaptations depending on the surrounding cellular *** important characteristic of peroxisomes is that they have a dynamic metabolism of reactive nitrogen and oxygen species(RNS and ROS)which generates two key molecules,nitric oxide(NO)and hydrogen peroxide(H2O2).These molecules can exert signaling functions by means of post-translational modifications that affect the functionality of target molecules like proteins,peptides or fatty *** review provides an overview of the endogenous metabolism of ROS and RNS in peroxisomes with special emphasis on polyamine and uric acid metabolism as well as the possibility that these organelles could be a source of signal molecules involved in the functional interconnection with other subcellular compartments.

Entropy model to assess sediment resuspension probability and trap efficiency of small dams

In the Brazilian drylands, there are tens of thousands of small dams. Despite their paramount importance to the rural population, they are rarely monitored. Water demand increases with time while, simultaneously, siltation reduces reservoir water capacity and availability. Reservoir siltation models are, therefore, mandatory to manage the numerous ungauged small dams in these *** objective of the current study is to improve sediment trap efficiency(TE) modeling by including resuspension as a key probabilistic process. The Shannon entropy was used to estimate the sediment resuspension probability, which was merged with the Camp model and generated the RETSED entropy model. To validate the RETSED model, an experimental check dam(ECD, 300 m3), located in the Gilbu es desertification site, Brazil, was monitored hourly during one hydrological year(July 2018-July 2019). measurements show that the annual volumetric decline of the check dam was 12%; and that the average trap efficiency equaled 86%. Only 9.5% of the hourly sediment concentration outflow exceeded the average plus one standard deviation, showing that the reservoir is well mixed; a fact which highlights the relevance of the resuspension process. Three empirical models failed to mimic the experimental results: Churchill(TE = 99%), Brune(TE = 75%), and maryland(TE = 94%). According to the RETSED entropy model, the resuspension probability during the experiment was 10%and TE = 81%, a value only 6% below the measured one. The Camp model simulated TE = 89%, only 3%higher than the measured value, but confirmed the Camp model’s tendency to overestimate TE due to a disregard of resuspension. The deterministic model showed low sensitivity concerning the hydrodynamic effects, whereas the entropy formulation proved to be more consistent with physical behavior: the resuspension probability clearly increased and trap efficiency decreased with rising reservoir discharge.

In this paper we prove the behaviour in weighted Lp spaces of the oscillation and variation of the Hilbert transform and the Riesz transform associated with the Hermite operator of dimension 1. We prove that this operator maps LP（R, w（x）dx） into itself when w is a weight in the Ap class for 1 〈 p 〈 ∞. For p = 1 we get weak type for the A1 class. Weighted estimated are also obtained in the extreme case p = ∞.

Improving outcomes in pancreatic cancer: Key points in perioperative management

This review focused in the perioperative management of patients with pancreatic cancer in order to improve the outcome of the disease. We consider that the most controversial points in pancreatic cancer management are jaundice management, vascular resection and neo-adjuvant therapy. Preoperative biliary drainage is recommended only in patients with severe jaundice, as it can lead to infectious cholangitis, pancreatitis and delay in resection, which can lead to tumor progression. The development of a phase III clinical trial is mandatory to clarify the role of neo-adjuvant radiochemotherapy in pancreatic adenocarcinoma. Venous resection does not adversely affect postoperative mortality and morbidity, therefore, the need for venous resection should not be a contraindication to surgical resection in selected patients. The data on arterial resection alone, or combined with vascular resection at the time of pancreatectomy are more heterogeneous, thus, patient age and comorbidity should be evaluated before a decision on operability is made. In patients undergoing R0 resection, arterial resection can also be performed.

mETA-R(multi-environment trial analysis in R)is a suite of R scripts linked by a graphical user interface(GUI)designed in Java *** objective of mETA-R is to accurately analyze multi-environment plant breeding trials(mETs)by fitting mixed and fixed linear models from experimental designs such as the randomized complete block design(RCBD)and the alpha-lattice/lattice ***-R simultaneously estimates the best linear and unbiased estimators(BLUEs)and the best linear and unbiased predictors(BLUPs).Additionally,it computes the variance-covariance parameters,as well as some statistical and genetic parameters such as the least significant difference(LSD)at 5%significance,the coefficient of variation in percentage(CV),the genetic variance,and the broad-sense *** parameters are very important in the selection of top performing genotypes in plant ***-R also computes the phenotypic and genetic correlations among environments and between traits,as well as their statistical *** genetic correlations between environments or traits can be visualized in a biplot graph or a tree diagram(dendrogram).Genetic correlations are very important for identifying environments with similar behavior or making indirect selection and identifying the most highly associated ***-R performs multi-environment analyses by using the residual maximum likelihood(REmL)method;these analyses can be done by environment,across environments by grouping factors(stress conditions,nitrogen content,etc.)and across environments;the analyses across environments can be done with a pre-defined degree of heritability.

Electron Confinement Effect of Laser Dyes within Dendritic Structures

Dendrimers are a novel class of nanometric-size macromolecules with a regular tree-dimensional like array of branch units.1,2 Their synthetic availability in a wide range of sizes combined with their peculiar architecture makes them versatile building blocks for a wide range of potential applications.3 Some years ago, meijer and co-workers reported that the modification of terminal amine functionalities of a fifth generation poly(propyleneimine) dendrimer (DAB-dendr-(NH2)64) with bulky substituents, (typically N-t-BOC protected phenylalanine), results in the formation of the so-called “dendritic box” (DAB-dendr-(NH-t-BOC-L-Phe)64).4 Within this macromolecular structure it is possible to encapsulate a variety of guest molecules due to the existence of internal cavities in the core. The photophysical properties of the guests can be modulated by the innovative electron confinement effect. In this respect, we wish to report that the emission frequency of organic dyes can be easily modulated by encapsulation in a dendritic box. The emission bands of dye molecules incorporated into a dendrimer can effectively be red shifted with respect to their emission in solution and contrary to other confined spaces of considerable hardness, the magnitude of this shifting can be regulated under appropriate experimental conditions. This peculiar effect could have unprecedented applications in the development of supramolecular devices relating to the frequency tuning of organic laser dyes.
